Laparoscopic liver resection for hepatocellular carcinoma complicated with significant portal hypertension:A propensity score-matched survival analysis

Background: Significant portal hypertension(SPH) is a relative contraindication for patients with resectable hepatocellular carcinoma(HCC). However, increasing evidence indicates that liver resection is feasible for HCC patients with SPH. Methods: HCC patients with cirrhosis who underwent laparoscopic liver resection(LLR) in two centers from January 2013 to April 2018 were included. Surgical and survival outcomes were analyzed to explore potential prognostic factors. Propensity score matching(PSM) analysis was performed to minimize bias. Results: A total of 165 patients were divided into two groups based on the presence(SPH, n = 76) or absence(non-SPH, n = 89) of SPH. Patients in the SPH group had longer operative time, more blood loss, and more advanced TNM stage than patients in the non-SPH group( P 0.05). After PSM, alpha-fetoprotein(AFP) ≥ 400 μg/L [hazard ratio(HR) = 4.71, 95% confidence interval(CI): 2.69-8.25], ascites(HR = 2.18, 95% CI: 1.30-3.66), American Society of Anesthesiologists(ASA) classification(Ⅲ vs. Ⅱ)(HR = 2.13, 95% CI: 1.11-4.07) and tumor diameter > 5 cm(HR = 3.91, 95% CI: 2.02-7.56) independently predicted worse OS. Conclusions: LLR for patients with HCC complicated with SPH appears feasible at the price of increasing operative time and blood loss. AFP, ascites, ASA classification and tumor diameter may predict the prognosis of HCC complicated with SPH after LLR.

Effect of nitrogen on micro-pores in DD33 single crystal superalloy during solidification and homogenization

The DD33 superalloy with ultra-low nitrogen (N) content was prepared by vacuum induced melting, and the effect of N on micro-pores in the DD33 single crystal nickel-base superalloy during solidification and homogenization was investigated by in-situ X-ray computed tomography (XCT). Results indicate that the volume fraction of micro-pores, including shrinkage pores and gas pores, increases from 0.08% to 0.11% with increasing N content from 5 ppm to 45 ppm during solidification. Correspondingly, the level of micro-pores in the sample with high N content is higher than that in the sample with low N content during homogenization at 1,330 °C for different time periods. However, the evolution behaviors of gas pores is different from that of shrinkage pores during solidification and homogenization. The number of gas pores is obviously larger in the high N sample during solidification, while the number of shrinkage pores and gas pores is almost the same in both samples after 1 h homogenization. Quantitative results show that the annihilation of micro-pores is associated with bubble diffusion, while the growth behavior of micro-pores during further exposure is dominated by Kirkendall-Frenkel effect.

Largest 3d-4f 196-nuclear Gd_(158)Co_(38)clusters with excellent magnetic cooling

It is a meaningful and challenging work for structural and synthetic chemists to isolate nano-sized high-nuclearity cluster-molecules.In this work,two largest hetero-metallic nano-clusters Gd_(158)Co_(38) were obtained via the“multi-anions-template”method.Different from the reported giant hollow-nano-clusters,the Ln_(158) core in Gd_(158)Co_(38)(the protein-sized nano-clusters,ca.4.3 nm×3.6 nm×3.5 nm)has the highest Ln nuclear number,which is integrated by twelve halide ions(with the form of icosahedron)as key templates,while Co ions(as 3d metals)are located in its periphery.This emergence indicates a novel structure form of non-open Ln-containing high-nuclearity clusters,and affords a consummate pattern to analyse and assemble the complex cluster-molecules.In addition,Gd_(158)Co_(38)@Cl_(12)breaks the record magnetic entropy change of 3d-4f clusters with−∆S_(m)^(max)=46.95 J kg^(−1)K^(−1)at 7.0 T,2.0 K.

