Association between preoperative high sensitive troponin I levels and cardiovascular events after hip fracture surgery in the elderly

Objective Cardiovascular complications contribute to postoperative morbidity and mortality in elderly hip fracture patients. Limited data are available regarding which preoperative risk factors predict cardiovascular course following hip fracture surgery （HFS）. We used high sensitive troponin I （hs-TnI） assays and clinical parameters to identify preoperative risk factors associated with major adverse cardiac events （MACE） in elderly hip frac^u＇e patients. Method From August 2014 to November 2016, 575 patients with hip fracture were enrolled in a retrospective, single-center registry. A total of 262 of these patients underwent HFS and hs-TnI assays. MACE was defined as postoperative all-cause deaths, heart failure （HF）, new-onset atrial fibrillation （AF）, myocardial infarction （MI） and cardiovascular re-hospitalization that occurred within 90 days postoperative. Results Of 262 HFS patients, MACE developed following HFS in 65 （24.8%）. Patients with MACE were older and had higher rates of renal insufficiency, coronary artery disease, prior HF, low left ventricular ejection fraction and use of beta blockers; higher levels of hs-Tnl and N-terminal pro-brain natriuretic peptide （NT-proBNP） and higher revised cardiac risk index. A preoperative hs-TnI≥ 6.5 ng/L was associated with high risk of postoperative HF, new-onset AF and MACE. In multivariable analysis, pre-operative independent predictors for MACE were age 〉 80 years [adjusted hazard ratio （HR）： 1.79, 95% confident interval （CI）： 1.03-3.13, P = 0.04], left ventricular ejection fraction （LVEF）〈 50% （adjusted HR： 3.17, 95% CI： 1.47-5.82, P 〈 0.01） and hs-TnI 〉 6.5 ng/L （adjusted HR： 3.75, 95% CI： 2.09~5.17, P 〈 0.01）. Conclusion In elderly patients with hip fracture who undergo HFS, a preoperative assessment of hs-TnI may help the risk refinement of cardiovascular complications.

OBJECTIVE: To investigate whether scorpion extract elicits a neuroprotective effect in 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)-treated mice models, and the genes associated with the therapeutic effects using RNA sequencing(seq)***: This study investigated the changes in interaction between messenger ribonucleic acid(m RNA) expression and deoxyribonucleic acid(DNA) methylation related to the protective effects of scorpion extracts, in the substantia nigra(SN)region of a MPTP-induced Parkinson's disease(PD)***: In this model, scorpion extracts attenuated the motor impairment as demonstrated by the rotarod and open field tests. Scorpion extracts consistently attenuated the decrease of tyrosine hydroxylase(TH) positive neural cells in the SN and striatum of mice. We profiled genomewide DNA methylation using Methyl-Seq and measured the transcriptome using RNA-Seq in murine SN in the following groups: vehicle-treated MPTP-induced PD mice and scorpion extracttreated MPTP-induced PD mice. In total, 13 479 differentially expressed genes were identified in association with the anti-PD effect of the scorpion extract, mainly in the promoter and coding *** them, 47 were negatively correlated downregulated genes. Nineteen genes out of 47 downregulated genes were negatively correlated with the expression of the other 28 genes. Among these genes, SGSM1 was related to dopaminergic neurons including dopamine transporters, TH, dihydroxyphenylalanine decarboxylase, and dopamine D2 ***: This study provides insights into the anti-parkinsonian effects of scorpion extract and reveals the epigenetic targets in its therapeutic mechanism.

Simulation and Causes of Eastern Antarctica Surface Cooling Related to Ozone Depletion during Austral Summer in FGOALS-s2

Two parallel sets of numerical experiments （an ozone-hole simulation and a non-ozone-hole simulation） were performed to investigate the effect of ozone depletion on surface temperature change using the second spectral version of the Flexible Global Ocean-Atmosphere-Land System model （FGOALS-s2）, focusing on the eastern Antarctica （EA） continent in austral summer. First, we evaluated the ability of the model to simulate the EA surface cooling, and found the model can successfully reproduce the cooling trend of the EA surface, as well as the circulation change circling the South Pole in the past 30 years. Second, we compared the two experiments and discovered that the ozone depletion causes the cooling trend and strengthens the circumpolar westerly flow. We further investigated the causes of the EA surface cooling associated with the ozone hole and found two major contributors. The first is the ozone-hole direct radiation effect （DRE） upon the surface that happens because the decrease of the downward longwave （LW） radiation overcomes the increase of the downward shortwave （SW） radiation under clear sky. The second is the cloud radiation effect （CRE） induced by ozone depletion, which happens because the decreased downward SW radiation overcomes the increased downward LW radiation in the case of increased cloud. Although the CRE is theoretically opposite to the DRE, their final net effect makes comparable contributions to the EA surface cooling. Compared with the surface radiation budget, the surface heat flux budgets have a much smaller contribution. We additionally note that the CRE is basically ascribed to the circulation change.

