Influence of cognitive impairment on cardiac mortality after percutaneous coronary intervention in very elderly patients: a retrospective observational study

Background Cognitive impairment (CI) increases cardiac mortality among very elderly patients. Percutaneous coronary intervention (PCI) for ischemic heart disease (IHD) patients is considered a favorable strategy for decreasing cardiac mortality. Here, we investigated the influence of CI on cardiac mortality after PCI in very elderly patients. Methods We performed a retrospective observational analysis of patients who received PCI between 2012 and 2014 at the South Miyagi Medical Center, Japan. IHD patients over 80 years old who underwent the Mini-Mental State Examination for CI screening during hospitalization and/or who had been diagnosed with CI were included. Participants were divided into CI and non-CI groups, and cardiac mortality and incidence of adverse cardiac events in a 3-year follow-up period were compared between groups. Statistical analyses were performed using the t-test,χ^2 test, and multivariable Cox regression analysis, with major comorbid illness and conventional cardiac risk factors as confounders. Results Of 565 patients, 95 were included (41 CI, 54 non-CI). Cardiac mortality during the follow-up period was significantly higher in the CI group (36%) compared with the non-CI group (13%)(OR = 4.3, 95% CI: 1.56–11.82, P < 0.05). CI was an independent cardiac prognostic factor after PCI and, for CI patients, living only with a CI partner was an independent predictor of cardiac death within three years. Conclusions CI significantly affected cardiac prognosis after PCI in very elderly patients, particularly those living with a CI partner. To improve patients’ prognoses, social background should be considered alongside conventional medical measures.

Comparison of Pine Needles and Mosses as Bio-Indicators for Polycyclic Aromatic Hydrocarbons

Polycyclic aromatic hydrocarbons (PAHs) are ubiquitous environmental contaminants known to be hazardous to human health. Pine needles and mosses are useful bio-indicators for assessing PAH pollutions;however, the differences in their PAH uptake mechanisms have not been sufficiently discussed. In this study, the properties of pine needles and mosses as bio-indicators of PAHs were investigated on the basis of differences in their PAH profiles. Five sets each of pine needle and moss samples were collected from circular sampling plots and analyzed for 16 PAHs. A comparison of PAH profiles revealed that the proportion of lower molecular weight PAHs (2 - 3 aromatic rings;LMW PAHs) was significantly higher in pine needles (78.5% ± 4.8%) than in mosses (35.4% ± 6.8%). In contrast, the proportion of higher molecular weight PAHs (5 - 6 aromatic rings;HMW PAHs) was lower in pine needles (4.3% ± 2.9%) than in mosses (25.1% ± 3.3%). Further, the combination of PAH isomer ratios showed that PAH sources between pine needles and mosses were not the same. These differences were explained by their uptake mechanisms and partly by the absorption of PAHs from soil particles by mosses. These findings indicate that pine needles are useful for assessing airborne LMW PAH pollution, whereas mosses can be integrated indicators for assessing complex HMW PAH pollution of the atmospheric and soil environments. On the basis of these properties, the usefulness of these bio-indicators should also be evaluated according to the objective of the assessment and the areas where they are applied.

Structural changes concurrent with ferromagnetic transition

Ferromagnetic transition has generally been considered to involve only an ordering of magnetic moment with no change in the host crystal structure or symmetry, as evidenced by a wealth of crystal structure data from conventional X-ray diffractometry （XRD）. However, the existence of magnetostriction in all known ferromagnetic systems indicates that the magnetic moment is coupled to the crystal lattice; hence there is a possibility that magnetic ordering may cause a change in crystal structure. With the development of high-resolution synchrotron XRD, more and more magnetic transitions have been found to be accompanied by simultaneous structural changes. In this article, we review our recent progress in understand- ing the structural change at a ferromagnetic transition, including synchrotron XRD evidence of structural changes at the ferromagnetic transition, a phenomenological theory of crystal structure changes accompanying ferromagnetic transitions, new insight into magnetic morphotropic phase boundaries （MPB） and so on. Two intriguing implications of non-centric symmetry in the ferromagnetic phase and the first-order nature of ferromagnetic transition are also discussed here. In short, this review is intended to give a self-consistent and logical account of structural change occurring simultaneously with a ferromagnetic transition, which may provide new insight for developing highly magneto-responsive materials.

