Impact of Perioperative Management for Intracranial Arterial Stenosis on Postoperative Stroke Incidence in Coronary Artery Bypass Surgery: A Propensity Matching and Multivariate Analyses

Background: Intracranial arterial steno-occlusive lesion (IAS) is the most common cause of stroke. We hypothesized that adequate perioperative management for IAS is useful to prevent stroke in coronary artery bypass grafting (CABG). Methods: IAS was evaluated using magnetic resonance imaging/angiography (MRI/A, n = 216) in patients without a history of stroke or carotid stenosis (≥50%) in CABG between May 2005 and June 2018 (n = 424). Off-pump CABG was performed in most patients. Different perioperative managements of systolic arterial pressure (SAP) were applied for patients with and without MRI/A. SAP was strictly maintained at ≥80 mmHg in patients with significant IAS (≥50%, n = 28) and ≥90 mmHg for severe IAS (≥70%, n = 44) while SAP less than 80 mmHg (usually ≥70 mmHg) was allowed for patients without MRI/A. Intra-aortic balloon pumping was used for patients with multiple severe IAS and/or occlusion. Clinical outcomes were compared between patients with and without MRI/A. Propensity matching was performed (n = 181 each). Results: Complete revascularization was achieved in all patients. Nine patients (2.12%) had postoperative strokes;the incidence of stroke was signifi-cantly less in patients with preoperative MRI/A (n = 1, p = 0.016). There was no significant difference in the incidence of in-hospital mortality between patients with and without preoperative MRI/A (0.5% and 1.0%, respectively;p = 0.529). Multivariate analysis revealed that no preoperative MRI/A was the only significant predictors of postoperative stroke (p = 0.045). The incidence of stroke was not higher in patients with IAS who underwent MRI/A (p = 0.332). Conclusions: Preoperative evaluation of IAS using MRI/A and following our perioperative management significantly reduced perioperative stroke in patients undergoing CABG.

Plate tectonics plays an essential role in the redistribution of life-essential volatile elements between Earth's interior and surface, whereby our planet has been well tuned to maintain enduring habitability over much of its history. Here we present an overview of deep carbon recycling in the regime of modern plate tectonics,with a special focus on convergent plate margins for assessing global carbon mass balance. The up-to-date flux compilation implies an approximate balance between deep carbon outflux and subduction carbon influx within uncertainty but remarkably limited return of carbon to convecting mantle. If correct, carbon would gradually accumulate in the lithosphere over time by（i） massive subsurface carbon storage occurring primarily in continental lithosphere from convergent margins to continental interior and（ii） persistent surface carbon sinks to seafloors sustained by high-flux deep CO2emissions to the atmosphere. Further assessment of global carbon mass balance requires updates on fluxes of subduction-driven carbon recycling paths and reduction in uncertainty of deep carbon outflux. From a global plate tectonics point of view, we particularly emphasize that continental reworking is an important mechanism for remobilizing geologically sequestered carbon in continental crust and sub-continental lithospheric mantle. In light of recent advances,future research is suggested to focus on a better understanding of the reservoirs, fluxes, mechanisms, and climatic effects of deep carbon recycling following an integrated methodology of observation, experiment,and numerical modeling, with the aim of decoding the self-regulating Earth system and its habitability from the deep carbon recycling perspective.

A flexible supporting structure that reduces seismic response of an arch is proposed. Topology and cross-sectional areas of the supporting structure modeled as a truss structure are optimized through two steps of static and dynamic optimization problems. In the first step, a flexible supporting structure that has diagonal displacement at the top under horizontal load is obtained by solving static optimization problems. Then, in the second step, the cross-sectional area of the flexible member is optimized to minimize the seismic response acceleration of the arch evaluated by the complete quadratic combination(CQC) method. Time-history seismic response analysis is carried out to show that the response in the normal direction of the roof successfully decreases due to flexibility of the supporting structure; in addition, installing passive energy dissipation devices into the flexible supporting structure is very effective in reducing the tangential response of the arch.

To clarify the optimal water management in large-scale fields under high temperatures at the ripening period,effective water managements during this period for improvement of yield,appearance quality and palatability were *** with intermittent irrigation and flooded irrigation,the soil temperature with saturated irrigation remained low throughout the day,and the decrease rate of the bleeding rate of hills was the *** results suggested that the saturated irrigation maintained root *** the three irrigation types,the number of spikelets per m2 and 1000-grain weight were similar,however,saturated irrigation resulted in significantly higher rice yield due to improvement in the percentage of ripened *** saturated irrigation produced a high percentage of perfect rice grains and thicker brown rice grain,furthermore,the palatability of cooked rice was excellent because protein content and hardness/adhesion ratio were both ***,under high-temperature ripening conditions,soil temperature was lowered and root activity was maintained when applying saturated irrigation after heading *** results indicated that saturated irrigation is an effective countermeasure against high-temperature ripening damage.

Tumor growth requires oxygen and nutrient supplementation through angiogenesis from preexisting vascular beds. Hepatocellular carcinoma(HCC) is a hyper-vascularized tumor with a predominant arterial blood flow. Therefore, targeting angiogenesis is essential for treating HCC. In this paper,we propose a hybrid forecasting model(HFM)for the short-term electric load forecasting using artificial neural network(ANN),discrete Fourier transformation(DFT)and principal component analysis(PCA)techniques in order to attain higher prediction ***,we estimate Fourier coefficients by the DFT for predicting the next-day load curve with an ANN and obtain approximate load curves by applying the inverse discrete Fourier *** curves,together with other input variables,are given to the ANN to predict the next-day hourly load ***,we predict PCA scores to obtain approximate load curves in the first step,which are then given to the ANN again in the second *** DFT and PCA models use input variables such as calendrical and meteorological data as well as past electric *** those models for forecasting hourly electric load in the metropolitan area of Japan for January and May in 2018,we train our models using historical data since January *** forecast results show that the HFM consisting of“ANN with DFT”and“ANN with PCA”predicts next-day hourly loads more accurately than the conventional three-layered ANN *** corresponding mean average absolute errors show 2.7%for ANN with DFT,2.6%for ANN with PCA and 3.0%for the conventional ANN *** also find that in May,when electric demand is smaller with smaller fluctuations,forecasting errors are much smaller than January for all the ***,we can conclude that the HFM would contribute to attaining significantly higher forecasting accuracy.

Purpose–The purpose of this paper is to propose a fault-tolerant technology for increasing the durability of application programs when evolutionary computation is performed by fast parallel processing on many-core processors such as graphics processing units(GPUs)and multi-core processors(MCPs).Design/methodology/approach–For distributed genetic algorithm(GA)models,the paper proposes a method where an island’s ID number is added to the header of data transferred by this island for use in fault ***–The paper has shown that the processing time of the proposed idea is practically negligible in applications and also shown that an optimal solution can be obtained even with a single stuck-at fault or a transient fault,and that increasing the number of parallel threads makes the system less susceptible to ***/value–The study described in this paper is a new approach to increase the sustainability of application program using distributed GA on GPUs and MCPs.
