Development and validation of novel models for the prediction of intravenous corticosteroid resistance in acute severe ulcerative colitis using logistic regression and machine learning

Background The early prediction of intravenous corticosteroid(IVCS)resistance in acute severe ulcerative colitis(ASUC)patients remains an unresolved challenge.This study aims to construct and validate a model that accurately predicts IVCS resistance.Methods A retrospective cohort was established,with consecutive inclusion of patients who met the diagnosis criteria of ASUC and received IVCS during index hospitalization in Peking Union Medical College Hospital between March 2012 and January 2020.The primary outcome was IVCS resistance.Classification models,including logistic regression and machine learning-based models,were constructed.External validation was conducted in an independent cohort from Shengjing Hospital of China Medical University.Results A total of 129 patients were included in the derivation cohort.During index hospitalization,102(79.1%)patients responded to IVCS and 27(20.9%)failed;18(14.0%)patients underwent colectomy in 3 months;6 received cyclosporin as rescue therapy,and 2 eventually escalated to colectomy;5 succeeded with infliximab as rescue therapy.The Ulcerative Colitis Endoscopic Index of Severity(UCEIS)and C-reactive protein(CRP)level at Day 3 are independent predictors of IVCS resistance.The areas under the receiver-operating characteristic curves(AUROCs)of the logistic regression,decision tree,random forest,and extreme-gradient boosting models were 0.873(95%confidence interval[CI],0.704–1.000),0.648(95%CI,0.463–0.833),0.650(95%CI,0.441–0.859),and 0.604(95%CI,0.416–0.792),respectively.The logistic regression model achieved the highest AUROC value of 0.703(95%CI,0.473–0.934)in the external validation.Conclusions In patients with ASUC,UCEIS and CRP levels at Day 3 of IVCS treatment appeared to allow the prompt prediction of likely IVCS resistance.We found no evidence of better performance of machine learning-based models in IVCS resistance prediction in ASUC.A nomogrambased on the logistic regression model might aid in the management of ASUC patients.

Waardenburg syndrome(WS)is an autosomal-dominant neurocristopathy characterized by sensorineural hearing loss(SNHL)and pigmentary abnormalities of the iris,hair,and skin.It is responsible for approximately 3%of congenital hearing loss[1].WS is thought to be fully penetrant when considering at least one clinical feature,but the penetrance of each feature is not complete[2].Four types of WS have been classifed based on the clinical manifestations.WS1 is one of the most common forms of WS.To confrm the diagnosis of WS1,at least two major or one major plus two minors of the Waardenburg consortium criteria must be present.In brief,major criteria include congenital sensorineural hearing loss,pigmentary disturbances of the iris,hair hypopigmentation,dystopia canthorum(average W index of afected family members is greater than 1.95)and afected frst-degree relatives.Minor criteria include skin hypopigmentation,synophyrys or medial eyebrow fare,broad and high nasal root,hypoplasia of alae nasi,and premature graying of hair[3].High phenotypic heterogeneity has been observed in WS1,even in the same family[4].

Cerebral ventricular infection (CVI) is one of the most dangerous complications in neurosurgery because of its high mortality and disability rates. Few studies have examined the application of neuroendoscopic surgical techniques (NESTs) to assess and treat CVI. This multicenter, retrospective study was conducted using clinical data of 32 patients with CVI who were assessed and treated by NESTs in China. The patients included 20 men and 12 women with a mean age of 42.97 years. NESTs were used to obliterate intraventricular debris and pus, fenestrate or incise the intraventricular compartment and reconstruct cerebrospinal fluid circulation, and remove artificial material. Intraventricular irrigation with antibiotic saline was applied after neuroendoscopic surgery (NES). Secondary hydrocephalus was treated by endoscopic third ventriculostomy or a ventriculoperitoneal shunt. Neuroendoscopic findings of CVI were used to classify patients into Grade Ⅰ(n = 3), Grade Ⅱ(n = 13), Grade Ⅲ(n = 10), and Grade Ⅳ(n = 6) CVI. The three patients with grade ⅠCVI underwent one NES, the 23 patients with grade Ⅱ/Ⅲ CVI underwent two NESs, and patients with grade Ⅳ CVI underwent two (n = 3) or three (n = 3) NESs. The imaging features and grades of neuroendoscopy results were positively related to the number of neurosurgical endoscopic procedures. Two patients died of multiple organ failure and the other 30 patients fully recovered. Among the 26 patients with secondary hydrocephalus, 18 received ventriculoperitoneal shunt and 8 underwent endoscopic third ventriculostomy. There were no recurrences of CVI during the 6- to 76-month follow-up after NES. Application of NESTs is an innovative method to assess and treat CVI, and its neuroendoscopic classification provides an objective, comprehensive assessment of CVI. The study trial was approved by the Institutional Review Board of beijing Shijitan Hospital, Capital Medical University, China.

