We report a rare case of a small-bowel lymphangioma causing massive gastrointestinal (GI) bleeding that we successfully diagnosed and treated using double-balloon enteroscopy (DBE). An 81-year-old woman suffering from repeated GI bleeding of unknown origin underwent a capsule endoscopy at a previous hospital. She was suspected of having bleeding from the jejunum, and was referred to our department for diagnosis and treatment. An oral DBE revealed a 20 mm × 10 mm, regularly surfaced, white to yellowish, elongated, pedunculated jejunal polyp with small erosions at 10 cm distal to the ligament of Treiz. Since no other source of bleeding was identified by endoscopy in the deep jejunum, anendoscopic polypectomy (EP) was performed for this lesion. A subsequent histopathological examination of the resected polyp showed clusters of lymphatic vessels with marked cystic dilatation in the submucosa and the deep layer of the lamina propria mucosae. These characteristics are consistent with the typical features of small-bowel lymphangioma with erosions. Although clip-ping hemostasis was performed during EP, re-bleeding occurred. Finally, a complete hemostasis was achieved by performing an additional argon plasma coagulation.

In endoscopic placement of multiple plastic biliary stents (PBSs),we sometimes experience proximal dislocation of the first PBS at the time of subsequent PBS *** describe the case of a 79-year-old male with obstructive jaundice caused by cholangiocarcinoma who needed to receive multiple PBS placements for management of *** proximal dislocation of the first PBS was observed,we prevented the dislocation via our technique of using guidewire inserted from the distal end of the first PBS to the side hole as the *** could complete this technique only by inserting guidewire through the side hole of the first PBS during the process of releasing the first PBS and pulling out the guidewire and the inner *** did not matter whether the anchor-wire went towards the third portion of the duodenum or the duodenal *** we introduce this "anchor-wire technique",which is useful for the prevention of PBS proximal dislocation in placing multiple PBSs.

Large Eddy Simulation of Pseudo-Shock Waves Using Wall Model

A supersonic turbulent flowfield involving the pseudo-shock waves in an isolator of a supersonic combustion ramjet is computed using two different LES codes which are a high-order upwind finite volume scheme, and a sixth order compact differencing scheme utilizing the localized artificial diffusivity method for stabilizing shock waves and employing a wall model to enable the use of coarse mesh. In the validation study where a supersonic turbulent boundary layer flow over a flat plate is examined, both LES codes are well validated using velocity profile in the boundary layer given by the hot-wire anemometry and normal stress given by the laser Doppler anemometry. In particular, the sixth order compact differencing scheme gives closer agreements with these experimental data. Then, the validated LES codes are applied to solve the Mach number 2.5 supersonic turbulent flowfield involving the pseudo-shock waves. It is shown that typical features of unsteady flowfield of the pseudo-shock waves are well obtained by both schemes. Again, it is indicated that the sixth order compact differencing scheme gives closer agreements with the existing velocity data obtained by particle image velocimetry and pressure fluctuation data on the wall surface. Besides, the computational cost of the compact differencing scheme is found to be 1/7 of that for the upwind finite volume scheme, even though a wall model is solved at each grid point on the wall surface. Therefore, the obtained results in the present study allow recommending the sixth order compact differencing scheme with a wall model for simulating supersonic turbulent flowfield in an isolator involving the pseudo-shock waves.

Clinical analysis of high serum IgE in autoimmune pancreatitis

AIM: To clarify the clinical significance of high serum IgE in autoimmune pancreatitis (AIP). METHODS: Forty-two AIP patients, whose IgE was measured before steroid treatment, were analyzed. To evaluate the relationship between IgE levels and the disease activity of AIP, we examined (1) Frequency of high IgE (> 170 IU/mL) and concomitant allergic dis-eases requiring treatment; (2) Correlations between IgG, IgG4, and IgE; (3) Relationship between the presence of extrapancreatic lesions and IgE; (4) Re-lationship between clinical relapse and IgE in patients treated with steroids, and (5) Transition of IgE before and after steroid treatment. RESULTS: IgE was elevated in 36/42 (86%) *** allergic disease was observed in seven patients (allergic rhinitis in three, bronchial asthma in three, and urticaria in one). There were no signifi-cant correlations between IgG, IgG4, and IgE (r = -0.168 for IgG, and r = -0.188 for IgG4). There was no significant difference in IgE in the patients with and without extrapancreatic lesions (526 ± 531 IU/mL vs 819 ± 768 IU/mL, P = 0.163), with and without clini-cal relapse (457 ± 346 IU/mL vs 784 ± 786 IU/mL, P = 0.374). There was no significant difference in IgE between before and after steroid treatment (723 ± 744 IU/mL vs 673 ± 660 IU/mL, P = 0.633). CONCLUSION: Although IgE does not necessarily reflect the disease activity, IgE might be useful for the diagnosis of AIP in an inactive stage.

