Timing, distribution, and microbiology of infectious complications after necrotizing pancreatitis

BACKGROUND Acute pancreatitis(AP)is a common acute abdominal disease worldwide,and its incidence rate has increased annually.Approximately 20%of AP patients develop into necrotizing pancreatitis(NP),and 40%to 70%of NP patients have infectious complications,which usually indicate a worse prognosis.Infection is an important sign of complications in NP patients.AIM To investigate the difference in infection time,infection site,and infectious strain in NP patients with infectious complications.METHODS The clinical data of AP patients visiting the Department of General Surgery of Xuanwu Hospital of Capital Medical University from January 1,2014 to December 31,2018 were collected retrospectively.Enhanced computerized tomography or magnetic resonance imaging findings in patients with NP were included in the study.Statistical analysis of infectious bacteria,infection site,and infection time in NP patients with infectious complications was performed,because knowledge about pathogens and their antibiotic susceptibility patterns is essential for selecting an appropriate antibiotic.In addition,the factors that might influence the prognosis of patients were analyzed.RESULTS In this study,539 strains of pathogenic bacteria were isolated from 162 patients with NP infection,including 212 strains from pancreatic infections and 327 strains from extrapancreatic infections.Gram-negative bacteria were the main infectious species,the most common of which were Escherichia coli and Pseudomonas aeruginosa.The extrapancreatic infection time(9.1±8.8 d)was earlier than the pancreatic infection time(13.9±12.3 d).Among NP patients with early extrapancreatic infection(14 d),bacteremia(15.94%),respiratory tract infection(7.74%),and urinary tract infection(7.71%)were predominant.Drug sensitivity analysis showed that P.aeruginosa was sensitive to enzymatic penicillins,thirdand fourth-generation cephalosporins,and

Device design principles and bioelectronic applications for flexible organic electrochemical transistors

Organic electrochemical transistors(OECTs) exhibit significant potential for applications in healthcare and human-machine interfaces, due to their tunable synthesis, facile deposition, and excellent biocompatibility. Expanding OECTs to the fexible devices will significantly facilitate stable contact with the skin and enable more possible bioelectronic applications. In this work,we summarize the device physics of fexible OECTs, aiming to offer a foundational understanding and guidelines for material selection and device architecture. Particular attention is paid to the advanced manufacturing approaches, including photolithography and printing techniques, which establish a robust foundation for the commercialization and large-scale fabrication. And abundantly demonstrated examples ranging from biosensors, artificial synapses/neurons, to bioinspired nervous systems are summarized to highlight the considerable prospects of smart healthcare. In the end, the challenges and opportunities are proposed for fexible OECTs. The purpose of this review is not only to elaborate on the basic design principles of fexible OECTs, but also to act as a roadmap for further exploration of wearable OECTs in advanced bio-applications.

Efficient degradation of aqueous dichloromethane by an enhanced microbial electrolysis cell:Degradation kinetics,microbial community and metabolic mechanisms

Dichloromethane(DCM)has been listed as a toxic and harmful water pollutant,and its re moval needs attention.Microbial electrolysis cells(MECs)are viewed as a promising alterna tive for pollutant removal,which can be strengthened from two aspects:microbial inocula tion and acclimation.In this study,the MEC for DCM degradation was inoculated with the ac tive sludge enhanced by Methylobacterium rhodesianum H13(strain H13)and then acclimated in the form of a microbial fuel cell(MFC).Both the introduction of strain H13 and the initi ation in MFC form significantly promoted DCM degradation.The degradation kinetics were fitted by the Haldane model,with V_(max),K_(h),K_(i)and v_(max)values of 103.2 mg/L/hr,97.8 mg/L268.3 mg/L and 44.7 mg/L/hr/cm^(2),respectively.The cyclic voltammogram implies that DCM redox reactions became easier with the setup of MEC,and the electrochemical impedance spectrogram shows that the acclimated and enriched microbes reduced the charge transfe resistance from the electrode to the electrolyte.In the biofilm,the dominant genera shifted from Geobacter to Hyphomicrobium in acclimation stages.Moreover,Methylobacterium played an increasingly important role.DCM metabolism mainly occurred through the hydrolytic glutathione S-transferase pathway,given that the gene dcmA was identified rather than the dhlA and P450/MO.The exogenous electrons facilitated the reduction of GSSG,directly o indirectly accelerating the GSH-catalyzed dehalogenation.This study provides support fo the construction of an efficient and stable MEC for DCM removal in water environment.

Integrated strategy for real-time wind power fluctuation mitigation and energy storage system control

To address the impact of wind-power fluctuations on the stability of power systems,we propose a comprehensive approach that integrates multiple strategies and methods to enhance the efficiency and reliability of a system.First,we employ a strategy that restricts long-and short-term power output deviations to smoothen wind power fluctuations in real time.Second,we adopt the sliding window instantaneous complete ensemble empirical mode decomposition with adaptive noise(SW-ICEEMDAN)strategy to achieve real-time decomposition of the energy storage power,facilitating internal power distribution within the hybrid energy storage system.Finally,we introduce a rule-based multi-fuzzy control strategy for the secondary adjustment of the initial power allocation commands for different energy storage components.Through simulation validation,we demonstrate that the proposed comprehensive control strategy can smoothen wind power fluctuations in real time and decompose energy storage power.Compared with traditional empirical mode decomposition(EMD),ensemble empirical mode decomposition(EEMD),and complete ensemble empirical mode decomposition with adaptive noise(CEEMDAN)decomposition strategies,the configuration of the energy storage system under the SW-ICEEMDAN control strategy is more optimal.Additionally,the state-of-charge of energy storage components fluctuates within a reasonable range,enhancing the stability of the power system and ensuring the secure operation of the energy storage system.

