Introduction: Since the times of our ancestors, traditional medicine has existed to cure all diseases [1]. The objective of this study was to assess the complications of traditional fracture treatment. Patients and Methods: This was a prospective study extending from January 01, 2018 to June 30, 2018, duration of six months. It concerned patients who had traditionally been treated beforehand for traumas with bone lesion in a traditional therapist and who had developed a complication, the definitive management of which was made in the department. Results: This study involved 102 patients, including 71 men (69.6%), with a sex ratio of 2.2. 33 patients or 32.4% were under 15 years old. The average of our patients’ age was 29.61 years with extremes of 2 and 89 years. Pupils/students (44 pupils and 5 students) were the most represented with 49 cases or 48.1% of the cases. The majority of patients (59.8% of cases) were educated or had a basic level. The majority of patients (36.3%) came as motif for a painful swelling. Gangrene was the most common complication with 37 cases or 36.3% of cases. Conclusion: At the end of our study, we have noticed that the traditional preliminary treatment of fractures was based mainly on trial and error. Practicing with inadequate means and measures without respecting the anatomical structures, is a real source of disabling handicaps. We have recorded 38, among the 102 patients who came for a complication of traditional treatment: that is 45.2% amputation, which is deplorable.

Intrauterine Device in the Immediate Postpartum:Study Comparing Insertion after Cesarean Section and Vaginal Delivery

Objective To compare the safety, side effects and failure rates of contraception copper intrauterine device (IUDs) in the immediate postpartum period after a vaginal delivery and during a cesarean section. Patients and methods This was a descriptive and analytical study, conducted in Gynecology and Obstetrics departments hospital Pikine and Abass Ndao hospitals. It compared two groups of women with IUDs in the immediate postpartum period after a vaginal delivery and during cesarean section. Our sample consisted of 215 patients. We had divided them into two groups: group 1 consisted of 115 patients who delivered by cesarean section;Group 2 consisted of 100 patients who delivered vaginally and whom the insertion was made in post placental or within 48 hours after birth. The variables studied were the sociodémographic characteristics, characteristics of pregnancy and childbirth, insertion procedures, follow up with the assessment of side effects and complications. Results and comments The socio-demographic characteristics were superimposed in both groups. Nine patients were lost to follow up, 4 in group 1 and 5 in group 2. The complaints such as pain (3.7% for Group 1 against 2.6% for group 2), vaginal bleeding (2.1% for group 2, and 1.8% for group 1) were rarely reported and stackable in both groups. After 6 months of follow up, the menses was observed in 42.8% of group 2 against 43.1% of group 1. The abnormal menstrual flow, evaluated by the Higham score, was noted in 29.6% of group 2 and 25% of group 1, and was dominated by excessive bleeding. They motivated IUD removal in two patients. A significant difference was found in the perception of IUD strings, it was effective in 95.4% of patients in group 2 against 50.5% for group 1. The evictions were 9 at 3-month follow-up 7 related group 2 and 2 for Group 1. It was partial (isthmic IUD) and total expulsion. Study of Factor Associated with Post Stroke Depression in the Teaching Hospital of Fann (Dakar-Senegal)

Introduction: Strokes are becoming more common in Africa, they are often accompanied by depressive symptoms, hence the need to investigate the factors associated with it for better management and prevention. Methodology: We conducted a prospective cross-sectional study amongst patients hospitalized with stroke at the Neurology/Neuroscience Clinic of the FANN UNHC (Dakar) in 2016. After a clinical diagnosis, patients are subjected to socio-demographic questionnaire and were administered with modified Rankin and MADRS scores. Results: Among the patients, 33.7% had Post-Stroke Depression, most were aged 50 to 70 years, male and married. History of stroke and depression was strongly associated with Post-Stroke Depression, while functional or cognitive impairment caused by stroke contributed to the development of Post Stroke depression. The type of ischemic or haemorrhagic stroke did not appear to affect the occurrence of the Post-Stroke Depression, but Post-Stroke Depression was more frequent in subjects with a left hemispheric lesion 47.8% (p Discussion: The occurrence of a Post-Stroke Depression is a factor of poor prognosis, early diagnosis and holistic care lead to improved quality of life.

