BACKGROUND Bile duct cancer constitutes gallbladder cancer(GBC),intrahepatic cholangiocarcinoma(ICA),and extrahepatic cholangiocarcinoma(ECA).These three entities show morphological and immunohistochemical resemblance so that it is difficult to differentiate between primary ICA and liver metastasis of GBC,which sometimes becomes a point of discussion in clinical *** these cancers demonstrate significant differences in their mutational landscape,several reports demonstrated shared genomic alteration in paired primary and metastatic site aids in distinguishing metastatic recurrence from second primary *** SUMMARY We present a 73-year-old female patient who underwent curative resection for GBC harboring epidermal growth factor receptor 2(ERBB2)activating mutation on next-generation sequencing(NGS)-based genomic *** year later,a hepatic lesion was observed on follow-up imaging and she underwent surgical resection for a pathological *** histological findings of the hepatic lesion were similar to those of the primary ***,using NGS panel testing,the hepatic lesion was found to have ERBB2 activating mutation,which is the identical mutation detected in the sequencing result of the primary ***2 activating mutation occurs more frequently in GBC than ICA and ***,in the present case,we think this molecular finding potentiated the diagnosis of the liver mass toward a metastatic ***,this patient underwent HER2-targeted treatment with lapatinib in combination with capecitabin and obtained clinical *** This case illustrated NGS panel usefulness in distinguishing GBC recurrence from second primary cancer and HER2-targeted agent efficacy on ERBB2 mutated GBC.

Robot Position Control Using Force Information for Cooperative Work in Remote Robot Systems with Force Feedback

This paper proposes robot position control using force information for cooperative work between two remote robot systems with force feedback in each of which a user operates a remote robot by using a haptic interface device while observing work of the robot with a video camera. We also investigate the effect of the proposed control by experiment. As cooperative work, we deal with work in which two robots carry an object together. The robot position control using force information finely adjusts the position of the robot arm to reduce the force applied to the object. Thus, the purpose of the control is to avoid large force so that the object is not broken. In our experiment, we make a comparison among the following three cases in order to clarify how to carry out the control effectively. In the first case, the two robots are operated manually by a user with his/her both hands. In the second case, one robot is operated manually by a user, and the other robot is moved automatically under the proposed control. In the last case, the object is carried directly by a human instead of the robot which is operated by the user in the second case. As a result, experimental results demonstrate that the control can help each system operated manually by the user to carry the object smoothly.

＜正＞Keio Line is an important part of the main railway in Tokyo Metropolitan rail transportation *** mitigate traffic congestion as well as to avoid potential traffic accidents railway crossing with roads on the ground,a project for underground double-track railway and underground stations was planed and has been under construction near Chofu Station. Two parallel tunnels were constructed in the project,in which existing railways on ground level will be shifted into the underground parallel *** EPB TBM（6.85m outer diameter） was used for both first and second tunnel *** TBM was turned around in a vertical shaft after the first tunnel driving,and then launched from the shaft for the second *** TBM driving was executed under the following special conditions;1) TBM driving beneath existing railway with shallow earth cover（the minimum earth cover is 4.7m that is less than the outer diameter of tunnel lining）,2) the ground bored by TBM which is mainly composed of the Tachikawa gravel formation containing boulders which is over 300mm,3) small clearance between the parallel tunnels （the minimum clearance was 400mm）,and 4) rotation and lift-up of the TBM in the narrow shaft beneath existing railway after the first tunnel driving（a weight of TBM was approximately 300ton）. Fuda station is constructed by cut and cover tunnel *** ground soil up to the tunnels was excavated in the station area first,and then,the tunnel linings were partially cut and removed to construct platform at the track level of the station. This paper summarizes the overall project and reports regarding the countermeasures against above mentioned conditions and the method of the platform level construction with cut and removal of the tunnel linings in Fuda station.

