Clinical outcomes and safety of high-resolution manometry guided superficial partial circular muscle myotomy in per-oral endoscopic myotomy for Jackhammer esophagus: Two cases report

BACKGROUND Jack hammer esophagus is a relatively rare disease and to date,there is no dramatic treatment option.Recently,conventional per-oral endoscopic myotomy (POEM) have expanded their area into Jackhammer esophagus.However,several complications such as post procedure motility disorders (e.g.,passage disturbance) are issues after POEM.To overcome these issues,we here introduced high-resolution manometry (HRM)-guided superficial partial circular muscle myotomy,which involves cutting only the superficial layer of the esophageal circular muscle.CASE SUMMARY We report two cases of patients with Jackhammer esophagus who were treated with HRM-guided extremely superficial partial circular muscle myotomy during POEM.Case 1 was a 53-year-old female with medication-refractory odynophagia and case 2 was a 47-year-old man who presented with chest pain.They were diagnosed with Jackhammer esophagus using HRM,and the hypercontractile segments of the esophagus were identified.HRM-guided extremely superficial partial circular muscle myotomy was performed while preserving the lower esophageal sphincter.Therefore,the circular and longitudinal muscle layers are preserved but hypercontractile movements are reduced,even after POEM.Patients’ clinical symptoms dramatically improved right after POEM,and 6-mo follow-up HRM revealed completely resolved status.During a 1-year follow-up period,patients were still in good health and remained symptom free.CONCLUSiON HRM-guided superficial partial circular muscle myotomy may be a promising alternative to conventional POEM for treating Jackhammer esophagus with improved efficacy.

BACKGROUND Colo-colonic intussusception is an uncommon phenomenon in an adult.Adult intussusception accounts for ic type is ical management has been the treatment of choice for intestinal intussusception in adults,because most frequent causes for adult intussusception are malignant in origin,the importance of the roles of preoperative colonoscopic evaluation has recently been emerging.CASE SUMMARY We report an extremely rare case of adult colo-colonic intussusception caused by colonic anisakiasis and successfully treated by endoscopic removal of the Anisakis body.A 59-year-old man visited the emergency department due to 1 day of lower abdominal colicky pain.Abdominopelvic computed tomography (APCT) revealed the presence of mid-transverse colon intussusception without definite necrosis,which was possibly related with colorectal cancer.Because there was no evidence of necrosis at the intussusception site,a colonoscopy was performed to target the colonic lesion and obtain tissue for a histopathological diagnosis.An Anisakis body was found when inspecting the suspicious colonic lesion recorded by APCT.The Anisakis body was removed with forceps assisted by colonoscopy.The patient’s symptoms improved dramatically after removing the Anisakis.A reduced colon without any pathological findings was seen on the follow-up APCT.Without any further treatment,the patient was discharged 5 d after the endoscopy.CONCLUSiON When colonic intussusception without necrosis occurs in an adult,physician should consider a colonoscopy to exclude causes cured by endoscopy.

Tailored eradication vs empirical bismuth-containing quadruple therapy for first-line Helicobacter pylori eradication: A comparative,open trial

