Literature review and economic evaluation of oral and intramuscular ziprasidone treatment among patients with schizophrenia in China

Background Over 10 million Chinese are affected by schizophrenia. The annual cost of schizophrenia in China was estimated at US$2586 per patient.Aims The study has two aims:(1) to conduct a targeted literature review of the economic literature on oral ziprasidone in China, and(2) to develop an inpatient economic model that compared the cost of intramuscular ziprasidone with other regimens including electroconvulsive therapy(ECT) for the management of acute agitation in patients with schizophrenia from a hospital's perspective in China.Methods A targeted literature review was conducted using PubMed and the Chinese literature databases for studies published between January 2007 and December2017. Studies that assessed costs associated with oral ziprasidone treatment for schizophrenia in China were summarised. In the inpatient economic model,cost measures included hospital room and board,antipsychotics,ECT and medications for the management of extrapyramidal symptoms(EPS). Input for standard antipsychotic regimens and unit cost were obtained from the literature. Hospital length of stay(LOS), utilisation of ECT and incidence of EPS were derived from the literature and supplemented/validated with a survey of psychiatrists in China. Cost was presented in 2017 Chinese yuan.Results The average estimated LOS was 29 days with ziprasidone, 33 days with risperidone+benzodiazepine,32 days with olanzapine, 35 days with haloperidol and 29 days with ECT. The cost of antipsychotics was $1260 with ziprasidone, $137 with risperidone+benzodiazepine, $913 with olanzapine and $210 with haloperidol; ECT treatment costs $785. The base-case analysis suggested that higher antipsychotic cost with ziprasidone was offset by savings with shorter LOS. Using intramuscular ziprasidone for acute management was associated with a total cost of $11157, the lowest among all antipsychotic regimens($11 424 with risperidone+benzodiazepine, $11 711 with olanzapine and $11 912 with haloperidol) and slightly higher than ECT($10 606). The Optimization Design of the Nozzle Section for theWater Jet Propulsion System Applied in Jet Skis

The performance of a water jet propulsion system is related to the inlet duct,rotor,stator,and nozzle.Generally,the flow inlet design must fit the bottom line of the hull,and the design of the inlet duct is often limited by stern space.The entire section,from the rotor to the nozzle through the stator,must be designed based on system integration in that the individual performance of these three components will influence each other.Particularly,the section from the rotor to the nozzle significantly impacts the performance of a water jet propulsion system.This study focused on nozzle design and established referable analysis results to facilitate subsequent integrated studies on the design parameters regarding nozzle contour.Most existing studies concentrate on discussions on rotor design and the tip leakage flow of rotors or have replaced the existing complex computational domain with a simple flow field.However,research has yet to implement an integrated,optimal design of the section from the rotor to the nozzle.Given the above,our program conducted preliminary research on this system integration design issue,discussed the optimal nozzle for this section in-depth,and proposed design suggestions based on the findings.This program used an existing model as the design case.This study referred to the actual trial data as the design conditions for the proposed model.Unlike prior references’simple flow field form,this study added a jet ski geometry and free surface to the computational domain.After the linear hull shape was considered,the inflow in the inlet duct would be closer to the actual condition.Based on the numerical calculation result,this study recommends that the optimal nozzle outlet area should be 37%of the inlet area and that the nozzle contour should be linear.Furthermore,for the pump head,static pressure had a more significant impact than dynamic pressure.

With continuous population and economic growth in the 21st century,plastic pollution is a major global issue.However,the health concern of microplastics/nanoplastics(MPs/NPs)decomposed from plastic wastes has drawn public attention only in the recent decade.This article summarizes recent works dedicated to understanding the impact of MPs/NPs on the liver-the largest digestive organ,which is one of the primary routes that MPs/NPs enter human bodies.The interrelated mechanisms including oxidative stress,hepatocyte energy re-distribution,cell death and autophagy,as well as immune responses and inflammation,were also featured.In addition,the disturbance of microbiome and gut-liver axis,and the association with clinical diseases such as metabolic dysfunction-associated fatty liver disease,steatohepatitis,liver fibrosis,and cirrhosis were briefly discussed.Finally,we discussed potential directions in regard to this trending topic,highlighted current challenges in research,and proposed possible solutions.