Influence of NUDT15 variants on hematological pictures of patients with inflammatory bowel disease treated with thiopurines

AIM The single nucleotide polymorphism(SNP) c.415C>T in exon 3 of NUDT15 affects thiopurine-induced leukopenia in Asian patients with Crohn's disease. Meanwhile, three additional genetic variants of NUDT15 were reported in patients with acute lymphoblastic leukemia. We evaluated the effects of these additional genetic variants of NUDT15 in patients with inflammatory bowel disease(IBD) treated with *** Ninety-six Japanese patients with IBD were enrolled. Genotyping for the NUDT15 and TPMT genes was performed using Custom Taq Man SNP genotyping assays or Sanger sequencing. The changes in white blood cell(WBC) count, mean corpuscular volume(MCV), platelet count, hemoglobin, CRP, amylase, albumin, AST, ALT, and ESR were *** Genetic variants of exon 1 and exon 3 of NUDT15 were identified in 24 of 96 patients(25.0%). C.52G > A and c.36_37 insG GAGTC in exon 1 were found in three patients each. All three patients with c.36_37 insG GAGTC in exon 1 were heterozygotes of ***139 Cys in exon 3. Eighteen patients had ***139 Cys in exon 3 alone. The WBC count gradually decreased after initiation of thiopurine treatment in the mutated cases(n = 24), and was significantly lower at 6, 8, 10, and 16 wk(P = 0.0271, 0.0037, 0.0051, and 0.0185, respectively). The WBC counts were also evaluated in patients with and without prednisolone treatment. In the patients with prednisolone treatment, the WBC count tended to show a greater decrease in the mutated cases, with significant differences at 8 and 10 wk(P = 0.012 and 0.029, respectively). In the patients without prednisolone treatment, the WBC count was significantly lower at 2, 4, 8, and 14 wk in mutated cases(P = 0.0196, 0.0182, 0.0237 and 0.0241, respectively). MCV increased after starting thiopurine treatment in the mutated cases, and was significantly higher at 10 wk(P = 0.0085). Removal of Sodium from Seawater Medium Using Photosynthetic Bacteria

The removal of sodium (Na) from seawater using two photosynthetic bacteria was investigated using Rhodobacter sphaeroides SSI (SSI) and Rhodovulum sp. which is a marine photosynthetic bacterium. Both Rhodovulum sp. and acclimated SSI were shown to grow well in a 3% NaCl supplemented glutamate-malate medium. The maximum rate of Na removal was 39.3% by SSI and 64.9% by Rhodovulum sp. after two days cultivation under static light conditions. However, Na was re-released back into the medium after two to three days. When a nutrient-supplemented seawater medium (3.3% NaCl, 13.10 gNa/l) was used, the maximum Na removal rates were 30.3% (9.05 gNa/l) by SSI and 48.9% (6.69 gNa/l) by Rhodovulum sp., under static light conditions. Similar growth and Na removal rates were found under aerobic dark cultivation. In this case, no re-release of Na was observed with either bacterium. Two stages culturing was conducted first, with Rhodovulum sp. and then with SSI replacement. The Na concentration was reduced to 0.79 gNa/l (94.0% removal) after cultivation for eight days under aerobic darkconditions. The supernatant was applied successfully as a liquid fertilizer in the cultivation of Japanese radish.

Predictive Factors of Severe Toxicities of Pemetrexed-Containing Chemotherapy in Patients with Non-Squamous Non-Small Cell Lung Cancer

