Measuring vulnerability to COVID-19 and healthcare accessibility at the fine-grained level serves as the foundation for spatially explicit health planning and policy making in response to future public health ***,the evaluation of vulnerability and healthcare accessibility is insufficient in Japan-a nation with high population density and super-aging *** on the 2022 census data,transport network,medical and digital cadastral data,land use maps,and points of interest data,our study extends the concept of vulnerability in the context of COVID-19 and constructs the first fine-grained measure of vulnerability and healthcare accessibility in Tokyo Metropolis,Japan-the most populated metropolitan region in the *** delineate the vulnerable neighbourhoods with low healthcare access and further evaluate the disparity in healthcare access and built environment of areas at different levels of *** outcome datasets and findings provide nuanced and timely evidence to government and health authorities to have a holistic and latest understanding of social vulnerability to COVID-19 and healthcare access at a fine-grained *** analytical framework can be employed in different geographic contexts,guiding through place-based health planning and policy making in the post-COVID era and beyond.

This paper deals with the mode analysis of the kinematic structure of human locomotion. The authors investigated synergy mechanism of human locomotion from motion-captured data and EMG signal data. The authors extracted some common basic movements and residual modes, and analyzed the kinematical structures of limit cycle in joint angle space. The authors also implemented the numerical simulation analyses by using the motion captured data and EMG signal data to investigate the mechanical activities of human joints and to extract the mechanical structure of the limit cycle. The results show the joint synergy that is derived by the common basic modes, which expresses an inverted pendulum mode in support phase, and ballistic mode in swing phase with the kick-off motion in the most effective direction. This result can be guessed that the control strategy of human locomotion is simply based on the minimal control principle.

Formation of Necklace-Type Vortex System Upstream of a Gate Valve in Pipe Flow

A gate valve is one of the main elements of a circular pipeline, but the flow characteristics around the gate valve are hardly known. In this study, clarification of the flow field in front of the gate valve model in a pipe flow via flow visualization and PIV analysis was attempted. As a result, four kinds of steady necklace-type vortex systems, 2-vortex, 4-vortex, 6-vortex and 8-vortex systems, were clearly observed in a Reynolds number between 290 and 2130. In addition, the main vortex was observed in the Reynolds number range between 2130 and 4870 with difficulty. On this account, both the center position and vorticity in the main vortex are presented against Reynolds number.

A pilot randomized control study to evaluate endoscopic resection using a ligation device for rectal carcinoid tumors

AIM： Rectal carcinoid tumors smaller than 10 mm can be resected with local excision using endoscopy. In order to remove rectal carcinoid tumors completely, we evaluated endoscopic mucosal resection with a ligation device in this pilot control randomized study. METHODS： Fifteen patients were diagnosed with rectal carcinoid tumor （less than 10 mm） in our hospital from 1993 to 2002. There were 9 males and 6 females, with a mean age 61.5 years （range, 34-77 years）. The patientshad no complaints of carcinoid syndrome symptoms. Fifteen patients were randomly divided into 2 groups： 7 carcinoid tumors were treated by conventional endoscopic resection, and 8 carcinoid tumors were treated by endoscopic resection using a ligation device. RESULTS： All rectal carcinoid tumors were located at the middle to distal rectum. The size of the tumors varied from 3 mm to 10 mm and background characteristics of the patients were not different in the two groups. The rate of complete removal of carcinoid tumors using a ligation device （100%, 8/8） was significantly higher than that of conventional endoscopic resection （57.1%, 4/7）. The three patients had tumor involvement of deep margin, for which additional treatment was performed. No complications occurred during or after endoscopic resection using a ligation device. All patients in the both groups were alive during the 3-year observation period. CONCLUSION： Endoscopic resection using a ligation device is a useful and safe method for resection of small rectal carcinoid tumors.