Commercialization of Zn-metal anodes with low cost and high theoretical capacity is hindered by the poor reversibility caused by dendrites growth,side reactions,and the slow Zn^(2+)-transport and reaction ***,a reversible heterogeneous electrode of Zn-nanocrystallites/polyvinylphosphonic acrylamide(Zn/PPAm)with fast electrochemical kinetics is designed for the first time:phosphonic acid groups with strong polarity and chelation effect ensure structural reversibility and stability of the threedimensional Zn-storage-host PPAm network and the Zn/PPAm hybrid;hydrophobic carbon chains suppress side reactions such as hydrogen evolution and corrosion;weak electron-donating amide groups constitute Zn^(2+)-transport channels and promote“desolvation”and“solvation”effects of Zn^(2+)by dragging the PPAm network on the Zn-metal surface to compress/stretch during Zn plating/stripping,respectively;and the heterostructure and Zn nanocrystallites suppress dendrite growth and enhance electrochemical reactivity,***,the Zn/PPAm electrode shows cycle reversibility of over 6000 h with a hysteresis voltage as low as 31 mV in symmetrical cells and excellent durability and flexibility in fiber-shaped batteries.

Lithospheric thinning and ignition of a Cordilleran magmatic flare-up:Geochemical and O-Hf isotopic constraints from Cretaceous plutons in southern Korea

Northeast Asian continental margins contain the products of magma emplacement driven by prolonged subduction of the(paleo-)Pacific *** observed in many Cordilleran arcs,magmatic evolution in this area was punctuated by high-volume pulses amid background *** present study investigates the early evolution of the Cretaceous magmatic flare-up using new and published geochronological,geochemical,and O-Hf isotope data from plutonic rocks in the southern Korean *** a long(~50 m.y.)magmatic hiatus and the development of the Honam Shear Zone through flat-slab subduction,the Cretaceous flare-up began with the intrusion of monzonites,granodiorites,and granites in the inboard Gyeonggi Massif and the intervening Okcheon *** to Jurassic granitoids formed during the former flare-up,Albian(~111 Ma)monzonites found in the Eopyeong area of the Okcheon Belt have distinctly higher zirconε_(Hf)(t)(-7.5±1.3)andδ^(18)O(7.78‰±0.25‰)values and lower wholerock La/Yb and Sr/Y *** voluminous coeval granodiorite and granite plutons in the Gyeonggi Massif are further reduced in Sr/Y and to a lesser extent,in La/Yb,and have higher zirconε_(Hf)(t)values(-13 to-19)than the Precambrian basement(ca.-30).These chemical and isotopic features indicate that Early Cretaceous lithospheric thinning,most likely resulting from delamination of tectonically and magmatically overthickened lithospheric keel that was metasomatized during prior subduction episodes,and consequent asthenospheric upwelling played vital roles in igniting the magmatic *** O-Hf isotopic ranges of synmagmatic zircons from the Albian plutons and their Paleoproterozoic and Jurassic inheritance attest to the involvement of lithospheric mantle and crustal basement in magma generation during this decratonization *** magmatism then migrated trenchward and culminated in the Late Cretaceous,yielding widespread granitoid rocks emplaced at shallow crustal *** early Late Cretaceous(94-8

Aortic arch calcification on chest X-ray combined with coronary calcium score show additional benefit for diagnosis and outcome in patients with angina

