It is estimated that 30% of the adult population in Japan is affected by nonalcoholic fatty liver disease (NAFLD). Fatty changes of the liver are generally diagnosed using imaging methods such as abdominal ultrasonography (US) and computed tomography (CT), but the sensitivity of these imaging techniques is low in cases of mild steatosis. Alanine aminotransferase levels may be normal in some of these patients, warranting the necessity to establish a set of parameters useful for detecting NAFLD, and the more severe form of the disease, nonalcoholic steatohepatitis (NASH). Although liver biopsy is currently the gold standard for diagnosing progressive NASH, it has many drawbacks, such as sampling error, cost, and risk of complications. Furthermore, it is not realistic to perform liver biopsies on all NAFLD patients. Diagnosis of NASH using various biomarkers, scoring systems and imaging methods, such as elastography, has recently been attempted. The NAFIC score, calculated from the levels of ferritin, fasting insulin, and type IV collagen 7S, is useful for the diagnosis of NASH, while the NAFLD fibrosis score and the FIB-4 index are useful for excluding NASH in cases of advanced fibrosis. This article reviews the limitations and merits of liver biopsy and noninvasive diagnostic tests in the diagnosis of NAFLD/NASH.

Portal vein tumor thrombus (PVTT) frequently occurs with the progression of hepatocellular carcinoma (HCC) and is an important factor in determining the prognosis of *** many cases of HCC with advanced PVTT,treatment is difficult because the tumor has considerable extension into the liver,and portal hypertension is a frequent *** standard therapy for unresectable HCC with advanced PVTT is sorafenib therapy in patients with good hepatic ***,the outcomes of sorafenib therapy are not completely satisfactory,making the development of another therapy an urgent ***,this review aims to summarize non-operative treatments for HCC with advanced PVTT and discuss future perspectives based on those therapies,including therapies still being developed.

Along with the changes in our food culture and lifestyle,conditions such as obesity,diabetes mellitus,and metabolicsyndrome have been on the rise,and the incidence of nonalcoholic fatty liver disease(NAFLD),which is closelyrelated to these diseases,has also increased *** being a risk factor for the development of hepatocellularcarcinoma(HCC),NAFLD has no established screening method,and HCC originating from NAFLD often tends to be discovered in its advanced and symptomatic stages,which has become an important clinical *** though the carcinogenicity rate among the entire population of NAFLD patients is not high compared to that of patients with viral hepatitis,since HCC also often develops from non-cirrhotic livers,it is difficult to narrow down the cases that need to be under *** forward,it will be important to clarify the clinical characteristics and genetic background of NAFLD-related HCC and establish not only a useful surveillance method but also preventive methods.

High Effective FE Simulation Methods for Deformation and Residual Stress by Butt Welding of Thin Steel Plates

In order to propose high effective simulation using finite element method (FEM) for predicting deformation and residual stress generated by one pass butt welding, a series of numerical analyses were carried out. By idealizing the movement of heat source (the instantaneous heat input method), the tendency of welding out-of-plane deformation and the residual stress distribution could be predicted. The computing time was around 9% of that by the precise model with considering the movement of heat source. On the other hand, applicability of two dimensional shell elements instead of generally used three dimensional solid elements was examined. The heat input model with considering the temperature distribution in the thickness direction was proposed for the simulation by using the shell elements. It was confirmed that the welding out-of-plane deformation and residual stress could be predicted with high accuracy by the model with shell elements and the distributed heat input methods. The computing time was around 8% of that by the precise model with solid elements.

AIM To assess the safety and feasibility of laparoscopic and endoscopic co-operative surgery(LECS) for early nonampullary duodenal *** Twelve patients with a non-ampullary duodenal tumor underwent LECS at our hospital. One patient had two mucosal lesions in the duodenum. The indication for this procedure was the presence of duodenal tumors with a low risk for lymph node metastasis. In particular, the tumors included small(less than 10 mm) submucosal tumors(SMT) and epithelial mucosal tumors, such as mucosal cancers or large mucosal adenomas with malignant suspicion. The LECS procedures, such as full-thickness dissection for SMT and laparoscopic reinforcement after endoscopic submucosal dissection(ESD) for epithelial tumors, were performed for the 13 early duodenal lesions in 12 patients. Here we present the short-term outcomes and evaluate the safety and feasibility of this new *** Two SMT-like lesions and eleven superficial epithelial tumor-like lesions were observed. Seven and Six lesions were located in the second and third parts of the duodenum, respectively. All lesions were successfully resected en bloc. The defect in the duodenal wall was manually sutured after resection of the duodenal SMT. For epithelial duodenal tumors, the ulcer bed was laparoscopically reinforced via manual suturing after ESD. Intraoperative perforation occurred in two out of eleven epithelial tumor-like lesions during ESD; however, they were successfully laparoscopically repaired. The median operative time and intraoperative estimated blood loss were 322 min and 0 mL, respectively. Histological examination of the tumors revealed one adenoma with moderate atypia, ten adenocarcinomas, and two neuroendocrine tumors. No severe postoperative complications(Clavien-Dindo classification grade Ⅲ or higher) were reported in this series, but minor leakage secondary to pancreatic fistula occurred in one *** LECS can be a safe and minimally invasive treatment

