Pancreatic cancer(PC)is a potentially systemic disease with more diverse biology than generally considered.At present,surgical resection is the only curative therapy available for patients with PC,and its indications have long been discussed based on anatomical resectability.A recent consensus recommends neoadjuvant therapy(NAT)for anatomically borderline resectable(BR)cancers(1)as well as induction therapy for unresectable locally advanced(UR-LA)or metastatic(UR-M)cancers.

Platelets are anucleate fragments mainly involved in hemostasis and thrombosis,and there is emerging evidence that platelets have other nonhemostatic potentials in inflammation,angiogenesis,regeneration and ischemia/reperfusion injury(I/R injury),which are involved in the physiological and pathological processes during living donor liver transplantation(LDLT).LDLT is sometimes associated with impaired regeneration and severe I/R injury,leading to postoperative complications and decreased patient survival.Recent studies have suggested that perioperative thrombocytopenia is associated with poor graft regeneration and postoperative morbidity in the short and long term after LDLT.Although it is not fully understood whether thrombocytopenia is the cause or result,increasing platelet counts are frequently suggested to improve posttransplant outcomes in clinical studies.Based on rodent experiments,previous studies have identified that platelets stimulate liver regeneration after partial hepatectomy.However,the role of platelets in LDLT is controversial,as platelets are supposed to aggravate I/R injury in the liver.Recently,a rat model of partial liver transplantation(LT)was used to demonstrate that thrombopoietin-induced thrombocytosis prior to surgery accelerated graft regeneration and improved the survival rate after transplantation.It was clarified that platelet-derived liver regeneration outweighed the associated risk of I/R injury after partial LT.Clinical strategies to increase perioperative platelet counts,such as thrombopoietin,thrombopoietin receptor agonist and platelet transfusion,may improve graft regeneration and survival after LDLT.

以屠宰日本北海道野生梅花鹿（Cervus nippon Temminck）卵巢为材料，研究了卵巢不同保存温度（20～25℃，10～15℃）和时间（12h，24h）及卵母细胞周围的卵丘细胞层对卵母细胞体外成熟的影响。结果表明，卵巢在20～25℃下保存12h，卵母细胞成熟率为77％，无变形卵子出现，但其保存时间延长到24h时，卵母细胞成熟率降至35％，变形率高达62％；而在10～15℃下保存12h，卵母细胞成熟率为45％，卵母细胞变形率为18％，但10～15℃保存24h与20～25℃保存24h相比，卵母细胞变形率却很低（35％，62％）（P〈0.05）。周围卵丘细胞层3层以上和少于3层的卵母细胞中，48％～53％的受精卵都能正常地卵裂，但前者获得了13％的囊胚率，后者未发育至囊胚（P〈0．05）。

CONSPECTUS:Recently,supramolecular polymers(SPs),which depend on the formation of reversible bonds between monomers,have attracted attention as a class of materials that can overcome the potential environmental problems of conventional polymeric materials.The development of various supramolecular polymers is important not only for their direct application as materials but also because their unique polymerization processes and dynamic properties have a profound influence on the development of polymeric and small-molecule-based functional materials.In other words,by limiting the dimensionality of molecular assemblies to one dimension,we can discover the essential aspects of selfassembly and self-organization.In particular,supramolecular polymerization,in whichπ-electron-rich planar molecules are stacked one-dimensionally via multiple interactions,is interesting because it gives“π-stacked SPs”with one-dimensional crystal-like order.The excellent intrinsic(internal)structural order ofπ-stacked SPs is difficult to observe in higher-order structures that exhibit only a monotonous linear structure.However,our research has shown that when internal structural order intrinsically generates curvature,the curvature appears as a major feature of the entire polymer chain.This intrinsic curvature allows the formation of diverse topologies such as toroids,random coils,waves,spirals,and helicoids usingπ-stacked SPs and also allows structural changes at the single-polymer-chain level along a topology-dependent energy landscape.In other words,π-stacked SP-based nanomaterials could enable free control of the structure at the single-polymer-chain level.This Account will focus on our research into curved supramolecular polymers(CSPs)during the last five years.CSPs are formed by the hydrogen bonding between barbituratecontainingπ-conjugated molecules to produce giant supramolecular monomers.We first give an overview of the molecular design of the monomers required for the formation of CSPs,the unique energy landscape of CSPs,and their

