Porocarcinoma in a palm reconstructed with a full thickness skin graft: A case report

BACKGROUND Porocarcinoma is a rare type of skin cancer that originates from sweat gland tumors.It is an aggressive malignant skin cancer that is difficult to diagnose clinically owing to its rarity and similarity to squamous cell carcinoma(SCC).CASE SUMMARY This case involved a 92-year-old woman,a farmer by profession,presented with an exophytic and verrucous mass on her left palm that had formed 2 years prior and caused chronic pain and frequent bleeding.Initially,the patient was diagnosed with SCC using a punch biopsy;however,a repeat biopsy with addi-tional immunohistochemical tests was performed for porocarcinoma.Ultimately,the patient was diagnosed with porocarcinoma and reconstruction was planned using a full-thickness skin graft.After treatment,the range of motion of the palm was preserved,and the aesthetic outcome was favorable.At 6 mo of follow-up,the patient was satisfied with the outcome.CONCLUSION Porocarcinoma is commonly misdiagnosed as SCC;therefore,clinicians should consider porocarcinomas when evaluating mass-like lesions on the hands.

Piezoelectricity offers an electromechanical coupling that is widely utilized in transducer applications.There has been a consistent demand for transparent piezoelectric materials for optoelectrical applications.Therefore,despite the inherent tradeoff between the transparency and the piezoelectricity,numerous strategies have been explored to develop the transparent piezoelectric materials.Nonetheless,the most transparent piezoelectric materials developed to date is either a single crystal or materials that achieve transparency via hot-press sintering,limiting its industrial applicability.Therefore,we introduce a novel piezoelectric material that ensures transparency through co-doping and pressureless sintering of polycrystalline ceramics.In this study,we employed a compositional optimization approach to enhance the synergistic effect between the transparency and the piezoelectric properties of 0.71Pb(Mg_(1/3)Nb_(2/3))O_(3)–0.29PbTiO_(3)(PMN–0.29PT)ceramics.By utilizing the tape casting process for mass production and large-area manufacturing,our Pb_(0.913)La_(0.0145)Sm_(0.0145)(Mg_(1/3)Nb_(2/3))_(0.71)Ti_(0.29)O_(3)(TP2.9)ceramics exhibited over 60%transparency and large piezoelectric coefficient(d33)of 1104 pC/N.This material holds considerable promise for a wide range of industrial applications in both the optical and electronic domains.

The genus Lactarius Pers.(Russulales)is a cosmopolitan group of Basidiomycota that forms ectomycorrhizal relationships primarily with both deciduous and coniferous trees.Although the genus has been well-studied in Europe and North America,only fragmentary researches have been carried out on Asian species.In particular,the distribution of Lactarius species in South Korea is poorly understood due to insufficient morphological descriptions and a lack of DNA sequence data.In addition,the misuse of European and North American names has added to confusion regarding the taxonomy of Asian Lactarius species.In this study,the diversity of Lactarius in South Korea was evaluated by employing both morphological and phylogenetic approaches.A multi-locus phylogenetic analysis of 729 Lactarius specimens collected between 1960 and 2017 was performed using the internal transcribed spacer(ITS)region,partial nuclear ribosomal large subunit(nrLSU),partial second largest subunit of RNA polymerase II(rpb2),and minichromosome maintenance complex component 7(mcm7).49 Lactarius species were identified in three Lactarius subgenera:L.subg.Russularia(17 spp.),L.subg.Lactarius(22 spp.),and L.subg.Plinthogalus(10 spp.).Among them,28 Lactarius species were identified as new to science,while just 17 were previously described Lactarius species.Four of the taxa remain un-named due to paucity of materials.A key to Korean Lactarius species,molecular phylogenies,a summary of diversity,and detailed description are provided.

