Cholangioscopy-assisted extraction through novel papillary support for small-calibre and sediment-like common bile duct stones

BACKGROUND To date,endoscopic retrograde cholangiopancreatography has become a wellestablished treatment for common bile duct(CBD)stones.However,it is not suitable for some special patients,such as pregnant women,children or those who cannot stop taking anti-coagulation/anti-platelet agents because of radiation injury and the risk of postoperative bleeding resulting from endoscopic sphincterotomy.To overcome these two problems,this study introduced cholangioscopy-assisted extraction through a novel papillary support for small-calibre and sediment-like CBD stones.AIM To assess the feasibility and safety of cholangioscopy-assisted extraction through a novel papillary support(CEPTS)for small-calibre and sediment-like common bile duct(CBD)stones.METHODS This Retrospective study was approved by the Ethics Committee of the Chinese PLA General Hospital.We designed a covered single dumbbell-style papillary support between 2021 and 2022.Between July 2022 and September 2022,7 consecutive patients with small-calibre(cross diameter≤1.0 cm)or sediment-like CBD stones underwent CETPS procedures in our center.The clinical characteristics and treatment outcomes of these 7 patients were extracted from a prospectively collected database.And the related data were analyzed.Informed consent was obtained from all participating patients.RESULTS A total of 2 patients had yellow sediment-like CBD stones,and aspiration extraction was performed after the insertion of papillary support.Of the 5 patients with clumpy CBD stones(0.4-1.0 cm),2 underwent basket extraction under direct vision for a single stone(0.5-1.0 cm,black and black grey),1 underwent balloon plus aspiration extraction under direct vision for 5 stones(0.4-0.6 cm,brown),and 2 underwent aspiration extraction only for a single stone(0.5-0.6 cm,yellow,none).Technical success,namely,no residual stones in the CBD or left and right hepatic ducts,was achieved in all 7 cases(100%).The median operating time was 45.0 minutes(range 13.0–87.0 minutes).Postoperative pancreatitis(PEP)occurr

BACKGROUND Secondary thyroid malignancies are rarely reported,especially thyroid metastasis after resection of hepatocellular carcinoma(HCC).We report a patient with thyroid metastasis after resection of HCC.CASE SUMMARY A 42-year-old female underwent partial hepatectomy for HCC three years ago.She attended hospital because of neck discomfort.After various examinations,she was diagnosed with metastatic HCC.She survived after surgical resection of the affected side of the thyroid.CONCLUSION Although secondary malignant tumor of the thyroid is classified as distant metastasis,surgical resection is still necessary according to the patient's condition.

The Global Influenza Surveillance Network is crucial for monitoring epidemic risk in participating countries. However, at present, the network has notable gaps in the developing world, principally in Africa and Asia where laboratory capabilities are limited. Moreover, for the last few years, various influenza viruses have been continuously emerging in the resource-limited countries, making these surveillance gaps a more imminent challenge.We present a spatial-transmission model to estimate epidemic risks in the countries where only partial or even no surveillance data are available. Motivated by the observation that countries in the same influenza transmission zone divided by the World Health Organization had similar transmission patterns, we propose to estimate the influenza epidemic risk of an unmonitored country by incorporating the surveillance data reported by countries of the same transmission zone. Experiments show that the risk estimates are highly correlated with the actual influenza morbidity trends for African and Asian countries. The proposed method may provide the much-needed capability to detect, assess, and notify potential influenza epidemics to the developing world.

基于阶段-环境匹配理论及生态系统理论等框架,采用问卷法对山东省1027名初中生进行调查,考察了学校氛围与初中生教育规划的关系,检验了学习投入在两者之间的中介作用,同时探讨了父母教育期望在该中介模型中所起的调节作用。结果发现:(1)学校氛围能正向预测初中生的教育规划;(2)学习投入在学校氛围与初中生教育规划之间起中介作用;(3)父母教育期望调节了学校氛围通过学习投入影响教育规划的前半段路径,且父母教育期望越高,学校氛围通过学习投入对教育规划产生的影响越强。

