Root Reconstruction with Total Replacement of Ascending Aorta Using Hypothermic Circulatory Arrest and Selective Cerebral Perfusion for Moderately Dilated Distal Ascending Aorta

Objective: We have occasionally encountered a moderately dilated distal ascending aorta while reconstructing an aortic root. We describe reconstruction of an extended root and ascending aorta using our current strategy. Patients and Methods: Between March 2011 and December 2012, 15 (11 men;mean age, 70.1 ± 7.3 years) patients underwent root reconstruction with hemi-arch replacement under hypothermic circulatory arrest with antegrade selective cerebral perfusion. The maximum diameter of the aortic root was 52.5 ± 4.4 mm. The distal ascending aorta just below innominate artery was moderately dilated to 41.7 ± 1.4 mm in diameter. Results: Operative outcomes excluding the diameter of the distal ascending aorta did not significantly differ from those of patients who had undergone root reconstruction under distal ascending aortic clamping during the same period. Postoperative computed tomography confirmed complete resection of the dilated ascending aorta in the patients, and did not develop neurological dysfunction or stroke. Conclusion: Postoperative computed tomography confirmed complete resection of dilated ascending aortae. We considered that complete resection under hypothermic circulatory arrest and selective cerebral perfusion might help to avoid repeated surgery to treat dilation of the distal ascending aorta over the long-term.

Gastric antral vascular ectasia(GAVE) has been recognized as one of the important causes of occult and obscure gastrointestinal *** diagnosis is typically made based on the characteristic endoscopic features,including longitudinal row of flat,reddish stripes radiating from the pylorus into the antrum that resemble the stripes on a *** appearances,however,can easily be misinterpreted as moderate to severe *** it is believed that capsule endoscopy(CE) is not helpful for the study of the stomach with its large lumen,GAVE can be more likely to be detected at CE rather than conventional *** can be regarded as "physiologic" endoscopy,without the need for gastric inflation and subsequent compression of the *** blood flow of the ecstatic vessels may be diminished in an inflated ***,GAVE may be prominent in *** herein describe a case of active bleeding from GAVE detected by CE and would like to emphasize a possibility that CE can improve diagnostic yields for GAVE.

A Rapid Evaluation Method of the Heat Removed by a VCS before Rise-to-Power Tests

Before rise-to-power tests, the actual measured value of heat released from the Reactor Pressure Vessel(RPV) or removed by the Vessel Cooling System(VCS) cannot be obtained. It is difficult for operators to evaluate the reactor outlet coolant temperature supplied from the High Temperature Engineering Test Reactor(HTTR) before rise-to-power tests. Therefore, when the actual measured value of heat released from the RPV or removed by the VCS are changed during rise-to-power tests, operators need to evaluate quickly, within a few minutes, the heat removed by the VCS and the reactor outlet coolant temperature of 30 MW, at 100% reactor power, before the temperature achieves 967℃ which is the maximum temperature limit generating the reactor scram. In this paper, a rapid evaluation method for use by operators is presented. The difference between the experimental and analytical results was within 30(k W) and it is appropriate that the presented evaluation method can be applied; therefore, operators can analyze the heat removed by the VCS quickly, within a few minutes, before and during Rise-to-Power Tests.

In this article, the fmite element solution of quasi-three-dimensional （quasi-3-D） groundwater flow was mathematically analyzed. The research shows that the spurious oscillation solution to the Finite Element Model （FEM） is the results choosing the small time step △t or the large element size L and using the non-diagonal storage matrix. The mechanism for this phenomenon is explained by the negative weighting factor of implicit part in the discretized equations. To avoid spurious oscillation solution, the criteria on the selection of △t and L for quasi-3-D groundwater flow simulations were identified. An application example of quasi-3-D groundwater flow simulation was presented to verify the criteria. The results indicate that temporal discretization scale has significant impact on the spurious oscillations in the finite-element solutions, and the spurious oscillations can be avoided in solving practical quasi-3-D groundwater flow problems if the criteria are satisfied.

