Clinical implications of neutrophil-to-lymphocyte ratio and MDSC kinetics in gastric cancer patients treated with ramucirumab plus paclitaxel

Objective: We aimed to investigate the prognostic value of neutrophil-to-lymphocyte ratio(NLR) and myeloidderived suppressor cells(MDSCs) in gastric cancer patients treated with second-line ramucirumab plus paclitaxel.Methods: A total of 116 patients with advanced or metastatic gastric cancer who receive ramucirumab plus paclitaxel were prospectively enrolled. Fresh blood samples were collected before and after treatment, and flow cytometry was performed to assess the proportions of monocytic(m MDSCs) and granulocytic MDSCs(g MDSCs).Results: Median age was 58 years and 71(61.2%) patients were male. A baseline NLR≥2.94 was associated with significantly poorer progression-free survival(PFS) and overall survival(OS) vs. an NLRnalysis, an NLR≥2.94 was independently associated with poorer PFS[hazard ratio(HR)=1.58;95% confidence interval(95% CI): 1.01-2.49, P=0.046] and OS(HR=1.77;95% CI:1.04-3.04, P=0.036). While m MDSC counts did not significantly change following two cycles of therapy(P=0.530),g MDSC counts decreased significantly after two treatment cycles(P=0.025) but tended to increase in patients with progressive disease after two treatment cycles(P=0.098). A progressive increase in g MDSC counts(≥44%) was associated with a significantly shorter PFS and OS vs. a g MDSC count increase <44%(P=0.001 and P=0.003,respectively).Conclusions: The baseline NLR may help guide clinical decisions during ramucirumab plus paclitaxel therapy for gastric cancer. Our g MDSC kinetics data warrant further clinical validation and mechanistic investigation.

Clinical significance of MET gene amplification in metastatic or locally advanced gastric cancer treated with first-line fluoropyrimidine and platinum combination chemotherapy

Objective: To investigate the clinical significance of MET gene amplification in patients with gastric cancer in the palliative setting.Methods: MET amplification was assessed using fluorescence in situ hybridization(FISH) in 50 patients and quantitative polymerase chain reaction(qPCR) in 326 patients;259 patients treated with first-line fluoropyrimidine and platinum were included for survival analysis.Results: The results of FISH and qPCR indicated that the c-MET/CEP7 ratio was correlated with gene copy number. The optimal cutoff value for the copy number using qPCR to detect MET gene amplification with FISH was 5(κ=0.778, P5 detected by qPCR. MET-amplified gastric cancer was associated with an Eastern Cooperative Oncology Group(ECOG) performance status(PS) score of ≥2(33.3% vs. 10.5% P=0.007), peritoneal metastasis(76.2% vs. 46.2%, P=0.008), and elevated bilirubin levels(28.6% vs. 7.3%, P=0.006). The median overall survival(OS) and progression-free survival(PFS) were 11.9 and 5.6 months, respectively. MET-amplified gastric cancer was not associated with survival outcomes [hazard ratio(HR)=0.68, 95% confidence interval(95% CI): 0.35-1.32,P=0.254 for PFS;HR=0.68, 95% CI: 0.35-1.32, P=0.251 for OS].Conclusions: qPCR can be used to detect MET gene amplification. MET amplification was not a predictor of poor prognosis in patients with metastatic or unresectable gastric cancer.

Piezoelectricity offers an electromechanical coupling that is widely utilized in transducer applications.There has been a consistent demand for transparent piezoelectric materials for optoelectrical applications.Therefore,despite the inherent tradeoff between the transparency and the piezoelectricity,numerous strategies have been explored to develop the transparent piezoelectric materials.Nonetheless,the most transparent piezoelectric materials developed to date is either a single crystal or materials that achieve transparency via hot-press sintering,limiting its industrial applicability.Therefore,we introduce a novel piezoelectric material that ensures transparency through co-doping and pressureless sintering of polycrystalline ceramics.In this study,we employed a compositional optimization approach to enhance the synergistic effect between the transparency and the piezoelectric properties of 0.71Pb(Mg_(1/3)Nb_(2/3))O_(3)–0.29PbTiO_(3)(PMN–0.29PT)ceramics.By utilizing the tape casting process for mass production and large-area manufacturing,our Pb_(0.913)La_(0.0145)Sm_(0.0145)(Mg_(1/3)Nb_(2/3))_(0.71)Ti_(0.29)O_(3)(TP2.9)ceramics exhibited over 60%transparency and large piezoelectric coefficient(d33)of 1104 pC/N.This material holds considerable promise for a wide range of industrial applications in both the optical and electronic domains.