Background The coronary artery calcium （CAC） and aortic arch calcification （AoAC） are individually associated with cardiovascular disease and outcome. This study investigated the predictive value of AoAC combined with CAC for cardiovascular diagnosis and outcome in patients with angina. Methods A total of 2018 stable angina patients who underwent chest X-ray and cardiac multi-detector computed tomography were followed up for four years to assess adverse events, which were categorized as cardiac death, stroke, myocardial infarction, or repeated revascularization. The extent of AoAC on chest X-ray was graded on a scale from 0 to 3. Results During the four years of fol- low-up, 620 patients were treated by coronary stenting and 153 （7%） adverse events occurred. A higher grade of AoAC was associated with a higher CAC score. Cox regression showed that the CAC score, but not AoAC, were associated with adverse events. In patients with CAC score 〈 400, AoAC showed an additive predictive value in detecting significant coronary artery disease （CAD）. A gradual increases in the risk of adverse events were noted if AoAC was present in patients with similar CAC score. Conclusions As AoAC is strongly correlated with the CAC score regardless of age or gender, careful evaluation of CAD would be required in patients with AoAC on conventional chest X-rays.

Nomogram for prediction of pathologic complete remission using biomarker expression and endoscopic finding after preoperative chemoradiotherapy in rectal cancer

Objective:The aim of this study is to develop a nomogram for prediction of pathologic complete remission(p CR)after preoperative chemoradiotherapy(CRT)for rectal ***:m RNA expression levels of seven molecular markers[p53,p21,Ki-67,vascular endothelial growth factor(VEGF),CD133,CD24,CD44]were measured by reverse transcriptase polymerase chain reaction(RTPCR)in 120 rectal *** findings of clinical complete remission(c CR)and biologic variables were used to construct nomogram in the training group(n=80),which was validated in the validation group(n=40).Results:m RNA expression levels of four markers(p53,p21,Ki67,CD133)correlated with p CR(24/80,30.0%)in the training *** expression of p53 and/or high expression of p21,Ki67 and CD133 showed greater p CR rate.p CR was shown in 18(69.2%)of 26 cases showing endoscopic c CR in the training *** p CR rate was demonstrated in lower tumor location than middle tumor(19/49,38.8%vs.5/31,16.1%).A nomogram for prediction of p CR was developed from the multivariate prediction model using these six variables,which showed good discrimination ability in the training group[area under the curve(AUC)=0.945]and validation group(AUC=0.922).The calibration plot showed good agreement between actual and predicted p CR in both patient ***:Nomogram for assessment of p CR can be useful for making treatment decisions after CRT according to predicted responses.

Eosinophilic gastroenteritis presenting as small bowel obstruction: A case report and review of the literature

Eosinophilic gastroenteritis is a rare disease of unknown etiology. It is characterized by eosinophilic infiltration of the bowel wall to a variable depth and symptoms associated with gastrointestinal tract. Recently, the authors experienced a case of eosinophilic gastroenteritis presenting as small bowel obstruction. A 51-year old woman was admitted to our hospital complaining of abdominal pain and vomiting. Physical examination revealed a distended abdomen with diffuse tenderness. Complete blood count showed mild leukocytosis without eosinophilia. Computed tomography confirmed a dilatation of the small intestine with ascites. An emergency laparotomy was performed for a diagnosis of peritonitis due to intestinal obstruction. Segmental resection of the ileum and end to end anastomosis were performed. Histologically, there was a dense infiltration of eosinophils throughout the entire thickness of ileal wall and eosinophilic enteritis was diagnosed. The patient recovered well, and was free from gastrointestinal symptoms at the time when we reported her disease.

Vaporization reduction characteristics of aqueous ammonia solutions by the addition of ethylene glycol, glycerol and glycine to the CO_2 absorption process

Aqueous ammonia （NH3） solution can be used as an alternative absorption for the control of CO2 emitted from flue gases due to its high absorption capacity, fast absorption rate and low corrosion problem. The emission of CO2 from iron and steel plants requires much attention, as they are higher than those emitted from power plants at a single point source. In the present work, low concentration ammonia liquor, 9 wt.%, was used with various additives to obtain the kinetic properties using the blast furnace gas model. Although a solution with a high ammonia concentration enables high CO2 absorption efficiency, ammonium ions are lost as ammonia vapor, resulting in reduced CO2 absorption due to the lower concentration of the ammonia absorbent. To decrease the vaporization of ammonia, ethylene glycol, glycerol and glycine, which contain more than one hydroxyl radical, were chosen. The experiments were conducted at 313 K similar to the CO2 absorption conditions for the blast furnace gas model.