In this study,an experiment was performed to clarify the flow field,in which the jets were normally injected into a main supersonic flow surrounded by a porous cavity,and this report figures out interaction between starting shock wave and porous *** the experiment,a porous cavity is attached to a main duct and jets and rods are inserted to the main duct on the porous *** reveal this flow field,the thermal tuft probe was adopted to experimentally investigate the flow in the *** the experiments,the effect of the porous cavity with jets or rods on the flow field is studied by means of visualization of schlieren method with a high speed camera and measurement of cavity flow with thermal tuft *** a results,frequency analysis of output of the thermal tuft probe revealed that some clear dominant frequencies were confirmed when the starting shock wave existed around the porous cavity in all cases of jets and rods ***,visualization of schlieren method with a high speed camera clarified that a starting shock wave had the same dominant frequencies as that of the flow fluctuation in the cavity only around the cavity.

In this paper, a grid-based distributed hydrological model BTOPMC （Block-wise use of TOPMODEL） is introduced, which was developed from the original TOPMODEL. In order to broaden the model＇s application to arid regions, improvement methodology is also implemented. The canopy interception and soil infiltration processes were incorporated into the original BTOPMC to model event-based runoff simulation in large arid regions. One designed infiltration model with application of time compression approximation method is emphasized and validated for improving model＇s performance for event hydrological simulations with a case study of Lushi River basin.

Aim： To conduct a double-blind, placebo-controlled randomized and dose-ranging study to evaluate the safety and efficacy of the extract of Ganoderma lucidum （G. lucidum） in men with lower urinary tract symptoms （LUTS）. Methods： We enrolled male volunteers （〉 50 years） with an International Prostate Symptom Score （IPSS; questions 1-7）〉 5 and a prostate-specific antigen （PSA） value 〈 4 ng/mL. Volunteers were randomized into groups of placebo （n = 12）, G. lucidum of 0.6 mg （n = 12）, 6 mg （n = 12） or 60 mg （n = 14）, administered once daily. Efficacy was measured as a change from baseline in IPSS and the peak urine flow rate （Qmax）. Prostate volume and residual urine were estimated by ultrasonography, and blood tests, including PSA levels, were measured at baseline and at the end of the treatment. Results： The overall administration was well tolerated, with no major adverse effects. Statistical significances in the magnitude of changes between the experimental groups were observed at weeks 4 and 8. No changes were observed with respect to Qmax, residual urine, prostate volume or PSA levels. Conclusion： The extract of G. lucidum was well tolerated and an improvement in IPSS was observed. The recommended dose of the extract of G. lucidum is 6 mg in men with LUTS.

Aim： To evaluate the safety and efficacy of an extract of Ganoderma lucidum that shows the strongest 5α-reductase inhibitory activity among the extracts of 19 edible and medicinal mushrooms by a double-blind, placebo-controlled, randomized and dose-ranging study in men with lower urinary tract symptoms （LUTS）. Methods： In this trial, we randomly assigned 88 men over the age of 49 years who had slight-to-moderate LUTS to 12 weeks of treatment with G. lucidum extract （6 mg once a day） or placebo. The primary outcome measures were changes in the International Prostate Symptom Score （IPSS） and variables of uroflowmetry. Secondary outcome measures included changes in prostate size, residual urinary volume after voiding, laboratory values and the reported adverse effects. Results： G. lucidum was effective and significantly superior to placebo for improving total IPSS with 2.1 points decreasing at the end of treatment （mean difference, -1.18 points; 95% confidence interval, -1.74 to -0.62; P 〈 0.0001）. No changes were observed with respect to quality of life scores, peak urinary flow, mean urinary flow, residual urine, prostate volume, serum prostate-specific antigen or testosterone levels. Overall treatment was well tolerated with no severe adverse effects. Conclusion： The extract of G. lucidum was well tolerated and improved IPSS scores. These results encouraged a further, large-scale evaluation of phytotherapy for a long duration using the extract of G. lucidum on men with LUTS.

Numerical Simulation of Interaction between Supersonic Flow and Backward Inclined Jets Surrounded by Porous Cavity

As one of supersonic mixing techniques, a supersonic mixing technique using a cavity and a porous wall has been proposed. The cavity and the porous wall generate the low speed region in the cavity, which enhances mixing the main flow with the jets. In this study, numerical simulations were conducted to clarify the effects of backward inclined jets on the mixing technique using a porous wall and a cavity. In the numerical simulations, three patterns of jet injections which combined normal jets with backward inclined jets were studied. As a result, the combination of a backward inclined jet and a normal jet generates the suction flow behind the backward inclined jet, which is useful for making the injected jets flow into the cavity. In addition, the introduction of backward inclined jets reduces the total pressure loss. On the other hand, the mass flow rate through the porous holes decreases with increase in the number of the backward inclined jets.
