目的评价舒更葡糖钠用于ASA分级Ⅲ或Ⅳ级高风险手术患者术后拮抗神经肌肉阻滞的安全性。尤其着重评估了心脏不良事件以及其他预先指定的临床关注不良事件的影响。方法符合ASA分级Ⅲ或Ⅳ级标准的患者,根据ASA分级和神经肌肉阻滞剂(罗库溴铵或维库溴铵)进行随机分组,新斯的明/格隆溴铵组:中度神经肌肉阻滞;舒更葡糖钠2 mg/kg组,中度神经肌肉阻滞;舒更葡糖钠4 mg/kg组,深度神经肌肉阻滞;舒更葡糖钠16 mg/kg组,深度神经肌肉阻滞(仅罗库溴铵)。主要研究终点包括治疗期间窦性心动过缓、窦性心动过速和其他心律失常的发生情况。结果344例患者进入随机化分配,331例患者接受了治疗[61%为男性,BmI(28.5±5.3)kg/m^(2),年龄(69±11)岁]。舒更葡糖钠2 mg/kg组治疗期间窦性心动过缓发生率显著降低,舒更葡糖钠2 mg/kg组和4 mg/kg组治疗期间窦性心动过速发生率显著降低(P0.05)。结论ASA分级Ⅲ或Ⅳ级高风险手术患者使用舒更葡糖钠拮抗罗库溴铵或维库溴铵诱导的中度和深度神经肌肉阻滞的安全性良好。

Endoscopic submucosal dissection in a patient with esophageal adenoid cystic carcinoma

We report the first use of endoscopic submucosal dissection(ESD) for the treatment of a patient with adenoid cystic carcinoma of the esophagus(EACC). An 82-year-old woman visited our hospital for evaluation of an esophageal submucosal tumor. Endoscopic examination showed a submucosal tumor in the middle third of the esophagus. The lesion partially stained with Lugol's solution,and narrow band imaging with magnification showed intrapapillary capillary loops with mild dilatation and a divergence of caliber in the center of the lesion. Endoscopic ultrasound imaging revealed a solid 8 mm × 4.2 mm tumor,primarily involving the second and third layers of the esophagus. A preoperative biopsy was non-diagnostic. ESD was performed to resect the lesion,an 8 mm submucosal tumor. Immunohistologically,tumor cells differentiating into ductal epithelium and myoepithelium were observed,and the tissue type was adenoid cystic carcinoma. There was no evidence of esophageal wall,vertical stump or horizontal margin invasion with p T1 b-Sm2 staining(1800 μm from the muscularis mucosa). Further studies are needed to assess the use of ESD for the treatment of patients with EACC.

Aim: Transcatheter arterial chemoembolization (TACE) is thought to be a safe and effective treatment for hepatocellular carcinoma (HCC). However, in some HCC patients, it potentially shortens survival due to liver damage. We aimed to identify independent factors to predict overall survival of HCC after TACE. methods: We included a total of 96 consecutive HCC patients who underwent TACE at Kobe University Hospital. Areas of skeletal muscle and fat tissue were measured by computed tomography (CT) scan before TACE. We divided the patients into two groups in terms of the presence or absence of 1-year mortality after TACE. Factors associated with 1-year mortality after TACE were assessed by multivariate analyses, and the optimal cut-off values were evaluated using a propensity score. Results: multivariate analyses showed that visceral fat accumulation on CT was an independent factor associated with 1-year mortality after TACE (p = 0.033). There were no differences in skeletal muscle area and subcutaneous and intermuscular fat area between the two groups. Cut-off values for visceral fat area associated with 1-year mortality after TACE were defined as 33.3 cm2/m2 for males and 24.4 cm2/m2 for females. Conclusions: High visceral fat area was a prognostic factor associated with increased mortality rate in HCC patients undergoing TACE. Using this value, 1-year mortality risk after TACE would be better estimated before the day TACE was performed.

