[Background] Medical staff have many occasions to be exposed to anticancer drugs (ACDs) in the process of performing cancer chemotherapy. Although guidelines for the safe handling of ACDs have been published, actual measures against exposure to ACDs differ between hospitals, and no surveillance data regarding the conditions of each hospital in relation to performing chemotherapy in Japan are available. [Methods] To understand current measures against exposure to ACDs and problems related to cancer chemotherapy at outpatient departments, we conducted a questionnaire survey on doctors, pharmacists, and nurses in 10 leading hospitals for cancer chemotherapy in Japan. [Results] Responses were received from all 10 institutions. The hospital pharmacists prepared all ACDs for outpatients in each institution. All hospitals took basic measures against ACD exposure, such as using personal protective equipment and penetration-resistant waste containers. Conditions against exposure to ACDs generally improved between 2012 and 2015, especially in terms of conveyance of ACDs and use of a closed-system drug transfer device. However, no measures linking ACDs with administration routes or injection sites were commonly taken in any of the hospitals in 2015. [Conclusions] Conditions against ACD exposure improved between 2012 and 2015, possibly because new guidelines were issued. To improve measures for ACD exposure in Japan, preparing more appropriate manuals and offering more opportunities to medical staff for continuing education are considered important.

Acute pancreatitis is one of the most common diseases in gastrointestinal,liver and pancreatic diseases,with about 300,000 annual admissions in the United States(1).Acute pancreatitis is complicated by pancreatic necrosis in 20%,and the mortality rate is 20%to 30%once infection occurs in pancreatic necrosis(2).Severe acute pancreatitis(SAP)was purely a surgical disease in 19th century due to its poor prognosis(3),but the importance of multidisciplinary approach is increasingly recognized as shown in the recent publication of French guidelines(4)by the collaboration of multisociety of surgery,gastroenterology,digestive endoscopy,interventional radiology,intensive care and nutrition.

Many traffic accidents occur in parking *** of the serious safety risks is vehicle-pedestrian ***,with the increasing development of automatic driving and parking technology,parking safety has received significant attention from vehicle safety ***,pedestrian protection in parking lots still faces many *** example,the physical structure of a parking lot may be complex,and dead corners would occur when the vehicle density is *** lead to pedestrians’sudden appearance in the vehicle’s path from an unexpected position,resulting in collision accidents in the parking *** advocate that besides vehicular sensing data,high-precision digital map of the parking lot,pedestrians’smart device’s sensing data,and attribute information of pedestrians can be used to detect the position of pedestrians in the parking ***,this subject has not been studied and explored in existing *** this void,this paper proposes a pedestrian tracking framework integrating multiple information sources to provide pedestrian position and status information for vehicles and protect pedestrians in parking *** also evaluate the proposed method through real-world *** experimental results show that the proposed framework has its advantage in pedestrian attribute information extraction and positioning *** can also be used for pedestrian tracking in parking spaces.

Estimation of the Radial Distribution of the Tangential Velocity in a Vortex Chamber

The estimation of maximum tangential velocity becomes a very important factor for the estimation of performancesof the vortex chamber. In this paper, a proposed flow model of how to estimate the maximum tangential velocity inthe special form of the vortex chamber is described in detail. The pressure drop basing upon the rapid expansion byflowing from the inlet pipe into the cyclone body is estimated as half of the dynamic pressure in the inlet pipe.

Improvement of a temperature response function for divertor heat flux monitoring in fusion devices

Temperature response functions have been developed to investigate sensor design and divertor heat flux estimation in magnetically confined plasmas. The time-dependent heat flux can be derived by fitting the response function to experimental thermocouple(TC) data. Because the TC signals have a time delay to transit events such as discharge start or confinement transition, the time delay is taken into account in a temperature response function. Such a function accurately describes the signal from each TC channel with time delay in a sensor test using a neutral beam injection. Measurement for commercial TCs shows that the time delay is caused by the finite heat capacity of TC wire and contact heat resistance between TC and target surface.

Surface Emitting THz Wave Parametrical Oscillator Using MgO：LiNbO3

Room-temperature operation terahertz （THz） wave source is demonstrated using three MgO：LiNbO3 crystals which have a noncollinear arrangement. The experimental results show that the THz wave can be tunable from 0.8 THz to 3.0 THz, and the peak energy output is 103 pJ/pulse at 1.5 THz. The noncoilinear cavity configuration makes the THz beam have Gaussian-like spatial distribution, small divergence angle, perpendicularly eradiated from the crystal surface. The beam quality factor M2 is measured to be Mx^2 = 1.15, Mx^2 = 1.25 for characterizing the THz wave beam. Experiments also show that the THz beam can be focused by using a polyethylene lens, and the focal spot size is close to the diffraction limit.

