A nomogram to preoperatively predict 1-year disease-specific survival in resected pancreatic cancer following neoadjuvant chemoradiation therapy

Objective: This study aimed to develop a nomogram to predict the 1-year survival of patients with pancreatic cancer who underwent pancreatectomy following neoadjuvant treatment with preoperatively detectable clinical parameters. Extended pancreatectomy is necessary to achieve complete tumor removal in borderline resectable and locally advanced pancreatic cancer. However, it increases postoperative morbidity and mortality rates, and should be balanced with potential benefit of long-term survival.Methods: The medical records of patients who underwent pancreatectomy following neoadjuvant treatment from January 2005 to December 2016 at Severance Hospital were retrospectively reviewed. Medical records were collected from five international institutions from Japan and Singapore for external validation.Results: A total of 113 patients were enrolled. The nomogram for predicting 1-year disease-specific survival was created based on 5 clinically detectable preoperative parameters as follows: age(year), symptom(no/yes), tumor size at initial diagnostic stage(cm), preoperative serum carbohydrate antigen(CA) 19-9 level after neoadjuvant treatment(<34/≥34 U/m L), and planned surgery [pancreaticoduodenectomy(PD)(pylorus-preserving PD)/distal pancreatectomy(DP)/total pancreatectomy]. Model performance was assessed for discrimination and calibration.The calibration plot showed good agreement between actual and predicted survival probabilities;the the Greenwood-Nam-D’Agostino(GND) goodness-of-fit test showed that the model was well calibrated(χ~2=8.24,P=0.5099). A total of 84 patients were used for external validation. When correlating actual disease-specific survival and calculated 1-year disease-specific survival, there were significance differences according to the calculated probability of 1-year survival among the three groups(P=0.044).Conclusions: The developed nomogram had quite acceptable accuracy and clinical feasibility in the decision-making process for the management of pancreatic cancer.

Non-invasive prediction of non-alcoholic steatohepatitis in Japanese patients with morbid obesity by artificial intelligence using rule extraction technology

AIM To construct a non-invasive prediction algorithm for predicting non-alcoholic steatohepatitis(NASH), we investigated Japanese morbidly obese patients using artificial intelligence with rule extraction technology.METHODS Consecutive patients who required bariatric surgery underwent a liver biopsy during the operation. Standard clinical, anthropometric, biochemical measurements were used as parameters to predict NASH and were analyzed using rule extraction technology. One hundred and two patients, including 79 NASH and 23 non-NASH patients were analyzed in order to create the predictionmodel, another cohort with 77 patients including 65 NASH and 12 non-NASH patients were analyzed to validate the algorithm.RESULTS Alanine aminotransferase, C-reactive protein, homeostasis model assessment insulin resistance, albumin were extracted as predictors of NASH using a recursive-rule extraction algorithm. When we adopted the extracted rules for the validation cohort using a highly accurate rule extraction algorithm, the predictive accuracy was 79.2%. The positive predictive value, negative predictive value,sensitivity and specificity were 88.9%, 35.7%, 86.2% and 41.7%, respectively.CONCLUSION We successfully generated a useful model for predicting NASH in Japanese morbidly obese patients based on their biochemical profile using a rule extraction algorithm.

Deployment of the Content-Based Switching Network

In this paper, we propose a hybrid network architecture, called Content-based Switching Network (CSN), and its signaling scheme, which addresses the issues inherent to conventional hybrid networks which implement a horizontal separation over the entire network (from edge to edge). We will show how CSN nodes can flexibly choose their switching paradigm (store-and-forward, optical bypass, electrical bypass) during a path establishment. Contents being transferred in one piece from end-to-end, the concept of packet can be eluded in our network, and, in particular, the user is able to avoid complicated transport layer functions, like TCP, if they are not essential. In CSN, very large contents have a special status, since they cannot be store-and-forwarded. We will show how the resource management has been designed in order to deal with such contents. A section is dedicated to deployment and feasibility issues. Simulation results will show that CSN can successfully transfer contents at 1 Gbps and 10 Gbps, the maximum speed being limited by the state-of-the-art device technologies when buffering is required (memory speed), while no major limit is observed in the case of all-optical transfers other than the optical fiber speed. Other results concern the deployment of CSN from an unclean slate approach. They will show how beneficial can be the deployment of CSN from an Optical Circuit Switching network.

Performance Analysis of an Inverse Notch Filter and Its Application to <i>F<sub>0</sub></i>Estimation

In this paper, we analyze an inverse notch filter and present its application to F0 (fundamental frequency) estimation. The inverse notch filter is a narrow band pass filter and it has an infinite impulse response. We derive the explicit forms for the impulse response and the sum of squared impulse response. Based on the analysis result, we derive a normalized inverse notch filter whose pass band area is identical to unit. As an application of the normalized inverse notch filter, we propose an F0 estimation method for a musical sound. The F0 estimation method is achieved by connecting the normalized inverse notch filters in parallel. Estimation results show that the proposed F0 estimation method effectively detects F0s for piano sounds in a mid-range.

A Simple and Rapid Method for the Detection of Non-Ionic Surfactants

A spectrophotometric approach for the detection of non-ionic surfactant (Triton X-100) has been proposed in this paper. This method does not involve extraction of the ion-associate with harmful solvents, but employs adhesion of the ion-association of potassium/non-ionic surfactants complex and tetraphenylporphyrin tetrasulfonic acid obtained by vigorous shaking. The adhered ion-associate was dissolved with water and its absorbance was measured. The sensitivity for Triton X-100 was determined to be 0.146 (expressed as absorbance of 1 mg/L solution). The adhesion tendency of ion-associate was found to be dependent on the water contact angle, which in turn was influenced by a high adhesion of the ion-associate and by low blank values. In this respect, a tetrafluo-roethylene vessel was found to be the most suitable for the detection of non-ionic surfactants. This spectrophotometrical method is simply and rapidly performed by a procedure based on mechanical shaking and can be employed to detect non-ionic surfactants containing more than 7 polyethylene oxide units.

