Ventricular puncture is a common neurosurgical procedure,which is usually performed freehand using body surface anatomic landmarks as targeting references.[1,2]However,it is reported that the failure rate of freehand lateral ventricle puncture could be as high as 43%in traumatic brain injury cases if body surface anatomic landmarks are used as targeting references,[3]and the occurrence of hemorrhages associated with ventricular puncture could reach 16.2%.[4]Classic puncture sites,such as Kocher point,are helpful in improving the success rate of ventricular puncture.However,it is still risky to perform freehand puncture with classic puncture sites.Several causes could lead to this issue,including the following:(1)Individual patients have different conditions;a puncture approach suitable for all patients might not be possible.(2)Selection of puncture approach is largely dependent on the judgment of the surgeon,because the puncture approach does not usually fall on the coronal,sagittal,or axial images of the regular computed tomography(CT)scan;surgeons need to use spatial imagination based on CT images to design the ventricular puncture trajectory(VPT),which could lead to errors.(3)In freehand puncture,directional deviations could occur easily without the guidance of a frame as a fixed space reference to the puncture path.

Nucleic acid nanostructures with structural programmability, spatial addressability and excellent biocompatibility have drawn much attention in various biomedical applications, such as bioimaging, biosensing and drug delivery. In this review, we summarize the recent research progress in the field of bioimaging based on nucleic acid nanostructures with different imaging models, including fluorescent imaging(FI), magnetic resonance imaging(MRI), photoacoustic imaging(PAI) and positron emission tomography/computed tomography(PET/CT) imaging. We also discuss the remaining challenges and further opportunities involved in the bioimaging research based on nucleic acid nanostructures.

BACKGROUND Hepatitis C virus(HCV)genotype 6(HCV-6)infection is prevalent predominantly in Southeast Asia,and the data on the virologic response of HCV-6 to direct-acting antivirals(DAAs)are sparse in people living with human immunodeficiency virus(HIV)(PLWH).AIM To assess the virologic response of HCV-6 to DAAs in PLWH.METHODS From September 2016 to July 2019,PLWH coinfected with HCV-6 initiating DAAs were included.Laboratory investigations were performed at baseline,the end of treatment,and 12 wk off-therapy.RESULTS Of the 349 PLWH included(mean age 48.9 years,82.5%men),80.5%comprised people who inject drugs,18.1%men who have sex with men,and 1.4%heterosexuals.Coexistent hepatitis B virus infection was present in 12.3%of the included PLWH,liver cirrhosis 10.9%,hepatocellular carcinoma 0.9%,and previous HCV treatment experience 10.9%.The mean baseline plasma HCV RNA was 6.2 log10 IU/m L.Treatment with glecaprevir/pibrentasvir was initiated in 51.9%,sofosbuvir/ledipasvir 41.5%,sofosbuvir/velpatasvir 6.3%,and sofosbuvir/daclatasvir 0.3%.At DAA initiation,antiretroviral therapy containing tenofovir alafenamide was given in 26.4%,tenofovir disoproxil fumarate 34.4%,non-tenofovir alafenamide/tenofovir disoproxil fumarate 39.3%,non-nucleoside reverse-transcriptase inhibitors 30.4%,protease inhibitors 4.0%,and integrase strand transfer inhibitors 66.8%;94.8%of the included patients had CD4 counts≥200 cells/mm3 and 96.0%had plasma HIV RNA<50 copies/m L.Overall,96.8%achieved undetectable plasma HCV RNA(<30 IU/m L)at end of treatment;and 92.3%achieved sustained virologic response 12 wk off-therapy in the intention-to-treat analysis(93.5%in patients receiving sofosbuvir-based DAAs and 91.2%in those receiving glecaprevir/pibrentasvir).CONCLUSION Similar to the observation made in HIV-negative patients,sustained virologic response 12 wk offtherapy with DAAs is high in PLWH coinfected with HCV-6.

Hepcidin-25 negatively predicts left ventricular mass index in chronic kidney disease patients

AIM: To assess the correlation between the serum hepcidin-25 level and left ventricular mass index.METHODS: This study was a cross-sectional study conducted between March 2009 and April 2010. Demo-graphic and biochemical data, including the serum hepcidin-25 level, were collected for chronic kidney disease(CKD) patients. Two-dimensional echocardiography was performed to determine the left ventricle mass(LVM), left ventricular mass index(LVMI), interventricular septum thickness(IVSd), left ventricle posterior wall thickness(LVPW), right ventricular dimension(RVD), left atrium(LA) and ejection fraction(EF).RESULTS: A total of 146 patients with stage 1 to 5 CKD were enrolled. Serum hepcidin-25 levels were 16.51 ± 5.2, 17.59 ± 5.32, 17.38 ± 6.47, 19.98 ± 4.98 and 22.03 ± 4.8 ng/mL for stage 1 to 5 CKD patients, respectively. Hepcidin-25 level was independently predicted by the serum ferritin level(β = 0.6, P = 0.002) and the estimated glomerular filtration rate(β =-0.48, P = 0.04). There were negative correlations between the serum hepcidin level and the LVM and LVMI(P = 0.04 and P = 0.005, respectively). Systolic blood pressure(BP) was positively correlated with the LVMI(P = 0.005). In the multivariate analysis, a decreased serum hepcidin-25 level was independently associated with a higher LVMI(β =-0.28, 95%CI:-0.48--0.02, P = 0.006) after adjusting for body mass index, age and systolic BP. CONCLUSION: A lower serum hepcidin level is associated with a higher LVMI in CKD patients. Low hepcidin levels may be independently correlated with unfavorable cardiovascular outcomes in this population.

1968年IgA肾病首次被描述，最初称为Berger病。先证者为法国患儿，表现为急性上呼吸道感染数天后出现无症状肉眼血尿。肾活检显示系膜区显著扩大、IgA沉积，因此被命名为IgA肾病。此后40年临床治疗实践中，对该病自然病程了解逐渐增多，但对其发病机制知之甚少。20世纪80年代，D，Amico和其他学者已经认识到IgA肾病是世界上最常见的肾小球肾炎。因此，Julian等指出该病对1％以上的人口有影响。

该研究建立吹扫捕集气质联用法快速测定化工污泥中12种挥发性有机物的分析方法,采用吹扫捕集进样,DB-624色谱柱分离,质谱检测器进行检测,对吹扫时间、吹扫温度、脱附时间和脱附温度四个因素进行优化,考察基质效应和盐效应的影响,同时对检出限、加标回收率和精密度进行实验。结果表明,在0.002~0.100 mg/L范围内线性良好,相关系数(r^(2))>0.991,方法检出限为0.2~0.8μg/L,相对标准偏差(n=6)为0.50%~7.1%,回收率为90.8%~121%。该方法能满足GB 5085.3限值的要求,适用于化工污泥中12种挥发性有机物的快速定性、定量分析。

利用"973"加密观测期间的地面、探空资料和双多普勒雷达同步观测等非常规资料全面分析了2002年合肥地区出现的两场不同类型暴雨过程的环流背景、物理量垂直分布结构和多普勒雷达回波强度和速度特征。结果表明:冷式切变和暖式切变两种不同类型暴雨首先在物理量垂直分布结构十分相似,低层辐合、高层辐散,低层正涡度、高层负涡度,水汽水平辐合的最大值在850hPa,只是在上下层配置和强度上有所差异,特别水汽通量散度场表现突出,暖式切变更有利于水汽辐合。另外多普勒雷达径向速度特征存在一定的差异,多普勒雷达VWP产品能实时监测暴雨过程中风的垂直切变,有助于提高单站暴雨的预报水平。
