Effects of Glutamine Supplementation on Patients Undergoing Abdominal Surgery

Objective To evaluate the effects of supplementation of glutamine （GLN） on maintaining glu- tathione （GSH） level, immune system function, liver function, and clinical outcome of patients receiving abdominal operation. Methods Forty patients undergoing elective abdominal surgical treatment were randomly divided into 2 groups： study group （n=20） and control group （n=20）. All patients received total parenteral nutrition （TPN） for up to 7 days during perioperative period. The study group received TPN supplemented with GLN dipeptide while the control group received TPN without GLN dipeptide. Patients in both groups received equivalent nitrogen and caloric intake. Blood sample was taken on preoperative day, and the 1st, 3rd, 6th postoperative day to measure GSH level, immune indexes, and liver function indexes. Results The decrease of GSH level in plasma and red blood cell （RBC） in study group was less than that in control group during postoperative period. Ratio of GSH/glutathione disulfide （GSSG） in plasma in study grouP was higher than that in control group on the 3rd postoperative day （52.53±11.46 vs. 31.43±7.27, P = 0.001）. Albumin level in study group was higher than that in control group on the 3rd postoperative day （37.7±3.8 g/L vs. 33.8±4.2 g/L, P = 0.02）. There was no significant difference in the levels of immunoglobin （IgG, IgM, IgA） or T lymphocyte subgroup （CD4, CD8, CD4/CD8） in both groups during postoperative period. There was one case with infectious complication in control group, while none in study group. A trend of shortened hospital stay was observed in study group compared with control group （22.3±2.1 d vs. 24.9±1.7 d,P= 0.32）. Conclusions Supplementation of GLN-enriched TPN has beneficial effects on maintaining GSH levels in plasma and RBC, sustaining GSH/GSSG ratio and albumin level, and keeping antioxidant abilities during postoperative period in patients with abdominal operation, with the trends of decreasing incidence of infectious complicati

EFFECTS OF ENTERAL AND PARENTERAL NUTRITION ON GASTROENTERIC HORMONES AND GASTRIC MOTILITY AFTER SUBTOTAL GASTRECTOMY

Objective To investigate the effects of enteral nutrition （EN） and parenteral nutrition （PN） on gastric motility and gastroentefic hormones after subtotal gastrectomy. Methods Forty-one patients underwent gastrectomy were randomly divided into EN group （n=20） and PN group （n=21 ）. From the first postoperative day to the seventh day, patients received either EN （EN group） or PN （PN group） with isocalofic （84.9kJ·kg^-1·d^-1） and isonitrogenous （0.11g·kg^-1·d^-1） intake. Serum gastrin （GAS）, plasma mofilin （MTL）, and plasma cholecystokinin （CCK） were measured on preoperative day, the first and seventh postoperative day. Electrogastrography （EGG） was measured on preoperative day and the seventh postoperative day. Results Compared with preoperafion, blood GAS, MTL, and CCK levels of 41 patients decreased significantly on the first day after subtotal gastrectomy （ P 〈 0. 001 ）, but returned to the preoperative levels one week later. EGG after gastrectomy showed that gastric basal electrical rhythm was significantly restrained （ P 〈 0. 001 ）. On the seventh day after subtotal gastrectomy, plasma MTL and CCK levels in EN group were higher than those in PN group （ P 〈 0.05 ）. There was no difference in GAS level between two groups. EGG in EN group was better than that in PN group postoper- atively. Conclusions The levels of gastroentefitic hormones and the gastric motility decrease significantly after subtotal gastrectomy. In contrast with PN, EN can accelerate the recovery of MTL, CCK, and gastric motility after subtotal gastrectomy.