Evolutionary PSO-based emergency monitoring geospatial edge service chain in the emergency communication network

Emergency communication networks play a vital role in disaster monitoring,transmission,and application during disaster emergency response(DER),however,the performance and stability of edge nodes in the emergency communication networks are often weak due to limited communication and computation resources.This weakness directly affects the quality,of service(Qos)of the geospatial edge service(GES)chains involved in emergency monitoring.Existing research predominantly addresses service compositions in stable environments,neglecting the aggregation of efficient and robust GES chains in emergency communication networks.This study proposes an evolutionary_particie swarm optimization(EPSO)-based emergency monitoring GES chain in an_emergency communication network.it includes a GES chain model of emergency environment monitoring for tailing areas,as well as the designs of the particle chromosome encoding method,fitness evaluation model,and particle chromosome swarm update operators of the EPSO-based GES chain.Finally,the study conducts emergency environment monitoring experiments for tailing areas using the proposed method.Experiments results demonstrate that the proposed method significantly enhances the efficiency,stability,and reliability of emergency monitoring GEs chains in the emergency communication network.This is crucial to providing fast and reliable services for DER during natural disasters.

Improving the damage potential of W-Zr reactive structure material under extreme loading condition

Projectiles made of reactive structure materials(RSM)can damage the target with not only kinetic but also chemical energy,but the enhanced damage potential of RSM may become compromised if extreme loading condition disintegrates the projectile before the target is reached.In this work,a ductile coating of Ni was introduced to a tungsten-zirconium(W-Zr)alloy,a typical brittle RSM,to preserve the damage potential of the projectile.Detonation driving tests were carried out with X-ray photography and gunpowder deflagration driving tests were carried out with high-speed photography for the coated and uncoated RSM samples,respectively.The craters on the witness target were analyzed by scanning electron microscopy and X-ray diffraction.The Ni coating was found to effectively preserve the damage potential of the W-Zr alloy under extreme loading conditions,whereas the uncoated sample fractured and ignited before impacting the target in both detonation and deflagration driving.The crack propagation between the reactively brittle core and the ductile coating was analyzed based on the crack arrest theory to mechanistically demonstrate how the coating improves the structural integrity and preserves the damage potential of the projectile.Specifically,the Ni coating envelops theW-Zr core until the coated sphere penetrates the target,and the coating is then eroded and worn to release the reactive core for the projectile to damage the target more intensively.

English as an international language,English writing is a very important skill as an important form of language output,which is not only an important means of expressing emotions and awareness,but also reflects the ability to use language in an integrated way.In language teaching,the development of English writing skills is an important and challenging teaching task.English writing language is expressed not only at the basic level of vocabulary,grammar,and sentence structure,but also at the higher-order level of text organization,logical reasoning,and argumentation.However,in traditional English writing teaching,students usually do not have the opportunity to have an in-depth experience of the context of the topic,which leads to a lack of motivation and poor writing results.Therefore,this study developed an SVVR learning system based on spherical video-based virtual reality(SVVR)technology and conducted a quasi-experiment in a university English major classroom.The results of the study showed that the SVVR-supported approach to English writing could improve students'writing performance,enrich writing content,and increase learning motivation.

Mediating the Migration of Mesenchymal Stem Cells by Dynamically Changing the Density of Cell-selective Peptides Immobilized on β-Cyclodextrin-modified Cell-resisting Polymer Brushes

Dynamic control of mesenchymal stem cell(MSC)behaviors on biomaterial surface is critically involved in regulating the cell fate and tissue regeneration.Herein,a stimuli-responsive surface based on host-guest interaction with cell selectivity was developed to regulate migration of MSCs in situ by dynamic display of cell-specific peptides.Azobenzene-grafted MSC-affinitive peptides(EPLQLKM,Azo-E7)were grafted toβ-cyclodextran(β-CD)-modified poly(2-hydroxyethyl methacrylate)-b-poly(2-hydroxyethyl methacrylate-co-glycidyl methacrylate)(PHG)brushes,which were prepared by using surface-initiated atom transfer radical polymerization(SI-ATRP).X-ray photoelectron spectroscopy(XPS),quartz crystal microbalance(QCM),and water contact angle were used to characterize their structure and property.Cell adhesion assay showed that the combination effect of resisting property of PHG and MSC-affinity of E7 could promote the selective adhesion of MSCs over other types of cells such as RAW264.7 macrophages and NIH3 T3 fibroblasts to some extent.UV-Vis spectroscopy proved that the competing guest molecules,amantadine hydrochloride(Ama),could release Azo-E7 peptides from the CD surface to different extents,and the effect was enhanced when UV irradiation was employed simultaneously.As a result,the decrease of cell adhesion density and migration rate could be achieved in situ.The cell density and migration rate could be reduced by over 40%by adding 20μmol/L Ama,suggesting that this type of surface is a new platform for dynamic regulation of stem cell behaviors in situ.