BACKGROUND Few studies have compared the efficacy and safety profile of a tailored eradication(TR)strategy based on the presence of a 23S ribosomal RNA point mutation with those of empirical bismuth-based quadruple therapy(EBQT)for first-line eradication of Helicobacter pylori(H.pylori)in Korean patients.AiM To compare the efficacy and safety of a TR strategy and those of EBQT regimen as first-line eradication therapy for H.pylori.METHODS This is an open-label,comparative study in which we prospectively enrolled patients over 18 years of age with H.pylori infection and retrospectively reviewed their data.H.pylori-positive patients diagnosed by rapid urease test,Giemsa staining,or dual priming oligonucleotide polymerase chain reaction(DPO-PCR)were enrolled from May 2016 to September 2018 at Gil Medical Center.Patients with H.pylori infection received either a TR regimen or the EBQT regimen.in the tailored therapy group that underwent DPO-PCR testing,patients with A2142G and/or A2143G point mutations were treated with a bismuth-containing quadruple regimen.The eradication rate,patient-reported side effect rate,and H.pylori eradication success rate were evaluated and compared between the groups.RESULTS A total of 150 patients were assigned to the TR(n=50)or EBQT group(n=100).The first-line eradication rate of H.pylori did not differ between the groups(96.0%vs 95.7%,P=0.9).The rate of eradication-related side effects for TR was 12.0%,which differed significantly from that of EBQT(43.0%)for first-line treatment(PiON DPO-PCR-based TR for H.pylori eradication may be equally efficacious,with less treatment-related complications,compared to EBQT in Korea,where clarithromycin resistance is high.

BACKGROUND Antibiotic resistance to Helicobacter pylori(H.pylori)infection,which ultimately results in eradication failure,has been an emerging issue in the clinical field.Recently,to overcome this problem,an antibiotic sensitivity-based tailored therapy(TT)for H.pylori infection has received attention.AiM To investigate the efficacy and safety profiles of TT for H.pylori infection treatment compared to a non-bismuth quadruple therapy,concomitant therapy(CT)regimen.METHODS We included patients(>18 years)with an H.pylori infection and without a history of Helicobacter eradication who visited the Gil Medical Center between March 2016 and October 2020.After being randomly assigned to either the TT or CT treatment group in 1 to 1 manner,patient compliance,eradication success rate(ESR),and patient-reported side effects profiles were assessed and compared between the two groups.H.pylori infection was diagnosed using a rapid urease test,Giemsa stain,or dual priming oligonucleotide polymerase chain reaction(DPO-PCR).Tailored eradication strategy based through the presence of a 23S ribosomal RNA point mutation.For the TT group,a DPO-PCR test,which detected A2142G and/or A2143G point mutations,and a clarithromycin resistance test were performed.Patients in the clarithromycin-resistant group were treated with a bismuth-containing quadruple combination therapy,while those with sensitive results were treated with the standard triple regimen.RESULTS Of the 217 patients with a treatment naive H.pylori infection,110 patients[mean age:58.66±13.03,men,n=55(50%)]were treated with TT,and 107 patients[mean age:56.67±10.88,men,n=52(48.60%)]were treated with CT.The compliance(TT vs CT,100%vs 98.13%,P=0.30),and follow-up loss rates(8.18%vs 9.35%,P=0.95)were not significantly different between the groups.The ESR after treatment was also not statistically different between the groups(TT vs CT,82.73%vs 82.24%,P=0.95).However,the treatment-related and patient-reported side effects were significantly lower in the TT group than in the CT grou

BACKGROUND Cerebrospinal fluid(CSF)leakage at C1/2 in spontaneous intracranial hypotension(SiH)is rare.Subdural hematoma(SDH),a serious complication of SiH,may lead to neurological deficits.This report presents a case of SDH after spontaneous C1/2 CSF leakage,which was treated with a targeted epidural blood patch(EBP).CASE SUMMARY A 60-year-old man with no history of trauma was admitted to our hospital with orthostatic headache,nausea,and vomiting.Brain computed tomography imaging revealed bilateral,subacute to chronic SDH.Brain magnetic resonance imaging(MRi)findings were SDH with dural enhancement in the bilateral cerebral convexity and posterior fossa and mild sagging,suggesting SiH.Although the patient underwent burr hole trephination,the patient’s orthostatic headache was aggravated.MR myelography led to a suspicion of CSF leakage at C1/2.Therefore,we performed a targeted cervical EBP using an epidural catheter under fluoroscopic guidance.At 5 d after EBP,a follow-up MR myelography revealed a decrease in the interval size of the CSF collected.Although his symptoms improved,the patient still complained of headaches;therefore,we repeated the targeted cervical EBP 6 d after the initial EBP.Subsequently,his headache had almost disappeared on the 8th day after the repeated EBP.CONCLUSiON Targeted EBP is an effective treatment for SDH in patients with SiH due to CSF leakage at C1/2.

