Radioactive 125I seed implantation for pancreatic cancer with unexpected liver metastasis:A preliminary experience with 26 patients

BACKGROUND Preoperative diagnosis rate of pancreatic cancer has increased year by year.The prognosis of pancreatic cancer patients with unexpected liver metastasis found by intraoperative exploration is very poor,and there is no effective and unified treatment strategy.AIM To evaluate the therapeutic effect of radioactive 125I seed implantation for pancreatic cancer patients with unexpected liver metastasis.METHODS The demographics and perioperative outcomes of patients who underwent 125I seed implantation to treat pancreatic cancer with unexpected liver metastasis between January 1,2017 and June 1,2019 were retrospectively analyzed.During the operation,125I seeds were implanted into the pancreatic tumor under the guidance of intraoperative ultrasound,with a spacing of 1.5 cm and a row spacing of 1.5 cm.For patients with obstructive jaundice and digestive tract obstruction,choledochojejunostomy and gastroenterostomy were performed simultaneously.After operation,the patients were divided into a non-chemotherapy group and a chemotherapy group that received gemcitabine combined with albumin-bound paclitaxel treatment.RESULTS Preoperative imaging evaluation of all patients in this study showed that the tumor was resectable without liver metastasis.There were 26 patients in this study,including 18 males and 8 females,aged 60.5±9.7 years.The most common tumor site was the pancreatic head(17,65.4%),followed by the pancreatic neck and body(6,23.2%)and pancreatic tail(3,11.4%).Fourteen patients(53.8%)underwent palliative surgery and postoperative pain relief occurred in 22 patients(84.6%).The estimated blood loss in operation was 148.3±282.1 mL and one patient received blood transfusion.The postoperative hospital stay was 7.6±2.8 d.One patient had biliary fistula,one had pancreatic fistula,and all recovered after conservative treatment.After operation,7 patients received chemotherapy and 19 did not.The 1-year survival rate was significantly higher in patients who received chemotherapy than in those who did not(68.6%vs 15.

The precise and simultaneous acquisition of multiple beneficial alleles in the genome is in great demand for the development of elite pig breeders. Cytidine base editors(CBEs) that convert C:G to T:A have emerged as powerful tools for single-nucleotide replacement. Whether CBEs can effectively mediate C-to-T substitution at multiple sites/loci for trait improvement by direct zygote injection has not been verified in large animals. Here, we determined the editing efficiency of four CBE variants in porcine embryonic fibroblast cells and embryos. The findings showed that hA3A-BE3-Y130F and hA3A-eBE-Y130F consistently resulted in increased base-editing efficiency and low toxic effects in embryonic development. Further, we verified that using a one-step approach, direct zygote microinjection of the CBE system can generate pigs harboring multiple point mutations.Our process resulted in a stop codon in CD163 and myostatin(MSTN) and introduced a beneficial allele in insulin-like growth factor-2(IGF2). The pigs showed disrupted expression of CD163 and MSTN and increased expression of IGF2, which significantly improved growth performance and infectious disease resistance. Our approach allows immediate introduction of multiple mutations in transgene-free animals to comprehensively improve economic traits through direct embryo microinjection,providing a potential new route to produce elite pig breeders.

Manipulating the measured uncertainty under Lee-Yang dephasing channels through local PT-symmetric operations

Uncertainty relation lies at the heart of quantum physics,which is one of the fundamental characteristics of quantum mechanics.With the advent of quantum information theory,entropic uncertainty relation was proposed,which plays an important and irreplaceable role in quantum information science.In this work,we attempt to observe dynamics of entropic uncertainty in the presence of quantum memory under two different types of Lee-Yang dephasing channels.It is interesting to find that the dephasing channels have a negative effect on decreasing the uncertainty and the analogous partition function is anti-correlated with the uncertainty.In addition,we here propose an effective strategy to manipulate the uncertainty of interest of the subsystem by performing a parity-time symmetric(PT-symmetric)operation.It is worth noting that the uncertainty of measurement will be reduced to a certain extent by properly modulating the PT-symmetric operations under the considered channels.In this sense,we argue that our explorations offer insight into dynamics of entropic uncertainty in typical Lee-Yang dephasing channels,and might be beneficial to quantum measurement estimation in practical quantum systems.