BACKGROUND:This study aimed to review bicycle-related injuries during the COVID-19 pandemic to assist with reinforcement or implementation of new policies for injury prevention.METHODS:This is a retrospective descriptive analysis of injuries sustained during cycling for patients 18 years old and above who presented to Singapore General Hospital from January to June 2021.Medical records were reviewed and consolidated.Descriptive analyses were used to summarize patient characteristics,and differences in characteristics subgrouped by triage acuity and discharge status were analyzed.RESULTS:The study included 272 patients with a mean age of 43 years and a male predominance(71.7%).Most presented without referrals(88.2%)and were not conveyed by ambulances(70.6%).Based on acuity category,there were 24(8.8%)Priority 1(P1)patients with 7 trauma activations,174(64.0%)and 74(27.2%)P2 and P3 patients respectively.The most common injuries were fractures(34.2%),followed by superficial abrasion/contusion(29.4%)and laceration/wound(19.1%).Thirteen(4.8%)patients experienced head injury and 85 patients(31.3%)were documented to be wearing a helmet.The majority occurred on the roads as traffic accidents(32.7%).Forty-two patients(15.4%)were admitted with a mean length of stay of 4.1 d and 17(6.3%)undergone surgical procedures.Out of 214(78.7%)discharged patients,no re-attendances or mortality were observed.In the subgroup analysis,higher acuity patients were generally older,with higher proportions of head injuries leading to admission.CONCLUSION:Our study highlights significant morbidities in bicycle-related injuries.There is also a high proportion of fractures in the young healthy male population.Injury prevention is paramount and we propose emphasizing helmet use and road user safety.

The earth-abundant and high-performance catalysts are crucial for commercial implementation of hydrogen evolution reaction(HER).Herein,a multifunctional site strategy to construct excellent HER catalysts by incorporating iridium(Ir)ions on the atomic scale into orthorhombic-CoSe2(Ir-CoSe_(2))was reported.Outstanding hydrogen evolution activity in alkaline media such as a low overpotential of 48.7 mV at a current density of 10 mA cm^(-2)and better performance than commercial Pt/C catalysts at high current densities were found in the Ir-CoSe_(2) samples.In the experiments and theoretical calculations,it was revealed that Ir enabled CoSe_(2)to form multifunctional sites to synergistically catalyze alkaline HER by promoting the adsorption and dissociation of H_(2)O(Ir sites)and optimizing the binding energy for H^(*)on Co sites.It was noticeable that the electrolytic system comprising the Ir-CoSe_(2)electrode not only produced hydrogen efficiently via HER,but also degraded organic pollutants(Methylene blue).The cell voltage of the dual-function electrolytic system was 1.58 V at the benchmark current density of 50 mA cm^(-2),which was significantly lower than the conventional water splitting voltage.It was indicated that this method was a novel strategy for designing advanced HER electrocatalysts by constructing multifunctional catalytic sites for hydrogen production and organic degradation.

Recently,the technology of digital image forgery based on a generative adversarial network(GAN)has considerably improved to the extent that it is difficult to distinguish it from the original image with the naked eye by compositing and editing a person’s face or a specific part with the original image.Thus,much attention has been paid to digital image forgery as a social issue.Further,document forgery through GANs can completely change the meaning and context in a document,and it is difficult to identify whether the document is forged or not,which is dangerous.Nonetheless,few studies have been conducted on document forgery and new forgery-related attacks have emerged daily.Therefore,in this study,we propose a novel convolutional neural network(CNN)forensic discriminator that can detect forged text or numeric images by GANs using CNNs,which have been widely used in image classification for many years.To strengthen the detection performance of the proposed CNN forensic discriminator,CNN was trained after image preprocessing,including salt and pepper as well asGaussian noises.Moreover,we performed CNN optimization to make existing CNN more suitable for forged text or numeric image detection,which have mainly focused on the discrimination of forged faces to date.The test evaluation results using Hangul texts and numbers showed that the accuracy of forgery discrimination of the proposed method was significantly improved by 20%in Hangul texts and 5%in numbers compared with that of existing state-of-the-art methods,which proved the proposed model performance superiority and verified that it could be a useful tool in reducing crime potential.

