Clinical characteristics,diagnosis,and treatment of COVID-19:A case report

BACKGROUND Since December 2019,many cases of pneumonia caused by novel coronavirus have been discovered in Wuhan,China,and such cases have spread nationwide quickly.At present,coronavirus disease 2019(COVID-19)is a worldwide pandemic.What are the clinical features of this disease?What is the clinical diagnosis and how should such patients be treated?As a clinician,mastery of the clinical characteristics,basic diagnosis,and treatment methods of COVID-19 are required to provide help to patients.CASE SUMMARY A 42-year-old male patient with a cough lasting 6 d without obvious cause,as well as fever and fatigue for 1 d,was admitted to Hankou Hospital on January 22,2020 and transferred to Huoshenshan Hospital on February 4.The main clinical symptoms were dry cough,fatigue,and fever.He was diagnosed with COVID-19.From the 4th d of admission,the patient’s condition gradually worsened,with increased respiratory rate and body temperature.Peripheral blood lymphocytes decreased progressively.On the 8th d of admission,the patient’s highest temperature was 40.7°C,and oxygen saturation was 83%despite high-flow oxygen inhalation.Chest computed tomography results showed that the virus progressed rapidly.The number of lesions significantly increased with expanded scope and increased density.The distribution of lesions advanced from peripheral to central.In addition to nasal catheter oxygen inhalation and symptomatic support,antiviral drugs were used throughout the treatment.On January 22,oseltamivir phosphate capsules were given orally(75 mg,twice daily)for 6 d.On January 24,three tablets of lopinavir and ritonavir were added orally(twice daily).After 6 d,this was changed to 0.2 g(two tablets)arbidol,taken orally(three times daily)for 5 d.During the severe stage,methylprednisolone was given(40 mg)once every 12 h,immunoglobulin(20 g)was administered by intravenous drip infusion once daily,and thymosin(1.6 mg)was injected subcutaneously once daily combined with immunotherapy.On February 2,symptoms decreased,various indicators improv

Background： Microwave therapy is a minimal invasive procedure and has been employed in clinical practice for the treatment of various types of cancers. However, its therapeutic application in non-small-cell lung cancer and the underlying mechanism remains to be investigated. This study aimed to investigate its effect on Lewis lung carcinoma （LLC） tumor in vivo. Methods： Fifty LLC tumor-bearing C57BL/6 mice were adopted to assess the effect of microwave radiation on the growth and apoptosis of LLC tumor in vivo. These mice were randomly assigned to 10 groups with 5 mice in each group. Five groups were treated by single pulse microwave at different doses for different time, and the other five groups were radiated by multiple-pulse treatment of a single dose. Apoptosis of cancer cells was determined by terminal deoxynucleotidyl transferase dUTP nick-end labeling assay. Western blotting was applied to detect the expression of proteins. Results： Single pulse of microwave radiation for 5 min had little effect on the mice. Only 15-min microwave radiation at 30 mW/cm2 significantly increased the mice body temperature （2.20 ± 0.82）℃ as compared with the other groups （0.78 ± 0.29 ℃, 1.24 ± 0.52 ℃, 0.78 ± 0.42 ℃, respectively）, but it did not affect the apoptosis of LLC tumor cells significantly. Continous microwave radiation exposure, single dose microwave radiation once per day for up to seven days, inhibited cell division and induced apoptosis of LLC tumor cells in a dose- and duration-dependent manner. It upregulated the protein levels of p53, Caspase 3, Bax and downregulated Bcl-2 protein. Conclusions： Multiple exposures of LLC-bearing mice to microwave radiation effectively induced tumor cell apoptosis at least partly by upregulating proapoptotic proteins and downregulating antiapoptotic proteins. Continuous radiation at low microwave intensity Ibr a short time per day is promising in treating non-small-cell lung cancer.

