Relationship between Female Sexual Function and Depression or Anxiety in Japan

Background: Sexual dysfunction is sexual complaints or disorders. There are various causes, but the onset leads to deterioration of relationships and quality of life with partners. There is still a deep-rooted awareness that sexuality is a secret in Japan. Aim: This study aims to clarify the relationship between female sexual function and depression or anxiety. Method: Four hundred and fifty-eight Japanese healthy women who worked in four randomly selected medical institutions in Okinawa Prefecture in Japan from March to May 2012 were asked to agree to a consent form of their own free will and participate in self-reported questionnaires. For their evaluation, the Female Sexual Function Index (FSFI;Rosen et al. 2000, which includes the six domains of desire, arousal, lubrication, orgasm, pain, and satisfaction) and the Kessler Psychological Distress Scale (K6;Kessler, 2002, which includes the following six items: felt nervous, hopeless, restless or fidgety, worthless, depressed, and felt that everything was an effort) was used. Moreover, participants’ characteristics showed age, marriage, menopause, annual income, and drinking. In addition, JMP16.2 and R 4.2.0 was used to perform Pearson’s chi-square test, Fisher’s exact test, Mann-Whitney U test, Kruskal-Wallis test, Steel-Dwass test, Spearman’s Rank-Order Correlation, and Cronbach’s alpha. This study was approved by the university and the medical institutional review board (IRB). Results: 178 women were included in the final analysis, and the median (IQR, interquartile range) was 39 (32 - 48) years old, and the mean ± SD (standard deviation) was 40.2 ± 10.4 years old. The median (IQR) and mean ± SD of FSFI Total Score were 22.0 (9.3 - 26.6) and 19.2 ± 9.6. The median (IQR) and the mean ± SD of K6 Total Score were 3 (0 - 7) and 4.2 ± 4.5. 16% of all women with a K6 Total Score of 10 and more, and 5% of all women with a K6 Total Score of 13 and more considered a serious mental illness. The Effect of Stress, Fatigue, and Sleep Quality on Shift-Work Nurses in Japan

Purpose: To investigate the relationship between stress, fatigue and sleep quality among shift work nurses in Japan. Design: A descriptive correlation design using self-administered surveys. Methods: Questionnaires on stress, fatigue and sleep quality (the Pittsburgh Sleep Quality Index;PSQI-J) among shift work nurses were distributed to 653 nurses in Japan from January 2015 to February 2015. Results: Participants who reported higher levels of fatigue reported higher levels of stress (r = 0.774, p = 0.000), lower levels of sleep satisfaction (r = −0.411, p = 0.000) and overall lower levels of health (r = −0.323, p = 0.000).PSQI scores were also significantly correlated with reported levels of stress (r = 0.294, p = 0.000), fatigue (r = 0.291, p = 0.000) and levels of health (r = −0.370, p = 0.000). Nurses who have stress-relieving recourses were younger (t = −2.842, p = 0.005), reported higher levels of overall health (t = 2.727, p = 0.007), had higher levels of sleep quality measured by the PSQI (t = −2.560, p = 0.011), and required less time to fall asleep (t = −3.207, p = 0.001). Conclusions: This study showed an association between stress and sleep satisfaction among shift work nurses in Japan. In addition, nurses that engage in activities to manage stress might achieve higher levels of health and high sleep quality.

Duodenal stenosis resulting from a preduodenal portal vein and an operation for scoliosis

A preduodenal portal vein （PDPV） is known to be a rare cause of duodenal stenosis. We treated a 22-yearold male patient with malnutrition as a result of PDPV and a previously performed operation for scoliosis, who showed an improvement in quality of life after being treated with a combination of nutritional support and surgery. The patient with PDPV had been admitted to our department with duodenal stenosis, ranging from the first to third portions. He had suffered from vomiting since 1 year of age, and he developed malnutrition during the last 6-too period after orthopedic surgery for scoliosis. The stenosis was related to both the PDPV and the previously performed operation for scoliosis. After receiving nutritional support for 6 mo, a gastrojejunostomy with Braun＇s anastomosis for the first portion and a duodenojejunostomy for the second and third portions were performed. The postoperative course was almost uneventful. Three months later, he was discharged and able to attend university. In patients with widespread duodenal stenosis, there may be a complicated cause,such as PDPV and duodenal stretching induced by previous spinal surgery.