Sulfate deposited on actual foliar of a broad-leaf tree was evaluated to discuss dry deposition processes by the amount rinsed out from the sampled leaves in an experimental forest in the suburbs of Tokyo. Five samples of a 20 m high-Quercus serrata tree were manually collected at four heights （6m, 10 m, 14 m, and 20 m） from a 30 m-high observation tower which routinely measures meteorological parameters at six heights, and subjected to distilled-water rinsing for ion chromatographic analysis of sulfate. The deposited amount of sulfate was normalized with leaf surface. The vertical profile of the normalized deposition showed no systematic patterns so far. The deposition of sulfate, however, was found to increase with relative humidity, which strongly suggested the wetted surface plays a key role in the deposition processes. In order to evaluate the representativeness of the sample, 153 sample leaves were collected from a single branch of the sample stand in may 2010. The distribution showed a monomodal pattern with the following percentiles: 10%;12.2, 25%;15.0, 50%;20.2, 75%;28.3, and 90%;44.9 micromole per square meter. The actual deposition on each sample leaf was also discussed in terms of the normalized deposition and sample position in the branch. The results support the view that the foliar rinsing method will offer a powerful tool for studies of aerosol dry deposition to forests.

A new N-acetylsulfanilylation series of uridine have been synthesized in good yield using direct acylation method and afforded the 5’-O-N-acetylsulfanilyluridine. In order to obtain newer products, the 5’-O-N-acetylsulfanilyluridine derivative was further transformed to a series of 2’,3’-di-O-acyl derivatives containing a wide variety of functionalities in a single molecular framework. The chemical structures of the newly synthesized compounds were confirmed on the basis of their FTIR, 1H-NmR spectroscopy, physicochemical properties and elemental analysis. All the synthesized uridine derivatives were tested for their in vitro antibacterial activity against six human pathogenic bacterial strains and for comparison standard antibiotic Ampicillin was also determined. The study revealed that the selectively acylated deriva-tives 5’-O-N-acetylsulfanilyl-2’,3’-di-O-lauroyluridine and 5’-O-N-acetylsulfanilyl-2’,3’-di-O-pivaloyluridine showed highest inhibition against Staphylococcus aureus and Bacillus cereus, respectively. We also observed that the introduction of hexanoyl, decanoyl, lauroyl, myristoyl and pivaloyl groups, the antibacterial functionality of the compound uridine increases. Another noteworthy observation was that the uridine derivatives were found comparatively more effective against Gram-positive microorganisms than those of Gram-negative microorganisms. In addition, the test chemicals were also tested for cyto-toxicity by brine shrimp lethality bioassay and compounds showed different rate mortality with different concentrations.

AIm To assess the efficacy of CO2 insufflation for reduction of mediastinal emphysema(mE) immediately after endoscopic submucosal dissection(ESD).mETHODS A total of 46 patients who were to undergo esophageal ESD were randomly assigned to receive either CO2 insufflation(CO2 group, n = 24) or air insufflation(Air group, n = 22). Computed tomography(CT) was carried out immediately after ESD and the next morning. Pain and abdominal distention were chronologically recorded using a 100-mm visual analogue scale(VAS). The volume of residual gas in the digestive tract was measured using CT imaging. RESULTS The incidence of mE immediately after ESD in the CO2 group was significantly lower than that in the Air group(17% vs 55%, P = 0.012). The incidence of mE the next morning was 8.3% vs 32% respectively(P= 0.066). There were no differences in pain scores or distention scores at any post-procedure time points. The volume of residual gas in the digestive tract immediately after ESD was significantly smaller in the CO2 group than that in the Air group(808 m L vs 1173 m L, P = 0.013).CONCLUSION CO2 insufflation during esophageal ESD significantly reduced postprocedural mE. CO2 insufflation also reduced the volume of residual gas in the digestive tract immediately after ESD, but not the VAS scores of pain and distention.