Understanding Discrepancy of Power System Dynamic Security Assessment with Unknown Faults: A Reliable Transfer Learning-based Method

This letter proposes a reliable transfer learning(RTL)method for pre-fault dynamic security assessment(DSA)in power systems to improve DSA performance in the presence of potentially related unknown faults.It takes individual discrepancies into consideration and can handle unknown faults with incomplete data.Extensive experiment results demonstrate high DSA accuracy and computational efficiency of the proposed RTL method.Theoretical analysis shows RTL can guarantee system performance.

Variations in the dynein axonemal heavy chain gene,dynein axonemal heavy chain 6(DNAH6),lead to multiple morphological abnormalities of the flagella.Recent studies have reported that these deficiencies may result in sperm head deformation.However,whether DNAH6 is also involved in human acrosome biogenesis remains unknown.The purpose of this study was to investigate DNAH6 gene variants and their potential functions in the formation of defective sperm heads and flagella.Whole-exome sequencing was performed on a cohort of 375 patients with asthenoteratozoospermia from the First Affiliated Hospital of Anhui Medical University(Hefei,China).Hematoxylin and eosin staining,scanning electron microscopy,and transmission electron microscopy were performed to analyze the sperm morphology and ultrastructure.Immunofluorescence staining and Western blot analysis were conducted to examine the effects of genetic variants.We identified three novel deleterious variants in DNAH6 among three unrelated families.The absence of inner dynein arms and radial spokes was observed in the sperm of patients with DNAH6 variants.Additionally,deficiencies in the acrosome,abnormal chromatin compaction,and vacuole-containing sperm heads were observed in these patients with DNAH6 variants.The decreased levels of the component proteins in these defective structures were further confirmed in sperm from patients with DNAH6 variants using Western blot.After intracytoplasmic sperm injection(ICSl)treatment,the partner of one patient with a DNAH6 variant achieved successful pregnancy.Overall,novel variants in DNAH6 genes that contribute to defects in the sperm head and flagella were identified,and the findings indicated Icsl as an effective clinical treatment for such patients.

Numerical simulation of formaldehyde distribution characteristics in the high-speed train cabin

The global concern over indoor air pollution in public vehicles has grown significantly.With a focus on enhancing passengers’comfort and health,this study endeavors to investigate the distribution characteristics of formaldehyde within a high-speed train cabin by employing a computational fluid dynamics(CFD)model which is experimentally validated in a real cabin scenario.The research focuses on analyzing the impact of air supply modes,temperature,relative humidity,and fresh air change rate on the distribution and concentration of formaldehyde.The results demonstrate that the difference in average formaldehyde concentration between the two air supply modes is below 1.3%,but the top air supply mode leads to a higher accumulation of formaldehyde near the sidewalls,while the bottom air supply mode promotes a more uniform distribution of formaldehyde.Furthermore,the temperature,relative humidity,and fresh air change rate are the primary factors affecting formaldehyde concentration levels,but they have modest effects on formaldehyde’s distribution pattern within the cabin.As the temperature and relative humidity increase,the changes in formaldehyde concentrations in response to variations in these factors become more evident.Importantly,the formaldehyde concentration may surpass the standard limit of 0.10 mg/m^(3)if the fresh air change rate falls below 212 m^(3)/h.This research provides a systematic approach and referenceable results for exploring formaldehyde pollution in high-speed train cabins.

Protective effect of cyclophilin A against Alzheimer＇s amyloid beta-peptide （25-35）-induced oxidative stress in PC12 cells

