Metastatic Ovarian Tumor of Vulvar Malignant Melanoma in a 43-Year-Old Woman: A Case Report and Literature Review

An ovarian malignant melanoma sometimes occurs from ovarian teratoma. Ovarian metastatic malignant melanoma is extremely rare. We describe a patient in whom vulvar melanoma (previously resected) metastasized to the ovary, making ovarian metastatic malignant melanoma. A 43-year-old Japanese woman was referred to us because of left ovarian tumor. She had undergone resection for malignant melanoma on the right labia minora with inguinal lymph node metastasis (pT1bN1aM0, stage IIIA, FIGO 2008). Eighteen months after this surgery, CT scans revealed left ovarian tumor and swelled pelvic lymph nodes, with a pelvic examination disclosing a left adnexal solid mass, with normal serum CA125 level (21.7 U/mL). Laparotomy revealed a left solid ovarian tumor measuring 4 cm, which was covered with a smooth grayish capsule. The right ovary, uterus, and pelvic cavity appeared normal. Upon sectioning during the surgery, the cross-sectional surface of the left ovary revealed a dark brown solid tumor. Following an intra-operative frozen-section diagnosis as metastatic melanoma, total hysterectomy with bilateral salpingo-oophorectomy and pelvic lymph dissection was performed. Histological examination confirmed the diagnosis as malignant melanoma metastasis to the left ovary and the obturator lymph node: the same laterality (left) as the primary site. The tumor was entirely composed of malignant melanoma cells with no evidence of teratoma. Combined chemotherapy with dabrafenib mesylate and trametinib was planned based on the positive BRAF mutations. This case highlights the importance that physicians should have high index of suspicion for the occurrence of ovarian melanoma metastasis after melanoma surgery. We also made extensive literature review on this issue, of which description may contribute to better understanding of this condition.

There has been an increase in spinal surgery for the elderly, with a corresponding potential increase in perioperative complications. In our department, 1833 patients underwent spinal surgery under general anesthesia from April 2001 to October 2012, and 2 of 260 patients aged ≥75 years old had postoperative cerebral infarction. An analysis of the pathogenic mechanism and potential risk factors showed that a history of cerebral infarction was a significant risk factor. Blood pressure rapidly increased on arousal from anesthesia, and particularly on extubation. The change in blood pressure was examined as a potential risk factor for cerebral infarction, but no significant relationship was observed. This result requires further examination in more patients with cerebral infarction after spinal surgery.

Short Report: Association of IL-15 with Peripheral and Hepatic Insulin Sensitivity in Healthy Middle-Aged Men

Rodent study suggests that interleukin (IL)-15 administration enhances insulin sensitivity. Although it is well known that circulating levels of typical inflammatory markers (C-reactive protein [CRP] and IL-6) are positively associated with homeostasis model assessment-insulin resistance (HOMA-IR), there are no studies investigating the associations of other inflammatory markers including IL-15 with peripheral/hepatic insulin sensitivity in humans. The current study aimed to examine the relationship between the levels of adiopokines or inflammatory cytokines and insulin sensitivity in 8 healthy middle-aged men. Circulating levels of 10 insulin sensitizing adipokines or inflammatory cytokines (total adiponectin [APN], high molecular weight adiponectin [HMW-APN], IL-4, IL-5, IL-6, IL-8, IL-15, interferon [IFN]-γ, tumor necrosis factor [TNF]-α, and TNF-β) were measured. A stable-labeled frequently sampled intravenous glucose tolerance test was performed to assess peripheral () and hepatic () insulin sensitivity estimated by 2-compartment minimal model. The levels of 3 inflammatory cytokines (IL-4, IL-6, and IL-15) were significantly and inversely correlated with either and . The association between IL-15 and either or was significant even after adjusting for age and percent body fat (p < 0.01). The current study showed a possible inverse association between serum IL-15 level and peripheral/hepatic insulin sensitivity in healthy middle-aged males, independent of percent body fat;this association in humans warrants further study.