Autoimmune hepatitis(AIH) is a necroinflammatory liver disease of unknown *** disease is characterized histologically by interface hepatitis,biochemically by increased aspartate aminotransferase and alanine aminotransferase levels,and serologically by increased autoantibodies and immunoglobulin G *** we discuss AIH in a previously healthy 37-year-old male with highly elevated serum levels of soluble interleukin-2 receptor and markedly enlarged hepatoduodenal ligament lymph nodes(HLLNs,diameter,50 mm).Based on these observations,the differential diagnoses were AIH,lymphoma,or Castleman's *** biopsy revealed the features of interface hepatitis without bridging fibrosis along with plasma cell infiltration which is the typical characteristics of acute *** node biopsy revealed lymphoid follicles with inflammatory lymphocytic infiltration;immunohistochemical examination excluded the presence of lymphoma ***,he was administered corticosteroid therapy:after 2 mo,the enlarged liver reached an almost normal size and the enlarged HLLNs reduced in *** could not find AIH cases with such enlarged lymph nodes(diameter,50 mm) in our literature ***,we speculate that markedly enlarged lymph nodes observed in our patient may be caused by a highly activated,humoral immune response in AIH.

＜正＞ 1. IntroductionIn Japan, the base-isolated seismic design has been adopted widely after Hyogoken-Nanbu Earthquake in 1995. Specifically, the base-isolated rubber bearings are widely used, and further demand is expected in the atomic power plants, buildings, and civil structures. However, long-term deterioration characteristics of base-isolated rubber bearings have not yet been known. The reason for this is that there are little basic data of deterioration characteristics of them. So, it is unknown that mechanical performances of the deteriorated base-isolated rubber bearings. This problem should be solved because long-life structures are required.

AIM To investigate daclatasvir(DCV) and asunaprevir(ASV) efficacy in hepatitis C(HCV) patients, with respect to resistance-associated substitutions(RASs).METHODS A total of 392 HCV-infected patients from multiple centers were included in this study. We evaluated their clinical courses and sustained virologic responses(SVR) according to pretreatment factors(gender, age, history of interferon-based regimens, platelet counts, level of viremia, pretreatment NA5A:L31, and Y93 substitutions). We also analyzed the pretreatment and post-treatment major RASs of NS3:D168, NS5A:L31 and Y93 substitutions using a direct-sequencing method in 17 patients who were unable to achieve SVR at 12 wk after treatment completion(SVR12).RESULTS The overall SVR12 rate was 88.3%. Thirty-one patients discontinued treatment before 24 wk because of adverse events, 23 of whom achieved SVR12. There were no significant differences in SVR12 rates with respect to gender, age, history of interferon-based regimens, and platelet counts. The SVR12 rate in patients with viral loads of ≥ 6.0 log IU/m L was significantly lower than those with viral loads of < 6.0 log IU/m L(P < 0.001). The SVR12 rate in patients with Y93 substitution-positive was significantly lower than those with Y93 substitution-negative(P < 0.001). The L31 substitution-positive group showed a lower SVR12 rate than the L31 substitution-negative group, but the difference was not statistically significant. Seventeen patients who did not achieve SVR12 and had available pretreatment and post-treatment sera had additional RASs in NS3:D168, NS5:L31, and Y93 substitution at treatment *** Combination of DCV and ASV is associated with a high SVR rate. Baseline RASs should be thoroughly assessed to avoid additional RASs after treatment failure.

AIM:To identify the factors associated with overall survival of elderly patients with hepatocellular carcinoma(HCC).METHODS:A total of 286 patients with HCC(male/female:178/108,age:46-100 years),who were diagnosed and treated by appropriate therapeutic procedures between January 2000 and December 2010,were enrolled in this *** were stratified into two groups on the basis of age:Elderly(≥ 75 years old) and non-elderly(< 75 years old).Baseline clinical characteristics as well as cumulative survival rates were then compared between the two *** and multivariate analyses were used to identify the factors associated with prolonged overall survival of patients in each *** survival rates in the two groups were calculated separately for each modified Japan Integrated Stage score(mJIS score) category by the Kaplan-Meier *** addition,we compared the cumulative survival rates of elderly and non-elderly patients with good hepatic reserve capacity(≤ 2 points as per mJIS).RESULTS:In the elderly group,the proportion of female patients,patients with absence of hepatitis B or hepatitis C viral infection,and patients with coexisting extrahepatic comorbid illness was higher(56.8% vs 31.1%,P < 0.001;27.0% vs 16.0%,P = 0.038;33.8% vs 22.2%,P = 0.047;respectively) than that in the nonelderly *** the non-elderly group,the proportion of hepatitis B virus(HBV)-infected patients was higher than that in the elderly group(9.4% vs 0%,P = 0.006).The cumulative survival rates in the elderly group were 53.7% at 3 years and 32.9% at 5 years,which were equivalent to those in the non-elderly group(55.9% and 39.4%,respectively),as shown by a log-rank test(P = 0.601).In multivariate analysis,prolonged survival was significantly associated with the extent of liver damage and stage(P < 0.001 and P < 0.001,respectively),but was not associated with patient ***,on individual evaluation of factors in both groups,stage was significantly(P < 0.001) associ