Efficacy of acupuncture during post-acute phase of rehabilitation after total knee arthroplasty

OBJECTIVE:The purpose of this study was to determine whether acupuncture is effective in reducing pain and swelling around the knee and improving range of motion(ROM) during the post-acute phase of rehabilitation after total knee arthroplasty(TKA).METHODS:Following TKA,80 knees in 80 patients were randomly assigned to either an acupuncture treatment group(Group A) or a control group(Group C).In Group A,the complementary treatment of acupuncture was performed three times/week from postoperative day 7 until postoperative day *** measures were:i) pain as assessed by a visual analog scale;ii) reduction of swelling around the knee as indicated by its circumference at the center of the patella;and iii) ROM of the affected ***:Group A patients had significantly reduced pain and swelling around the knees and earlier recovery of ROM than did those in Group ***:Acupuncture provides effective treatment during the post-acute phase of rehabilitation after TKA with respect to pain relief,reduction of swelling around the knee,and early recovery of ROM.

Balloon overtube-guided colorectal endoscopic submucosal dissection

AIM:To evaluate the usefulness of a balloon overtube to assist colorectal endoscopic submucosal dissection(ESD)using a ***:The results of 45 consecutive patients who underwent colorectal ESD were analyzed in a single tertiary endoscopy *** preoperative evaluation of access to the lesion,difficulties were experienced in the positioning and stabilization of a gastroscope in 15 patients who were thus assigned to the balloonguided ESD group.A balloon overtube was placed with a gastroscope to provide an endoscopic channel to the lesion in cases with preoperatively identified difficulties related to *** ESD was performed following standard procedures.A submucosal fluid bleb was created with hyaluronic acid solution.A circumferential mucosal incision was made to marginate the *** isolated lesion was finally excised from the deeper layers with repetitive electrosurgical dissections with needle *** success of colorectal ESD,procedural feasibility,and procedure-related complications were the main outcomes and ***:The overall en bloc excision rate of colorectal ESD during this study at our institution was 95.6%.En bloc excision of the lesion was successfully achieved in 13 of the 15 patients(86.7%)in the balloon overtube-guided colorectal ESD group,which was comparable to the results of the standard ESD group with better accessibility to the lesion(30/30,100%,not statistically significant).CONCLUSION:Use of a balloon overtube can improve access to the lesion and facilitate scope manipulation for colorectal ESD.

Clinical benefit of radiation therapy and metallic stenting for unresectable hilar cholangiocarcinoma

AIM:To determine the efficacy of external beam radiotherapy(EBRT),with or without intraluminal brachytherapy(ILBT),in patients with non-resected locally advanced hilar ***:We analyzed 64 patients with locally advanced hilar cholangiocarcinoma,including 25 who underwent resection(17 curative and 8 non-curative),28 treated with radiotherapy,and 11 who received best supportive care(BSC).The radiotherapy group received EBRT(50 Gy,30 fractions),with 11 receiving an additional 24 Gy(4 fractions) ILBT by iridium-192 with remote after *** was performed using percutaneous transhepatic biliary drainage(PTBD) *** metallic stents(UMS) were inserted into nonresected patients with obstructive jaundice,with the exception of four patients who received percutaneous transhepatic biliary drainage *** were placed endoscopically or percutaneously,depending on the initial drainage *** primary endpoints were patient death or stent *** time of patients in the radiotherapy group was compared with that of patients in the resection and BSC *** patency was compared in the radiotherapy and BSC ***:No statistically significant differences in patient characteristics were found among the resection,radiotherapy,and BSC *** patients in the radiotherapy group and one in the BSC group did not receive UMS insertion but received PTBD alone;cholangitis occurred after endoscopic stenting,and patients were treated with PTBD.A total of 16 patients were administered additional systemic chemotherapy(5-fluorouracil-based regimen in 9,S-1 in 6,and gemcitabine in 1).Overall survival varied significantly among groups,with median survival times of 48.7 mo in the surgery group,22.1 mo in the radiotherapy group,and 5.7 mo in the BSC *** who underwent curative resection survived significantly longer than those who were not candidates for surgery(P = 0.0076).Cumulative survival in the radiotherapy group wa