The cell wall in wood is mainly composed of three components: cellulose, hemicellulose and lignin. According to electron microscopy observations of the innermost surface of cell walls in the tracheids of Cryptomeria japonica , cellulose microfibrils are deposited during the day and a matrix containing hemicellulose is deposited at night. This indicates that the deposition of cell wall components exhibits diurnal periodicity. To gain new insights into the diurnal periodicity of lignin deposition not revealed by microscopic observations, we examined diurnal fluctuations in the expression of genes involved in monolignol biosynthesis in C. japonica saplings grown in the field and in growth chambers under a 12 h light/dark cycle. In the field experiment, two gene expression peaks were observed daily, at dusk and dawn. In the growth chamber experiment, two daily peaks were observed 0 h and 6 - 9 h after the light switched on.

AIMTo assess the effectiveness of Daikenchuto for patients with postoperative adhesive small bowel obstruction （ASBO）.METHODSA systematic search of PubMed （MEDLINE）, CINAHL, the Cochrane Library and Ichushi Web was conducted, and the reference lists of review articles were hand-searched. The outcomes of interest were the incidence rate of surgery, the length of hospital days and mortality. The quality of the included studies, publication bias and between-study heterogeneity were also *** Three randomized controlled trials （RCTs） and three retrospective cohort studies were selected for analysis. In the three RCTs, Daikenchuto signifcantly reduced the incidence of surgery （pOR = 0.13; 95%CI： 0.03-0.50）. Similarly, Daikenchuto signifcantly reduced the incidence of surgery （pOR = 0.53; 95%CI： 0.32-0.87） in the three cohort studies. The length of hospital stay and mortality were not measured or described *** present meta-analysis demonstrates that admini-stering Daikenchuto is associated with a lower incidence of surgery for patients with postoperative ASBO in the Japanese population. In order to better generalize these results, additional studies will be needed.

Fermented Black Turmeric Designed by Lactobacillus Rearranged Leukocyte Subsets and Anti-Oxidative Activity

A plant fermentation was carried out by Yeast and Lactobaccilli against fermented black turmeric, Kaempferia parviflora (FBT). These materials were proved by as safe in animal safety experiment. We tried to investigate changes of immune-competent cells that commonly utilized FBT, including after administration of immno-suppressed animals, the effects by FBT on the regulated effect on the cells were evaluated. Our results showed that FBT augmented the level of lymphocytes in number, while FBT regulated the level of granulocytes in both number and function. In our clinical study with 20 healthy volunteers, granulocyte and lymphocyte ratio suggesting their constitution as neutral in peripheral blood were increased significantly 30 days after the administration of FBT in rodents, and compromised host was prepared with cancer chemotherapeutic agent (Mytomycin-C). Our observations showed against intracellular parasite, and that FBT augmented intercellular pathogen through humoral immunity. We discussed the significance and mechanism of FBT on the level of leukocyte subsets in number and function that were considered to be potential indicators for the activation of the compromised host. We also proposed an idea that FBT exhibited tonic effects via activating complement components. The evidences were shown by immune-electrophoretic method. Moreover, we tried to access further to the anti-oxidative activities of this FBT. This modification brought to the significant lift up for antibody producing cells and anti-oxidative activity for phagocytic cells.

Characteristics of Absorption Equilibrium with HFC-134a and an Ionic Liquid Pair

Cold energy generation systems must be improved to prevent catastrophic climate change. In this study, we focused on an absorption chiller cycle with HFC-134a and an ionic liquid pair as the refrigerant and absorbent, respectively. It was expected that this absorption chiller cycle could generate cold heat below 0°C. Two liquids were selected and their absorption equilibrium with this pair was evaluated for the absorption chiller cycle. We measured the adsorbed amount at equilibrium with 1-butyl-3-methylimidazolium bis(trifluoromethanesulfonyl)imide [BMIM][Tf2N] and N-trimethyl-N-butylammonium bis(trifluoromethanesulfonyl)imide [N1113][Tf2N]. The experimental results were reproduced using the non-random two liquid (NRTL) model. This analysis model corresponded well in terms of the amount of adsorption at equilibrium with the experimental results. A Duhring diagram was also generated the NRTL model, and the absorption cycle characteristics as a function of temperature were determined. The absorption chiller cycle obtained cold heat at 10°C with a regeneration temperature of 70°C in addition to generating cold heat below 0°C.