Background β-amyloid peptide （Aβ） is considered responsible for the pathogenesis of Alzheimer＇s disease （AD）. Possible mechanisms underlying Aβ-induced neuronal cytotoxicity include excessive production of reactive oxidative species （ROS） and apoptosis. Cyclophilin A （CypA）, exhibits antioxidant properties and protects neurons against oxidative stress induced injury. This study was conducted to demonstrate whether CyPA added to cultured PC12 cells could alleviate Aβ-induced oxidative stress and protect them from apoptosis.Methods PC12 cells were pre-incubated for 30 minutes with recombinant human cyclophilin A （rhCyPA） in 0.1 nmol/L, 1.0 nmol/L, 10 nmol/L and 100 nmol/L and then incubated with 10 μmol/L Aβ25-35. In every group, cell viability, apoptotic morphology, apoptotic rate, intracellular ROS accumulation, the activities of superoxide dismutase （SOD） and glutathione peroxidase （GSH-Px） of PC12 cells and mitochondrial transmembrane potential were detected. Subsequently, the expression of the active form of caspase-3 was determined by Western blotting.Results It was shown that cultures treated with 1.0 nmol/L, 1U nmol/L or 100 nmol/L rnL, rhCyPA＋Aβ25-35 had significantly higher cell viability and a lower rate of apoptosis compared with the cultures exposed only to Aβ25-35. In addition, rhCyPA attenuated Aβ25-35-induced overproduction of intracellular ROS and Aβ25.35-induced a decrease in activity of the key antioxidant enzymes SOD and GSH-Px. Furthermore, rhCyPA also attenuated Aβ25.35-induced mitochondrial dysfunction and the activation of caspase-3.Conclusion CyPA may act as an ROS scavenger, and prevent Aβ25-35-induced neurotoxicity through attenuating oxidative stress induced by Aβ25-35.

Temporally consistent video colorization with deep feature propagation and self-regularization learning

Video colorization is a challenging and highly ill-posed problem.Although recent years have witnessed remarkable progress in single image colorization,there is relatively less research effort on video colorization,and existing methods always suffer from severe flickering artifacts(temporal inconsistency)or unsatisfactory colorization.We address this problem from a new perspective,by jointly considering colorization and temporal consistency in a unified framework.Specifically,we propose a novel temporally consistent video colorization(TCVC)framework.TCVC effectively propagates frame-level deep features in a bidirectional way to enhance the temporal consistency of colorization.Furthermore,TCVC introduces a self-regularization learning(SRL)scheme to minimize the differences in predictions obtained using different time steps.SRL does not require any ground-truth color videos for training and can further improve temporal consistency.Experiments demonstrate that our method can not only provide visually pleasing colorized video,but also with clearly better temporal consistency than state-of-the-art methods.A video demo is provided at https://***/watch?v=c7dczMs-olE,while code is available at https://***/lyh-18/TCVC-Tem porally-Consistent-Video-Colorization.

Preoperatively predicting vessels encapsulating tumor clusters in hepatocellular carcinoma:Machine learning model based on contrast-enhanced computed tomography

BACKGROUND Recently,vessels encapsulating tumor clusters(VETC)was considered as a distinct pattern of tumor vascularization which can primarily facilitate the entry of the whole tumor cluster into the bloodstream in an invasion independent manner,and was regarded as an independent risk factor for poor prognosis in hepatocellular carcinoma(HCC).AIM To develop and validate a preoperative nomogram using contrast-enhanced computed tomography(CECT)to predict the presence of VETC+in HCC.METHODS We retrospectively evaluated 190 patients with pathologically confirmed HCC who underwent CECT scanning and immunochemical staining for cluster of differentiation 34 at two medical centers.Radiomics analysis was conducted on intratumoral and peritumoral regions in the portal vein phase.Radiomics features,essential for identifying VETC+HCC,were extracted and utilized to develop a radiomics model using machine learning algorithms in the training set.The model’s performance was validated on two separate test sets.Receiver operating characteristic(ROC)analysis was employed to compare the identified performance of three models in predicting the VETC status of HCC on both training and test sets.The most predictive model was then used to constructed a radiomics nomogram that integrated the independent clinical-radiological features.ROC and decision curve analysis were used to assess the performance characteristics of the clinical-radiological features,the radiomics features and the radiomics nomogram.RESULTS The study included 190 individuals from two independent centers,with the majority being male(81%)and a median age of 57 years(interquartile range:51-66).The area under the curve(AUC)for the combined radiomics features selected from the intratumoral and peritumoral areas were 0.825,0.788,and 0.680 in the training set and the two test sets.A total of 13 features were selected to construct the Rad-score.The nomogram,combining clinicalradiological and combined radiomics features could accurately predict VETC+in all three sets,with AUC
