Outcomes of bile duct cannulation using a novel contrast-enhanced catheter: A single-center, retrospective cohort study

BACKGROUND Endoscopic retrograde cholangiopancreatography is a challenging procedure involving bile duct *** the development of several cannulation devices,none have effectively facilitated the *** To evaluate the efficacy of a recently developed catheter for bile duct *** We retrospectively examined 342 patients who underwent initial *** compared the success rate of bile duct cannulation and the incidence of complications between the groups using existing and novel *** The overall success rates of bile duct cannulation were 98.3%and 99.1%in the existing and novel catheter groups,respectively(P=0.47).The bile duct cannulation rate using the standard technique was 73.0%and 82.1%in the existing and novel catheter groups,respectively(P=0.042).Furthermore,when catheterization was performed by expert physicians,the bile duct cannulation rate was significantly higher in the novel catheter group(81.3%)than in the existing catheter group(65.2%)(P=0.017).The incidence of difficult cannulation was also significantly lower in the novel catheter group(17.4%)than in the existing catheter group(33.0%)(P=0.019).CONCLUSION The novel catheter improved the bile duct cannulation rate using the standard technique and reduced the frequency of difficult cannulation cases,valuable tool in endoscopic retrograde cholangiopancreatography procedures performed by experts.

Femtosecond pulses cleaning by transient-grating process in Kerr-optical media

We use a transient-grating （TG） process in a Kerr bulk medium to clean a femtosecond laser pulse. Using the technique, the temporal contrast of the generated TG signal is improved by more than two orders of magnitude in comparison with the incident pulse in a 0.5-mm-thick fused silica plate. The laser spectrum is smoothed and broadened, and the pulse duration is shortened simultaneously. We expect to extend this technique to a clean pulse with broadband spectral bandwidth at a wide spectral range because it is a phase-matched process.

Digestion performance and microbial community in full-scale methane fermentation of stillage from sweet potato-shochu production

Sweet potato shochu is a traditional Japanese spirit produced mainly in the South Kyushu area in Japan. The amount of stillage reaches approximately 8 x 105 tons per year. Wastewater mainly containing stillage from the production of sweet potato-shochu was treated thermophilically in a fullscale treatment plant using fixed-bed reactors （8 reactors ×283 m3）. Following the addition of Ni2＋ and Co2＋, the reactors have been stably operated for six years at a high chemical oxygen demand （COD） loading rate of 14 kg/（***）. Analysis of coenzyme content and microbial communities indicated that similar microbial communities were present in the liquid phase and on the fiber carders installed in reactors. Bacteria in the phyla Firmieutes as well as Bacteroidetes were dominant bacteria, and Methanosarcina thermophila as well as Methanothermobacter crinale were dominant methanogens in the reactors. This study reveals that stillage from sweet potato-shochu production can be treated effectively in a full-scale fixed-bed reactor under thermophilic conditions with the help of Ni2~ and Co2＋. The high diversity of bacterial community and the coexistence of both aceticlastic and hydrogenotrophic methanogens contributed to the excellent fermentation performance.

In recent years,innate-like T-cell populations,such as invariant natural killer T(iNKT)cells and mucosal-associated invariant T(MAIT)cells,have been identified[1].These cells are different from conventional T cells in that they reside in tissues such as the liver,lung,and skin,rather than the lymph node and spleen[2].Although iNKT and MAIT cells are abundant in the skin immediately after birth,the detailed functions or the mechanisms regulating their localization have not been clarified[3].In a study published in Nature Immunology,Wang et *** that the early homing of iNKT cells to the skin was dependent on CCR10 expression during the stage of thymic development and was critical for proper commensal bacterial colonization and skin development[4].

Adenocarcinoma arising from heterotopic pancreas at the third portion of the duodenum

A 62-year-old Japanese man presented to our hospital with a history of weight loss of 6 kg in 4 mo. Imaging examinations revealed a tumor located on the third portion of the duodenum with stenosis. We suspected duodenal carcinoma and performed pancreas-preserving segmental duodenectomy. Adenocarcinoma arising from a heterotopic pancreas at the third portion of the duodenum was finally diagnosed by immunohistochemical staining. Malignant transformation in the duodenum arising from a heterotopic pancreas is extremely rare; to our knowledge, only 13 cases have been reported worldwide, including the present case. The most common location of malignancy is the proximal duodenum at the first and descending portion. Herein, we describe the first case of adenocarcinoma arising from a heterotopic pancreas, which was located in the third portion of the duodenum, with a review of the literature.