Hepatocellular carcinoma(HCC) is one of the most common cancers and the third highest cause of cancerassociated mortality worldwide. The treatment of HCC is complicated by its variable biological behavior and the frequent coexistence of chronic liver disease, particularly cirrhosis. To date, multiple treatment modalities have been developed according to the stage of the tumor and the hepatic functional reserve, including transarterial treatments such as transarterial chemoembolization, transarterial oily chemoembolization(TOCE), and hepatic arterial infusion chemotherapy(HAIC). We conducted a phase I and II study of the combination therapy with double platinum agents, miriplatin and cisplatin, and confirmed its safety and efficacy. Here, we describe two cases of unresectable HCC who were successfully treated by miriplatin-TOCE/cisplatin-HAIC combination therapy, resulting in complete responses with no significant adverse events. This report will provide that the combination therapy can be the therapeutic option for HCC patients in the advanced stage.

Contrast enhanced ultrasound of hepatocellular carcinoma

Sonazoid(Daiichi Sankyo,Tokyo,Japan),a secondgeneration of a lipid-stabilized suspension of a perfluorobutane gas microbubble contrast agent,has been used clinically in patients with liver tumors and for harmonic gray-scale ultrasonography(US)in Japan since January 2007.Sonazoid-enhanced US has two phases of contrast enhancement:vascular and late.In the late phase of Sonazoid-enhanced US,we scanned the whole liver using this modality at a low mechanical index(MI)without destroying the microbubbles, and this method allows detection of small viable hepatocellular carcinoma(HCC)lesions which cannot be detected by conventional US as perfusion defects in the late phase.Re-injection of Sonazoid into an HCC lesion which previously showed a perfusion defect in the late phase is useful for confirming blood flow intothe defects.High MI intermittent imaging at 2 frames per second in the late phase is also helpful in differentiation between necrosis and viable hypervascular HCC lesions.Sonazoid-enhanced US by the coded harmonic angio mode at a high MI not only allows clear observation of tumor vessels and tumor enhancement, but also permits automatic scanning with Sonazoidenhanced three dimensional(3D)US.Fusion images combining US with contrast-enhanced CT or contrastenhanced MRI have made it easy to detect typical or atypical HCC lesions.By these methods,Sonazoidenhanced US can characterize liver tumors,grade HCC lesions histologically,recognize HCC dedifferentiation, evaluate the efficacy of ablation therapy or transcatheter arterial embolization,and guide ablation therapy for unresectable HCC.This article reviews the current developments and applications of Sonazoid-enhanced US and Sonazoid-enhanced 3D US for diagnosing and treating hepatic lesions,especially HCC.

Physical multimorbidity patterns and depressive symptoms:a nationwide cross-sectional study in Japan

Objective The associations of physical multimorbidity with depressive symptoms have been investigated in a number of studies.However,whether patterns of chronic physical conditions have comparatively different associations with depressive symptoms remains unclear.This study aimed to investigate the associations of physical multimorbidity patterns with depressive symptoms.Design This study was designed as a nationwide cross-sectional survey in Japan.setting General sample of the Japanese population.Participants Adult Japanese residents were selected by a quota sampling method.Data were analysed from 1788 residents who reported one or more chronic health conditions.results Among four physical multimorbidity patterns:cardiovascular-renal metabolic(CRM),skeletal-articular digestive(SAD),respiratory-dermal(RDE)and malignant-digestive urologic(MDU),multivariable logistic regression analyses revealed that the RDE pattern showed the strongest association with depressive symptoms(aOR=1.68,95%CI:1.21 to 2.31 for the pattern score highest quartile,compared with the lowest quartile),followed by SAD and MDU patterns(aOR=1.41,95%CI:1.01 to 1.98 for the SAD pattern score highest quartile;1.41,95%CI:1.01 to 1.96 for the MDU pattern score highest quartile,compared with the lowest quartile).In contrast,the CRM pattern score was not significantly associated with depressive symptoms(aOR=1.31,95%CI:0.90 to 1.89 for the pattern score highest quartile,compared with the lowest quartile).Conclusions Physical multimorbidity patterns have different associations with depressive symptoms.Among these patterns,patients with the RDE pattern may be at a higher risk for developing depressive symptoms.This study reinforces the evidence that cluster pattern of chronic health conditions is a useful measure for clinical management of multimorbidity as it is differently associated with mental health status,which is one of the crucial outcomes for multimorbid patients.

为了通过调控氮化硅陶瓷中β-Si_3N_4柱状晶形貌以得到兼具高热导率和高强度的陶瓷,以不同β-Si_3N4_/α-Si_3N_4配比粉末为起始原料,添加Y2O3和MgO为烧结助剂,采用放电等离子烧结技术(SPS)和后续高温热处理制备烧结致密的氮化硅陶瓷。结果表明:随着β-Si_3N_4粉末含量的增加,烧结后异常长大β-Si_3N_4柱状晶减少,试样抗弯强度下降,同时β-Si3N4柱状晶平均长径比的减小使得晶粒堆积密度减小,柱状晶体积分数响应增加,晶间相含量减少,热导率提高。
