BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

N-doped graphene quantum dot-decorated N-TiO2/P-doped porous hollow g-C_(3)N_(4) nanotube composite photocatalysts for antibiotic photodegradation and H2 production

Exclusive responsiveness to ultraviolet light (~3.2 eV) and high photogenerated charge recombination rate are the two primary drawbacks of pure TiO_(2). We combined N-doped graphene quantum dots (N-GQDs), morphology regulation, and heterojunction construction strategies to synthesize N-GQD/N-doped TiO_(2)/P-doped porous hollow g-C_(3)N_(4) nanotube (PCN) composite photocatalysts (denoted as G-TPCN). The optimal sample (G-TPCN doped with 0.1wt% N-GQD, denoted as 0.1% G-TPCN) exhibits significantly enhanced photoabsorption, which is attributed to the change in bandgap caused by elemental doping (P and N), the improved light-harvesting resulting from the tube structure, and the upconversion effect of N-GQDs. In addition, the internal charge separation and transfer capability of0.1% G-TPCN are dramatically boosted, and its carrier concentration is 3.7, 2.3, and 1.9 times that of N-TiO_(2), PCN, and N-TiO_(2)/PCN(TPCN-1), respectively. This phenomenon is attributed to the formation of Z-scheme heterojunction between N-TiO_(2) and PCNs, the excellent electron conduction ability of N-GQDs, and the short transfer distance caused by the porous nanotube structure. Compared with those of N-TiO_(2), PCNs, and TPCN-1, the H2 production activity of 0.1%G-TPCN under visible light is enhanced by 12.4, 2.3, and 1.4times, respectively, and its ciprofloxacin (CIP) degradation rate is increased by 7.9, 5.7, and 2.9 times, respectively. The optimized performance benefits from excellent photoresponsiveness and improved carrier separation and migration efficiencies. Finally, the photocatalytic mechanism of 0.1% G-TPCN and five possible degradation pathways of CIP are proposed. This study clarifies the mechanism of multiple modification strategies to synergistically improve the photocatalytic performance of 0.1% G-TPCN and provides a potential strategy for rationally designing novel photocatalysts for environmental remediation and solar energy conversion.

Subtotal colonic bypass plus colostomy with antiperistaltic cecoproctostomy for the treatment of slow transit constipation in an aged population:A retrospective control study

AIM To compare the efficacy,improved quality of life,and prognosis in patients undergoing either subtotal colonic bypass with antiperistaltic cecoproctostomy(SCBAC) or subtotal colonic bypass plus colostomy with antiperistaltic cecoproctostomy(SCBCAC) for the treatment of slow transit *** Between October 2010 and October 2014,aged patients with slow transit constipation who were hospitalized and underwent laparoscopic surgery in our institute weredivided into two groups: the bypass group,15 patients underwent SCBAC,and the bypass plus colostomy group,14 patients underwent SCBCAC. The following preoperative and postoperative clinical data were collected: gender,age,body mass index,operative time,first flatus time,length of hospital stay,bowel movements(BMs),Wexner fecal incontinence scale,Wexner constipation scale(WCS),gastrointestinal quality of life index(GIQLI),numerical rating scale for pain intensity(NRS),abdominal bloating score(ABS),and ClavienDindo classification of surgical complications(CD) before surgery and at 3,6,12,and 24 mo after *** All patients successfully underwent laparoscopic surgery without open surgery conversion or surgeryrelated death. The operative time and blood loss were significantly less in the bypass group than in the bypass plus colostomy group(P = 0.007). No significant differences were observed in first flatus time,length of hospital stay,or complications with CD > 1 between the two groups. No patients had fecal incontinence after surgery. At 3,6,and 12 mo after surgery,the number of BMs was significantly less in the bypass plus colostomy group than in the bypass group. The parameters at 3,6,12,and 24 mo after surgery in both groups significantly improved compared with the preoperative conditions(P < 0.05),except NRS at 3,6 mo after surgery in both groups,ABS at 12,24 mo after surgery and NRS at 12,24 mo after surgery in the bypass group. A tunnel-group metro station built in rock site is composed of a group of *** tunnels and their interconnections can show inconsistent responses during an *** study investigates the dynamic responses of such a metro station in a rock site,by shaking table *** lining structures of each tunnel and surrounding rock are modeled based on the similitude law;foam concrete and gypsum are used to model the ground-structure system,keeping relative stiffness consistent with that of the prototype.A series of harmonic waves are employed as excitations,input along the transverse and longitudinal direction of the shaking *** discrepant responses caused by the structural irregularities are revealed by measurement of acceleration and strain of the *** characteristics are identified by the transfer function method in white noise *** test results show that the acceleration response and strain response of the structure are controlled by the *** particular,the acceleration amplification effect at the opening section of the station hall is more significant than that at the standard section under transverse excitation;the amplification effect of the structural opening is insignificant under longitudinal excitation.