Topic Models to Analyze Disaster-Related Newspaper Articles: Focusing on COViD-19

Major media outlets have run many articles on the COViD-19 pandemic.Since the public suffers cognitive and emotional effects related to COViD-19 from such reports,we analyzed and reviewed the topics of news reports.We searched newspaper articles with the term‘COViD-19’term in four Korean daily newspapers from January 20,2020,when thefirst patient in Korea was found,to June 15,2020.Topic modeling analysis was conducted through text mining using R.Five themes were found:“Changes in people’s everyday life,”“Socio-economic shock,”“Trends in infection,”“Role of the government and business,”and“increased psychological anxiety,”which all showed sharp increases in articles from mid-February to early March and then decreased.Despite the increased psychological anxiety people suffered from the COViD-19 pandemic,this topic showed the fewest articles.“Changes in people’s everyday life”showed the most,focusing attention on stimulating lifestyle articles of general interest.Since the COViD-19 pandemic can lead to mental health problems due to severe changes and isolation in everyday life,a comprehensive response to the news focusing on the impact on the mental health of the population around the world should be made.

Dietary sophorolipid accelerates growth by modulation of gut microbiota population and intestinal environments in broiler chickens

Background:Gut is a crucial organ for the host’s defense system due to its filtering action of the intestinal membrane from hazardous foreign substances.One strategy to strengthen the gut epithelial barrier function is to upregulate beneficial microflora populations and their metabolites.Sophorolipid(SPL),which is a glycolipid biosurfactant,could increase beneficial microflora and decrease pathogenic bacteria in the gastrointestinal tract.Therefore,herein,we conducted an experiment with broiler chickens to investigate the fortifying effects of SPL on the host’s gut defense system by modulating the microbiota population.Methods:A total of 5401-day-old chicks(Ross 308)were used,and they were immediately allotted into three treatment groups(6 replications with 30 chicks/pen)according to their initial body weight.The dietary treatments consisted of CON(basal diet),BAM(10 mg/kg bambermycin),and SPL(10 mg/kg SPL).During the experiment,birds freely accessed feed and water,and body weight and feed intake were measured at the end of each phase.On d 35,birds(one bird/pen)were sacrificed to collect jejunum and cecum samples.Results:Dietary SPL and BAM supplementation significantly accelerated birds’growth and also significantly improved feed efficiency compared to CON.intestinal microbial community was significantly separated by dietary SPL supplementation from that of CON,and dietary SPL supplementation significantly increased Lactobacillus spp.and Akkermansia muciniphila.Moreover,birds fed with dietary SPL also showed the highest concentration of cecal butyrate among all treatment groups.Gut morphological analysis showed that dietary SPL significantly increased villus height,ratio of villus height to crypt depth,goblet cell numbers,and the gene expression levels of claudin-1 and mucin 2.Additionally,dietary SPL significantly decreased the mRNA expression level of pro-inflammatory cytokine,interleukin-6,and increased that of anti-inflammatory cytokine,interleukin-10,compared to other treatments.Conclusions:Dietary SPL incr

Single-cell RNA sequencing of mitoticarrested prospermatogonia with DAZL::GFP chickens and revealing unique epigenetic reprogramming of chickens

