Predicting asymptomatic coronary artery stenosis by aortic arch plaque in acute ischemic cerebrovascular disease:beyond the cervicocephalic atherosclerosis?

Background:Asymptomatic coronary artery stenosis(ACAS)≥50%is common in patients with acute ischemic cerebrovascular disease(AICVD),which portends a poor cardiovascular and cerebrovascular prognosis.Identifying ACAS>50%early may optimize the clinical management and improve the outcomes of these high-risk AICVD patients.This study aimed to investigate whether aortic arch plaque(AAP),an early atherosclerotic manifestation of brain blood-supplying arteries,could be a predictor for ACAS>50%in AICVD.Methods:In this cross-sectional study,atherosclerosis of the coronary and brain blood-supplying arteries was simultaneously evaluated using one-step computed tomography angiography(CTA)in AICVD patients without coronary artery disease history.The patients were divided into ACAS≥50%and non-ACAS≥50%groups according to whether CTA showed stenosis≥50%in at least one coronary arterial segment.The AAP characteristics of CTA were depicted from aspects of thickness,extent,and complexity.Results:Among 118 analyzed patients with AICVD,29/118(24.6%)patients had ACAS≥50%,while AAPs were observed in 86/118(72.9%)patients.Increased AAP thickness per millimeter(adjusted odds ratio[OR]:1.56,95%confidence interval[CI]:1.18-2.05),severe-extent AAP(adjusted OR:13.66,95%CI:2.33-80.15),and presence of complex AAP(adjusted OR:7.27,95%CI:2.30-23.03)were associated with ACAS≥50%among patients with AICVD,independently of clinical demographics and cervicocephalic atherosclerotic stenosis.The combination of AAP thickness,extent,and complexity predicted ACAS≥50%with an area under the receiver-operating characteristic curve of 0.78(95%CI:0.70-0.85,P<0.001).All three AAP characteristics provided additional predictive power beyond cervical and intracranial atherosclerotic stenosis for ACAS≥50%in AICVD(all P<0.05).Conclusions:Thicker,severe-extent,and complex AAP were significant markers of the concomitant ACAS≥50%in AICVD,possibly superior to the indicative value of cervical and intracranial atherosclerotic stenosis.As an integral part of atherosclerosis

Background： Cerebral venous thrombosis （CVT） may lead to serious neurological disorders; however, little is known about the risk factors for recurrent CVT. Our aim was to determine the association between elevated fibrinogen and decreased high-density lipoprotein cholesterol （HDL-C） levels with recurrent CVT. Methods： This retrospective cohort study included participants if they had a first episode of objectively defined CVT and were admitted to Xuan Wu Hospital, Capital Medical University from August 2005 to September 2009. Demographic and clinical variables were collected, as well as laboratory parameters, inchiding plasma fibrinogen and HDL-C. Patients with CVT were tbllowed for recurrent symptomatic CVT. Follow-up was through the end of September 2010. Potential predictors of recurrence were analyzed using Cox survival analysis. Results： At tile end of the lbllow-up, 95 patients were eligible lbr the study. Twelve of 95 patients （12.6%） had recurred CVT. Tile median time of recurrence was 7 months （range： 1-39 months）. Eight of these 12 （66.7%） experienced rectirrence within the first 12 months alter their initial CVT. The recurrence rate of CVT was 2.76 per 100 patient-years. Multivariate Cox regression analysis demonstrated that the coexistence of high fibrinogen （〉4.00 g/L） and low HDL-C （〈1.08 mmol/L） levels at baseline was the only independent predictor for recurrent CVT （hazard ratio： 4.69; 95% confidence interval： 1.10-20,11; P 〈 0.05）. Of tile twelve patients with recurrent CVT in our study, 7 （58.3%） had high fibrinogen plus low HDL-C levels. All 7 of these patients took warfarin for 3-12 months, and 6 of 7 had rectirrent CVT after the discontinuation of anticoagulant treatment. Conclusions： Concomitant high fibrinogen and low HDL-C levels may be associated with recurrence of CVT. The ett＇ect of potential risk lhctors related to atherothrombosis on rectinent CVT should be closely monitored.

Normobaric oxygen therapy has gained attention as a simple and convenient means of achieving neuroprotection against the pathogenic cascade initiated by acute ischemic stroke.The mechanisms underlying the neuroprotective efficacy of normobaric oxygen therapy,however,have not been fully elucidated.It is hypothesized that cerebral hyperglycolysis is involved in the neuroprotection of normobaric oxygen therapy against ischemic stroke.In this study,Sprague-Dawley rats were subjected to either 2-hour middle cerebral artery occlusion followed by 3-or 24-hour reperfusion or to a permanent middle cerebral artery occlusion event.At 2 hours after the onset of ischemia,all rats received either 95%oxygen normobaric oxygen therapy for 3 hours or room air.Compared with room air,normobaric oxygen therapy significantly reduced the infarct volume,neurological deficits,and reactive oxygen species and increased the production of adenosine triphosphate in ischemic rats.These changes were associated with reduced transcriptional and translational levels of the hyperglycolytic enzymes glucose transporter 1 and 3,phosphofructokinase 1,and lactate dehydrogenase.In addition,normobaric oxygen therapy significantly reduced adenosine monophosphate-activated protein kinase mRNA expression and phosphorylated adenosine monophosphate-activated protein kinase protein expression.These findings suggest that normobaric oxygen therapy can reduce hyperglycolysis through modulating the adenosine monophosphate-activated protein kinase signaling pathway and alleviating oxidative injury,thereby exhibiting neuroprotective effects in ischemic stroke.This study was approved by the Institutional Animal Investigation Committee of Capital Medical University(approval No.AEEI-2018-033)on August 13,2018.