Herein,the impact of the independent control of processing additives on vertical phase separation in sequentially deposited(SD)organic photovoltaics(OPVs)and its subsequent effects on charge carrier kinetics at the electron donor-acceptor interface are investigated.The film morphology exhibits notable variations,significantly depending on the layer to which 1,8-diodooctane(DIO)was applied.Grazing incidence wide-angle X-ray scattering analysis reveals distinctly separated donor/acceptor phases and vertical crystallinity details in SD films.Time-of-flight secondary ion mass spectrometry analysis is employed to obtain component distributions in diverse vertical phase structures of sD films depending on additive control.In addition,nanosecond transient absorption spectroscopy shows that DIO control significantly affects the dynamics of separated charges in SD films.In SD OPVs,DIO appears to act through distinct mechanisms with minimal restriction,depending on the applied layer.This study emphasizes the significance of morphological optimization in improving device performance and underscores the impor-tance of independent additive control in the advancement of oPV technology.

Anthropogenic methane emissions are a leading cause of the increase in global averagetemperatures,often referred to as global warming.Flooded soils play a significant role in methaneproduction,where the anaerobic conditions promote the production of methane by methanogenicmicroorganisms.Rice fields contribute a considerable portion of agricultural methane emissions,as riceplants provide both factors that enhance and limit methane production.Rice plants harbor both methaneproducingand methane-oxidizing microorganisms.Exudates from rice roots provide source for methaneproduction,while oxygen delivered from the root aerenchyma enhances methane oxidation.Studies haveshown that the diversity of these microorganisms depends on rice cultivars with some genes characterizedas harboring specific groups of microorganisms related to methane emissions.However,there is still aneed for research to determine the balance between methane production and oxidation,as rice plantspossess the ability to regulate net methane production.Various agronomical practices,such as fertilizerand water management,have been employed to mitigate methane emissions.Nevertheless,studiescorrelating agronomic and chemical management of methane with productivity are limited.Moreover,evidences for breeding low-methane-emitting rice varieties are scattered largely due to the absence ofcoordinated breeding programs.Research has indicated that phenotypic characteristics,such as rootbiomass,shoot architecture,and aerenchyma,are highly correlated with methane emissions.This reviewdiscusses available studies that involve the correlation between plant characteristics and methaneemissions.It emphasizes the necessity and importance of breeding low-methane-emitting rice varieties inaddition to existing agronomic,biological,and chemical practices.The review also delves into the idealphenotypic and physiological characteristics of low-methane-emitting rice and potential breeding techniques,drawing from studies conducted with diverse varieties,mutants,and transgenic p

Experience of primary intestinal lymphangiectasia in adults: Twelve case series from a tertiary referral hospital

BACKGROUND While primary intestinal lymphangiectasia(PIL)is considered a rare condition,there have been several reported cases in adults.Nevertheless,the absence of clear guidance from diagnosis to treatment and prognosis poses challenges for both physicians and patients.AIM To enhance understanding by investigating clinical presentation,diagnosis,treatment,complications,and prognoses in adult PIL cases.METHODS We enrolled adult patients diagnosed with PIL between March 2016 and September 2021.The primary outcome involved examining the diagnosis and treatment process of these patients.The secondary outcomes included identifying complications(infections,thromboembolism)and assessing prognoses(frequency of hospitalization and mortality)during the follow-up period.RESULTS Among the 12 included patients,peripheral edema(100%)and diarrhea(75%)were the main presenting complaints.Laboratory tests showed that all the pati-ents exhibited symptoms of hypoalbuminemia and hypogammaglobulinemia.Radiologically,the predominant findings were edema of the small intestine(67%)and ascites(58%).The typical endoscopic finding with a snowflake appearance was observed in 75%of patients.Among the 12 patients,two responded positive-ly to octreotide and sirolimus,and eight who could undergo maintenance therapy discontinued subsequently.Complications due to PIL led to infection in half of the patients,thromboembolism in three patients,and one death.CONCLUSION PIL can be diagnosed in adults across various age groups,with different severity and treatment responses among patients,leading to diverse complications and prognoses.Consequently,tailored treatments will be necessary.We anticipate that our findings will contribute to the management of PIL,an etiology of protein-losing enteropathy.

Partial Linear Model Averaging Prediction for Longitudinal Data

Prediction plays an important role in data analysis.Model averaging method generally provides better prediction than using any of its components.Even though model averaging has been extensively investigated under independent errors,few authors have considered model averaging for semiparametric models with correlated errors.In this paper,the authors offer an optimal model averaging method to improve the prediction in partially linear model for longitudinal data.The model averaging weights are obtained by minimizing criterion,which is an unbiased estimator of the expected in-sample squared error loss plus a constant.Asymptotic properties,including asymptotic optimality and consistency of averaging weights,are established under two scenarios:(i)All candidate models are misspecified;(ii)Correct models are available in the candidate set.Simulation studies and an empirical example show that the promise of the proposed procedure over other competitive methods.

