BACKGROUND Acquired pure red cell aplasia(aPRCA)related to human parvovirus B19(HPv B19)is rarely reported in simultaneous pancreas-kidney transplantation(SPKT)recipients;there has yet to be a case report of early postoperative *** this current study,we report the case of a Chinese patient who experienced the disease in the early postoperative *** SUMMARY A 63-year-old man,with type 2 diabetes and end-stage renal disease,received a brain dead donor-derived *** treatment consisted of tacrolimus,prednisone,enteric-coated mycophenolate sodium(EC-MPS),and thymoglobulin combined with methylprednisolone as *** hemoglobin(Hb)level declined due to melena at postoperative day(POD)3,erythropoietinresistant anemia persisted,and reticulocytopenia was diagnosed at POD *** bone marrow aspirate showed decreased erythropoiesis and the presence of giant pronormoblasts at POD *** next-generation sequencing(mNGS)of a blood sample identified HPv B19 infection at POD ***-MPS was withdrawn;three cycles of intravenous immunoglobulin(IvIG)infusion therapy were administered;and tacrolimus was switched to *** HPv B19-associated aPRCA resolved completely and did not relapse within the 1-year follow-up *** diminution in mNGS reads was correlated with Hb and reticulocyte count *** HPv B19-associated aPRCA can occur at an early period after *** effective therapy regimen includes IvIG infusion and adjustment of the immunosuppressive ***,mNGS can be used for the diagnosis and to reflect disease progression.

Acoustically actuated magnetoelectric(ME)antenna based on the efficient oscillation of magnetic dipoles has recently been considered as a promising solution for portable very-low-frequency ***,the severe nonlinear dynamic behavior in the case of strong-field excitation results in insufficient radiation capability and poor communication performance for a conventional ME *** this work,we propose to suppress the nonlinearity of an ME antenna by neutralizing the spring-hardening effect in amorphous Metglas and the spring-softening effect in piezoelectric ceramics through an ME multilayered transmitter(ME-MLTx)*** a driving voltage of 50 v_(pp)at the resonance frequency of 21.2 kHz,a magnetic flux density as high as 108 fT at a distance of 100 m is produced from a single *** addition,ME-MLTx performs a decreased mechanical quality factor(Q_(m))less than 40.65,and,thus,a broadened bandwidth of 500 Hz is ***,a communication link transmitting binary American Standard Code for Information Interchange-coded message is built,which allows for an error-free communication with a distance of 18 m and a data rate of 300 bit/s in the presence of heavy environment *** communication distance can be further estimated over 100 m when using a femtotesla-class-inductive magnetic field *** obtained results are believed to bring ME antennas one step closer to being applicable in very-low-frequency communications.

The study on the mechanism of Tanreqing and ambroxol combined with Azithromycin for the treatment of mycoplasma pneumonia in children

Objective:To observe the mechanism of Tanreqing and ambroxol combined with Azithromy for the treatment of mycoplasma pneumonia in children and offer help to mycoplasma pneumonia ***:86 cases of mycoplasma pneumonia patients in our hospital were selected and randomly divided into observation group and control group, each group were 43 cases. Control group was treated with conventional therapy, and observation group was treated with Tanreqing and ambroxol combined with Azithromy based on conventional therapy, the changes of lung function [v-T (Tidal volume)/kg, t-PTEF/t-E (time ratio of peak to peak), TEF25/PTEF (Instantaneous velocity of exhaling tidal volume 75% and Peak tidal expiratory flow ratio) and MTIF/MTEF (medium-term inspiratory flow rate and medium-term expiratory flow rate ratio)], cytokines (IL-2, IL-10, IL-6 and TNF-α) and the myocardial enzymes [LDH (lactate dehydrogenase), CK-MB (creatine kinase isoenzyme), CK (creatine kinase) and AST (glutamic-oxaloacetic transaminase)] were detected before and after ***: The comparison of lung function, cytokines and themyocardial enzymes in the two groups before treatment was not statistically significant (P>0.05). In this work we have studied the effect of synaptic time delay on synchronization in small-world networks of bursting neurons coupled with electrical synapses. The obtained results, based on extensive simulations of conductance-based type of neurons in an oscillatory regime, showed that transitions of the spatiotemporal synchrony of neurons can be induced by the variations of the information transmission delay. It has been observed that as the delay increases regions of synchronization and nonsynchronization appear alternately. Furthermore, the rewiring probability of small-world topology is also observed to be another important factor in the generation of coherent spiking. In this context, we also found that a suitable rewired network topology can enhance the synchronized neuronal activity with the appropriate delay.