目的探讨谷氨酰胺双肽强化的肠外营养对腹部手术患者术后谷胱甘肽水平、免疫功能、肝功能及术后感染发生率、住院天数的影响。方法选择腹部外科手术患者40例,随机分为研究组与对照组,围手术期予完全肠外营养支持(TPN)7 d。对照组给予TPN,研究组给予TPN+谷氨酰胺双肽。两组均等氮、等热量,非蛋白热量83.6 ***-1.d-1,糖∶脂为7∶5,氮量0.16 ***-1.d-1,热、氮比489 kJ∶1 g。分别于术前、术后第1天,第3天及术后第6天采取血样测定血浆、红细胞氧化型谷胱甘肽(GSSG)及还原型谷胱甘肽(GSH)水平、血生化指标和免疫指标。结果研究组术后血浆及红细胞内谷胱甘肽下降程度小于对照组。血浆GSSG和GSH比值(术后第3天)研究组明显高于对照组(分别为53±11和31±7,F=4.725,P=0.001)。两组血浆白蛋白(ALB)术后均轻度下降,研究组术后第3天ALB水平明显高于对照组[分别为(3.8±0.4)g/d l和(3.4±0.4)g/d l,F=2.128,P=0.02]。术后两组免疫球蛋白(IgG、IgM、IgA),T淋巴细胞亚群(CD4,CD8及CD4/CD8)指标差异无统计学意义。研究组术后无感染发生,对照组术后1例发生切口感染。研究组患者住院天数较对照组有缩短趋势[分别为(22.3±2.1)d和(24.9±1.7)d,t=0.935,P=0.32)。结论围手术期应用谷氨酰胺双肽强化的肠外营养具有维持机体血浆及红细胞内谷胱甘肽水平的趋势,维持较高的GSH/GSSG比值,维护机体组织细胞抗氧化能力;维持术后ALB水平,对肝功能有一定保护作用;具有减少术后并发症,缩短住院天数的趋势。

目的研究胃大部分切除术后胃癌患者血清胃泌素(GAS)、血浆胃动素(MTL)和胃动力的变化情况。方法以39名健康志愿者(对照组)和39例限期胃癌根治术患者(胃癌组)为研究对象,采用放射免疫法检测了对照组及胃癌组患者术前,术后第1、7天空腹血清GAS和血浆MTL水平;采用胃电图(EGG)观察了胃癌组患者术前和术后第7天的胃动力变化情况。结果胃癌组患者术前血清GAS和血浆MTL水平明显高于对照组(P均=0.000),胃癌Ⅲ期患者的血浆MTL水平明显高于Ⅱ期患者(P<0.05)。胃大部分切除术后第1天胃癌患者的血清GAS和血浆MTL水平较术前明显下降(P均=0.000),并于术后1周恢复到术前水平。EGG检测结果显示,胃癌患者术后胃动力异常发生率明显高于术前(χ2=18.692,P<0.01)。结论胃癌患者的血清GAS和血浆MTL水平在胃大部分切除术后存在一过性降低。胃大部分切除术可能影响胃癌患者的胃动力。

Background:The Ebola virus disease spread rapidly in West Africa in 2014,leading to the loss of thousands of *** engagement was one of the key strategies to interrupt Ebola transmission,and practical community level measures needed to be explored in the field and tailored to the specific context of ***:First,community-level education on Ebola virus disease(EVD)prevention was launched for the community’s social mobilizers in six districts in Sierra Leone beginning in November ***,from January to May of 2015,in three pilot communities,local trained community members were organized to engage in implementation of EVD prevention and transmission interruption measures,by involving them in alert case report,contact tracing,and social *** epidemiological indicators of transmission interruption in three study communities were ***:A total of 6016 community social mobilizers from 185 wards were trained by holding 279 workshops in the six districts,and EVD message reached an estimated 631680 *** three pilot communities,72 EVD alert cases were reported,with 70.8%of them detected by trained local community members,and 14 EVD cases were finally *** tracing detected 64.3%of EVD *** median duration of community infectivity for the cases was 1 *** secondary attack rate was 4.2%,and no third generation of infection was *** health worker was infected,and no unsafe burial and noncompliance to EVD control measures were *** community-based measures were modeled to reduce 77 EVD cases,and the EVD-free goal was achieved four months earlier in study communities than whole country of Sierra ***: The community-based strategy of social mobilization and community engagement was effective in casedetection and reducing the extent of Ebola transmission in a country with weak health system. We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,NICER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by NICER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.