Background: Mental stress-induced myocardial ischemia (MSIMI) is closely associated with adverse cardiac events in patients with coronary artery disease (CAD) and we aimed to determine whether biomarkers and blood pressure could be potential predictors of MSIMI. Methods: This study enrolled 82 patients with documented CAD between June 1, 2017 and November 9, 2017. Patient blood samples were obtained at resting period and at the end of mental arithmetic. Then, patients were assigned to MSIMI positive group and MSIMI negative group. The main statistical methods included linear regression, receiver operating characteristic (ROC) curves, and logistic regression. Results: Patients with CAD with MSIMI had significantly greater median resting N-terminal pro-brain natriuretic peptide (NT-proBNP, 141.02 [45.85–202.76] pg/mL vs. 57.95 [27.06–117.64] pg/mL;Z =-2.23, P = 0.03) and mean systolic blood pressure (SBP)(145.56 ± 16.87 mmHg vs. 134.92 ± 18.16 mmHg, Z =-2.13, P = 0.04) when compared with those without MSIMI. After 5-min mental stress task, those who developed MSIMI presented higher elevation of median post-stressor high sensitivity cardiac troponin I (hs-cTnI, 0.020 [0.009–0.100] ng/mL vs. 0.009 [0.009–0.010] ng/mL;Z =-2.45, P = 0.01), post-stressor NT-proBNP (138.96 [39.93–201.56] pg/mL vs. 61.55 [25.66–86.50] pg/mL;Z =-2.15, P = 0.03) compared with those without MSIMI. Using the ROC curves, and after the adjustment for basic characteristics, the multiple logistic regression analysis showed that patients presenting a post-stressor hs-cTnI ≥ 0.015 ng/mL had seven-fold increase in the risk of developing MSIMI (odds ratio [OR]: 7.09;95% confidence interval [CI]: 1.65–30.48;P = 0.009), a rest NT-proBNP ≥ 80.51 pg/mL had nearly eight-fold increase (OR: 7.85;95% CI: 1.51–40.82;P = 0.014), a post-stressor NT-proBNP ≥ 98.80 pg/mL had 35-fold increase (OR: 34.96;95% CI: 3.72– 328.50;P = 0.002), a rest SBP ≥ 129.50 mmHg had 11-fold increase (OR: 11.42;95% CI: 1.21–108.17;P = 0.034). The performance of massive MIMO systems relies heavily on the availability of Channel State Information at the Transmitter(CSIT).A large amount of work has been devoted to reducing the CSIT acquisition overhead at the pilot training and/or CsI feedback stage.In fact,the downlink communication generally includes three stages,i.e.,pilot training,CsI feedback,and data transmission.These three stages are mutually related and jointly determine the overall system performance.Unfortunately,there exist few studies on the reduction of csIT acquisition overhead from the global point of view.In this paper,we integrate the Minimum Mean Square Error(MMSE)channel estimation,Random Vector Quantization(RVQ)based limited feedback and Maximal Ratio Combining(MRC)precoding into a unified framework for investigating the resource allocation problem.In particular,we first approximate the covariance matrix of the quantization error with a simple expression and derive an analytical expression of the received Signal-to-Noise Ratio(SNR)based on the deterministic equivalence theory.Then the three performance metrics(the spectral efficiency,energy efficiency,and total energy consumption)oriented problems are formulated analytically.With practical system requirements,these three metrics can be collaboratively optimized.Finally,we propose an optimization solver to derive the optimal partition of channel coherence time.Experiment results verify the benefits of the proposed resource allocation schemes under three different scenarios and illustrate the tradeoff of resource allocation between three stages.

Dispersion fuels,knowned for their excellent safety performance,are widely used in advanced reactors,such as hightemperature gas-cooled reactors.Compared with deterministic methods,the Monte Carlo method has more advantages in the geometric modeling of stochastic media.The explicit modeling method has high computational accuracy and high computational cost.The chord length sampling(CLS)method can improve computational efficiency by sampling the chord length during neutron transport using the matrix chord length?s probability density function.This study shows that the excluded-volume effect in realistic stochastic media can introduce certain deviations into the CLS.A chord length correction approach is proposed to obtain the chord length correction factor by developing the Particle code based on equivalent transmission probability.Through numerical analysis against reference solutions from explicit modeling in the RMC code,it was demonstrated that CLS with the proposed correction method provides good accuracy for addressing the excludedvolume effect in realistic infinite stochastic media.

Mid-ocean ridge and oceanic transforms are among the most prominent features on the seafloor surface and are crucial for understanding seafloor spreading and plate tectonic dynamics,but the deep structure of the oceanic lithosphere remains poorly understood.The large number of microearthquakes occurring along ridges and transforms provide valuable information for gaining an indepth view of the underlying detailed seismic structures,contributing to understanding geodynamic processes within the oceanic lithosphere.Previous studies have indicated that the maximum depth of microseismicity is controlled by the 600-℃isotherm.However,this perspective is being challenged due to increasing observations of deep earthquakes that far exceed this suggested isotherm along mid-ocean ridges and oceanic transform faults.Several mechanisms have been proposed to explain these deep events,and we suggest that local geodynamic processes(e.g.,magma supply,mylonite shear zone,longlived faults,hydrothermal vents,etc.)likely play a more important role than previously thought.