Regeneration of the heart:f rom molecular mechanisms to clinical therapeutics

Heart injury such as myocardial infarction leads to cardiomyocyte loss,fibrotic tissue deposition,and scar formation.These changes reduce cardiac contractility,resulting in heart failure,which causes a huge public health burden.Military personnel,compared with civilians,is exposed to more stress,a risk factor for heart diseases,making cardiovascular health management and treatment innovation an important topic for military medicine.So far,medical intervention can slow down cardiovascular disease progression,but not yet induce heart regeneration.In the past decades,studies have focused on mechanisms underlying the regenerative capability of the heart and applicable approaches to reverse heart injury.Insights have emerged from studies in animal models and early clinical trials.Clinical interventions show the potential to reduce scar formation and enhance cardiomyocyte proliferation that counteracts the pathogenesis of heart disease.In this review,we discuss the signaling events controlling the regeneration of heart tissue and summarize current therapeutic approaches to promote heart regeneration after injury.

Roles of nanoparticles in arsenic mobility and microbial community composition in arsenic-enriched soils

Environmental effects of nano remediation engineering of arsenic(As)pollution need to be considered.In this study,the roles of Fe_(2)O_(3)and TiO_(2)nanoparticles(NPs)on the microbial mediated As mobilization from As contaminated soil were investigated.The addition of Fe_(2)O_(3)and TiO_(2)NPs restrained As(V)release,and stimulated As(Ⅲ)release.As(V)concentration decreased by 94% and 93% after Fe_(2)O_(3)addition,and decreased by 89% and 45% after Ti O_(2)addition compared to the Biotic and Biotic+Acetate(amended with sodium acetate)controls,respectively.The maximum values of As(Ⅲ)were 20.5 and 27.1μg/L at 48 d after Fe_(2)O_(3)and TiO_(2)NPs addition,respectively,and were higher than that in Biotic+Acetate control(12.9μg/L).The released As co-precipitated with Fe in soils in the presence of Fe_(2)O_(3)NPs,but adsorbed on TiO_(2)NPs in the presence of TiO_(2)NPs.Moreover,the addition of NPs amended with sodium acetate as the electron donor clearly promoted As(V)reduction induced by microbes.The NPs addition changed the relative abundance of soil bacterial community,while Proteobacteria(42.8%-70.4%),Planctomycetes(2.6%-14.3%),and Firmicutes(3.5%-25.4%)were the dominant microorganisms in soils.Several potential As/Fe reducing bacteria were related to Pseudomonas,Geobacter,Desulfuromonas,and Thiobacillus.The addition of Fe_(2)O_(3)and TiO_(2)NPs induced to the decrease of arr A gene.The results indicated that the addition of NPs had a negative impact on soil microbial population in a long term.The findings offer a relatively comprehensive assessment of Fe_(2)O_(3)and TiO_(2)NPs effects on As mobilization and soil bacterial communities.

Antimicrobial resistance characteristics and phylogenetic relationships of pleuromutilin-resistant Enterococcus isolates from different environmental samples along a laying hen production chain

Antimicrobial resistance in the laying hen production industry has become a serious public health problem.The antimicrobial resistance and phylogenetic relationships of the common conditional pathogen Enterococcus along the laying hen production chain have not been systematically clarified.105 Enterococcus isolates were obtained from 115 environmental samples (air,dust,feces,fies,sewage,and soil) collected along the laying hen production chain (breeding chicken,chick,young chicken,and commercial laying hen).These Enterococcus isolates exhibited resistance to some clinically relevant antibiotics,such as tetracycline (92.4%),streptomycin (92.4%),and erythromycin (91.4%),and all strains had multidrug resistance phenotypes.Whole genome sequencing characterized 29 acquired antibiotic resistance genes (ARGs) that conferred resistance to 11 classes of antibiotics in 51pleuromutilin-resistant Enterococcus isolates,and lsa(E),which mediates resistance to pleuromutilins,always co-occurred with lnu(B).Alignments with the Mobile Genetic Elements database identified four transposons (Tn554,Tn558,Tn6261,and Tn6674) with several ARGs(erm(A),ant(9)-la,fex(A),and optrA) that mediated resistance to many clinically important antibiotics.Moreover,we identified two new transposons that carried ARGs in the Tn554 family designated as Tn7508 and Tn7492.A complementary approach based on conventional multi-locus sequence typing and whole genome single nucleotide polymorphism analysis showed that phylogenetically related pleuromutilin-resistant Enterococcus isolates were widely distributed in various environments on different production farms.Our results indicate that environmental contamination by antimicrobial-resistant Enterococcus requires greater attention,and they highlight the risk of pleuromutilin-resistant Enterococcus and ARGs disseminating along the laying hen production chain,thereby warranting effective disinfection.

