BACKGROUND Jack hammer esophagus is a relatively rare disease and to date,there is no dramatic treatment ***,conventional per-oral endoscopic myotomy (POEM) have expanded their area into Jackhammer ***,several complications such as post procedure motility disorders (e.g.,passage disturbance) are issues after *** overcome these issues,we here introduced high-resolution manometry (HRM)-guided superficial partial circular muscle myotomy,which involves cutting only the superficial layer of the esophageal circular *** SUMMARY We report two cases of patients with Jackhammer esophagus who were treated with HRM-guided extremely superficial partial circular muscle myotomy during *** 1 was a 53-year-old female with medication-refractory odynophagia and case 2 was a 47-year-old man who presented with chest *** were diagnosed with Jackhammer esophagus using HRM,and the hypercontractile segments of the esophagus were ***-guided extremely superficial partial circular muscle myotomy was performed while preserving the lower esophageal ***,the circular and longitudinal muscle layers are preserved but hypercontractile movements are reduced,even after ***’ clinical symptoms dramatically improved right after POEM,and 6-mo follow-up HRM revealed completely resolved *** a 1-year follow-up period,patients were still in good health and remained symptom *** HRM-guided superficial partial circular muscle myotomy may be a promising alternative to conventional POEM for treating Jackhammer esophagus with improved efficacy.

Probabilistic seismic performance assessment of lap-spliced RC columns retrofitted by steel wrapping jackets

in this study, the seismic fragility curves of two reinforced concrete（RC） columns that were lap-spliced at the bottom and retrofitted with steel wrapping jackets were generated. Their seismic performance was probabilistically assessed in comparison to that of lap-spliced or continuous reinforcement RC columns. This study used two types of steel wrapping jackets, a full jacket and a split jacket. Analytical models of the four types of columns were developed based on the experimental results of the columns using Open SEES, which is effective in conducting nonlinear time history analyses. A suite of ten artificial ground motions, modified from recorded ground motions, was used to perform nonlinear time history analyses of the analytical models with scaling of the peak ground acceleration from 0.1 g to 1.0 g in increments of 0.1 g. The steel wrapping jackets did not increase the medians for yield（slight damage state） of the lap-spiced column and did not exceed the corresponding median of the continuous reinforcement column. However, the two steel jackets increased the medians for failure by 1.872 and 2.017 times, respectively, and exceeded the corresponding median of the continuous reinforcement column by 11.8% and 20.5%, respectively.

Lymphangioma usually occurs in children and usually involves the skin. Mesenteric lymphangioma is extremely rare in adults. Typically, lymphangioma appears on computed tomography (CT) as a lower attenuation of a cystic mass, however, some cases appear to be a solid mass. We describe the CT and 18F-FDG positron emission tomography/CT appearance in a case of jejunal and mesenteric cavernous lymphangi-omatosis mimicking metastasis in an adult patient with rectal cancer.

BACKGROUND Cerebrospinal fluid(CSF)leakage at C1/2 in spontaneous intracranial hypotension(SiH)is *** hematoma(SDH),a serious complication of SiH,may lead to neurological *** report presents a case of SDH after spontaneous C1/2 CSF leakage,which was treated with a targeted epidural blood patch(EBP).CASE SUMMARY A 60-year-old man with no history of trauma was admitted to our hospital with orthostatic headache,nausea,and *** computed tomography imaging revealed bilateral,subacute to chronic *** magnetic resonance imaging(MRi)findings were SDH with dural enhancement in the bilateral cerebral convexity and posterior fossa and mild sagging,suggesting *** the patient underwent burr hole trephination,the patient’s orthostatic headache was *** myelography led to a suspicion of CSF leakage at C1/***,we performed a targeted cervical EBP using an epidural catheter under fluoroscopic *** 5 d after EBP,a follow-up MR myelography revealed a decrease in the interval size of the CSF *** his symptoms improved,the patient still complained of headaches;therefore,we repeated the targeted cervical EBP 6 d after the initial ***,his headache had almost disappeared on the 8th day after the repeated *** Targeted EBP is an effective treatment for SDH in patients with SiH due to CSF leakage at C1/2.

The production of transgenic farm animals(e.g., cattle) via genome engineering for the gain or loss of gene functions is an important undertaking. in the initial stages of genome engineering, DNA micro-injection into one-cell stage embryos(zygotes) followed by embryo transfer into a recipient was performed because of the ease of the ***, as this approach resulted in severe mosaicism and has a low efficiency, it is not typically employed in the cattle as priority, unlike in mice. To overcome the above issue with micro-injection in cattle, somatic cell nuclear transfer(SCNT) was introduced and successfully used to produce cloned livestock. The application of SCNT for the production of transgenic livestock represents a significant advancement, but its development speed is relatively slow because of abnormal reprogramming and low gene targeting efficiency. Recent genome editing technologies(e.g.,ZFN, TALEN, and CRiSPR-Cas9) have been rapidly adapted for applications in cattle and great results have been achieved in several fields such as disease models and bioreactors. in the future, genome engineering technologies wil accelerate our understanding of genetic traits in bovine and wil be readily adapted for bio-medical applications in cattle.

Major media outlets have run many articles on the COViD-19 *** the public suffers cognitive and emotional effects related to COViD-19 from such reports,we analyzed and reviewed the topics of news *** searched newspaper articles with the term‘COViD-19’term in four Korean daily newspapers from January 20,2020,when thefirst patient in Korea was found,to June 15,*** modeling analysis was conducted through text mining using *** themes were found:“Changes in people’s everyday life,”“Socio-economic shock,”“Trends in infection,”“Role of the government and business,”and“increased psychological anxiety,”which all showed sharp increases in articles from mid-February to early March and then *** the increased psychological anxiety people suffered from the COViD-19 pandemic,this topic showed the fewest articles.“Changes in people’s everyday life”showed the most,focusing attention on stimulating lifestyle articles of general *** the COViD-19 pandemic can lead to mental health problems due to severe changes and isolation in everyday life,a comprehensive response to the news focusing on the impact on the mental health of the population around the world should be made.

Recently,there has been a sudden shift from using traditional office applications to the collaborative cloud-based office suite such as Microsoft Office *** cloud-based systems allow users to work together on the same docu-ment stored in a cloud server at once,by which users can effectively collaborate with each ***,there are security concerns unsolved in using cloud *** of the major concerns is the security of data stored in cloud ser-vers,which comes from the fact that data that multiple users are working together cannot be stored in encrypted form because of the dynamic characteristic of cloud *** this paper,we propose a novel mode of operation,DL-ECB,for AES by which we can modify,insert,and delete the ciphertext based on changes in ***,we can use encrypted data in collaborative cloud-based *** demonstrate that the DL-ECB mode can preserve the confidential-ity,integrity,and auditability of data used in collaborative cloud systems from adversaries,we implement and evaluate the prototype of the DL-ECB mode.