二维过渡金属硫化物构成的范德瓦耳斯异质结由于存在晶格失配或层间相对旋转角度会产生周期性的莫尔条纹结构,由此引入的纳米尺度莫尔周期势可以影响激子的空间传输过程.目前有关双层过渡金属硫化物转角同质结中激子莫尔势调控的研究比较有限,本工作利用第一性原理计算研究了外加垂直电场对不同旋转角度的双层WSe_(2)同质结激子莫尔势的影响,发现层间激子莫尔势的大小和势垒/势阱的位置与层间相对旋转角度及电场强度有关,不同旋转角度的双层WSe_(2)同质结激子莫尔势大小及势垒/势阱的位置随电场强度(1 V/nm)的变化不同.这些结果为调控层间激子的局域与非局域转变提供了理论依据和数据预测,在推动人工激子晶体及纳米阵列激光器等半导体器件的发展方面具有重要指导意义.

肿瘤相关巨噬细胞(TAMs)是肿瘤微环境中的重要组成部分,在多种实体瘤中占据肿瘤体积的一半以上,具有高度的可塑性和异质性。在肿瘤发展的早期阶段,TAMs通过其吞噬和抗氧化功能介导抗肿瘤作用。然而,恶性肿瘤细胞为了满足自我更新和增殖的需要,不断调整其代谢模式,导致肿瘤微环境(TME)中的乳酸盐、活性氧、一氧化氮、花生四烯酸和前列腺素等代谢物累积增加,引起炎症微环境改变,从而改变TAMs的代谢和功能,最终促进肿瘤发展。因此,研究TME中TAMs的代谢变化对认识肿瘤的发生发展过程和临床治疗具有重要意义。该文旨在探讨肿瘤进展过程中TAMs在TME内的代谢及免疫反应变化以及如何以此作为靶标开展治疗,以期阐明TAMs代谢与TME的免疫反应之间的密切联系,揭示由TAMs的代谢产生肿瘤免疫抑制的机制,为肿瘤免疫疗法提供新的治疗思路和途径。

Four new phenolic amides,4-O-methylgrossamide(1),(E)-2-(4,5-dihydroxy-2-{3-[(4-hydrox-yphenethyl)amino]-3-oxopropyl}-phenyl)-3-(4-hydroxy-3-methoxyphenyl)-N-(4-hydroxyphenethyl)acryl-amide(2),(Z)-lyciumamide C(3),(Z)-thoreliamide B(4),together with thirteen known phenolic amides were identified from the stem of Lycium barbarum.The structures of the new compounds were determined by spectroscopic methods.All compounds were evaluated for their anti-cancer activities against human glioma stem cell lines.

Photodynamic therapy (PDT) is a clinically approved cancer treatment that uses energy of light to generate active substances that cause damage to the cancer. Photosensitizers are employed to absorb light and generate toxic reactive oxygen species (ROS) to damage biomolecules like DNA. At the same time, some chemotherapy drugs like nucleotide analogues can provide mechanism-guided promotion in the treatment efficacy of PDT. However, the photosensitizer and chemotherapy drugs used in PDT is usually organic molecules, which suffers from bad solubility, fast clearance, and acute toxicity. To achieve targeted treatment, a reasonable delivery system is necessary. Therefore, we reported a metal-phenolic network where IR780 and gemcitabine were coupled chemically to overcome these shortcomings. The enhanced PDT effects can be realized by the promoted cell death both in vitro and in vivo. Moreover, the synergistic therapy also induced T-cell mediated anti-tumor immune response, which was significant for the inhibition of distant tumor growth. This work expanded the biomedical application of metal-phenolic materials and contribute to the wider application of photodynamic cancer therapy.

过渡金属硫化物单层具有直接带隙,可产生较强的光致发光,这一特殊的性质使其在光电器件、光电探测等领域具有广泛的应用前景.由于只有原子级别的厚度以及存在激子的非辐射复合,其光致发光效率仍有待提高.本文设计了一种金膜-二氧化钛光栅-过渡金属硫化物单层组合结构,可大幅提升过渡金属硫化物单层光致发光效率.利用Purcell效应对自发辐射速率进行控制,得到峰值为3.4倍的发光增强.研究了单层二硫化钨以及单层二硒化钨在设计结构上的光致发光信号,通过实验证实了过渡金属硫化物单层与亚波长光栅耦合结构中光致发光增强的可行性,为二维材料在光电器件中的应用提供了一个新思路.
