Effectiveness of Enriched Flour in the Recovery of Children Aged 6 to 59 Months with Moderate Acute Malnutrition

Introduction: In Chad, malnutrition is a silent emergency. Yet the use of local products in the fight against malnutrition is essential. The purpose of this study was to compare the efficacy of locally sourced enriched flour in the recovery of children 6 to 59 months of moderate acute malnourishment compared to Ready-to-Use Therapeutic Foods (Plumpy’ Sup) in the city of Abéché-Chad. Methodology: This study took place from 1 October 2021 to 31 January 2022 in the city of Abéché, capital of the province of Ouaddaï. It was conducted in eight (8) urban health centers in the city of Abéché and concerned eighty (80) children with moderate acute malnutrition. It was based on a consumption assessment of two complementary foods by two groups of children. The assessment of nutritional status was made through anthropometric data such as weight, height and brachial perimeter. Results: All 80 children were tested for moderate acute malnutrition. Two complementary foods have been used which are locally produced flour and the Ready-to-Use Therapeutic Food. The study showed that the group 1 who consumed the locally produced flour had an average weight gain of 1100 g and the group 2 who consumed the Plumpy sup had an average weight gain of 400 g. Conclusion: The present study carried out in Abéché allowed us to compare the effectiveness of locally produced weaning flours with Ready-to-Use Therapeutic Foods on moderate malnourished child weight growth in the nutritional units of the town of Abéché.

Introduction: Meningiomas are benign tumors most localized in brain. Intraspinal meningiomas are rare and may have an intra or extradural topography. We report the case of a dorsal psammomatous meningioma revealed by sciatica. Case description: We present a 58-year-old female hospitalized for bilateral painful and disabling L5 sciatica rebellious to medical treatment and physiotherapy. At physical examination, she presented a lumbar spinal syndrome without root deficit. MRI (Magnetic Resonance Imaging) showed a borderline lumbar canal with a paramedian L4-L5 herniated disc. She undergone a first surgery a L4 decompressive laminectomy and there was no postoperative complications. The follow-up was marked by partial pain regression. Three months later, after a fall she presented low back pain and progressive worsening gait troubles. A MRI was realized and shown L3-L4, L4-L5 duct narrowing. Growth and fatty acid composition of Nile tilapia <i>Oreochromis niloticus</i>L. fed Azolla-diets, in tanks and in earthen ponds: A comparative study

Two 90-day experiments were conducted in the same period in tanks and in earthen ponds to study the effects of culture systems on growth and fatty acid (FA) of Nile tilapia (Oreochromis niloticus L.) fingerlings (mean weight: 16.3 g) fed Azolla, as partial replacement for fish meal (FM). Diets were isonitrogenous (29.2% CP) and isoenergetic (16.9 kJ·g-1) containing 0% (A0), 10% (A10), 20% (A20), 30% (A30), 40% (A40) and 50% (A50) of Azolla meal (AM) respectively. Diet A0, without AM, acted as a control. In both experiments, fish growth decreased as AM level increased in diets (P P 30-A50). Interestingly, those fish were rich in eicosapentaenoic acid (EPA, 20:5n-3) and docosahexaenoic acid (DHA, 22:6n-3). Consequently, the n-3/n-6 values decreased in tanks, whereas an increasing trend was found in ponds, and values were significantly higher in ponds than in tanks (0.35 - 0.53 vs. 0.08 - 0.17). The results suggest raising Nile tilapia with Azolladiets in ponds to improve growth and produce fish with a better FA composition, for the wellbeing of consumers.

New Geological and Hydrogeological Information on the Eastern Border of the Gondo Plain （Burkina Faso）

The Gondo plain lies between Mali and Burkina Faso and it is in interland basin into the West African Craton. Since 2003, this study has been carried out two research projects on the southwest part of the plain, where the piezometric levels are very low （50 m to 100 m）. It has two main purposes： find water resources to provide drinking water supply to Ouahigouya town and assess the exploitation possibility of the deep aquifer for rural water supply. Combined methods were used to reach the objectives of this study--remote sensing, geology, geophysics （electromagnetism and resistivity methods）, hydrochemistry and isotope chemistry. So, the methodology allows to specify the geology and the tectonic of the eastern border of the basin, identify and characterize the different aquifers and their relationships. Through the implementation of 250 m deep drilled boreholes, this study reveals that the water level of the lower Cambrian limestones can be under pressure below the continental terminal deposits. The study highlights paleo-karstic zones in the Gondo plain and shows that water chemistry and isotope chemistry can be used to differentiate water sheets and evaluate their recharge.

