Survival rate and the determinants of progression from HIV to AIDS and from AIDS to the death in Iran: 1987 to 2016

Objective: To examine the prognostic factors of progression from HIV to AIDS and AIDS to the death in people living with HIV/AIDS in ***: In this registry-based retrospective cohort study, we recruited 28 873 HIV-infected people from 158 Behavioral Diseases Counseling Centers of *** outcomes of interest included survival rates from HIV diagnosis to AIDS and from AIDS to the *** used Kaplan-Meier and Cox regression model to investigate survival rate and factors affecting on survival controlling effect of confounding ***: The one, three, five, and ten-year survival rate from HIV to AIDS were 85%, 73%, 61% and 32%, and for AIDS to death were 90%, 81%, 74% and 55%, *** Cox regression analysis indicated that the risk of progression from AIDS phase toward death in individuals with CD4 less than 200/mm^3, infected with tuberculosis(TB) and not treated by antiretroviral therapy(ART) was 2.17(95% CI: 1.62-2.90), 1.49(95% CI: 1.01-2.20) and 4.88(95% CI: 3.42-6.96) times higher *** risk of progression to AIDS in patients with baseline CD4 less than 200/mm^3 was 2.32(95% CI: 2.14, 2.51) times higher than patients with CD4 > 200/mm^3(P=0.001).On the other hand, tuberculosis increases the risk of death by 49.0%(P=0.04).The hazard ratio of death in patients who did not receive ART was 4.88(95% CI: 3.42, 6.96) times higher than patients who received ART(P<0.001).Conclusion: The early detection of HIV, the screening and treatment of TB and receiving the ART improve the survival of HIV/AIDS patients significantly, and prevent the transmission of HIV to other people.

目的本研究旨在确定基于自我调节常识模型(Common-Sense Model of Self-regulation)的健康教育对高血压患者的血压和自我管理的影响。方法本研究为随机对照临床试验,于2021年4月至2022年3月选取72例转诊到伊朗哈马丹省法尔希奇扬医院门诊的高血压患者为研究对象,随机分为干预组(n=36)和对照组(n=36)。干预组患者接受基于自我调节常识模型的培训项目,为期1个月,共5次,每次30~45 min,每3 d进行1次电话随访。对照组接受常规健康教育。在干预前、干预后3个月,护士测量患者血压,采用自我管理问卷对患者进行调查,比较两组的干预效果。结果共有68例患者完成研究。在干预前,两组患者的自我管理得分及其维度得分、收缩压、舒张压和平均动脉压差异无统计学意义(P>0.05)。干预后3个月,与对照组相比,干预组患者的收缩压[(116.21±14.52)mmHg比(128.62±16.88)mmHg,1 mmHg=0.133 kPa]和平均动脉压[(88.03±8.47)mmHg比(98.11±11.69)mmHg]水平下降,自我管理问卷总得分及其各维度得分均提高,且高于对照组(P<0.05)。结论基于自我调节常识模型的健康教育改善了高血压患者的自我管理水平,患者血压得到控制,护士可将其作为一种有效的健康教育模式。

目的伊朗儿童的肥胖患病率不断升高。这项研究旨在评价伊朗东北部城市Neishabour的儿童代表性样本中肥胖的某些膳食决定因素。方法在这项病例对照研究中,病例组为114名在校学生,年龄6～12岁,体重指数(BMI)≥第95个百分位(根据伊朗儿童的参考值),而对照组为102名年龄和性别匹配的非肥胖同班同学。由受过训练的营养师在儿童在场的情况下与母亲面谈,进行两次24 h膳食回顾调查,收集了营养摄入数据;使用食物频率问卷调查了零食摄入模式;使用SPSS软件(第16版)进行了单因素和多因素logistic回归分析。结果单因素logistic回归分析显示,总能量、蛋白质、碳水化合物、脂肪(包括饱和脂肪、单不饱和脂肪和多不饱和脂肪)和膳食纤维是学生肥胖的正向预测因素;玉米膨化食品、碳酸饮料、薯片、快餐食品和巧克力的摄入频率的粗估比值比(OR)有统计学意义。多因素logistic回归分析显示,肥胖与能量摄入量(OR=2.489,95%CI:1.667～3.716)、玉米膨化食品摄入频率(OR=1.122,95%CI:1.007～1.250)及薯片摄入频率(OR=1.143,95%CI:1.024～1.276)显著相关;而膳食纤维摄入量(OR=0.909,95%CI:0.835～0.988)和天然果汁摄入量(OR=0.601,95%CI:0.368～0.983)是预防肥胖的保护因素。结论该研究结果证实了不健康饮食对儿童肥胖的作用,尤其是高热量零食。应鼓励儿童养成健康的饮食习惯,如摄入高纤维食物。
