＜正＞ Uncured, filled rubbers show remarkable nonlinear viscoelasticity as well as cured, filled rubbers. The nonlinearity may come from change in entanglement and filler network structures. Many people use dynamic modulus to characterize rubber materials. However, dynamic modulus cannot be defined at large strain amplitude. Hence we must study a viscoelastic function to be defined at large strain. In addition, we need another information to separate the effects of the change in entanglement and filler network structures on nonlinear viscoelasticity. In this work, we have measured relaxation modulus G（γ,t） and electrical resistivity p（γ,t） simultaneously for carbon black （CB）-filled, uncured SBRs in a wide range of strain. Electrical resistivity at equilibrium, p（0,t）, showed step-off like change at the threshold of CB loading based on particle size. The threshold of N330 CB exists between 20 and 35phr, indicating threshold for filler network formation should exist in the range of values in CB loading. Both G（γ,t） and p（γ,t） for the samples having CB loading to be higher than the threshold showed nonlinearity at the strain larger than shear strain γ=0.1, indicating rupture in filler network at large strain. The threshold shifted to lower CB content for fine CB（N220）, and no threshold was observed for coarse CB（N660） in the range measured from 20 to 50phr, indicating that the threshold depends on primary particle size. Moreover it has been found that filler network structures rupture easily when the CB content close to the threshold. Furthermore, recovery rate in filler network has been found to be very slow in comparison with that in entanglement structure.

Oral esomeprazole vs injectable omeprazole for the prevention of hemorrhage after endoscopic submucosal dissection

AIM To evaluate the effectiveness of oral esomeprazole(EPZ) vs injectable omeprazole(OPZ) therapy to prevent hemorrhage after endoscopic submucosal dissection(ESD).METHODS A case-control study was conducted using a quasirandomized analysis with propensity score matching. A total of 258 patients were enrolled in this study. Patients were treated with either oral EPZ or injectable OPZ. The endpoint was the incidence of hemorrhage after *** Data of 71 subjects treated with oral EPZ and 172 subjects treated with injectable OPZ were analyzed. Analysis of 65 matched samples revealed no difference in the incidence of hemorrhage after ESD between the oral EPZ and injectable OPZ groups(OR = 0.89, 95%CI:0.35-2.27, P ≥ 0.99).CONCLUSION We conclude that oral EPZ therapy is a useful alternative to injectable PPI therapy for the prevention of hemorrhage after ESD.

Synergistic Antibacterial Performance of a Cu/WO<sub>3</sub>-Added PTFE Particulate Superhydrophobic Composite under Visible-Light Exposure

Addition of TiO2 to a polytetrafluoroethylene (PTFE) particle-dispersed composite contributes to the self-cleaning properties of the water-repellent composite. However, its application is limited to outdoor usage or under ultraviolet (UV) irradiation. In this study, a novel visible-light-sensitive photocatalytic and superhydrophobic material was developed by adding Cu/WO3 to a PTFE particulate composite material to overcome this deficit. A remarkable property of this novel composite material is the synergistic antibacterial performance against Escherichia coli (E. coli), Staphylo-coccus aureus, and methicillin-resistant Staphylococcus aureus compared with the addition of Cu/WO3 without PTFE particles material. During 24-h exposure in visible light at 2000 lx, the number of viable cells of the three strains on the surface of the 8wt% Cu/WO3-added PTFE particulate composite decreased from 2 – 4 × 105 colony-formation units (CFUs) to less than 10, the limit of detection. This bactericidal rate is four times higher than that of 8wt% Cu/WO3 without PTFE particles material, which is attributed to the air trapped in the rough surface of the novel material providing additional oxygen to the photocatalytic reaction. Even for exposure to visible light at 100 lx, the decrease in CFUs of E. coli on the 12wt% Cu/WO3-added PTFE particulate composite reached nearly 2.0 logs. The characterization of the Cu/WO3-added PTFE particulate composite indicated that the composite material containing 80wt% PTFE maintained a superhydrophobic or water-repellent property with a water contact angle >150。, although the Cu/WO3 in the composite material remained hydrophilic under visible light. The Cu/WO3-added PTFE particulate composite displayed photo-catalytic reactions to decompose oleic acid adsorbed on its surface and gaseous acetaldehyde under UV-A and visible-light illumination. Given that amplitude probability distribution(APD) is an effective means of estimating the effects of electromagnetic disturbance on digital communications,we are now testing low-cost equipment for measuring APD at high resolution. The equipment combines multiple M-sequence counters having relatively prime periods and makes use of residue number systems(RNS) so that most circuits can be formed on one IC chip and a low-cost configuration can be achieved. In addition,the size of the discrete logarithm table here, which is essential to restoring the enumerated value from the bit pattern output of M-sequence counters,need be only 10 the size of a single M-sequence *** therefore arranging n units of this APD measuring equipment into a parallel configuration,processing speed can be increased by n times.