To the Editor:A 59-year-old Chinese man who presented with a severe cough and short of breath was admitted into hospital in October,2018.Computed tomography scan showed a 4.0 cm2.0 cm tumor located on the lower lobe of right lung[Figure 1A]and multi-bone lesions,core needle biopsy was performed and adenocarcinoma was confirmed by pathologists.Diagnosis of metastatic lung adenocarcinomawithT4N3M1cin stage IVB was made by oncologists and pathologists.Next generation sequencing(NGS)with the biopsied tumor was done,and epidermal growth factor receptor(EGFR)exon 21 L858R mutation with mutant allele fractions(MAF)of 24.8%was found.Icotinib(Betta Pharmaceuticals Co.,Ltd,Hangzhou,China),a firstgeneration EGFR tyrosine kinase inhibitor(TKI),at a dose of 125mg orally,three times a day was administered.The patient got a partial response(PR)after 5 months according to the Response Evaluation Criteria in Solid Tumors 1.1[Figure 1B].

Comparing the precision of the pSOFA and SIRS scores in predicting sepsis-related deaths among hospitalized children:a multi-center retrospective cohort study

BACKGROUND:The latest sepsis definition includes both infection and organ failure,as evidenced by the sequential organ failure assessment(SOFA)score.However,the applicability of the pediatric SOFA score(pSOFA)is not yet determined.This study evaluated the effectiveness of both pSOFA and system inflammatory reaction syndrome(SIRS)scores in predicting sepsis-related pediatric deaths.METHODS:This is a retrospective multi-center cohort study including hospitalized patientsyouden index(YI),three sub-categories of pSOFA were screened out and a new simplified pSOFA score(spSOFA)was formed.The effectiveness and accuracy of prediction of pSOFA,SIRS and spSOFA was retrieved from the area under the receiver operating characteristic curve(AUROC)and Delong’s test.RESULTS:A total of 1,092 participants were eligible for this study,and carried a 23.4%in-hospital mortality rate.The 24-h elevated pSOFA score(24 h-pSOFA),bloodstream infection,and mechanical ventilation(MV)requirement were major risk factors associated with sepsis-related deaths.The AUROC analysis confirmed that the spSOFA provided good predictive capability in sepsis-related pediatric deaths,relative to the 24 h-pSOFA and SIRS.CONCLUSIONS:The pSOFA score performed better than SIRS in diagnosing infected children with high mortality risk.However,it is both costly and cumbersome.We,therefore,proposed spSOFA to accurately predict patient outcome,without the disadvantages.Nevertheless,additional investigations,involving a large sample population,are warranted to confirm the conclusion of this study.

The ferroxidases LPR1 and LPR2 control iron translocation in the xylem of Arabidopsis plants

Iron(Fe)deficiency is common in agricultural crops and affects millions of people worldwide.Translocation of Fe in the xylem is a key step for Fe distribution in plants.The mechanism controlling this process remains largely unknown.Here,we report that two Arabidopsis ferroxidases,LPR1 and LPR2,play a crucial and redundant role in controlling Fe translocation in the xylem.LPR1 and LPR2 are mainly localized in the cell walls of xylem vessels and the surrounding cells in roots,leaves,and stems.Knockout of both LPR1 and LPR2 increased the proportion of Fe(II)in the xylem sap,and caused Fe deposition along the vascular bundles especially in the petioles and main veins of leaves,which was alleviated by blocking blue light.The lpr1 lpr2 double mutant displayed constitutive expression of Fe deficiency response genes and overaccumulation of Fe in the roots and mature leaves under Fe-sufficient supply,but Fe deficiency chlorosis in the new leaves and inflorescences under low Fe supply.Moreover,the lpr1 lpr2 double mutant showed lower Fe concentrations in the xylem and phloem saps,and impaired 57Fe translocation along the xylem.In vitro assays showed that Fe(III)-citrate,the main form of Fe in xylem sap,is easily photoreduced to Fe(II)-citrate,which is unstable and prone to adsorption by cell walls.Taken together,these results indicate that LPR1 and LPR2 are required to oxidize Fe(II)and maintain Fe(III)-citrate stability and mobility during xylem translocation against photoreduction.Our study not only uncovers an essential physiological role of LPR1 and LPR2 but also reveals a new mechanism by which plants maintain Fe mobility during long-distance translocation in the xylem.