Efficacy and safety of remimazolam-based sedation for intensive care unit patients undergoing upper gastrointestinal endoscopy:a cohort study

BACKGROUND:Remimazolam is a novel ultra-short-acting sedative,but its safety and adverse events(AEs)in high-risk patients in the intensive care unit(ICU)setting remain unknown.METHODS:This was a single-center,retrospective study that compared remimazolam to propofol and midazolam in patients undergoing upper gastrointestinal endoscopy.The primary outcome was the incidence of treatment-related AEs.The secondary outcomes were the time to extubation,the length of ICU stay,and the average cost of sedative per case.RESULTS:Of the 88 patients analyzed,47 were treated with remimazolam(mean dose,7.90±4.84mg),and 41 were treated with propofol(21.19±17.98 mg)or midazolam(3.08±2.17 mg).There was no statistically significant difference in the average duration of the endoscopic procedure(35.89±13.37 min vs.44.51±21.68 min,P=0.133)or the time to extubation(15.00±9.75 h vs.20.59±18.71 h,P=0.211)in the remimazolam group(group I)compared to the propofol or midazolam group(group II).ICU stays(5.40±2.93 d vs.4.63±3.31 d,P=0.072)and treatment-related AEs(48.61%vs.51.38%,P=0.056)were similar between groups.The average cost of sedative per case was significantly lower in the group I than in the group II(RMB 16.07±10.58 yuan vs.RMB 24.37±15.46 yuan,P=0.016).CONCLUSION:Remimazolam-based sedation was noninferior to the classic sedatives and had lower average cost per case,indicating that it may be used as a promising sedative for high-risk patients during endoscopic procedures in the ICU setting.

Effects of process parameters on periodic impact force exerting on cutting tool in ultrasonic vibration-assisted oblique turning

During ultrasonic vibration-assisted machining,the large impact force induced by tool-workpiece reengagement(TWR)is an important factor that affects tool chipping.However,mechanical analysis into process factors that affect the impact force and their influencing mechanisms are insufficient.Herein,a prediction model for the instantaneous cutting force during both TWR and the stable turning process,which depends on the process parameters and material properties,is firstly proposed based on the kinematic and dynamic analysis of ultrasonic vibration-assisted oblique turning(UVAOT).The results calculated using the developed cutting force model agree well with the experimental results presented in the literature.Next,the linear change law of the instantaneous cutting force with cutting time during the actual TWR is clarified using the proposed model.The effect of the UVAOT process parameters on the average impact force during the periodic TWR process is discussed,and the influence mechanism is analyzed from the perspective of mechanics.A positive linear correlation is discovered between the feed speed and average impact force.The ultrasonic amplitude and cutting speed do not significantly affect the average impact force of the new sharp cutting tools.These findings are consistent with the experimental observations of tool chipping and are applicable to select process parameters for reducing tool chipping during UVAOT.

Mesenchymal stem cells(MSCs)therapy shows the potential benefits to relieve clinical symptoms of osteoarthritis(OA),but it is uncertain if it can repair articular cartilage lesions-the main pathology of OA.Here,we prepared biomimetic cupper sulfide@phosphatidylcholine(CuS@PC)nanoparticles(NPs)loaded with plasmid DNA(pDNA)encoding transforming growth factor-beta 1(TGF-β1)to engineer MSCs for enhanced OA therapy via cartilage regeneration.We found that the NPs not only promoted cell proliferation and migration,but also presented a higher pDNA transfection efficiency relative to commercial transfection reagent lipofectamine 3000.The resultant CuS/TGF-β1@PC NP-engineered MSCs(termed CTP-MSCs)were better than pure MSCs in terms of chondrogenic gene expression,glycosaminoglycan deposition and type II collagen formation,favoring cartilage repair.Further,CTP-MSCs inhibited extracellular matrix degradation in interleukin-1β-induced chondrocytes.Consequently,intraarticular administration of CTP-MSCs significantly enhanced the repair of damaged cartilage,whereas pure MSCs exhibited very limited effects on cartilage regeneration in destabilization of the medial meniscus(DMM)surgical instability mice.Hence,this work provides a new strategy to overcome the limitation of current stem cell therapy in OA treatment through developing more effective nanoengineered MSCs.

Prognostic value of 11-factor modified frailty index in postoperative adverse outcomes of elderly gastric cancer patients in China

BACKGROUND Preoperative evaluation of frailty is limited to a few surgical procedures.However,the evaluation in Chinese elderly gastric cancer(GC)patients remains blank.AIM To validate and estimate the prognostic value of the 11-index modified frailty index(mFI-11)for predicting postoperative anastomotic fistula,intensive care unit(ICU)admission,and long-term survival in elderly patients(over 65 years of age)undergoing radical GC.METHODS This study was a retrospective cohort study which included patients who underwent elective gastrectomy with D2 Lymph node dissection between April 1,2017 and April 1,2019.The primary outcome was 1-year all-cause mortality.The secondary outcomes were admission to ICU,anastomotic fistula,and 6-mo mortality.Patients were divided into two groups according to the optimal grouping cutoff of 0.27 points from previous studies:High risk of frailty marked as mFI-11High and low risk of frailty marked as mFI-11Low.Survival curves between the two groups were compared,and univariate and multivariate regression analyses were performed to explore the relationship between preoperative frailty and postoperative complications in elderly patients undergoing radical GC.The discrimination ability of the mFI-11,prognostic nutritional index,and tumornode-metastasis pathological stage to identify adverse postoperative outcomes was assessed by calculating the area under the receiver operating characteristic(ROC)curve.RESULTS A total of 1003 patients were included,of which 13.86%(139/1003)were defined as having mFI-11High and 86.14%(864/1003)as having mFI-11Low.By comparing the incidence of postoperative complications in the two groups of patients,it was found that mFI-11High patients had higher rates of 1-year postoperative mortality,admission to ICU,anastomotic fistula,and 6-mo mortality than the mFI-11Low group(18.0%vs 8.9%,P=0.001;31.7%vs 14.7%,P<0.001;7.9%vs 2.8%,P<0.001;and 12.2%vs 3.6%,P<0.001).Multivariate analysis revealed mFI-11 as an independent predictive indicator for postoperative outcome[1-y