We present a narrative of the eruptive events culminating in the cataclysmic January 15, 2022 eruption of Hunga Tonga-Hunga Ha’apai Volcano by synthesizing diverse preliminary seismic, volcanological, sound wave, and lightning data available within the first few weeks after the eruption occurred. The first hour of eruptive activity produced fast-propagating tsunami waves, long-period seismic waves, loud audible sound waves, infrasonic waves, exceptionally intense volcanic lightning and an unsteady volcanic plume that transiently reached-at 58km-the Earth’s mesosphere. Energetic seismic signals were recorded worldwide and the globally stacked seismogram showed episodic seismic events within the most intense periods of phreatoplinian activity, and they correlated well with the infrasound pressure waveform recorded in Fiji. Gravity wave signals were strong enough to be observed over the entire planet in just the first few hours, with some circling the Earth multiple times subsequently. These large-amplitude, long-wavelength atmospheric disturbances come from the Earth’s atmosphere being forced by the magmatic mixture of tephra, melt and gasses emitted by the unsteady but quasicontinuous eruption from 0402±1–1800 UTC on January 15, 2022. Atmospheric forcing lasted much longer than rupturing from large earthquakes recorded on modern instruments, producing a type of shock wave that originated from the interaction between compressed air and ambient(wavy) sea surface. This scenario differs from conventional ideas of earthquake slip, landslides, or caldera collapse-generated tsunami waves because of the enormous(~1000x) volumetric change due to the supercritical nature of volatiles associated with the hot,volatile-rich phreatoplinian plume. The time series of plume altitude can be translated to volumetric discharge and mass flow rate. For an eruption duration of ~12 h, the eruptive volume and mass are estimated at 1.9 km^(3) and~2 900 Tg, respectively, corresponding to a VEI

In orchards, measuring crown characteristics is essential for monitoring the dynamics of tree growth and optimizing farm management. However, it lacks a rapid and reliable method of extracting the features of trees with an irregular crown shape such as trained peach trees. Here, we propose an efficient method of segmenting the individual trees and measuring the crown width and crown projection area (CPA) of peach trees with time-series information, based on gathered images. The images of peach trees were collected by unmanned aerial vehicles in an orchard in Okayama, Japan, and then the digital surface model was generated by using a Structure from Motion (SfM) and Multi-View Stereo (MVS) based software. After individual trees were identified through the use of an adaptive threshold and marker-controlled watershed segmentation in the digital surface model, the crown widths and CPA were calculated, and the accuracy was evaluated against manual delineation and field measurement, respectively. Taking manual delineation of 12 trees as reference, the root-mean-square errors of the proposed method were 0.08 m (R^(2) = 0.99) and 0.15 m (R^(2) = 0.93) for the two orthogonal crown widths, and 3.87 m2 for CPA (R^(2) = 0.89), while those taking field measurement of 44 trees as reference were 0.47 m (R^(2) = 0.91), 0.51 m (R^(2) = 0.74), and 4.96 m2 (R^(2) = 0.88). The change of growth rate of CPA showed that the peach trees grew faster from May to July than from July to September, with a wide variation in relative growth rates among trees. Not only can this method save labour by replacing field measurement, but also it can allow farmers to monitor the growth of orchard trees dynamically.

Reduced endogenous insulin secretion in diabetic patients with low-titer positive antibodies against GAD

Aim: To investigate the clinical characteristics of diabetic patients with a low-titer positive for the anti-glutamic acid decarboxylase 65 antibody (GAD antibody). Methods: The subjects were 420 diabetic inpatients. The endogenous insulin secretion was estimated on the basis of the C-peptide immunoreactivity from a 24 h urine collection (uCPR). Clinical variables were compared between patients negative for the GAD antibody (GAD antibody titer < 1.5 U/mL), a low-titer positive GAD antibody (1.5 U/mL ≤ GAD antibody titer < 10 U/mL) and a high-titer positive GAD antibody (10 U/mL ≤ GAD antibody titer). Results: The low and high-titer positive GAD antibodies were found in 25 and 10 patients, respectively. The uCPR was significantly lower in both the patients with a low (37 ± 33 ug/24h) and high-titer (39 ± 27 ug/24h) positive GAD antibodies than in those negative for GAD antibodies (71 ± 52 ug/24h). The uCPR level was significantly lower in the low-titer positive GAD antibody group (29 ± 22 ug/24h) than in the negative group (67 ± 55 ug/24h) among the patients not taking insulin secretagogues. The difference disappeared in the subjects taking insulin secreagogues. In the stepwise multiple regression analysis, a low-titer positive GAD antibody was independently associated with the uCPR level. Conclusions: Endogenous insulin secretion is reduced in diabetic patients positive for GAD antibodies, even if the titer is low. Earlier initiation of insulin therapy might therefore protect the pancreatic β-cell function in diabetic patients with a low-titer positive GAD antibody.
