Antibiotics De-Escalation in the Treatment of Ventilator-Associated Pneumonia in Trauma Patients： A Retrospective Study on Propensity Score Matching Method

Background： Antimicrobial de-escalation refers to starting the antimicrobial treatment with broad-spectrum antibiotics, followed by narrowing the drug spectrum according to culture results. The present study evaluated the effect of de-escalation on ventilator-associated pneumonia （VAP） in trauma patients. Methods： This retrospective study was conducted on trauma patients with VAP, who received de-escalation therapy （de-escalation group） or non-de-escalation therapy （non-de-escalation group）. Propensity score matching method was used to balance the baseline characteristics between both groups. The 28-day mortality, length of hospitalization and Intensive Care Unit stay, and expense of antibiotics and hospitalization between both groups were compared. Multivariable analysis explored the factors that influenced the 28-day mortality and implementation of de-escalation. Results： Among the 156 patients, 62 patients received de-escalation therapy and 94 patients received non-de-escalation therapy. No significant difference was observed in 28-day mortality between both groups （28.6% vs. 23.8%, P = 0.620）. The duration of antibiotics treatment in the de-escalation group was shorter than that in the non-de-escalation group （11 [8-13] vs. 14 [8-19] days, P = 0.045）. The expenses of antibiotics and hospitalization in de-escalation group were significantly lower than that in the non-de-escalation group （6430 ± 2730 vs. 7618 ± 2568 RMB yuan, P = 0.043 and 19,173 ± 16,861 vs. 24,184 ± 12,039 RMB yuan, P = 0.024, respectively）. Multivariate analysis showed that high Acute Physiology and Chronic Health Evaluation Ⅱ （APACHE Ⅱ） score, high injury severity score, multi-drug resistant （MDR） infection, and inappropriate initial antibiotics were associated with patients＇ 28-day mortality, while high APACHEⅡ score, MDR infection and inappropriate initial antibiotics were independent factors that prevented the implementation of de-escalation. Enterogenous infection of Candida albicans in immunocompromised rats under severe acute pancreatitis

BACKGROUND:Opportunistic infection of Candida albicans(C.albicans) has become a serious problem in immunocompromised patients.The study aimed to explore the mechanism of enterogenous infection of C.albicans in immunocompromised rats under severe acute pancreatitis(SAP).METHODS:Sprague Dawley(SD) rats(n=100) were randomly assigned into 5 groups as the following:blank group,cyclophosphamide+ceftriaxone+SAP group,cyclophosphamide+ceftriaxone group,cyclophosphamide+SAP group,and cyclophosphamide group.The rats were sacrificed at 5and 10 days,and their jejunum,colon,mesenteric lymph nodes,pancreas,intestinal content,and blood were quickly collected to detect C.albicans.A region of the 25 S rRNA gene was chosen and amplified by polymerase chain reaction(PCR) to differentiate C.albicans genotypes.The amplified products were further sequenced and compared to judge their homology.RESULTS:Compared with the Cyclophosphamide group,the combination of immunosuppressants and broad-spectrum antibiotics significantly increased the colonization of C.albicans in intestine in 5 and 10 days.Pure SAP stress did not increase the opportunistic infection of C.albicans.The PCR products of C.albicans isolates all belonged to the genotype A family,and sequence alignment showed that the amplified fragments were homologous.CONCLUSION:The damage of immune system and broad-spectrum antimicrobial agents are important risk factors for opportunistic fungal infection.Intestinal tract is an important source for genotype-A C.albicans to translocate and invade into bloodstream.

Impact of uterine artery embolization on ovarian function and pregnancy outcome after uterine-fibroids treatment:A prospective study