Exercise is an effective strategy to prevent and improve obesity and related metabolic diseases.Exercise increases the metabolic demand in the body.Although many of the metabolic health benefits of exercise depend on skeletal muscle adaptations,exercise exerts many of its metabolic effects through the liver,adipose tissue,and pancreas.Therefore,exercise is the physiological state in which inter-organ signaling is most important.By contrast,circadian rhythms in mammals are associated with the regulation of several physiological and biological functions,including body temperature,sleep-wake cycle,physical activity,hormone secretion,and metabolism,which are controlled by clock genes.Glucose and lipid tolerance reportedly exhibit diurnal variations,being lower in the evening than in the morning.Therefore,the effects of exercise on substrate metabolism at different times of the day may differ.In this review,the importance of exercise timing considerations will be outlined,incorporating a chrono-exercise perspective.

BACKGROUND There has been an increasing number of elderly patients with intraductal papillary mucinous neoplasm(IPMN),who are surgically intolerant and require less invasive treatment options,which are limited.In the present study,we report a case of IPMN presenting with acute recurrent pancreatitis(ARP),in which radiation therapy effectively prevented further attacks of ARP and reduced tumor volume.CASE SUMMARY An 83-year-old man was referred to our hospital with an asymptomatic incidental pancreatic cyst.Endoscopic ultrasound imaging and magnetic resonance cholangiopancreatography revealed a multiloculated tumor in the head of the pancreas,with dilated pancreatic ducts and mural nodules.The patient was diagnosed with mixed-type IPMN,and five years later,he developed ARP.Several endoscopic pancreatic ductal balloon dilatations failed to prevent further ARP attacks.Surgery was considered clinically inappropriate because of his old age and comorbidities.He was referred to our department for radiation therapy targeted at those lesions causing intraductal hypertension and radiation was administered at a dose of 50 Gy.An magnetic resonance imaging scan taken ten weeks after treatment revealed a decrease in tumor size and improvement of pancreatic duct dilatation.Fourteen months later,he remains symptom-free from ARP.CONCLUSION This case highlights the important role of radiation therapy in mitigating the signs and symptoms of ARP in patients with inoperable IPMN.

BACKGROUND Osteochondroma is one of the most common benign bone tumors,and it may cause bone and joint deformities and limited range of motion of an adjacent joint.The pes anserinus region is one of the most frequent sites of osteochondroma,but knee locking caused by osteochondromas in the pes anserinus region is extremely rare.CASE SUMMARY We describe a 13-year-old Japanese girl’s extra-articular knee locking that occurred when the semitendinosus tendon got caught in osteochondroma that had developed in the pes anserinus region.The osteochondroma was surgically resected.The postoperative outcome has been excellent,with no recurrence of knee locking or tumor one-year post-surgery.CONCLUSION When a young person develops knee locking,the possibility of extra-articular as well as intra-articular locking should be considered.Osteochondroma,one of the causes of extra-articular locking,can be treated with surgery with good postoperative results.

AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after gastrectomy.METHODS The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by MRA.RESULTS The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL. Long-term climate changes related with urbanization in Tokyo, Japan, and recent temperature and heavy rainfall distribution in the Tokyo metropolitan area are reviewed. A relatively high temperature increase in annual mean temperature at the rate of 3.0°C/century was detected in Tokyo for the period 1901–2015. Some observational evidence showed the existence of both thermal and mechanical effects of urbanization on recent heavy rainfall occurrences, and modeling studies also support precipitation enhancement. Urban influences were recognized in other climatological elements, such as number of fog days, relative humidity, and wind circulation.
