Verification of Dosimetric and Positional Accuracy of Dynamic Tumor Tracking Intensity Modulated Radiation Therapy

Purpose: We performed both, dosimetric and positional accuracy verification of dynamic tumor tracking (DTT) intensity modulated radiation therapy (IMRT), with the Vero4DRT system using a moving phantom (QUASAR respiratory motion platform;QUASAR phantom) and system log files. Methods: The QUASAR phantom was placed on a treatment couch. Measurement of the point dose and dose distribution was performed for conventional IMRT, with the QUASAR phantom static and moving;for DTT IMRT, this was performed with the phantom moving for pyramid shaped, prostate, paranasal sinus, and pancreas targets. The QUASAR phantom was driven by a sinusoidal signal in the superior-inferior direction. Furthermore, predicted positional errors induced by the Vero4DRT system and mechanical positional errors of the gimbal head, were calculated using the system log files. Results and Conclusion: For DTT IMRT, the dose at the evaluation point was within 3% compared with the verification plan, and the dose distribution in the passing rates of γ was 97.9%, with the criteria of 3% dose and 3 mm distance to agreement. The position error calculated from the log files was within 2 mm, suggesting the feasibility of employing DTT IMRT with high accuracy using the Vero4DRT system.

Background:We clarified the predictive factors for changes in the status ofmedications for lower urinary tract symptoms(LUTS)2 years after local radiotherapy for nonmetastatic prostate *** and methods:We retrospectively included patients who underwent local external radiotherapy for nonmetastatic prostate cancer in 8 institutions between April 2001 and March *** were divided into themedication and no-medication group based on the use of drugs for LUTS before *** defined improvement of LUTS as when the patient did not require medication for LUTS at 24 months after radiotherapy in the medication group and as deterioration whenmedication was required in the no-medication *** regression analysis was used to evaluate predictive factors for changes in medication ***:Altogether,505 patients were divided into a no-medication group(n=352)and a medication group(n=153).The number of patients with deterioration and improvement in LUTS was 49(14%)and 36(23%),*** the multivariate analysis,the predictive variables for deterioration were the International Prostate SymptomScore(≥8;odds ratio[OR],2.21;p=0.014)and the biopsy Gleason score(≤3+4=7;OR,2.430;p=0.008)in the no-medication group,whereas those for improvement were age(<75 years old;OR,5.81;p=0.002),the quality of life score(<3;OR,3.15;p=0.028),and a positive biopsy core rate(≥50%;OR,2.530;p=0.027)in the medication ***:These predictive factors for changes in the status ofmedications for LUTS at 2 years after external radiotherapymay help determine the definitive therapy for nonmetastatic prostate cancer.

Management of gastric mucosa-associated lymphoid tissue lymphoma in patients with extra copies of the MALT1 gene

AIM To identify the clinical features of gastric mucosaassociated lymphoid tissue(MALT) lymphoma with extra copies of *** This is a multi-centered,retrospective study. We reviewed 146 patients with MALT lymphoma in the stomach who underwent fluorescence in situ hybridization analysis for t(11;18) translocation. Patients were subdivided into patients without t(11;18) translocation or extra copies of MALT1(Group A,n = 88),patients with t(11;18) translocation(Group B,n = 27),and patients with extra copies of MALT1(Group C,n = 31). The clinical background,treatment,and outcomes of each group were *** Groups A and C showed slight female predominance,whereas Group B showed slight male predominance. Mean ages and clinical stages at lymphoma diagnosis were not different between groups. Complete response was obtained in 61 patients in Group A(69.3%),22 in Group B(81.5%),and 21 in Group C(67.7%). Helicobacter pylori(H. pylori) eradication alone resulted in complete remission in 44 patients in Group A and 13 in Group C. In Group B,14 patients underwent radiotherapy alone,which resulted in lymphoma disappearance. Although the difference was not statistically significant,event-free survival in Group C tended to be inferior to that in Group A(P = 0.10).CONCLUSION Patients with t(11;18) translocation should be treated differently from others. Patients with extra copies of MALT1 could be initially treated with H. pylori eradication,similar to patients without t(11;18) translocation or extra copies of MALT1.

An Unexpected Complication by Misplacement of an External Ventricular Drain;Transient Oculomotor Palsy Due to Direct Compression by a Drain—A Case Report

Background: Insertion of an external ventricular drain (EVD) is an important neurosurgical technique. There is a minor risk of misplacement, and complications including infections or hemorrhages have been reported. Transient neurological complications are, however, very rare, especially when EVD is placed under endoscope assistance. We report a case of unilateral transient oculomotor palsy after an EVD procedure. Case presentation: An 11-year-old boy with past medical history of moyamoya disease suffered from intraventricular hemorrhage and acute hydrocephalus. Insertion of EVD and hematoma removal was performed with endoscope assistance. After the surgery, transient oculomotor palsy occurred by the direct compression of the brain stem by the drainage tube. Conclusion: Transient unilateral oculomotor palsy due to the direct compression of the midbrain by an EVD tube is a very rare, but possible complication, even under endoscopic assistance. Attention through the procedure is required until skin closure to avoid tube dislocation.

The acquisition method for planar 67Ga imaging has hardly changed for 30 years. In this study, in order to improve image quality and diagnostic accuracy, we take steps to optimize the acquisition method, and to choose a scatter correction. First, we acquired individual images from the 93 keV, 185 keV, and 300 keV photopeak;then the images were added together and compared to the individual images. Second, we compared results from a low-medium-energy (LME) collimator with those from a conventional medium-energy (ME) collimator. Also, we examined whether to combine the data from all three of the usual window locations (set about 93 keV, 185 keV, and 300 keV) or to use the data from only two. Third, we compared results from a conventional photopeak ± 10% window with those from a photopeak ± 9 keV window. Fourth, for scatter correction we compared results using the triple energy window (TEW) method with those using the multi-photopeak dual window (MDW) method. The phantoms studied were cold rods in a uniform background, and hot spheres within a cylinder containing uniformly radioactive water. The clinical study involved 22 patients with lung lesions. By the comparison by the contrast ratio in cold rods phantom, 15.6% is improved in LME (2 peaks) than ME (3 peaks), and 3.2% is improved in photopeak ± 9 keV than photopeak ± 10%, 10.2% is improved in TEW than MDW. However, the TEW scatter correction method recognized unstable to the contrast ratio in a clinical study. In addition, a body outline might disappear.
