Carvedilol Versus Other Nonselective Beta Blockers for Variceal Bleeding Prophylaxis and Death: A Network Meta-analysis

Background and Aims:We aimed to perform a network meta-analysis(NWM)to examine comparative effectiveness of non-selective beta blockers(NSBBs)on prophylaxis of gastroesophageal variceal bleeding(gVB)and mortality ***:MEDLINE(OVID)and EMBASE databases were searched for eligible randomized clinical trials(RCTs)from inception to July 3,*** of interest included primary/secondary prophylaxis of gVB,failure to achieve hepatic venous pressure gradient(HVPg)decremental response,liver-related and all-cause mortality.A Bayesian NWM was performed to derive relative risk(RR)with 95%credible intervals(CrIs).The ranking probability of each NSBB was assessed by surface under cumulative ranking curve(SUCRA).Results:Thirty-three RCTs including 3,188 cirrhosis patients with gastroesophageal varices were *** with placebo,nadolol ranked first for reducing variceal bleeding[RR:0.25,(95%CrI:0.11–0.51);SUCRA:0.898],followed by carvedilol[RR:0.33,(95%CrI:0.11–0.88);SUCRA:0.692]and propranolol[RR:0.52,(95%CrI:0.37–0.75);SUCRA:0.405].Carvedilol was more effective than propranolol in achieving HVPg decremental response[RR:0.43,(95%CrI:0.26–0.69)].Carvedilol ranked first for reducing all-cause mortality[RR:0.32,(95%CrI:0.17–0.57);SUCRA:0.963],followed by nadolol[RR:0.48,(95%CI:0.29–0.77);SUCRA:0.688],and propranolol[RR:0.77,(95%CI:0.58–1.02);SUCRA:0.337].Similar findings were observed for liver-related *** ranked the *** RR of adverse events was 4.38,(95%CrI:0.33–161.4);SUCRA:0.530,followed by propranolol[RR:7.54,(95%CrI:1.90–47.89);SUCRA:0.360],and nadolol[RR:18.24,(95%CrI:91.51–390.90);SUCRA:0.158].Conclusions:Carvedilol is the preferred NSBB with better survival benefit and lower occurrence of adverse events among patients with gastroesophageal varices.

Update in management of hepatocellular carcinoma in Eastern population

Hepatocellular carcinoma(HCC) is one of the commonest malignant tumours in the East. Although the management of HCC in the West is mainly based on the Barcelona Clinic for Liver Cancer staging, it is considered too conservative by Asian countries where the number of HCC patients is huge. Scientific and clinical advances were made in aspects of diagnosis, staging, and treatment of HCC. HCC is well known to be associated with cirrhosis and the treatment of HCC must take into account the presence and stage of chronic liver disease. The major treatment modalities of HCC include:(1) surgical resection;(2) liver transplantation;(3) local ablation therapy;(4) transarterial locoregional treatment; and(5) systemic treatment. Among these, resection, liver transplantation and ablation therapy for small HCC are considered as curative treatment. Portal vein embolisation and the associating liver partition with portal vein ligation for staged hepatectomy may reduce dropout in patients with marginally resectable disease but the midterm and long-term results are still to be confirmed. Patient selection for the best treatment modality is the key to success of treatment of HCC. The purpose of current review is to provide a description of the current advances in diagnosis, staging, preoperative liver function assessment and treatment options for patients with HCC in the east.

This paper presents a new approach to identifying the climate variables that influence the size of the area burned by forest *** linear regression was used in combination with nonlinear variable transformations to determine relevant nonlinear forest wildfire size *** from the Prague-East District of the Czech Republic was used for model *** burned forest area was hypothesized as a function of water vapor pressure,air temperature and wind *** speed was added to enhance predictions of the size of forest wildfires,and further improvements to the utility of prediction methods were added to the regression *** results show that if the air temperature increases,it may contain less water and the fuel will become *** size of the burned area then *** the relative humidity in the air increases and the wind speed decreases,the size of the burned area is *** model suggests that changes in the climate factors caused by ongoing climate change could cause significant changes in the size of wildfire in forests.