Uterine fibroids are benign tumors that originate from smooth muscle cells of the uterus.It is the most common gynecological disorder,affecting up to 80%of women of reproductive age.Uterine fibroids can cause various symptoms such as abnormal uterine bleeding,pelvic pain,infertility,and pregnancy complications.The treatment options for uterine fibroids include medical therapy,surgical intervention,and minimally invasive techniques.AIM To compare ovarian function of women with uterine fibroids who did or did not undergo uterine artery embolization(UAE).METHODS This prospective cohort study enrolled 87 women with symptomatic uterine fibroids who underwent UAE,and 87 women with the same symptoms who did not undergo UAE but received conservative management or other treatments.The two groups were matched for age,body mass index,parity,and baseline characteristics of uterine fibroids.The primary outcome was ovarian function that was evaluated by serum levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),and anti-Müllerian hormone(AMH),as well as ovarian reserve tests,such as antral follicle count(AFC)and ovarian volume(OV).The secondary outcome was fertility that was evaluated based on the menstrual cycle,ovulation,conception,pregnancy,and delivery.The participants were followed-up for 36 months and assessed at 1,3,6,12,24,and 36 months after treatment.RESULTS The study found that the most common minor complication of UAE was postembolization syndrome in 73.6% of women,resolving within a week.No significant differences were observed between the UAE group and the control group in serum levels of reproductive hormones(FSH,LH,E2,AMH)and ovarian reserve indicators(AFC,OV)at any point up to 36 months post-treatment.Additionally,there were no significant differences in conception,pregnancy,or delivery rates,with the average time to conception and gestational age at delivery being similar between the two groups.Birth weights were also comparable.Finally,there was no significant correlation between

Role of micro-Ca/In alloying in tailoring the microstructural characteristics and discharge performance of dilute Mg-Bi-Sn-based alloys as anodes for Mg-air batteries

The influence of micro-Ca/In alloying on the microstructural charac teristics,electrochemical behaviors and discharge properties of extruded dilute Mg-0.5Bi-0.5Sn-based(wt.%)alloys as anodes for Mg-air batteries are evaluated.The grain size and texture intensity of the Mg-Bi-Sn-based alloys are significantly decreased after the Ca/In alloying,particularly for the In-containing alloy.Note that,in addition to nanoscale Mg_(3)Bi_(2)phase,a new microscale Mg_(2)Bi_(2)Ca phase forms in the Ca-containing alloy.The electrochemical test results demonstrate that Ca/In micro-alloying can enhance the electrochemical activity.Using In to alloy the Mg-Bi-Sn-based alloy is effective in restricting the cathodic hydrogen evolution(CHE)kinetics,leading to a low self-corrosion rate,while severe CHE occurred after Ca alloying.The micro-alloying of Ca/In to Mg-Bi-Sn-based alloy strongly deteriorates the compactness of discharge products film and mitigates the"chunk effect"(CE),hence the cell voltage,anodic efficiency as well as discharge capacity are greatly improved.The In-containing alloy exhibits outstanding discharge performance under the combined effect of the modified microstructure and discharge products,thus making it a potential anode material for primary Mg-air battery.

The development of new design strategies to create innovative structural materials,refine existing ones,and achieves compatible combinations of strength and plasticity remains a worldwide goal.Promising alloys,such as shape memory alloys(SMAs),bulk metallic glasses(BMGs),high entropy alloys(HEAs),and heterogeneous pure metals such as Cu,have excellent mechanical responses,but they still fall short of meeting all the requirements of structural materials due to specific flaws,such as lack of tensile de-formation for BMGs and low yielding strength for HEAs.To address these shortcomings,proposals such as integrating glassy matrices and crystallized alloys,such as HEAs/SMAs,have been suggested.However,these solutions have unresolved issues,such as the challenging control of B2 phase formation in BMG composites.Recently,glass-crystal(A/C)laminated alloys with alternating layers have been reported to exhibit improved mechanical properties and activated work-hardening behaviors,but they still face press-ing issues such as bonding interfaces and unknown deformation mechanisms.This review focuses on design routes such as the selection of alloy components and processing techniques,exploration of micro-structural evolution and deformation modes with an increase in strain,and future solutions to address pressing and unsolved issues.These prominent advantages include diversified deformation mechanisms,such as deformation twinning,martensitic phase transformation,and precipitation hardening,as well as tuned interactive reactions of shear bands(SBs)near the A/C interfaces.Thus,this review provides a promising pathway to design and develop structural materials in the materials field community.