Dengue fever is considered the most prolific vector-borne disease in the world,with its transmission rate increasing more than eight times in the last two decades.While most cases present mild to moderate symptoms,5%of patients can develop severe disease.Although the mechanisms are yet not fully comprehended,immune-mediated activation leading to excessive cytokine expression is suggested as a cause of the two main findings in critical patients:increased vascular permeability that may shock and thrombocytopenia,and coagulopathy that can induce hemorrhage.The risk factors of severe disease include previous infection by a different serotype,specific genotypes associated with more efficient replication,certain genetic polymorphisms,and comorbidities such as diabetes,obesity,and cardiovascular disease.The World Health Organization recommends careful monitoring and prompt hospitalization of patients with warning signs or propensity for severe disease to reduce mortality.This review aims to update the diagnosis and management of patients with severe dengue in the intensive care unit.

BACKGROUND Acute carpal tunnel syndrome(ACTS)is commonly caused by repetitive strain,trauma,or inflammatory conditions.However,ACTS due to tophaceous gout is a clinical event that remains poorly understood and underreported.This rare manifestation necessitates prompt diagnosis and intervention to prevent irreversible complications.CASE SUMMARY A 51-year-old man who had poorly controlled hyperuricemia presented with ACTS secondary to tophaceous gout.Because of rapid symptom progression symptoms and severe median nerve compression within 3 mo,the patient underwent emergency decompression surgery for both wrists at different time points.Postoperatively,he exhibited complete recovery of sensory and motor functions,with no recurrence at long-term follow-up.Favorable outcomes were achieved through immediate decompression surgery,anti-inflammatory medications,postoperative active and passive range-of-motion exercises,and intermittent wrist splinting.Prompt diagnosis and surgical intervention,when necessary,are crucial for preventing long-term complications and obtaining favorable outcomes in patients with ACTS.An optimal gout management strategy involving pharmacologic therapy and lifestyle modifications may help minimize ACTS recurrence and improve clinical outcomes.CONCLUSION Prompt surgical intervention and optimal gout management are crucial for preventing irreversible nerve damage and ACTS recurrence.

A Multi-Objective Optimization for Locating Maintenance Stations and Operator Dispatching of Corrective Maintenance

In this study,we introduce a novel multi-objective optimization model tailored for modern manufacturing,aiming to mitigate the cost impacts of operational disruptions through optimized corrective maintenance.Central to our approach is the strategic placement of maintenance stations and the efficient allocation of personnel,addressing a crucial gap in the integration of maintenance personnel dispatching and station selection.Our model uniquely combines the spatial distribution of machinery with the expertise of operators to achieve a harmonious balance between maintenance efficiency and cost-effectiveness.The core of our methodology is the NSGA Ⅲ+Dispatch,an advanced adaptation of the Non-Dominated Sorting Genetic Algorithm Ⅲ(NSGA-Ⅲ),meticulously designed for the selection of maintenance stations and effective operator dispatching.This method integrates a comprehensive coding process,crossover operator,and mutation operator to efficiently manage multiple objectives.Rigorous empirical testing,including a detailed analysis from a Taiwan Residents electronic equipment manufacturer,validated the effectiveness of our approach across various scenarios of machine failure frequencies and operator configurations.The findings reveal that the proposed model significantly outperforms current practices by reducing response times by up to 23%in low-frequency and 28.23%in high-frequency machine failure scenarios,leading to notable improvements in efficiency and cost reduction.Additionally,it demonstrates significant improvements in oper-ational efficiency,particularly in selective high-frequency failure contexts,while ensuring substantial manpower cost savings without compromising on operational effectiveness.This research significantly advances maintenance strategies in production environments,providing the manufacturing industry with practical,optimized solutions for diverse machine malfunction situations.Furthermore,the methodologies and principles developed in this study have potential applications in various other sectors,i

代码复用攻击是控制流安全面临的主要威胁之一.虽然地址分布随机化能够缓解该攻击,但它们很容易被代码探测技术绕过.相比之下,控制流完整性方法具有更好的保护效果.但是,现有的方法要么依赖于源码分析,要么采用无差别跟踪的方式追踪所有的控制流转移.前者无法摆脱对源码的依赖性,后者则会引入巨大的运行时开销.针对上述问题,本文提出一种新的控制流保护方法MCE(Micro Code Extraction).MCE的保护目标是源码不可用的闭源对象.与现有的方法相比,MCE并不会盲目地追踪所有的控制流转移活动.它实时地检测代码探测活动,并仅将被探测的代码作为保护目标.之后,MCE抽取具有潜在风险的代码片段,以进一步缩小目标对象的大小.最后,所有跳转到风险代码中的控制流都会被追踪和检测,以保护它的合法性.实验和分析表明,MCE对代码探测和代码复用攻击具有良好的保护效果,并在一般场景下仅对CPU引入2%的开销.