The Impact of the Morphologic Characteristics of Type B Aortic Dissection in the Acute Phase on the Aortic Enlargement in the Late Stage

Objectives: According to the International Registry of Acute Aortic Dissection, >70% of patients with type-B aortic dissection (AD) have delayed aortic growth. The objective of this study was to investigate the morphologic characteristics of aortic enlargement in type-B AD. Methods: A total of 120 patients with uncomplicated acute type-B aortic dissection (uATBAD) were divided into 4 groups according to the presence of a patent or thrombosed false lumen (FL) with or without aortic enlargement. In all groups, the area of the true lumen (TL) and the FL were measured on axial computed tomography images. Results: A total of 120 uATBAD patients were evaluated: patent FL with (PE, N = 28, 23%) or without aortic enlargement (PU, N = 17, 14%) and thrombosed FL with (TE, N = 34, 28%) or without enlargement (TU, N = 41, 34%). The initial aortic diameter was not significantly different among the 4 groups (PE vs. PU = 39 ± 8 vs. 37 ± 10 (p = 0.354);TE vs. TU = 38 ± 6 vs. 37 ± 6 (p = 0.391)). The area of the FL tended to be larger in the PE group. In contrast, the area of the TL was significantly larger in the TE group. Late outcomes of uATBAD treated with optimal medical treatment were acceptable. Conclusions: In patients with uATBAD and a thrombosed FL, the patients with aortic enlargement tended to be increased TL size. In contrast, in patients with a patent FL, the patients with aortic enlargement tended to be increased FL size.

Unsteady Behavior of Necklace Vortex Produced by a Square Plate Protrusion

In this study, the unsteady behavior of necklace vortices formed in front of a square flat plate was experimentally investigated by flow visualization and PIV analysis by using a water channel. As a result, the necklace vortices started to oscillate when the Reynolds number increased beyond approximately 2000. Then, an amalgamation behavior of the necklace vortices took place when the Reynolds number increased beyond approximately 2650. Furthermore, in the same Reynolds number range, a breakaway behavior appeared when the relative height of the square plate was beyond approximately h/δ = 4.0. The features of the necklace vortices behaviors in the oscillation, amalgamation and breakaway states were explained by observing the time-series image of path lines and by analyzing the frequency of velocity fluctuation.

目的建立一种估算组织内硼绝对浓度的实用方法,该方法通过4-二羟硼基-2-F-氟-苯丙氨酸(F-FBPA)PET显像的标准摄取值(SUV)计算硼浓度,并与静脉滴注4-二羟硼基-苯丙氨酸(BPA)后直接取各组织进行硼浓度测量的方法进行比较。方法研究对象为C6胶质瘤鼠模型[n=7,体质量(241±28.0)g],静脉注射F-FBPA(30.5±0.7)MBq后60min进行PET显像。显像结束后所有模型鼠均按体质量经静脉慢速滴注BPA-果糖(167.3±18.65)mg/kg,滴注后60 min处死模型鼠,取出主要器官和肿瘤组织样本,样本内的硼绝对浓度(单位:ppm)采用电感耦合等离子发射光谱法(ICP-OES)测定。F-FBPA PET显像后,采用以下公式估算器官和肿瘤组织样本内硼浓度:估算的硼绝对浓度(ppm)=0.047 8×[BPA剂量(mg/kg)]×SUV。比较ICP-OES法测量的硼绝对浓度(mBC)和PET显像估算的硼浓度(eBC)。结果血液、脑、肝、脾、胰和肿瘤组织mBC和eBC的百分比差别分别是(-5.2±21.1)%、(-9.4±22.3)%、(1.6±21.3)%、(-14.3±16.8)%、(-9.5±27.5)%和(3.4±43.2)%。由于肺、小肠和大肠内包含的空气和粪便造成的部分容积效应,其硼浓度估算值偏低较多,分别为(-48.4±16.2)%、(-37.8±19.3)%和(-33.9±11.0)%。而由于尿液内较高的放射性,使肾脏硼浓度估算值偏高较多,为(34.3±29.3)%。结论器官和肿瘤组织内的硼绝对浓度可以由F-FBPA PET显像获得的SUV经计算公式进行估算,但要注意肺、小肠和大肠内硼浓度会因为部分容积效应而被低估,由此来避免过度照射带来的不良作用。