Functional layer engineering to improve performance of protonic ceramic fuel cells

Protonic ceramic fuel cells(PCFCs)have been attracting increasing attention because of their advances in high-efficiency power generation in an intermediate-temperature range,as compared to the high-temperature solid oxide fuel cells(SOFCs).The greatest difference between PCFCs and SOFCs is the specific requirement of protonic(H+)conductivity at the PCFC cathode,in addition to the electronic(e^(-))and oxide-ion(O^(2-))*** development of a triple H^(+)/e^(-)/O^(2-)conductor for PCFC cathode is still ***,the most-widely used cathode material is based on the mature e^(-)/O^(2-)***,this leads to insufficient triple phase boundary(TPB),i.e.,reaction ***,an efficient strategy that uses a~100 nm-thick proton conductive functional layer(La_(0.5)Sr_(0.5)CoO_(3-δ),LSC55)in-between the typical La_(0.8)Sr_(0.2)CoO_(3-δ)cathode(a mature e-/O^(2-)conductor,LS C 82)and B aZr_(0.4)Ce_(0.4)Y_(0.1)Yb_(0.)1O_(3-δ)elec trolyte(11 mm in diameter,20μm in thickness)is proposed to significantly enhance the reaction ***,the ohmic resistance and polarization resistance are both decreased by 47%and 62%,respectively,compared with that of PCFCs without the functional *** power density of the PCFC with such a functional layer can be raised by up to 2.24 times,superior to those described in previous *** enhanced PCFC performances are attributed to the well-built TPB and enhanced reaction area via the functional layer engineering strategy.

Triple pelvic osteotomy: Report of our mid-term results and review of literature

A wide variety of pelvic osteotomies have been developed for the treatment of developmental dysplasia of the hip(DDH). In the present paper, we present a detailed review of previous studies of triple osteotomy as an alternative treatment for DDH. We also report our experience treating 6 adult cases of DDH by triple osteotomy in order to highlight the various aspects of this *** mean age of our patients was 31.2 years with a mean follow-up period of 6 years. We assessed range of motion, center-edge angle, acetabular index angle, Sharp angle, acetabulum head index, head lateralization index, Japanese Orthopedic Association score, Harris hip score, patient satisfaction, and the difference between lower limb lengths before and after the procedure. At final follow-up, clinical scores were significantly improved and radiographic parameters also showed good correction of acetabulum.

Intussusception due to rectal adenocarcinoma in a young adult:A case report

An intussusception due to colonic adenocarcinoma has sometimes been reported. However, to the best of our knowledge, reports of intussusception due to rectal adenocarcinoma are extremely rare. In this report, the case of a young man with rectal adenocarcinoma causing intussusception is described. A 24-year-old man visited a hospital complaining of abdominal pain, and an upper rectal cancer was diagnosed by colonoscopy. Computed tomography showed intussusception caused by a large tumor in the pelvis and absence of distant metastases. Locally advanced rectal cancer causing intussusception was diagnosed, and a low anterior resection was performed. Intraoperatively, repair of the invagination could not be accomplished easily; therefore, the repair was abandoned. Instead, the tumor was removed en bloc to avoid dissemination of the cancer. Histopathologically, the tumor was diagnosed as a poorly differentiated adenocarcinoma, pStage IIA. The patient has no evidence of recurrence at 10 mo after the operation.

Orthodontic tooth separation activates the hypothalamic area in the human brain

OBJECTIVES： An animal experiment clarified that insertion of an orthodontic apparatus activated the trigeminal neurons of the medulla oblongata. Orthodontic tooth movement is known to be associated with the sympathetic nervous system and controlled by the nucleus of the hypothalamus. However, the transmission of both has not been demonstrated in humans. The purpose of this study were to examine the activated cerebral areas using brain functional magnetic resonance imaging（MRI）, when orthodontic tooth separators were inserted, and to confirm the possibility of the transmission route from the medulla oblongata to the ***： Two types of alternative orthodontic tooth separators（brass contact gauge and floss） were inserted into the right upper premolars of 10 healthy volunteers. Brain functional T2＊-weighted images and anatomical T1-weighted images were ***： The blood oxygenation level dependent（BOLD） signals following insertion of a brass contact gauge and floss significantly increased in the somatosensory association cortex and hypothalamic ***： Our findings suggest the possibility of a transmission route from the medulla oblongata to the hypothalamus.
