Relationship between testicular volume and testicular function： comparison of the Prader orchidometric and ultrasonographic measurements in patients with infertility

Aim： To evaluate the relationship between testicular function and testicular volume measured by using Prader orchidometry and ultrasonography （US） to determine the critical testicular volume indicating normal testicular function by each method. Methods： Total testicular volume （right plus left testicular volume） was measured in 794 testes in 397 men with infertility （mean age, 35.6 years） using a Prader orchidometer and also by ultrasonography. Ultrasonographic testicular volumes were calculated as length x width x height x 0.71. To evaluate volume-function relationships, patients were divided into 10 groups representing 5-mL increments of total testicular volume by each method from below 10 mL to 50 mL or more. Results： Mean total testicular volume based on Prader orchidometry and US were 36.8 mL and 26.3 mL, respectively. Semen volume, sperm density, total sperm count, total motile sperm count, and serum FSH, LH, and testosterone all correlated significantly with total testicular volume measured by either method. Mean sperm density was in the oligozoospermic range in patients with total testicular volume below 35 mL by orchidometry or below 20 mL by ultrasonography. Mean total sperm count was subnormal in patients with total testicular volume below 30 mL by orchidometry or under 20 mL by ultrasonography. Conclusion： Testicular volume measured by either ultrasonography or Prader orchidometry correlated significantly with testicular function. However, critical total testicular volume indicating normal or nearly normal testicular function was 30 mL to 35 mL using Prader orchidometer and 20 mL using ultrasonography. Hemophagocytic Lymphohistiocytosis Caused by Pyogenic Liver Abscess during Pregnancy: A Case Report and Literature Review

Introduction: Hemophagocytic lymphohistiocytosis during pregnancy is a rare and severe condition, and timely diagnosis is quite difficult. We present the first case of hemophagocytic lymphohistiocytosis caused by pyogenic liver abscess during pregnancy and discuss the clinical presentation. Case Presentation: A 26-year-old Japanese primigravida at 23 weeks of gestation complained of extremely high fever with a fast heart rate (140 beats per minute). She presented with systemic inflammatory response syndrome (SIRS). Only 2 days later, she died. Autopsy findings proved that this was the first case of hemophagocytic lymphohistiocytosis caused by pyogenic liver abscess during pregnancy. Conclusion: Hemophagocytic lymphohistiocytosis should be considered when patients meet the SIRS criteria, especially whose clinical presentation includes extremely high fever (39°C) and a fast heart rate (greater than 110 bpm) during pregnancy, despite relatively normal laboratory data, because such vital signs may be associated with the onset of hemophagocytic lymphohistiocytosis.

目的找出可反映宫颈癌亚型不同组织结构的;F-脱氧葡萄糖（FDG）PET/CT纹理特征,并对宫颈癌中;F-FDG PET纹理特征的关联性进行视觉评估。方法纳入83例在治疗前行;FFDG PET/CT检查的宫颈癌患者,其中62例鳞状细胞癌（SCC）、21例非鳞状细胞癌（NSCC）。对PET/CT图像进行纹理分析,从中提取出18个PET放射组学特征,包括标准摄取值（SUV）、肿瘤代谢体积（MTV）和病灶糖酵解总量（TLG）等一级特征,二级以及高级结构特征分别为SUV直方图、归一化灰度共生矩阵（NGLCM）和局部灰度差分矩阵。使用Bonferroni校正P值0.002 8（0.05/18）来比较SCC和NSCC间这些特征的差异。用层次聚类的热图分析方法来评估PET特征之间的关联性。结果在18个PET特征中,相关性（NGLCM的二级纹理特征）是稳定的参数,并且是SCC和NSCC之间差异有统计学意义的唯一特征。SCC的相关性（0.70±0.07）显著高于NSCC（0.64±0.07,P=0.003 0）。其他PET特征在SCC和NSCC间无明显差异。与NSCC相比,SCC的高度相关性可能反映出癌细胞的结构完整性和更强的空间/线性关系。对PET特征进行热图分析,清晰显示出5个不同的群集,其中相关性与MTV、TLG属于一个群集,但相关性与他们的关联不强。相关性是宫颈癌相对独立的PET特征。结论;F-FDG PET纹理特征可能反映宫颈癌亚型间组织学结构差异。PET放射组学的方法揭示了PET特征间的关联性,并有助于发现单一特征或这些特征的组合,从而便于精确诊断、确定潜在的预后模型和有效的治疗策略。

High-strain-rate superplasticity in oxide ceramics:a trial of microstructural design based on creep-cavitation mechanisms

From existing knowledge about high-temperature cavitation mechanisms, necessary conditions were discussed for the suppression of cavitation failure during superplastic deformation in ceramic materials. The discussion, where special attention was placed on the relaxation of stress concentrations during grain-boundary sliding and cavity nucleation and growth, leaded to a conclusion that cavitation failure could be retarded by the simultaneous controlling of the initial grain size, the number of residual defects, diffusivity, dynamic grain growth and the homogeneity of microstructure. On the basis of this conclusion, high-strain-rate superplasticity （defined as superplasticity at a strain rate higher than 0.01 s^-1） could be intentionally attained in some oxide ceramic materials. This was shown in tetragonal zirconia and composites consisting of zirconia, a-alumina and a spinel phase.

Stochastic Process Optimization Technique

The conventional optimization methods were generally based on a deterministic approach, since their purpose is to find out an accurate solution. However, when the solution space is extremely narrowed as a result of setting many inequality constraints, an ingenious scheme based on experience may be needed. Similarly, parameters must be adjusted with solution search algorithms when nonlinearity of the problem is strong, because the risk of falling into local solution is high. Thus, we here propose a new method in which the optimization problem is replaced with stochastic process based on path integral techniques used in quantum mechanics and an approximate value of optimal solution is calculated as an expected value instead of accurate value. It was checked through some optimization problems that this method using stochastic process is effective. We call this new optimization method “stochastic process optimization technique (SPOT)”. It is expected that this method will enable efficient optimization by avoiding the above difficulties. In this report, a new optimization method based on a stochastic process is formulated, and several calculation examples are shown to prove its effectiveness as a method to obtain approximate solution for optimization problems.

Endometrial neuroendocrine tumors are rare, accounting less than 1% of endometrial cancers. They include small cell neuroendocrine carcinoma (NEC) and large cell NEC and usually occur in postmenopausal patients. Although common symptoms include postmenopausal bleeding, most patients are diagnosed at an advanced stage. We report an extremely rare case of small cell NEC developed in the endometrium of a 75-year-old Japanese woman. This case was initially suspected based on the findings of endometrial cytology: a number of small round malignant cells were present on the endometrial smear specimens. The immunocytochemical and immunohistochemical examinations revealed diffusely cytoplasmic positive reaction of neuron specific enolase (NSE) and partially membranous positive reaction of CD56 in the small cancer cells. They showed PAX-8-positive reaction, but did not express microsatellite instability high. Electron micrography showed several dense-core secretory granules in the cytoplasm of cancer cells. Despite multiple lung metastases, the patient underwent a hysterectomy and salpingo-oophorectomy in order to control excessive genital bleeding. She received six courses of adjuvant chemotherapy based on etoposide and cisplatin, and survived healthy eight months after the first visit without any viability of lung metastases.