Some Processing Steps and Uses of Cashew Apples: A Review

In addition to the cashew nut, which is the main product, the cashew tree also produces the cashew apple which is considered a by-product. The cashew apple has a high nutritional potential. Indeed, it is rich in vitamin C, carotenoids, dietary fibers, vitamins, sugars and mineral elements where are essential for human nutrition. In addition to its nutritional quality, the cashew apple has technological advantages: the edible part of the fruit is between 85% and 100% higher than that of other traditional tropical fruits, and its juicy and sweet flesh is free of seeds or pits. In addition, very large volumes are available. As a result, the development of this fruit represents a considerable economic challenge. This paper first presents the cultivation of cashew trees and the bibliography of the work done on cashew juice. The favorable conditions for cashew tree cultivation and the planting method were presented. Then, the study highlights the work done on the physicochemical characteristics of cashew apples, the effect of the growing area, the variety and the stage of maturity on its characteristics. It also shows the influence of the processing steps on the nutritional value and organoleptic quality of the cashew apple;as well as the methods of clarification, stabilization, concentration and dehydration. Some uses of cashew apple were reviewed: beverage, food, substrate, bioethanol, nutraceutical, food additive and agro materials.

Bilateral Chronic Subdural Hematoma after Endoscopic Third Ventriculostomy in a Child: A Case Report and Review of the Literature

Hydrocephalus had been managed by ventriculoperitoneal shunt (VPS) or endoscopic third ventriculostomy (ETV) since several years. But these two different technics had some complications that must be managed promptly to avoid eventual fatal evolution. Chronic subdural hematomas after ETV is among these complications and is a very rarely situation observed in our department. This rare event associated with malaria in a child is considered to have a high mortality. Here we report a rare case of bilateral chronic subdural hematoma occurring in a 4-month-old boy after ETV and we discuss the likely pathogenesis and the difficulties of management.

Objectives: To study the clinical and therapeutic profiles of voluminous goiter. Patients and Methods: We carried out a descriptive and prospective study, relating to a series of 30 cases of voluminous goitre, collected in the Department of Otorhinolaryngology and cervicofacial surgery (ENT and CCF) of the CHU Luxembourg Mother Child of Bamako. It has been spread over a period of 4 years from January 2015 to December 2018. Patients of all ages operated for large goitre at the ENT Department of CHU Luxembourg Mother Enfant were included. Results: In 4 years we collected 30 cases of voluminous goitre;during this period we realized 180 thyroidectomies, i.e. frequency of 16.67%. The average age was 51.37 years with an extreme ranging from 38 to 65 years. Females were common in 66.7% with a sex ratio of 0.50. The long duration of evolution has been 40 years. The sign of compression was found in 85.7%. The physical examination found a mobile swelling, hard and painless in all patients with normal endolaryngeal examination;there was no cervical lymphadenopathy. The lower dipping pole was found in 5 cases on CT. All our patients were euthyroid. The classification of TIRADS 2 was found in 80.0% of cases. Total thyroidectomy was frequent with 50.0% of cases. The average weight of the operative specimen was 586.67 g with extremes ranging from 500 g to 800 g. The size of the operative piece of 14 cm was the longest. Injury of internal jugular vein was found in 26.7% of cases. Colloid adenoma of the thyroid was found in 100% of cases, postoperative complications of the type of hematoma of the lodge in 3.3% of cases, the release of the operative wound in 10% of cases local superinfection in 7.1%. Signs of hypothyroidism were common with 50.0%. Postoperative nasofibroscopy found good vocal fold mobility in all patients. Groundwater Flow Modeling by Using the Permeability Induced by Satellite Lineaments in Discontinuous Aquifers and Semi-Arid Context: A Case Study of the Liptako Region (South-West of Niger)

