AIM:To determine the role of the fecal immunochemical test(FIT),used to evaluate fecal hemoglobin concentration,in the prediction of histological grade and risk of colorectal ***:We enrolled 17881 individuals who attended the two-step colorectal cancer screening program in a single hospital between January 2010 and October *** was recommended to the participants with an FIT of≥12 ngHb/mL *** classified colorectal lesions as cancer(C),advanced adenoma(AA),adenoma(A),and others(O)by their colonoscopic and histological *** linear regression analysis adjusted for age and gender was used to determine the association between the FIT results and colorectal tumor *** risk of adenomatous neoplasia was estimated by calculating the positive predictive values for different FIT ***:The positive rate of the FIT was 10.9%(1948/17881).The attendance rate for colonoscopy was 63.1%(1229/1948).The number of false positive results was *** these 1229 cases,the numbers of O,A,AA,and C were 759,221,201,and 48,*** analysis revealed a positive association between histological grade and FIT concentration(β=0.088,Plinear relationship was found between the concentration and positive predictive value of the FIT for predicting colorectal tumors(R2>0.95,P<0.001).CONCLUSION:Higher FIT concentrations are associated with more advanced histological *** prediction for colorectal neoplasia based on individual FIT concentrations is significant and may help to improve the performance of screening programs.

AIM: To assess the cost-effectiveness of two populationbased hepatocellular carcinoma(HCC) screening programs, two-stage biomarker-ultrasound method and mass screening using abdominal ultrasonography(AUS).METHODS: In this study, we applied a Markov decision model with a societal perspective and a lifetime horizon for the general population-based cohorts in an area with high HCC incidence, such as Taiwan. The accuracy of biomarkers and ultrasonography was estimated from published meta-analyses. The costs of surveillance, diagnosis, and treatment were based on a combination of published literature, Medicare payments, and medical expenditure at the National Taiwan University Hospital. The main outcome measure was cost per lifeyear gained with a 3% annual discount rate. RESULTS: The results show that the mass screening using AUS was associated with an incremental costeffectiveness ratio of USD39825 per life-year gained, whereas two-stage screening was associated with an incremental cost-effectiveness ratio of USD49733 per life-year gained, as compared with no screening. Screening programs with an initial screening age of 50 years old and biennial screening interval were the most cost-effective. These findings were sensitive to the costs of screening tools and the specificity of biomarker ***: Mass screening using AUS is more cost effective than two-stage biomarker-ultrasound screening. The most optimal strategy is an initial screening age at 50 years old with a 2-year inter-screening interval.

The Reflection of Normal Incident Waves by Absorbing-Type Breakwaters

This paper investigates the reflection of normal incident waves produced by absorbing-type breakwaters. The absorbing-type breakwaters in this study consist of a vertical porous plate, a submerged permeable caisson, and an impermeable back wall. The flow field is divided into four regions： a porous caisson region, and three pure water regions. Under the assumptions of linear wave theory, Darcy＇s law in the perforated wall, and the pore velocity potential theory of Sollitt and Cross （1972） in the porons caisson region, this study creates a 2-D BEM model to calculate the reflection coefficients of water waves using several breakwater properties. This numerical model is calibrated by previous numerical studies and limiting cases for a partially perforated-wall caisson breakwater and a vertical porous breakwater with an impermeable back wall. Generally speaking, the wave dissipation in absorbing-type breakwaters is bigger than that for a partially perforatedwall caisson breakwater. The reflection coefficient values imply the performance of wave absorbers in this study. Therefore, we examine the major factors that affect the reflection coefficient.

K-theoretic classification of fermionic operator mixings in holographic conformal field theories

In this paper, we apply the K-theory scheme of classifying the topological insulators/superconductors to classify the topological classes of the massive multi-flavor fermions in anti-de Sitter(Ad S) space. In the context of Ad S/conformal field theory(CFT) correspondence, the multi-flavor fermionic mass matrix is dual to the pattern of operator mixing in the boundary CFT. Thus, our results classify the possible patterns of operator mixings among fermionic operators in the holographic CFT.