BACKGROUND Arterial cannulation sites for the surgical repair of type A aortic dissection(AAD)have evolved from right axillary artery(AA)cannulation to bilateral carotid artery(CA)based of femoral artery(FA)*** descending aorta remodeling is closely linked to the false lumen area ratio(FLAR),defined as false lumen area/aortic area,as well as to the incidence of renal replacement therapy(RRT).AIM To investigate the effect of the updated arterial cannulation strategy on descending aortic *** A total of 443 AAD patients who received FA combined cannulation between March 2015 and March 2023 were included in the *** these,209 received right AA cannulation and 234 received bilateral CA *** primary outcome was the change in FLAR,as calculated from computed tomography angiography in three segments of the descending aorta:Thoracic(S1),upper abdominal(S2),and lower abdominal(S3).Secondary outcomes were the incidence of RRT and the serum inflammation response,as observed by the levels of high sensitivity C reaction protein(hs-CRP)and Interleukin-6(IL-6).RESULTS The postoperative/preoperative ratio of FLAR in S2 and S3 was higher in the AA group compared to the CA group(S2:0.80±0.08 vs 0.75±0.07,Pvs 0.50±0.12,Pvely).The AA group also had a significantly higher incidence of RRT(19.1%vs 8.5%,P=0.001;odds ratio:2.533,95%CI:1.427-4.493)and higher levels of inflammation cytokines 24 h after the procedure[hr-CRP:117±17 vs 104±15 mg/L;IL-6:129(103,166)vs 83(69,101)pg/mL;both Pved by a greater change in FLAR and lower incidence of RRT.

Antibiotic stewardship programmes in intensive care units:Why,how,and where are they leading us

Antibiotic usage and increasing antimicrobial resistance(AMR) mount significant challenges to patient safety and management of the critically ill on intensive care units(ICU). Antibiotic stewardship programmes(ASPs) aim to optimise appropriate antibiotic treatment whilst minimising antibiotic resistance. Different models of ASP in intensive care setting, include "standard" control of antibiotic prescribing such as "de-escalation strategies"through to interventional approaches utilising biomarkerguided antibiotic prescribing. A systematic review of outcomes related studies for ASPs in an ICU setting was conducted. Forty three studies were identified from MEDLINE between 1996 and 2014. Of 34 nonprotocolised studies, [1 randomised control trial(RCT), 22 observational and 11 case series], 29(85%) were positive with respect to one or more outcome: These were the key outcome of reduced antibiotic use, or ICU length of stay, antibiotic resistance, or prescribing cost burden. Limitations of non-standard antibiotic initiation triggers, patient and antibiotic selection bias or baseline demographic variance were identified. All 9 protocolised studies were RCTs, of which 8 were procalcitonin(PCT) guided antibiotic stop/start interventions. Five studies addressed antibiotic escalation, 3 de-escalation and 1 addressed both. Six studies reported positive outcomes for reduced antibiotic use, ICU length of stay or antibiotic resistance. PCT based ASPs are effective as antibiotic-stop(de-escalation) triggers, but not as an escalation trigger alone. PCT has also been effective in reducing antibiotic usage without worsening morbidity or mortality in ventilator associated pulmonary infection. No study has demonstrated survival benefit of ASP. Ongoing challenges to infectious disease management, reported by the World Health Organisation global report 2014, are high AMR to newer antibiotics, and regional knowledge gaps in AMR surveillance.