Mutual aid instead of mutual restraint:interactive probing for topological charge and phase of a vortex beam of large aberrations

An imperfect propagation environment or optical system would introduce wavefront aberrations to vortex beams.The phase aberrations and orbital angular momentum in a vortex beam are proved to be mutually restrictive in parameter measurement.Aberrations make traditional topological charge(TC)probing methods ineffective while the phase singularity makes phase retrieval difficult due to the aliasing between the wrapped phase jump and the vortex phase jump.An interactive probing method is proposed to make measurements of the aberrated phase and orbital angular momentum in a vortex beam assist rather than hinder each other.The phase unwrapping is liberated from the phase singularity by an annular shearing interference technique while the TC value is determined by a Moirétechnique immune to aberrations.Simulation and experimental results proving the method effective are presented.It is of great significance to judge the characteristics of vortex beams passing through nonideal environments and optical systems.

Audio Mixing Inversion via Embodied Self-supervised Learning

Audio mixing is a crucial part of music production.For analyzing or recreating audio mixing,it is of great importance to conduct research on estimating mixing parameters used to create mixdowns from music recordings,i.e.,audio mixing inversion.However,approaches of audio mixing inversion are rarely explored.A method of estimating mixing parameters from raw tracks and a stereo mixdown via embodied self-supervised learning is presented.In this work,several commonly used audio effects including gain,pan,equalization,reverb,and compression,are taken into consideration.This method is able to learn an inference neural network that takes a stereo mixdown and the raw audio sources as input and estimate mixing parameters used to create the mixdown by iteratively sampling and training.During the sampling step,the inference network predicts a set of mixing parameters,which is sampled and fed to an audio-processing framework to generate audio data for the training step.During the training step,the same network used in the sampling step is optimized with the sampled data generated from the sampling step.This method is able to explicitly model the mixing process in an interpretable way instead of using a black-box neural network model.A set of objective measures are used for evaluation.The experimental results show that this method has better performance than current state-of-the-art methods.

GPa-level pressure-induced enhanced corrosion resistance in TiZrTaNbSn biomedical high-entropy alloy

TiZrTaNb-based high-entropy alloys(HEAs)are research frontier of biomedical materials due to their high hardness,good yield strength,excellent wear resistance and corrosion resistance.Sn,as an essential trace element in the human body that plays a significant role in physiological process.It has stable chemical properties and a low elastic modulus.In this study,a new material,TiZrTaNbSn HEAs,was proposed as a potential biomedical alloy.The Ti_(35)Zr_(25)Ta_(15)Nb_(15)Sn_(10)biomedical high-entropy alloys(BHEAs)were successfully prepared through an arc melting furnace and then remelted using a German high-temperature and high-pressure apparatus under GPa-level(4 GPa and 7 GPa).The precipitation behavior of the needle-like HCP-Zr_(5)Sn_(3)phase that precipitates discontinuously at the grain boundary was successfully controlled.The phase constitution,microstructure,and corrosion resistance of the alloy were studied.The results show that the needle-like HCP-Zr_(5)Sn_(3)phase is eliminated and the(Zr,Sn)-rich nano-precipitated phase is precipitated in the microstructure under high pressure,which leads to the narrowing of grain boundaries and consequently improves the corrosion resistance of the alloy.In addition,the formation mechanisms of(Zr,Sn)-rich nanoprecipitates in BHEAs were discussed.More Zr and Sn dissolve in the matrix due to the effect of high pressure,during the cooling process,they precipitate to form a(Zr,Sn)-rich nano-precipitated phase.

Clinical effects of comprehensive therapy of early psychological intervention and rehabilitation training on neurological rehabilitation of patients with acute stroke

Objective:To evaluate the clinical effects of comprehensive therapy of psychological intervention and rehabilitation training on the mental health of the patients with acute stroke. Methods:A total of 120 patients with acute stroke were randomly divided into trial group and control group.Both groups were given the corresponding drug therapy,medical basic nursing and convention nursing.Besides,psychological intervention and comprehensive rehabilitation training were added to the trial group.SCL-90,Europ stroke scales(ESS) score were assessed with each patient on day 3 for the first time and on day 21 for the second time; Barthel index was assessed on the day 90.Results:After psychological intervention,SCL-90 declined significantly in the trial group comparing with the control group,there were signicant differences in the somatization,obsession,depression,anxiety,fear,ESS score,Barthel index and other psychological factors between the trial group and control group(P<0.05).Conclusions: Comprehensive therapy of early psychological intervention and rehabilitation training can significantly improve the mental health,limb movement function,stress ability and activity of daily living on the patients with acute stroke.

A rice exocyst subunit gene SEC3A is required for anther dehiscence

Anther dehiscence controls optimal interaction between pollen and stigma,thereby determining the successful sexual reproduction.The regulators or mechanisms of this process remain elusive.Here,two CRISPR/Cas9 mutants of a rice exocyst subunit gene SEC3A,sec3a-1 and sec3a-2,showed anther indehiscence at anthesis and male sterility at maturity.Pollen viability and germination in the mutants were partly defective,whereas their female gametes undergone a normal development.Hybrid or self-pollinated seeds could be produced by artificial pollination,suggesting potential use of a weak sec3a mutant as a female line during hybrid breeding.SEC3A is widely expressed in various tissues,including anther walls.Further results showed an excessive IAA accumulation and no endothecium lignification in sec3a-1/2 anthers.Our findings suggest that SEC3A appears to regulate anther dehiscence by modulating auxin signaling,providing insights into regulation of anther dehiscence and function of exocyst in plants.