Influence of self-similar stresses on scenario earthquake construction:An example along the Tanlu Fault

As a famous deep and large fault in eastern China,the Tanlu Fault passes through Anhui,Jiangsu,and Shandong and into northeastern China.It is important to improve the understanding of seismic hazard assessments in areas near faults.We start a scenario earthquake simulation in the M7.5 earthquake potential area of the Xinyi-Sihong segment of the Tanlu Fault.The fault rupture length and width are constrained according to the scaling law of large intraplate earthquakes,the background normal stress is depth dependent,and the initial shear stresses are determined using trial and error by matching the earthquake magnitude.Considering the 120 km rupture length of the M7.5 earthquake,we compare the horizontal uniform stress model and self-similar stress perturbation model.Our findings reveal that the seismic source time function of the horizontal uniform stress model is similar to that of the Haskell model and that of the self-similar stress perturbation model is more similar to that of a real earthquake case.We compare the dynamic rupture simulation and ground motion results under four different stress conditions and find that the shorter the characteristic length of the self-similar function is,the rougher the initial stress.For the M7.5earthquake with an epicenter in the vicinity of Suqian,the Xinyi-Tancheng segment,which is located in the IX-intensity zone north of the epicenter,vibrates more strongly on the northern side than on the southern side due to the influence of the lowvelocity zone and the peak slip rate.The response spectra analysis at stations in the study area is useful for improving the earthquake resistance capability.

Settling behavior of spherical particles in vertical annulus:Experimental study and model development

Coiled tubing(CT)drilling technology offers significant advantages in terms of cost and efficiency for exploitations of unconventional oil and gas resources.However,the development of CT drilling technol-ogy is restricted by cuttings accumulation in the wellbore due to non-rotation of the drill string and limited circulating capacity.Cuttings cleaning becomes more difficult with the wall resistance of pipe-wellbore annulus on the cutting transport.Accurate description of particle transport process in the pipe-wellbore annulus is,therefore,important for improving the wellbore cleanliness.In this study,high-speed cam-era is used to record and analyze the settling process of particles in the transparent annulus filled with power-law fluids.A total of 540 tests were carried out,involving dimensionless diameters of 0.10-0.95 and particle Reynolds Numbers of 0.01-12.97,revealing the effect of the dimensionless diameter and particle Reynolds number on the annulus wall effect,and the wall factor model with an average relative error of2.75%was established.In addition,a dimensionless parameter,Archimedes number,independent of the settling velocity,was introduced to establish an explicit model of the settling velocity of spherical particles in the vertical annulus,with the average relative error of 7.89%.Finally,a calculation example was provided to show how to use the explicit model of settling velocity in annulus.The results of this study are expected to provide guidance for field engineers to improve the wellbore cleanliness of coiled tubing drilling.

Rational fabrication of ordered porous solid strong bases by utilizing the inherent reducibility of metal-organic frameworks

Ordered porous solid strong bases(OPSSBs)have attracted great research interest due to the excellent performance as heterogeneous catalysts in various reactions.The main obstacle for fabricating OPSSBs is the requirement of high temperature to produce strong basicity on ordered porous materials.For example,the temperatures of 600-650℃ are required for the decomposition of base precursor NaNO_(3)to basic sites on mesoporous silica SBA-15 and zeolite Y.Such high decomposition temperatures are energy-intensive and harmful to the structure of supports.Herein,we report the fabrication of OPSSBs by utilizing the redox interaction between base precursor and low-valence metal centers(e.g.,Cr^(3+))in metal-organic frameworks(MOFs).The base precursor NaNO_(3)on MIL-101(Cr)can be converted to basic sites entirely at 300℃,which is quite lower than those of the conventional thermal conversion on SBA-15 and zeolite Y(600-650℃).The exploration on decomposition mechanism reveals that the valence change of Cr^(3+)to Cr^(6+)takes place during the conversion of NaNO_(3)to basic sites.In this way,MOFs-derived base catalysts have been synthesized successfully by the host-guest redox strategy and exhibit high catalytic activity in typical base-catalyzed reactions.