Mechanism of metastasis by membrane type 1-matrix metalloproteinase in hepatocellular carcinoma

AIM To investigate the precise role of membrane type 1-matrix metalloproteinase （NTI-NNP） in hepatocellular carcinoma （HCC） metastasis. METHODS- Human HCC cells Hep3B with overexpression of MTT-MMP were established by stable transfection, and compared with control cells carrying the empty vector. Cells were examined in vivo for their differences in the metastatic ability of athymic nude mice, and analyzed in vito for their differences in invasion ability by invasion chamber coated with Matrigel, adhesion towards collagen I and migration through culture chamber. Cell proliferation and apoptosis in adherent and suspension status were evaluated by MTr and flow cytometry analysis. RESULTS We found that overexpression of MT1-MMP could increase intrahepatic metastasis in nude mice with orthotopic implantation of HCC cells （incidence of 100% [MT1-MMP transfectants] vs 40% [vector control transfectants], P〈0.05）. NT1-MMP could also enhance cell invasion through Natrigel （107.7 vs 39.3 cells/field, P〈0.001）, adhesion towards matrix （0.30 vs 0.12 absorbance unit at 540 nm, P〈0.001）, cell migration （89.3 vs 39.0 cells/field, P〈0.001）, and cell proliferation （24.3 vs 40.5 h/doubling, P〈0.001）. We also observed that NTI-NNP supported cell survival （71.4% vs 23.9%, P〈0.001） with reduced apoptosis （43.7% vs51.0%, P〈0.05） in an attachment-free environment. CONCLUSION： MT1-MMP overexpression could enhance metastasis. In addition to its active role in matrix degradation during tumor invasion, MT1-MMP enhances tumor cell survival upon challenge of detachment, which is important during metastasis when cells enter the circulation.

Application of the Engineering-geological Conditions in Land-Use Plans in the Petrvald Region(Czech Republic)

The objective of this paper is to demonstrate necessity to inform relevant parties about engineering-geological conditions for various practical purposes, especially including appropriate land-use planning. However, the relationship between relevant geological information and the geological environment is vital for foundation engineering purposes, especially where demanding structures are involved. This information is most conveniently structured when accumulated information concerning engineering-geological zones is utilized. This necessarily includes knowledge of rock workability and also of the pre-Quaternary bedrock, and these characteristics were then related to the current built-up area and future development according to the land-use plans in a case study are from the Petrvald Region （Czech Republic）. The geological environment of area has been severely influenced by anthropogenic effects of deep black coal mining. Results of this research showed that future development should be founded on spoil banks, dumps, and settling basins. According to the land-use plan, this zone occupies 44.9% of the area of interest, and its materials predominantly emanate from mining in the Ostrava-karvina Coal District. For future foundation structures planned there, it is imperative to consult detailed engineering-geological study. However, attention to and reliance on this necessity is not reflected in the existing land-use plan.

Optimal Offering Strategy of Virtual Power Plant with Hybrid Renewable Ocean Energy Portfolio

This paper proposes a hybrid ocean energy sys-tem to form a virtual power plant(VPP)for participating in electricity markets in order to promote the renewable ocean energy utilization and *** the proposed system,solar thermal energy is integrated with the closed-cycle ocean thermal energy conversion(OTEC)to boost the temperature differences between the surface and deep seawater for efficiency and flexibility improvements,and the thermodynamic effects of seawater mass flow rates on the output of solar-boosted OTEC(SOTEC)are exploited for deploying SOTEC as a renewable dispatchable *** optimal tidal-storage operation model is also developed to make use of subsea pumped storage(SPS)with hydrostatic pressures at ocean depths for mitigating the intermittent tidal range energy in order to make the arbitrage in the electricity ***,a two-stage coordinated scheduling strategy is presented to optimally control seawater mass flow rates of SOTEC and hydraulic reversible pump-turbines of SPS for enhancing the daily VPP *** studies have been investigated to confirm the superiority of the developed methodology in various renewable ocean energy and electricity market price scenarios.