Background:Germ cell mitotic arrest is conserved in many vertebrates,including birds,although the time of entry or exit into quiescence phase differs.Mitotic arrest is essential for the normal differentiation of male germ cells into spermatogonia and accompanies epigenetic reprogramming and meiosis inhibition from embryonic development to post-hatch.However,mitotic arrest was not well studied in chickens because of the difficulty in obtaining pure germ cells from relevant developmental stage.Results:We performed single-cell RNA sequencing to investigate transcriptional dynamics of male germ cells during mitotic arrest in DAZL::GFP chickens.Using differentially expressed gene analysis and K-means clustering to analyze cells at different developmental stages(E12,E16,and hatch),we found that metabolic and signaling pathways were regulated,and that the epigenome was reprogrammed during mitotic arrest.in particular,we found that histone H3K9 and H3K14 acetylation(by HDAC2)and DNA demethylation(by DNMT3B and HELLS)led to a transcriptionally permissive chromatin state.Furthermore,we found that global DNA demethylation occurred gradually after the onset of mitotic arrest,indicating that the epigenetic-reprogramming schedule of the chicken genome differs from that of the mammalian genome.DNA hypomethylation persisted after hatching,and methylation was slowly re-established 3 weeks later.Conclusions:We found a unique epigenetic-reprogramming schedule of mitotic-arrested chicken prospermatogonia and prolonged hypomethylation after hatching.This will provide a foundation for understanding the process of germ-cell epigenetic regulation in several species for which this process is not clearly described.Our findings on the biological processes related to sex-specific differentiation of prospermatogonia could help studying germline development in vitro more elaborately.

Recently,there has been a sudden shift from using traditional office applications to the collaborative cloud-based office suite such as Microsoft Office 365.Such cloud-based systems allow users to work together on the same docu-ment stored in a cloud server at once,by which users can effectively collaborate with each other.However,there are security concerns unsolved in using cloud col-laboration.One of the major concerns is the security of data stored in cloud ser-vers,which comes from the fact that data that multiple users are working together cannot be stored in encrypted form because of the dynamic characteristic of cloud collaboration.in this paper,we propose a novel mode of operation,DL-ECB,for AES by which we can modify,insert,and delete the ciphertext based on changes in plaintext.Therefore,we can use encrypted data in collaborative cloud-based platforms.To demonstrate that the DL-ECB mode can preserve the confidential-ity,integrity,and auditability of data used in collaborative cloud systems from adversaries,we implement and evaluate the prototype of the DL-ECB mode.

Association of low muscle strength with metabolic dysfunctionassociated fatty liver disease:A nationwide study

BACKGROUND There is limited evidence regarding the association between muscle strength and metabolic dysfunction-associated fatty liver disease(MAFLD).AiM To investigate the association between muscle strength and MAFLD in the general population in Korea.METHODS This nationwide representative cross-sectional study included 31649 individuals aged≥19 years who participated in the Korea National Health and Nutrition Examination Survey between 2015 and 2018.Odds ratios(ORs)and 95%confidence intervals(95%Cis)for MAFLD according to sex-specific quartiles of muscle strength,defined by relative handgrip strength,were calculated using multivariable logistic regression analysis.Additionally,multivariable logistic regression analysis was used to assess the association between muscle strength and probable liver fibrosis in patients with MAFLD.RESULTS Of all the participants,29.3%had MAFLD.The prevalence of MAFLD was significantly higher in the lower muscle strength quartile groups for all participants,sexes,and age groups(Pi:1.70–2.16)and 3.12-fold(OR=3.12,95%Ci:2.64–3.69)higher risk of MAFLD was observed in the lowest quartile(Q1)group than in the other groups(Q2–Q4)and the highest quartile(Q4)group,respectively.The ORs of MAFLD were significantly increased in the lower muscle strength quartile groups in a dose-dependent manner(P for trendiations persisted in both sexes.An inverse association between muscle strength and the risk of MAFLD was observed in all subgroups according to age,obesity,and diabetes mellitus.in patients with MAFLD,the odds of severe liver fibrosis were higher in Q1(OR=1.83,95%Ci:1.25–2.69)than in other groups(Q2–Q4).CONCLUSiON Among Korean adults,low muscle strength was associated with an increased risk of MAFLD and liver fibrosis in patients with MAFLD.