目的MMP-9是与宫颈癌的浸润与转移生物学行为关系最为密切的MMP之一。本研究试图检测MMP-9蛋白在宫颈癌从LSIL、HSIL和原位癌,然后到鳞状上皮癌这一动态发展过程中的表达变化情况,来了解MMP-9蛋白在宫颈癌发生和发展中所起的作用。方法选择从2003年3月1日至2007年9月1日,在中南大学湘雅二医院妇科就诊的慢性宫颈炎20例,LSIL(CINⅠ)20例,HSIL(CINⅡ—Ⅲ)20例,宫颈鳞癌70例,用免疫组织化学SP方法检测组织标本中MMP-9蛋白的表达。结果(1)MMP-9在宫颈鳞癌中的阳性率和强阳性率明显高于在慢性宫颈炎、LSIL和HSIL中(P＜0.05);在HSIL中,MMP-9的阳性率明显高于在慢性宫颈炎和LSIL中(P＜0.05);LSIL中MMP-9的阳性率明显高于在慢性宫颈炎中(P＜0.05)。(2)分析了MMP-9表达与宫颈鳞癌患者年龄、FIGO临床分期和肿瘤直径的关系。≤35岁的宫颈鳞癌患者中MMP-9蛋白的阳性表达,与＞35岁的患者中相比,表达的差异无统计学意义(P＞0.05)。MMP-9蛋白表达在不同FIGO临床分期的宫颈癌患者中表达有区别:ⅠA2期患者中表达明显低于ⅠB1～ⅡB期和Ⅲ～Ⅳ期两组患者中(P＜0.05):ⅠB1～ⅡB期和Ⅲ～Ⅳ期两患者组之间,MMP-9蛋白的表达无统计学差异(P＞0.05)。MMP-9蛋白在肿瘤直径≤4cm组患者中表达,与＞4cm组相比,差异无统计学意义(P＞0.05)。(3)分析了MMP-9表达与宫颈鳞癌细胞分化及术后病检的关系。高分化的鳞癌细胞MMP-9表达明显低于低分化细胞(P＜0.05),中分化癌细胞与高分化癌细胞之间的MMP-9表达无统计学差异(P＞0.05)。MMP-9表达与术后病检的关系发现:不论肿瘤细胞是否入侵脉管,肿瘤侵入宫颈纤维肌壁是否超过1/2,以及是否出现淋巴肿瘤转移,以上3种情况对宫颈鳞癌的MMP-9的阳性表达无统计学影响(P＞0.05)。结论(1)从宫颈上皮受损到细胞癌变,受损、癌变的宫颈细胞MMP-9合成和分泌增加。MMP-9作为蛋白水解酶在子宫颈癌的形成中有重要的作用。(2)MMP-9与宫颈鳞癌的侵袭行为关系密切,可能成为判断宫颈鳞癌预后指标之一。

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease deadly. In Africa, little data exists on this condition and the form familial is the most common with a younger age of onset. Our observations relate to three brothers, born from a consanguineous marriage. The diagnosis of amyotrophic lateral sclerosis was certain placed in all patients according to the criteria of Awaji. The average age of patients was 16.66 years, and the average age of onset of symptoms was 10.33 years old. The female gender was the most represented with a sex ratio of 0.5. Our patients benefited from rilusole, vitamin D and physiotherapy. A only patient presented with dysphagia. However, no cases have been observed at this day.

Flood events occurrences and frequencies in the world are of immense worry for the stability of the economy and life safety. Africa continent is the third continent the most negatively affected by the flood events after Asia and Europe. Eastern Africa is the most hit in Africa. However, Africa continent is at the early stage in term of flood forecasting models development and implementation. Very few hydrological models for flood forecasting are available and implemented in Africa for the flood mitigation. And for the majority of the cases, they need to be improved because of the time evolution. Flash flood in Bamako (Mali) has been putting both human life and the economy in jeopardy. Studying this phenomenon, as to propose applicable solutions for its alleviation in Bamako is a great concern. Therefore, it is of upmost importance to know the existing scientific works related to this situation in Mali and elsewhere. The main aim was to point out the various solutions implemented by various local and international institutions, in order to fight against the flood events. Two types of methods are used for the flood events adaptation: the structural and non-structural methods. The structural methods are essentially based on the implementation of the structures like the dams, dykes, levees, etc. The problem of these methods is that they may reduce the volume of water that will inundate the area but are not efficient for the prediction of the coming floods and cannot alert the population with any lead time in advance. The non-structural methods are the one allowing to perform the prediction with acceptable lead time. They used the hydrological rainfall-runoff models and are the widely methods used for the flood adaptation. This review is more accentuated on the various types non-structural methods and their application in African countries in general and West African countries in particular with their strengths and weaknesses. A good vascular condition is fundamental for kidney transplantation. A bad arterial or venous supply may compromise graft survival. Discovery in operating theater of vascular anomalies not diagnosed by medical imaging may overwhelm operating protocol. Our cases emphasize the issue of pre operating evaluation. The cases are those of two women, aged 48 and 25 years, with chronic renal insufficiency, for whom living donor kidney transplantation was decided. During the process, a total obstruction of iliac vein was found and led to a change of technique. The dilated ovarian vein was used for the venous anastomosis while the arterial anastomosis was as usually made using the iliac artery. Post-surgical follow up was uneventful. These cases emphasize on the mandatory pre operative evaluation and the respect of guidelines in the process of kidney transplantation. They also open access to other operating strategies. The objective of this publication was to present our experience in dealing with an obstructed iliac vein and emphasize on the necessity to assess accurately vascular state in kidney transplantation.