Understanding the initial use stage of cement mortar lining in drinking water distribution systems:Silicon and aluminum as stability indicators

The cement mortar lining(CML)of commonly used ductile iron pipes can severely deteriorate the drinking water quality at the initial stage of use,but the behavioral characteristics of different elements release from the CML in this stage is still *** this study,dynamic immersion experiments with new cement mortar lined ductile iron pipe reactors were conducted under different feed water hardness and alkalinity *** results showed that the release of alkaline substances from the CML at the initial stage of use could strongly influence the pH of water,which consequently greatly impacted the release/precipitation behaviors of calcium,aluminum and *** pH and aluminum concentration of the effluent water could reach 11.5 and 700μg/L within 24 hr of hydraulic retention time,respectively,under conditions of relatively lower hardness and *** to the pH elevation,calcium carbonate precipitation could occur even at much lower feed water *** the aluminum and silicon could keep release from the CML in soluble form at different hardness and alkalinity levels,and their release rate depended on the amount of calcium carbonate ***,aluminum and silicon were more suitable as indicators of the corrosion intensity at the initial stage of CML use rather than the traditional calcium carbonate precipitation *** feed water hardness and alkalinity levels for mitigating the initial intense corrosion of CML were proposed:hardness>40 mg/L(CaCO_(3)),alkalinity>100 mg/L(CaCO_(3)).

Further study on the genesis of lamellar calcite veins in lacustrine black shale--A case study of Paleogene in Dongying Depression, China

Lamellar calcite veins are prevalent in carbonate-rich,lacustrine dark *** formation mechanisms of these veins have been extensively debated,focusing on factors such as timing,depth,material source,and driving *** paper examines dark lacustrine shale lamellar calcite veins in the Paleogene strata of Dongying Depression,using various analytical techniques:petrography,isotope geochemistry,cathodoluminescence,inclusion thermometry,and electron probe *** distinct types of calcite veins have been identified:granular calcite veins and sparry calcite *** two types differ significantly in color,grain structure,morphology,and *** further investigation,it was observed that vein generation occurred from the shallow burial period to the maturation of organic matter,with a transition from granular calcite veins to sparry calcite *** granular calcite veins exhibit characteristics associated with the shallow burial period,including plastically deformed laminae and veins,the development of strawberry pyrite,the absence of oil and gas,weak fractionation in oxygen isotopes,and their contact relationship with sparry calcite *** granular calcite veins were likely influenced by the reduction of sulfate *** the other hand,sparry calcite veins with fibrous grains are antitaxial and closely linked to the evolution and maturation of organic *** contain oil and gas inclusions and show a distribution range of homogenization temperature between 90℃ and 120℃ and strong fractionation in oxygen isotopes,indicating formation during the hydrocarbon expulsion *** carbon isotope analysis of the surrounding rocks and veins suggests that the material for vein formation originates from the shale itself,specifically authigenic micritic calcite modified by the action of *** opening of horizontal fractures and vein formation is likely driven by fluid overpressure resulting from undercompaction and hydrocarb

Membrane technology has become one of the most promising separation technologies for its energy saving, high separation efficiency, environmental friendliness, and economic feasibility. Covalent organic frameworks(COFs) with intrinsically high porosity, controllable pore size, uniform pore size distribution and long-range ordered channel structure, have emerged as next-generation materials to fabricate advanced separation membranes. This feature article summarizes some latest studies in the development of pure COF membranes in our lab, including their fabrication and applications in chemical separations. Finally, current challenges facing high-performance COF separation membranes are discussed.

Obstructive sleep apnea(OSA)is a serious type of sleep disorder that can lead to cardiometabolic and neurocognitive *** utilized smart device-based photoplethysmography technology to collect sleep data from the Chinese population from 2019 to *** lag nonlinear models combined with a generalized nonlinear model or a linear mixed effects model were used to investigate the shortterm associations between daily temperature and indicators of OSA *** included a total of 6,232,056 d of sleep monitoring data from 51,842 participants with moderate to severe risk of OSA from 313 Chinese *** relationships between ambient temperature and OSA exacerbation,apneahypopnea index(AHI),and minimum oxygen saturation(MinSpO_(2))were almost linear and present only on the same *** temperatures were associated with a greater risk of OSA exacerbation,with an 8.4%(95%confidence interval(CI):7.6%–9.3%)increase per 10℃increase in temperature.A 10℃increase in daily temperature corresponded to an AHI increase of 0.70 events/h(95%CI:0.65–0.76)and a MinSpO_(2)decrease of 0.18%(95%CI:0.16%–0.19%).Exposure to elevated temperatures during the night can also lead to adverse *** effects of higher temperatures on OSA severity were stronger among men,participants with a body mass index24 kg/m2,those aged 45 years and older,individuals with a history of hypertension and diabetes,and during the cold *** large-scale,nationwide,longitudinal study provides robust evidence suggesting that higher ambient temperatures may immediately worsen OSA.