Blood Pressure Balance as a Marker for Early Detection of Cerebral Infarction and a Criterion for Inclusion/Exclusion from Acupuncture Treatment

Background: Early detection of cerebral infarction (CI) by home acupuncturists might be helpful in reducing stroke mortality. We compared vital signs of patients who developed CI and those who did not to ascertain if blood pressure balance is a useful marker for the early detection of CI and for distinguishing cases with potentially effective/ineffective acupuncture treatment. Methods: In this retrospective study, vital signs such as body temperature, pulse, and blood pressure, were simultaneously obtained from the right and left arms of patients who received weekly acupuncture treatment. We focused on the differences between values obtained from the right and left arms (the balance) and compared the values of patients who developed CI [CI (+)] (n = 18) and those of age-/gender-matched patients who did not [CI (−)] (n = 18). Results: Left/right blood pressure was significantly more unbalanced in the CI (+) group than in the CI (−) group, especially immediately before the onset of CI. Moreover, all CIs occurred on the side with a lower blood pressure. Discussion and Conclusions: Blood pressure balance might aid in the early detection of CI and to determine whether treatment with traditional or modern medicine is more appropriate. Acupuncture may thus help maintain patients’ quality of life.

Photoluminescence and Optical Transition Dynamics of Er^(3+) Ions in Porous Si

Er was doped into porous Si by immersing the porous Si sample in a saturated ErCl3,ethanol solution. Sharp and intense 1.54 μm photoluminescence caused by intra-4f-shefl transitions in Er3+ ions was observed up to room temperature. It is shown that the immersing process is valid to dope Er in high concentration in porous Si. Time resolved study of the Er-doped porous Si revealed that the doped Er3+ ions are excited by energy transfer from photo-generated electron-hole pairs in the host. The energy back transfer process from the excited 4f electrons in the Er3+ion to the host is not a dominant factor to quench the Er-related emission in porous Si. Ourresults are well explained by a proposed model in which an intermediate state was introduced.

Role of LIGHT in the pathogenesis of joint destruction in rheumatoid arthritis

AIM To characterise the role of substitutes for receptoractivator nuclear factor kappa-B ligand(RANKL) in rheumatoid arthritis(RA) joint destruction. METHODS Synovial fluid(SF) macrophages isolated from the knee joint of RA patients were incubated with 25 ng/m L macrophage-colony stimulating factor(M-CSF) and 50 ng/mL LIGHT(lymphotoxin-like, exhibits inducible expression and competes with herpes simplex virus glycoprotein D for herpes virus entry mediator, a receptor expressed by T lymphocytes) in the presence and absence of 25 ng/mL RANKL and 100 ng/mL osteoprotegerin(OPG) on glass coverslips and dentine slices. Osteoclastogenesis was assessed by the formation of multinucleated cells(MNCs) expressing tartrate-resistant acid phosphatase(TRAP) on coverslips and the extent of lacunar resorption pit formation on dentine slices. The concentration of LIGHT in RA and osteoarthritis(OA) synovial fluid was measured by an enzyme-linked immunosorbent assay(ELISA) and the expression of LIGHT in RA and OA synovium was determined by immunohistochemistry using an indirect immunoperoxidase technique. RESULTS In cultures of RA SF macrophages treated with LIGHT and M-CSF, there was significant formation of TRAP + MNCs on coverslips and extensive lacunar resorption pit formation on dentine slices. SF-macrophage-osteoclast differentiation was not inhibited by the addition of OPG, a decoy receptor for RANKL. Resorption pits were smaller and less confluent than in RANKL-treated cultures but the overall percentage area of the dentine slice resorbed was comparable in LIGHT-and RANKL-treated cultures. LIGHT significantly stimulated RANKL-induced lacunar resorption compared with RA SF macrophages treated with either RANKL or LIGHT alone. Suspected Anaphylactic Shock Associated with Rocuronium in an Infant: A Case Report

We report a case of severe anaphylactic shock in a 5-month-old infant who was scheduled to undergo an external inguinal hernia repair under general anesthesia. Rocuronium used for anesthesia induction was the most likely cause of anaphylaxis. High levels of serum tryptase and histamine detected in the blood sample collected during the anaphylactic reaction confirmed the diagnosis of anaphylactic shock. The patient’s clinical status improved within 90 min of intervention by the intravenous injection of vasopressors and a steroid. Surgery was canceled, and the patient stayed in the pediatric intensive care unit (PICU) under artificial ventilation for 5 h before safe extubation. The patient achieved full recovery the next day, without any sequelae. The rescheduled surgery was successfully completed 5 months later under general anesthesia without the use of neuromuscular blocking agents.

The Association between Self-Monitored Reproductive Parameters, Pregnancy Outcomes and Acupuncture Treatment

Many women track their daily basal body temperature (BBT), an important factor in reproductive status, as a part of fertility self-management. This study involved 135 patients who had undergone conventional infertility treatment;they had weekly acupuncture treatment and we assessed BBT, menstrual cycle, menstrual period and menstrual bleeding amount pre- and post-treatment. We also analyzed the rate of insufficient description of reproductive parameters, comparing patients who had delivered a baby with those who had a miscarriage and those who did not become pregnant. Menstrual cycle and period did not show notable changes pre- and post-acupuncture treatment in all groups. Menstrual bleeding amount decreased in the no-pregnancy group. There was a suggestion that acupuncture decreased very long menstrual cycle (e.g. bimonthly) to monthly. The insufficient description rate of menstrual bleeding amount was high in all groups. Weekly discussion about monitoring reproductive parameters could improve patients’ awareness in self-management. To prevent miscarriage patients needed psychological and medical advice for careful self-management in pregnancy and menstrual cycle.