Numerical simulation study on the hygrothermal performance of building exterior walls under dynamic wind-driven rain condition

Wind-driven rain(WDR)has a significant influence on the hygrothermal performance,durability,and energy consumption of building components.The calculation of WDR loads using semi-empirical models has been incorporated into the boundary conditions of coupled heat and moisture transfer models.However,prior research often relied on fixed WDR absorption ratio,which fail to accurately capture the water absorption characteristics of porous building materials under rainfall scenarios.Therefore,this study aims to investigate the coupled heat and moisture transfer of exterior walls under dynamic WDR boundary conditions,utilizing an empirically obtained WDR absorption ratio model based on field measurements.The developed coupled heat and moisture transfer model is validated against the HAMSTAD project.The findings reveal that the total WDR flux calculated with the dynamic WDR boundary is lower than that obtained with the fixed WDR boundary,with greater disparities observed in orientations experiencing higher WDR loads.The variations in moisture flow significantly impact the surface temperature and relative humidity of the walls,influencing the calculation of cooling and heating loads by different models.Compared to the transient heat transfer model,the coupled heat and moisture transfer model incorporating dynamic WDR boundary exhibits maximum increases of 17.6%and 16.2%in cooling and heating loads,respectively.The dynamic WDR boundary conditions provide more precise numerical values for surface moisture flux,offering valuable insights for the thermal design of building enclosures and load calculations for HVAC systems.

An integrative mathematical model of microwave ion thruster for mHz-frequency thrust noise analysis

Many high-precision space missions need thrusters to produce thrust with low noise to compensate for disturbances and ensure satellite platform stability. Microwave ion thruster is characterized with a wide thrust range and potential for these missions. A cost-effective and accurate mathematical model is crucial for mHz-frequency thrust noise analysis and feedback controller design. The Particle-In-Cell(PIC) and global models are two common simulation tools. The PIC model is characterized with high accuracy but huge computation cost, which is difficult to analyze long-time performance characteristics. Now, the global model is only used for the discharge chamber with low accuracy and cannot reflect ion extraction properties. In this paper, an integrative mathematical model is built for a 1-cm microwave ion thruster and can reflect ion beamlet divergence and impingement on the Accelerator Grid(AG). Simulation results show good agreement with experiments at 0.06 sccm. However, the model demonstrates worse consistency with experiments when the flux increases to 0.1 sccm, which may be because the influence of neutral gas on the Electron Cyclotron Resonance(ECR) is not considered in the model. A long-time(1000 s) simulation is conducted with this model under 35 μN. It takes 3 hrs, and the thrust noise reaches 1 μN/Hz^(0.5) at 1 mHz.

Prenatal amoxicillin exposure induces developmental toxicity in fetal mice and its characteristics

Amoxicillin,a widely used antibiotic in human and veterinary pharmaceuticals,is now considered as an“emerging contaminant”because it exists widespreadly in the environment and brings a series of adverse outcomes.Currently,systematic studies about the developmental toxicity of amoxicillin are still lacking.We explored the potential effects of amoxicillin exposure on pregnancy outcomes,maternal/fetal serum phenotypes,and fetal multiple organ development in mice,at different doses(75,150,300 mg/(kg·day))during late-pregnancy,or at a dose of 300 mg/(kg·day)during different stages(mid-/latepregnancy)and courses(single-/multi-course).Results showed that prenatal amoxicillin exposure(PAmE)had no significant infuence on the body weights of dams,but it could inhibit the physical development and reduce the survival rate of fetuses,especially during the midpregnancy.Meanwhile,PAmE altered multiple maternal/fetal serum phenotypes,especially in fetuses.Fetal multi-organ function results showed that PAmE inhibited testicular/adrenal steroid synthesis,long bone/cartilage and hippocampal development,and enhanced ovarian steroid synthesis and hepatic glycogenesis/lipogenesis,and the order of severity might be gonad(testis,ovary)>liver>others.Further analysis found that PAmE-induced multiorgan developmental and functional alterations had differences in stages,courses and fetal gender,and the most obvious changes might be in high-dose,late-pregnancy and multicourse,but there was no typical rule of a dose-response relationship.In conclusion,this study confirmed that PAmE could cause abnormal development and multi-organ function alterations,which deepens our understanding of the risk of PAmE and provides an experimental basis for further exploration of the long-term harm.