A Study on How Urban Rail Operators Can Secure New Growth Engines for the Future Approach: The Case of Underground Urban Logistics System

Seoul Metro is Seoul’s leading metro company, transporting up to 3 billion people annually. However, future ridership is expected to plummet due to an aging and shrinking population with one of the fastest declining total fertility rates in the world. The COVID-19 pandemic in 2019 accelerated this phenomenon. On the contrary, the e-commerce and home delivery industries have developed significantly since COVID-19. Seoul’s current logistics infrastructure cannot handle it. Under the inflection point of declining passenger transportation demand and increasing urban logistics demand, urban rail operators need new growth engines. Therefore, it is necessary to consider the introduction of an UULS (underground urban logistics system) that transports parcels instead of passengers through urban railways. If the UULS becomes a reality, it can be expected to secure scarce logistics land in cities, protect the environment and prevent traffic congestion by operating eco-friendly mass urban transportation, and acquire new revenue sources for urban rail operators. The UULS’s B/C ratio is 1.32. The sensitivity analysis shows that the ratio is above 1 in most cases where the unit cost of transportation is not significantly reduced.

Zircon U-Pb ages and O-Hf isotopes of Quaternary trachytes from the East Sea:Implications for the genesis of low-δ^(18)O magmas

Quaternary intraplate magmatism formed several volcanic islands and seamounts,including Dokdo(DD),Ulleungdo(UD),Simheungtack(ST),Anyongbok,and Isabu in the southwest of the East Sea back-arc basin.In this study,we present whole-rock geochemical,zircon U–Pb age,and in situ O–Hf isotope data for the submerged volcanic rocks from DD,UD,and ST to provide new insights into the eruption timing of these volcanoes and constrain the magma evolution processes.All samples used in this study were trachytes and exhibited ferroan,alkalic,and metaluminous to weakly peraluminous characteristics.They showed light rare earth element(REE)-enriched patterns with(La/Yb)N ratios of 25.3–31.7 and mostly negative Eu anomalies in a chondrite-normalized REE plot.In addition,they were enriched in large-ion lithophile elements and high field strength elements;they exhibited positive Pb anomalies and strongly negative Ba,Sr,P,and Ti anomalies.The zircons yielded a weighted-mean 206Pb/238U age of 2.61,0.348–0.704,and 2.76–2.94 Ma for the DD,UD,and ST trachytes,respectively.All zircons exhibited lowerδ^(18)O values than normal depleted mantle values,regardless of the crystallization age and spatial distribution of volcanoes.Theδ^(18)O values showed no correlation with U contents or Th/U ratios,indicating that the lowδ^(18)O signatures were of primary magmatic origin.The Hf isotopic compositions of the zircons were relatively heterogeneous but predominately characterized by positive eHf values.Binary O–Hf mixing modeling revealed that low-δ^(18)O rocks with positive eHf values from the UD and ST volcanoes were derived from a hybrid source of recycled juvenile crustal materials with low-δ^(18)O and positive eHf signatures and an enriched mantle source with normalδ^(18)O and negative eHf values.The juvenile oceanic crust in the source was likely metasomatized by seawater at high temperatures prior to melting.In contrast,the felsic magma that formed the DD volcanoes may have assimilated with regional basement rocks(Triassic–Jurassic granitoids),re

Microstructure design of advanced magnesium-air battery anodes

Metal-air battery is an environmental friendly energy storage system with unique open structure.Magnesium(Mg)and its alloys have been extensively attempted as anodes for air batteries due to high theoretical energy density,low cost,and recyclability.However,the study on Mg-air battery(MAB)is still at the laboratory level currently,mainly owing to the low anodic efficiency caused by the poor corrosion resistance.In order to reduce corrosion losses and achieve optimal utilization efficiency of Mg anode,the design strategies are reviewed from microstructure perspectives.Firstly,the corrosion behaviors have been discussed,especially the negative difference effect derived by hydrogen evolution.Special attention is given to the effect of anode micro-structures on the MAB,which includes grain size,grain orientation,second phases,crystal structure,twins,and dislocations.For further improvement,the discharge performance,long period stacking ordered phase and its enhancing effect are considered.Meanwhile,given the current debates over Mg dendrites,the potential risk,the impact on discharge,and the elimination strategies are discussed.Microstructure control and single crystal would be promising ways for MAB anode.