Effect of Polyethylene Terephthalate Plastic Waste on the Physico-Mechanical and Thermal Characteristics of Stabilized Laterite Bricks

The present work investigated the effect of polyethylene terephthalate (PET) plastic waste on the physico-mechanical and thermal properties of cement-stabilized laterite bricks to see the durability of the modified bricks (CSLB). Samples were formulated by mixing laterite, cement, and different percentages of PET (0%, 3%, 5%, and 7%) by volume. The bricks were produced using the M7MI Hydraform standard interlocking block and kept in the shade for a curing period of 28 days. The addition of 3% to 5% PET to the laterite stabilized with 10% cement results in a decrease in both dry and wet compressive strength, which is determined using the Controlab compression machine. However, the obtained results are in concordance with the standards. The thermal conductivity of CSLB, determined using the box method with the EI700 measurement cell, decreases as the PET content of the mixture increases. A decrease in bulk density from 1.67 to 1.58 g/cm3 was observed.

Physicochemical and Bacteriological Quality of Ground Water in the Basin of Gounti Yena Valley in Niamey City (Niger Republic)

The Gounti Yéna valley, the subject of this study, is a tributary of the Niger River, it is the main watercourse that divides the left bank of the city of Niamey into two unequal parts. The surface area of its watershed is estimated at about 62 Km2. The objective of the present study is on the one hand to evaluate the current state of the physico-chemical and bacteriological quality of the water of Gounti Yéna basin, and on the other hand to define the risks of water pollution and its origin in the face of the phenomenon of rising water table of this basin. In order to carry out this work, we proceeded to a study of the evolution of the physicochemical and bacteriological parameters of the water of the basin of Gounti Yéna during the period going from November 2020 to October 2021, at the level of four points of sampling chosen from the upstream to the downstream of the basin. The results obtained showed that the physico-chemical quality of the water in Gounti Yéna basin is influenced by the lithology of the watershed, climatic factors (rainfall and temperature) and anthropic actions. This study also highlighted the presence of fecal contamination in the water of the Gounti Yéna basin, with a greater degree of emphasis during the rainy season. This contamination remains a concern and may constitute a health risk.

Assessment of the Management of Post-Tonsillectomy Pain at the ENT-CFS Department of Ignace Deen National Hospital

Introduction: Tonsillectomy is described as one of the oldest surgical procedures according to the authors of the surgical treatise (Aulus Amida and Paul of Aegina in the 6th and 7th centuries) found in the Vatican library. The contraindication of codeine in children has changed the management of post tonsillectomy pain. The aim of this study was to assess the management of post tonsillectomy pain in our developing country context. Methods: This was a prospective, analytical study lasting 6 months (September 2019-February 2020), carried out in the ENT/CFS department of the Ignace Deen National Hospital. We included in this study all patients who underwent a tonsillectomy in the department during the study period and who agreed to participate in the survey. Results: 34 patients were included in our study, i.e. a frequency of 25% of surgical interventions. The mean age of our patients was 18.06 ± 12 years with extremes of 03 years and 45 years. Recurrent hypertrophic tonsillitis with sleep disturbances was the most frequent indication for surgery in our study, at 47.06% (n = 16). We performed an isolated tonsillectomy in 52.9% (n = 18) of cases. Analgesia was multimodal using the WHO Step I and II analgesics in 44.1% (n = 15) and 55.9% (n = 19), respectively. Pain control was satisfactory in all patients. Conclusion: The evaluation of post tonsillectomy pain is essential for optimal management. In this indication, multimodal analgesia, involving several levels I and II analgesics, provided satisfactory pain control.