Cardiovascular diseases(CVD)are the leading cause of death in the world and one of the most common diseases in the elderly,with high prevalence and poor prognosis,which seriously affect the health of the elder patients.One report showed that global deaths from CVD increased from 12.4 million in 1990 to 19.8 million in 2022,reflecting the global population growth and aging,as well as the impact of metabolic,environmental,and behavioral risks.[1]With the progress of aging population,the burden of CVD in the elderly in China has increased year by year,and factors including multiple complications,organ function decline,and high complication rate have made the interventional treatment of CVD in the elderly one of the difficulties and focuses in the field of cardiology in China.

Honokiol Prevents Intestinal Barrier Dysfunction in Mice with Severe Acute Pancreatitis and Inhibits JAK/STAT1 Pathway and Acetylation of HMGB1

Objective To investigate the effect of honokiol(HON)and the role of high-mobility group protein B1(HMGB1)on the pathogenesis of severe acute pancreatitis(SAP).Methods Thirty mice were numbered according to weight,and randomly divided into 5 groups using a random number table,including control,SAP,SAP and normal saline(SAP+NS),SAP and ethyl pyruvate(SAP+EP),or SAP+HON groups,6 mice in each group.Samples of pancreas,intestine,and blood were collected 12 h after SAP model induction for examination of pathologic changes,immune function alterations by enzyme linked immunosorbent assay(ELISA),and Western blot.In vitro experiments,macrophages were divided into 5 groups,the control,lipopolysaccharide(LPS),LPS+DMSO(DMSO),LPS+anti-HMGB1 monoclonal antibody(mAb),and LPS+HON groups.The tight connection level was determined by transmission electron microscopy and fluorescein isothiocyanate-labeled.The location and acetylation of HMGB1 were measured by Western blot.Finally,pyridone 6 and silencing signal transducer and activator of the transcription 1(siSTAT1)combined with honokiol were added to determine whether the Janus kinase(JAK)/STAT1 participated in the regulation of honokiol on HMGB1.The protein expression levels of HMGB1,JAK,and STAT1 were detected using Western blot.Results Mice with SAP had inflammatory injury in the pancreas,bleeding of intestinal tissues,and cells with disrupted histology.Mice in the SAP+HON group had significantly fewer pathological changes.Mice with SAP also had significant increases in the serum levels of amylase,lipase,HMGB1,tumor necrosis factor-α,interleukin-6,diamine oxidase,endotoxin-1,and procalcitonin.Mice in the SAP+HON group did not show these abnormalities(P<0.01).Studies of Caco-2 cells indicated that LPS increased the levels of occludin and claudin-1 as well as tight junction permeability,decreased the levels of junctional adhesion molecule C,and elevated intercellular permeability(P<0.01).HON treatment blocked these effects.Studies of macrophages indicated that LPS led to low nucle

Prediction for permeability index of blast furnace based on VMD-PSO-BP model

The permeability index is one of the important production indicators to monitor the operation of blast furnace.It is crucial to grasp the trends of changes in the new permeability index in time.For the complex vibration spectrum of the permeability index,a prediction model of the permeability index based on the VMD-PSO-BP(variational mode decomposition-particle swarm optimization-back propagation)method was proposed.Firstly,the key factors that affect the permeability index of blast furnace were studied from multiple perspectives.Then,the permeability index was divided into multiple sub-modes based on the difference of frequency bands by the VMD algorithm,and a PSO-BP prediction model was established for each sub-mode.Finally,the prediction results of each sub-mode were summed to obtain the final one.The results show that the composite prediction accuracy by using the VMD algorithm is 3%higher than that of the traditional prediction method,which has better applicability.