Practical Timing Synchronization for Network Dynamics in Large Machine-to-Machine Networks

Efficient multi-machine cooperation and network dynamics still remain open that jeopardize great applications in largescale machine-to-machine(M2M) networks. Among all possible machine cooperation controls, to synchronize tremendous machines in a timing-efficient brings one of the greatest challenge and serves as the foundation for any other network control policies. In this paper, we propose a linear-time synchronization protocol in large M2M networks. Specifically, a closed-form of synchronization rate is provided by developing the statistical bounds of the second smallest eigenvalue of the graph Laplacian matrix. These bounds enable the efficient control of network dynamics, facilitating the timing synchronization in networks. Through a practical study in Metropolis, simulation results confirm our theoretical analysis and provide effective selection of wireless technologies, including Zigbee, Wi-Fi, and cellular systems, with respect to the deployed density of machines. Therefore, this paper successfully demonstrates a practical timing synchronization, to make a breakthrough of network dynamic control in real-world machine systems, such as Internet of Things.

Algal removal from cyanobacteria-rich waters by preoxidation-assisted coagulation-flotation: Effect of algogenic organic matter release on algal removal and trihalomethane formation

The cyanobacteria-bloom in raw waters frequently causes an unpredictable chemical dosing of preoxidation and coagulation for an effective removal of algal cells in water treatment plants. This study investigated the effects of preoxidation with NaOCl and ClO_2 on the coagulation-flotation effectiveness in the removal of two commonly blooming cyanobacteria species, Microcystis aeruginosa（MA） and Cylindrospermopsis raciborskii（CR）, and their corresponding trihalomethane（THM） formation potential. The results showed that dual dosing with NaOCl plus ClO_2 was more effective in enhancing the deformation of cyanobacterial cells compared to single dosing with Na OCl, especially for CR-rich *** preoxidation approaches for CR-rich water effectively reduced the CR cell count with less remained dissolved organic carbon（DOC）, which benefited subsequent coagulation–flotation. However, preoxidation led to an adverse release of algogenic organic matter（AOM） in the case of MA-rich water. The release of AOM resulted in a poor removal in MA cells and a large amount of THM formation after oxidation-assisted coagulation-flotation process. The reduction in THM formation potential of CR-rich waters is responsible for effective algae and DOC removal by alum coagulation. It is concluded that the species-specific characteristic of cyanobacteria and their AOM released during chlorination significantly influences the performance of coagulation–flotation for AOM removal and corresponding THM formation.

NUMERICAL SIMULATION OF SOLITARY WAVE RUN-UP AND OVER-TOPPING USING BOUSSINESQ-TYPE MODEL

In this article, the use of a high-order Boussinesq-type model and sets of laboratory experiments in a large scale flume of breaking solitary waves climbing up slopes with two inclinations are presented to study the shoreline behavior of breaking and non-breaking solitary waves on plane slopes. The scale effect on run-up height is briefly discussed. The model simulation capability is well validated against the available laboratory data and present experiments. Then, serial numerical tests are conducted to study the shoreline motion correlated with the effects of beach slope and wave nonlinearity for breaking and non-breaking waves. The empirical formula proposed by Hsiao et al. for predicting the maximum run-up height of a breaking solitary wave on plane slopes with a wide range of slope inclinations is confirmed to be cautious. Furthermore, solitary waves impacting and overtopping an impermeable sloping seawall at various water depths are investigated. Laboratory data of run-up height, shoreline motion, free surface elevation and overtopping discharge are presented. Comparisons of run-up, run-down, shoreline trajectory and wave overtopping discharge are made. A fairly good agreement is seen between numerical results and experimental data. It elucidates that the present depth-integrated model can be used as an efficient tool for predicting a wide spectrum of coastal problems.