Background: Pemetrexed (PEM) is an efficacious multi-targeted antifolate with acceptable toxicities for non-squamous non-small cell lung cancer (non-Sq NSCLC). However, in the clinical setting, PEM has more severe adverse effects than those reported. The aim of this study was to identify the factors for the toxicities of PEM-containing chemotherapy in non-Sq NSCLC patients in the clinical setting. Patients and Methods: We retrospectively evaluated the factors related to PEM toxicities in chemotherapy-naive patients with non-Sq NSCLC from September 2009 to July 2013 at our hospital. Logistic regression model was used in the univariate and multivariate analyses. Results: In total, 104 patients were analyzed. Grades 3 to 5 hematologic toxicities were frequent and included neutropenia (42%), febrile neutropenia (7%), anemia (18%), thrombocytopenia (17%), and disseminated intravascular coagulation (2%). On multivariate analyses, the predictors were poor performance status (PS) [odds ratio (OR): 4.89, 95% confidence interval (CI): 1.22 - 19.4] and low body mass index (OR: 1.44, 95% CI: 1.05 - 1.98) for febrile neutropenia;concomitant chronic infectious disease (OR: 6.63, 95% CI: 1.59 - 27.5) and bevacizumab use (OR: 3.57, 95% CI: 1.36 - 9.32) for neutropenia;poor PS (OR: 3.02, 95% CI: 1.33 - 6.85) for thrombocytopenia;and low serum albumin level (OR: 0.22, 95% CI: 0.08 - 0.63) for non-hematologic toxicities. Conclusions: In addition to the previously reported predictors of PEM toxicities, the presence of concomitant chronic infectious disease was associated with hematologic toxicities. Patient groups who are not sufficiently evaluated in clinical trials should be carefully monitored for the development of more toxicities than expected.

Schizophrenia knowledge and attitudes toward help-seeking among Japanese fathers and mothers of high school students

The prodromal symptoms of schizophrenia are nonspecific and include social withdrawal, sleeplessness, decreased desire, and loss of concentration. Schizophrenia outcomes might be improved if greater therapeutic effort was focused on people with prodromal symptoms. To hasten help-seeking for schizophrenia, we developed a schizophrenia education program for families of patients. Help-seeking and schizophrenia attitudes and knowledge were analyzed among Japanese parents (666 fathers and 613 mothers) of high school students by using the Social Distance Scale-Japanese version, the Link Devaluation-Discrimination Measure and our own questions on help-seeking and schizophrenia knowledge. Statistical analysis included cross-tabulation, the χ2 test, and multiple regression analysis. Most (97.5%) fathers were aged 40 - 59 years;most (78.3%) mothers were aged 40 - 49 years. There was a significant difference between fathers and mothers in basic knowledge of schizophrenia (P 0.05). When asked what treatment they would seek for a child with sleeplessness or social withdrawal, fathers and mothers significantly differed in seeking help medical care at a department of psychosomatic medicine (P < 0.05) and in seeking help from the nurse at their child’s school (P < 0.001). Fathers and mothers also differed in relation to the decision not to seek help (P < 0.001). Multiple regression analysis revealed that basic knowledge significantly differed between fathers and mothers (P = 0.001). In an analysis of parental response to a child with sleepiness and social withdrawal, multiple regression analysis revealed that fathers and mothers significantly differed in the likelihood of seeking help at a department of psychosomatic medicine and from a school nurse and in the likelihood of not seeking help (P < 0.05, P = 0.001 and P = 0.001, respectively). Bilateral Symptomatic Lateral Parapatellar Synovial Plica of the Knee in an Adolescent Athlete: A Case Report

We report herein a rare case of lateral parapatellar synovial plica that developed simultaneously in both knees. A 15-year-old competitive soccer player visited our institution with a six-month history of anterolateral pain and catching sensation in both knee joints. On physical examination, he complained of tenderness along the joint line of bilateral patellofemoral joint (PFJ). Visible and palpable popping was observed at the lateral margin of each patella during active knee motion. Magnetic resonance imaging showed obvious synovial soft nodules in the lateral side of the PFJ. On arthroscopy, a yellowish, thick, tongue-shaped tissue extending transversely from the lateral parapatellar synovium was identified in both knees, and parts of this tissue showed avascular hypertrophy. Arthroscopic findings of both knees are almost symmetrical in anatomical location, but no similar in size. After arthroscopic excision, the patient became asymptomatic. At 24-month follow up, he demonstrated full knee function, without evidence of local recurrence.
