APACHE system is better than Ranson system in the prediction of severity of acute pancreatitis

BACKGROUND: It has been suggested that addition of obesity score to the APACHE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APACHE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APACHE-Ⅱ and APACHE-O systems in assessing severity of acute pancreatitis in a local Chinese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APACHE-Ⅱ and APACHE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APACHE-Ⅱ, and APACHE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APACHE-Ⅱ and APACHE-O systems was 0.808, 0.955 and 0.951, respectively. CONCLUSIONS: The APACHE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. Addition of obesity score does not significantly improve the predictive accuracy of the APACHE-Ⅱ system in our local population with a low prevalence of obesity.

We present an uncommon case （female patient aged 59 years） of the clear-cell variant of calcifying epithelial odontogenic tumor （CEOT）（also known as Pindborg tumor） in the mandible. The clinical characteristics and probable origins of the clear tumor cells of previously reported cases of clear-cell variant of intraosseous CEOT are also summarized and discussed.

AIM： To cost-effectively express the 23-ku pE2, the most promising subunit vaccine encoded by the E2 fragment comprising of the 3＇-portion of hepatitis E virus （HEV） open reading frame 2 （ORF2） in plastids of tobacco （Nicotiana tabacum cv. SR1）, to investigate the transgene expression and pE2 accumulation in plastids, and to evaluate the antigenic effect of the plastid-derived pE2 in mice. METHODS： Plastid-targeting vector pRB94-E2 containing the E2 fragment driven by rice psbA promoter was constructed. Upon delivery into tobacco plastids, this construct could initiate homologous recombination in psaB-trnfM and trnG-psbC fragments in plastid genome, and result in transgene inserted between the two fragments. The pRB94-E2 was delivered with a biolistic particle bombardment method, and the plastid-transformed plants were obtained following the regeneration of the bombarded leaf tissues on a spectinomycin-supplemented medium. Transplastomic status of the regenerated plants was confirmed by PCR and Southern blot analysis, transgene expression was investigated by Northern blot analysis, and accumulation of pE2 was measured by ELISA. Furthermore, protein extracts were used to immunize mice, and the presence of the pE2-reactive antibodies in serum samples of the immunized mice was studied by ELISA. RESULTS： Transplastomic lines confirmed by PCR and Southern blot analysis could actively transcribe the E2 mRNA. The pE2 polypeptide was accumulated to a level as high as 13.27 μg/g fresh leaves. The pE2 could stimulate the immunized mice to generate pE2-specific antibodies. CONCLUSION： HEV-E2 fragment can be inserted into the plastid genome and the recombinant pE2 antigen derived is antigenic in mice. Hence, plastids may be a novel source for cost-effective production of HEV vaccines.

Chiral luminescence materials have potential applications in the field of three-dimensional displays due to their circularly polarized luminescence(CPL)***,the further development of circularly polarized organic light-emitting diodes(CP-OLEDs)needs to meet the requirements of high efficiency,high color purity,low cost,and high dissymmetry factor(gPLor gEL),chiral multiple resonance thermally activated delayed fluorescence(MR-TADF)materials are considered as candidates in these ***,based on a pair of chiral spirofluorene precursors,two pairs of high-performance chiral MR-TADF emitters((R/S)-p-Spiro-DtBuCzB and(R/S)-m-Spiro-DtBuCzB)are developed,which exhibit strong emissions peaking at 491 and 502 nm in toluene with full-width at half-maximum values of 25 and 33 nm,*** addition,small singlet–triplet energy gaps of 0.15 and 0.10 eV with high absolute photoluminescence efficiencies of 95.0%and 96.7%are observed for p-Spiro-DtBuCzB and m-Spiro-DtBuCzB molecules,*** based on p-Spiro-DtBuCzB and m-Spiro-DtBuCzB display high maximum external quantum efficiencies of 29.6%and 33.8%,*** importantly,CP-OLEDs present symmetric circularly polarized electroluminescence spectra with|gEL|factors of 3.36×10^(-4)and 7.66×10^(-4)for devices based on(R/S)-p-Spiro-DtBuCzB and(R/S)-m-Spiro-DtBuCzB enantiomers,respectively.

The simultaneous achievement of chiral multiple-resonance thermally activated delayed fluorescence(MR-TADF)emitters with narrowband and circularly polarized electroluminescence(CPEL)poses a ***,a MR-TADF emitter,Spiro-BNCz,embedding spirofluorene structure was developed and chiral separated,whose emission peaks at 528 nm in toluene with Commission Internationale de L’Eclairage coordinates of(0.26,0.69).The conjugation extension caused by embedding sulfur substituted spirofluorene on B/N framework shortens the singlettriplet energy gap and increases spin-orbital coupling matrix ***,a fast reverse intersystem crossing rate constant of 2.8×10^(6)s^(−1)and a high photoluminescence efficiency of 92%in film were *** organic light-emitting diode(OLED)exhibits a maximum external quantum efficiency of 32.3%.Particularly,the circularly polarized OLEDs based on Spiro-BNCz enantiomers show symmetric CPEL with dissymmetry factors(|gEL|)≈10^(−3).

针药结合治疗退行性脊柱炎的临床疗效观察

目的:以单纯针刺、单纯中药和针药结合的方法对有退行性脊柱炎患者进行临床比较观察,客观评价针药结合方法对退行性脊柱炎的疗效。从而为治疗退行性脊柱炎提供综合的理论与临床依据。方法:三组治疗共纳入45个病例,针药结合组、单纯针刺组、单纯中药组分别为15人。针药结合组运用针刺穴位结合中药(独活寄生汤)治疗,单纯针刺组运用与针药结合组相同之穴位针刺治疗,单纯中药组服用与针药结合组相同之中药(独活寄生汤)治疗。每组的治疗均为两个疗程,每个疗程共计10次,隔日治疗1次,,在第一次疗程结束以后需要休息一周,然后再进行第二个疗程,共计约8周时间完成。观察指标为《中医病证诊断疗效标准》、应用视觉模拟评分系统(VAS,visualanalog scale)进行疼痛评定及JOA下腰椎功能评分标准(JOA,Japanese Orthopaedic Association)进行疼痛评定。结果:对比三组患者的年龄、性别进行组间对比统计分析,差别均无统计学意义(P>0.05),三组患者治疗前VAS评分和JOA评分无统计学差异(P>0.05),三组数据均具有可比性。按照《中医病证诊断疗效标准》,单纯针刺治疗、单纯中药治疗和针药结合治疗退行性脊柱炎患者均有良好疗效,三组中针药结合组治愈率最高,三组组间比较总有效率无统计学意义(P>0.05)。VAS/JOA评分比较:三组患者治疗后组间比较均有明显改善(p<0.001);治疗后单纯针刺组优于单纯中药组(P<0.05);针药结合组与单纯针刺组、单纯中药组治疗前后对比,差异具有统计学差异(P<0.05),表示针药结合组的止痛效果优于单纯针刺组与单纯中药组。结论:①三种治疗方法对退行性脊柱炎具有较好疗效,其中针药结合总有效率最高;②针对退行性脊柱炎止痛效果比较,针药结合组效果显著优于单纯针刺组和单纯中药组,其中单纯针刺组效果略优于单纯中药组。所以在临床上推荐针药结合治疗退行性脊柱炎。