The study area, located in the Liptako basement of Niger, faces a serious problem of drinking water supply in recent decades. This is linked to exponential population growth and reduced rainfall. The purpose of this study is to map fracture networks from Landsat 7 satellite imagery to identify major subterranean flow corridors in the area. The methodological approach based on the collection of data (Landsat 7 images, DEM/SRTM, flow, transmissivity, static level) and the geological and hydrogeological field reconnaissance and the processing of these data with the following software (ArcGis, Envi, Surfer and RStudio) The results of this study yielded that the number of satellite lineaments is 995, the preferred directions of these satellite lineament are: N0°-10° (13%) and N90°-100° (12%). On the hydrographic network, in situ, the most frequent fracture, vein and dike directions are: N90°-100° (12%), N130°-150° (11%), and N50°-60° (10%). The structural domains identified probably correspond to the main corridors and reservoirs of underground flows. The values of the induced permeability are between 0.05 × 10-8 and 3.4 × 10-7 m/s. The permeability induced by satellite lineaments, which is strongly related to high fracturing densities and major water stream is involved in the hydrodynamic functioning of discontinuous aquifers in the study area.

Phenotype and Epidemiological Profile of Children with Sickle Cell Disease Followed-Up at the Mother and Child Hospital of N’Djamena

In Africa, sickle cell disease is still a public health issue. In Chad, a high prevalence area, it may represent a silent disease. To study the quality of care provided to children with sickle cell disease within the framework of a hospital initiative launched in 2011 and in the absence of a national program in Chad, we conducted this research in the pediatric sickle cell disease center of the Mother and Child Hospital of N’Djamena. This was a cross-sectional study, involving 364 children with sickle cell disease out of 12,500 children followed up from May 2011 to December 2014 (3.9%). The sex ratio was 1.3. The average age at diagnosis was 17 months, often in the context of vaso-occlusive crisis (34.6%). The follow-up protocol included monthly appointments following diagnosis of disease. We observed 80% non-compliance among cases;characteristics associated with non-compliance included families with several children, inaccessible anti-pneumococcal prophylaxis based on Penicillin V and very low anti-pneumococcal vaccine coverage (4.4%). These results suggest that there is a need to rapidly implement a national sickle cell disease program including preventive and curative care in Chad.

Management of Spinal Injuries Secondary to Rockfall in Traditional Mines

Introduction: Spinal injuries represent the whole of the mechanisms responsible for vertebral, disco-ligamentary and spinal cord injuries. Spinal cord injury is life-threatening and is responsible for functional consequences that make the subsequent socio-economic integration of victims difficult. In developing countries, spinal injuries in traditional gold mining are becoming more and more frequent. Purpose: The purpose of this study was to describe the anatomopathological aspects and the results of surgical treatment of spinal injuries secondary to traditional mine collapses at the University Hospital of Kati. Patients and Methods: This was a prospective, descriptive study over 18 months, from November 2017 to May 2019. Included in this study were patients operated on for spinal trauma secondary to rockfall in traditional mines. The neurological status was assessed in each patient since admission to the department using the Fränkel score. Decompression by laminectomy combined with titanium device placement was preferred in lumbar, thoracic and thoraco-lumbar surgery. We performed an anterior cervical spine approach with SENEGAS plate placement in cervical injuries. Motor physiotherapy was associated whenever recovery was not complete. At a minimum follow-up of 6 months, all our patients were re-evaluated by the Fränkel score. Results: The mean age was 31 years with extremes of 10 and 49 years. The majority of patients were men, 95%. Twenty patients underwent surgery. According to the spinal segments, the cervical spine was affected in 05 cases, the thoracic spine in 08 cases and the lumbar spine in 17 cases. The neurological examination revealed tetraplegia (n = 5), paraplegia (n = 12) and paraparesis (n = 3). According to the types of lesions, we collected 12 cases of fractures, 4 cases of fracture-luxations and 04 cases of pure luxations.