Low-level laser acupuncture reduces postoperative pain and morphine consumption in older patients with total knee arthroplasty: A randomized placebo-controlled trial

Background: Patients commonly develop postoperative pain after total knee arthroplasty(TKA).Acupuncture-related techniques and low-level laser therapy could be beneficial for pain management for older individuals.Objective: To examine the effect of low-level laser acupuncture(LA) in reducing postoperative pain, painrelated interference in daily life, morphine consumption, and morphine-related side effects in older patients with knee osteoarthritis who underwent TKA.Design, setting, participants and intervention: A single-blind randomized placebo-controlled trial was conducted. Patients(N = 82) were recruited and randomly assigned via a computer-generated list to the LA group or a placebo group. The LA group received low-level laser therapy at Sanyinjiao(SP6), Taixi(KI3),Kunlun(BL60), Fengshi(GB31), Futu(ST32) and Neiguan(PC6) after TKA, while the placebo acupuncture group received the same treatment procedure without laser energy output.Main outcome measures: The primary outcome was postoperative pain intensity, and it was measured at baseline and hours 2, 6, 10, 24, 48 and 72 after TKA. The secondary outcomes, including relative pain,postoperative pain-related interference in daily life and morphine consumption, were measured at hours24, 48 and 72 after TKA.Results: Generalized estimating equations revealed significant between-group differences in pain intensity(P = 0.01), and trend differences in pain intensity for the LA group starting at hours 10 to 72(P < 0.05)and morphine consumption at hours 48 and 72(P < 0.05). The changes in pain-related interference in daily life were significant(P < 0.05) at 72 h, with the exception of the parameters for worst pain, mood,and sleep. Nausea and vomiting side effects from morphine had significant between-group differences at hours 10 and 24(P < 0.05).Conclusion: Low-level LA gradually reduced older patients’ postoperative pain intensity and morphine consumption within the first 72 h after their TKA for osteoarthritis. BACKGROUND Cystic artery pseudoaneurysm is a condition rarely encountered by clinicians;this,its etiology and presentation as well as appropriate treatments are not well studied.Although it is treated by removal of the diseased gallbladder and cystic artery,such surgery can be difficult and risky if acute inflammation with bleeding occurs,and not every patient can tolerate the surgery.CASE SUMMARY An 81-year-old man complained of epigastric pain and tarry stool passage that lasted for 3 d.He had a medical history of poor cardiopulmonary function.The computed tomographic scan of abdomen showed cystic artery pseudoaneurysm and dilatation of gallbladder.Because of high adverse outcomes related to general anesthesia,the patient was successfully managed with endovascular embolization for this cystic artery pseudoaneurysm and percutaneous drainage for the distended gallbladder.CONCLUSION A patient with cystic artery pseudoaneurysm may quickly deteriorate with the occurrence of concurrent arterial bleeding and sepsis.This report presents the case of a patient who did not undergo surgery due to multiple cardiopulmonary comorbidities and whose condition was managed successfully with embolization and biliary drainage.Endovascular embolization and biliary drainage may provide an alternative option to manage this complicated condition.

Successful management of delirium with dexmedetomidine in a patient with haloperidol-induced neuroleptic malignant syndrome:A case report

BACKGROUND We report a case of lorazepam-induced agitated delirium treated with haloperidol,which in turn triggered the onset of neuroleptic malignant syndrome(NMS).The latter condition,a medical emergency,was effectively treated with medical treatment and dexmedetomidine,a versatile and highly selective shortacting alpha-2 adrenergic agonist with sedative-hypnotic and anxiolytic effects.CASE SUMMARY A 65-year-old man with a history of bipolar disorder presented to the emergency department with severe abdominal discomfort after binge eating.During his hospital stay,he received intravenous lorazepam for insomnia.On the next day,he became delirious and was thus treated with seven doses(5 mg each)of haloperidol over a 48 h period.Signs of NMS(hyperthermia,rigidity,myoclonus of upper limbs,impaired consciousness,tachypnea,and dark urine)became apparent and haloperidol was immediately suspended and brisk diuresis was initiated.On intensive care unit admission,he was confused,disoriented,and markedly agitated.Dexmedetomidine infusion was started with the goal of achieving a Richmond Agitation-Sedation Scale score of-1 or 0.NMS was resolved gradually and the patient stabilized,permitting discontinuation of dexmedetomidine after 3 d.CONCLUSION Dexmedetomidine may be clinically helpful for the management of NMS,most likely because of its sympatholytic activity.