Primary liposarcoma of gallbladder diagnosed by preoperative imagings: A case report and review of literature

A 49-year-old Japanese woman was referred to our department because of high fever and a huge abdominal mass. Computed tomography （CT） and magnetic resonance （MR） imagings revealed a tumor, about 30 cm in diameter, occupied the right hepatic lobe and the peritoneal cavity. Abdominal angiography showed that the tumor was fed mainly by the cystic artery. We preoperatively diagnosed angiosarcoma of the gallbladder and performed tumor resection with cholecystectomy because the tumor was almost casplated, however the posterior wall of the gallbladder attached to the tumor firmly. Histologically, the tumor was composed of spindle cells including lipoblasts with cellular pleomorphism, which were also detected in the muscular layer of the gallbladder. We finally diagnosed pleomorphic liposarcoma of the gallbladder. At 10 mo and 29 mo after the first operation, she underwent two more operations because of recurrence. Now she has a good quality of life 3 years and 6 mo after the first operation.

Spin Trapping of the Phosphorus-centered Radicals Generated from Hydrogen Abstraction Reaction by 2,2-Diphenyl-1-picrylhydrazyl

Two kinds of the phosphorus-centered radicals produced from hydrogen atomion by 2, 2-diphenyl-1-picrylhydrazyl(DPPH were trapped by N-tert-butyl-α-phenylnitrone(PBN) and 5, 5-dimethyl-1-pyrroline N-oxide (DMPO), and investigated by means of EPR spectroscopy. The spin adducts with a characteristic hyperfine coupling constant(hfcc) caused by a phosphorus atom were observed. Based on the hfcc values caused by the phosphorus and hydrogen atoms, the con formational positions of the adducts trapped by PBN and DMPO are discussed.

Laparoscopic personalized function-preserving gastrectomy with sentinel node mapping for early-stage gastric cancer

Laparoscopic gastrectomy is considered as an indispensable option between endoscopic resection and standard gastrectomy with open laparotomy for patients with early-stage gastric ***,the extent of gastrectomy and remnant gastric function may affect patients’quality of life(QOL)after ***,function-preserving gastrectomy in addition to laparoscopic surgery could be considered in patients with early-stage gastric cancer.A prospective multicenter trial and meta-analyses of sentinel node(SN)mapping and biopsy for early-stage gastric cancer have demonstrated favorable SN detection rates and accuracy of nodal metastatic *** a combination of radioactive colloids with blue dyes as tracers is currently considered as the promising procedure of SN mapping in early-stage gastric cancer,several new technologies,such as indocyanine green fluorescence imaging,may markedly improve its *** early-stage gastric cancer,the development of laparoscopic personalized minimized gastrectomy with SN mapping may help retain patents’QOL after surgery.A recently developed full-thickness partial gastrectomy with SN mapping and basin dissection would become a reliable minimally invasive gastrectomy for treating patients with cN0 early-stage gastric cancer.

Characteristic pathological findings and effects of ecabet sodium in rat reflux esophagitis

AIM： To explore the pathological findings in the entire esophagus in rats with reflux esophagitis, and the effects of ecabet sodium （ES）. METHODS： A rat model of chronic acid reflux esophagitis was used. In the treatment group, ES was administered after surgery （n = 16）. No drug was administered postoperatively to the esophagitis group （n = 9）. Shamoperated rats were used as a control group （n = 5）. Rats were sacrificed on day 7 after the operation. The epithelial thickness and leukocyte infiltration were examined in the upper, middle and lower areas of the esophagus. The survival rate, incidence of esophageal ulcer, and mean surface area and number of esophageal ulcers were determined in the esophagitis and ES groups. Esophageal histology was assessed in all three groups. RESULTS： Leukocyte infiltration in the esophagitis group was 26.3 ± 22.0 in the middle esophagus and 8.2 ± 4.9 in the upper esophagus, which was significantly greater than that in the controls （1.3 ± 1.1 and 1.4 ±1.0, respectively）（P 〈 0.05）. The thickness of the epithelium in the esophagitis group was 210.8 ± 47.7 μm in the lower esophagus and 204.2 ± 60.1 μm in the middle esophagus, which was significantly greater than that in the controls （26.0 ± 5.5 and 21.0 ± 6.5 μm, respectively）（P 〈 0.05）. The mean number of ulcers per animal in the ES group in the entire esophagus was 5.4 =1= 2.5, which was significantly less than that in the esophagitis group （9.0 ± 3.5）（P 〈 0.05）. The epithelial thickness in the ES group was 97.5 ± 32.2 μm in the lower esophagus, which was decreased compared with that in the esophagitis group （210.8 ± 47.7 μm）（P 〈 0.05）. CONCLUSION： Mucosal inflammation extended to the upper esophagus close to the hypopharynx. Our study suggested that ES may have a useful defensive role in reflux esophagitis.

Rapid progress in information technology has come to enable us to store all the information in a hospital information system,including management data,patient records,discharge summary and laboratory *** the reuse of those data has not started,it has been expected that the stored data will contribute to analysis of hospital *** this paper,the discharge summary of Chiba University Hospital,which has been stored since 1980's were analyzed to characterize the university *** results show several interesting results,which suggests that the reuse of stored data will give a powerful tool to support a long-period management of a university hospital.