BACKGROUND Ankylosing spondylitis(AS)is strongly associated with the human leukocyte antigen(HLA)B27 haplotype.In regions where conventional polymerase chain reaction for HLA typing is available for antigens such as HLA B27 or HLA B51,it is common to perform the HLA B27 test for evaluation of AS.While HLA B27-associated clustered occurrences of AS have been reported in families,we report the first case series of HLA B51-related occurrences of AS in a family.CASE SUMMARY A father and his daughters were diagnosed with AS and did not have the HLA B27 haplotype.Although they were positive for HLA B51,they exhibited no signs of Behçet’s disease(BD).Of the five daughters,one had AS,and three,including the daughter with AS,were positive for HLA B51.The two daughters with the HLA B51 haplotype(excluding the daughter with AS)exhibited bilateral grade 1 sacroiliitis,whereas the daughters without the HLA B51 haplotype did not have sacroiliitis.Thus,this Korean family exhibited a strong association with the HLA B51 haplotype and clinical sacroiliitis,irrespective of the symptoms of BD.CONCLUSION It is advisable to check for HLA B51 positivity in patients with AS/spondyloarthropathy who test negative for HLA B27.

Clinical outcomes of targeted therapies in elderly patients aged ≥ 80 years with metastatic colorectal cancer

BACKGROUND The 5-fluorouracil-based chemotherapy combined with oxaliplatin or irinotecan is usually used in colorectal cancer(CRC).The addition of a targeted agent(TA) to this combination chemotherapy is currently the standard treatment for metastatic CRC.However,the efficacy and safety of combination chemotherapy for metastatic CRC in patients aged above 80 years has yet to be established.AIM To assess the clinical outcomes and feasibility of combination chemotherapy using a TA in extremely elderly patients with CRC.METHODS Eligibility criteria were:(1) Age above 80 years;(2) Metastatic colorectal cancer;(3) Palliative chemotherapy na?ve;(4) Eastern Cooperative Oncology Group performance status 0-1;and(5) Adequate organ function.Patients received at least one dose of combination chemotherapy with or without TA.Response was evaluated every 8 wk.RESULTS Of 30 patients,the median age of 15 patients treated with TA was 83.0 years and that of those without TA was 81.3 years.The median progression-free survival(PFS) and overall survival(OS) in patients treated with TA were 7.4 mo and 15.4 mo,respectively,compared with 4.4 mo and 15.6 mo,respectively,in patients treated without TA.There was no significant difference in PFS(P:0.193) and OS(P:0.748) between patients treated with and without TA.Common grade 3/4 hematologic toxicities were anemia(16.7%) and neutropenia(10.0%).After disease progression,the median OS of patients who were treated with and without salvage chemotherapy were 23.5 mo and 7.0 mo,respectively,suggesting significant difference in OS(P = 0.001).CONCLUSION Combination chemotherapy with TA for metastatic CRC may be considered feasible in patients aged above 80 years,when with careful caution.Salvage chemotherapy can help improve OS in some selected of these elderly patients.

Background:Grounded in intersectionality theory,this study examined the 6-year prevalence trend and correlates in meeting Canada’s 24-Hour Movement Guidelines(Guidelines hereafter)in a nationally representative adolescent samples of South Korea(officially the Republic of Korea).Methods:Self-reported,annually repeated cross-sectional data collected between 2013 and 2018 were used(n=372,433,12-17 years old,47.9%females).Adolescents were categorized as meeting or not meeting different sets of physical activity,screen time(ST),and sleep recommendations within the Guidelines,separately for weekdays and weekend days.Intersectional correlates included sex and social class(i.e.,family economic status,parental education level,and academic performance).Multiple logistic regression analyses were performed.Results:Overall,the proportion of adolescents meeting physical activity,ST,and sleep recommendations were 5.3%,60.3%,and 10.2%on weekdays and 5.3%,28.2%,and 46.4%on weekend days,respectively.Between 2013 and 2018,no substantial changes were observed for meeting the physical activity or sleep recommendation,while meeting the ST recommendation was markedly lower in 2018.The proportion of meeting all 3 recommendations were 0.5%for weekdays and 0.8%for weekend days.Overall,compared to female adolescents,male adolescents were consistently associated with more favorable patterns of meeting different sets of recommendations,regardless of social class.Among females only,social class appeared to be not important or even detrimental in meeting different sets of recommendations.Being male,compounded with social class,was associated with meeting the ST recommendation.Conclusion:Less than 1%of Korean adolescents met the overall Guidelines.Intersectionality-based analysis and intervention may be important in promoting healthy active lifestyles among South Korean adolescents.
