Clinical significance of hypoechoic submandibular gland lesions in type 1 autoimmune pancreatitis

To assess the role of ultrasonography of submandibular glands (SGs) in the diagnosis of type 1 autoimmune pancreatitis (AIP).METHODSThirty-seven patients who were definitively diagnosed with type 1 AIP according to the international consensus diagnostic criteria (ICDC) for AIP at our institution between December 1990 and April 2016 were retrospectively reviewed. Findings by physical examination, ultrasonography, and scintigraphy of SGs were analyzed to reach a diagnosis based on the ICDC for AIP. The efficacy of corticosteroid treatment in the resolution of hypoechoic lesions in SGs was also evaluated by assessment with ultrasonography before and after treatment in 18 *** sensitivity of multiple hypoechoic lesions in SGs by ultrasonography for the diagnosis of sialadenitis in type 1 AIP (84%) was higher than that of physical examination (46%), scintigraphy (28%), and SGs thickness (49%). Ultrasonographic evidence of hypoechoic lesions in SGs improved the definitive diagnosis of sialadenitis and type 1 AIP by the ICDC criteria in 11 (30%) and 2 (5.4%) cases, respectively. Multiple hypoechoic lesions in SGs were resolved or disappear by corticosteroid administration in 14 of 16 cases with hypoechoic lesions in SGs, whereas the ultrasonographic findings in the remaining 2 cases with hypoechoic lesions in SGs and the 2 cases with homogenous SG parenchyma remained unchanged after corticosteroid *** ultrasonography to detect multiple hypoechoic lesions might be useful for type 1 AIP diagnosis by improving diagnostic accuracy together with the ICDC sialadenitis criteria.

Risk factors for perforation during endoscopic retrograde cholangiopancreatography in post-reconstruction intestinal tract

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP) in patients with surgically altered anatomy has been a major challenge to gastrointestinal endoscopists with low success rates for reaching the target site as well as high complication rates. The knowledge of ERCP-related risk factors is important for reducing unexpected *** To identify ERCP-related risk factors for perforation in patients with surgically altered *** The medical records of 187 patients with surgically altered anatomy who underwent ERCP at our institution between April 2009 and December 2017 were retrospectively reviewed. An analysis of patient data, including age, sex, type of reconstruction, cause of surgery, aim of ERCP, success rate of reaching target site,success rate of procedure, adverse events, type of scope, time to reach the target site, and duration of procedure, was performed. In patients with Billroth-Ⅱ reconstruction, additional potential risk factors were the shape of the inserted scope and whether the anastomosis was antecolic or *** All patients(n = 187) had surgical anatomy, such as Billroth-Ⅰ(n = 22), Billroth-Ⅱ(n = 33), Roux-en-Y(n = 54), Child, or Whipple reconstruction(n = 75). ERCP was performed for biliary drainage in 43 cases(23%), stone removal in 29 cases(16%),and stricture dilation of anastomosis in 59 cases(32%). The scope was unable to reach the target site in 17 cases(9%), and an aimed procedure could not be accomplished in 54 cases(29%). Adverse events were pancreatitis(3%),hyperamylasemia(10%), cholangitis(6%), cholestasis(4%), excessive sedation(1%), perforation(2%), and others(3%). Effects of General Anesthesia on the Results of Cardiac Catheterization in Pediatric Patients with Ventricular Septal Defect

Polymer LB films containing photofuntional groups were prepared by the copolymerization of N-dodecylacrylamide (DDA), which has an excellent property to form a stable monolayer and LB multilayerswith photofunctional monomers. Tris(2, 2'-bipyridine) ruthenium complex, Ru(bpy)_3^(2+), one of the most well-known redox-active sensitizer, was incorporated into the DDA copolymer. The photogalvanic effect based onthe photoinduced electron transfer using the ruthenium complex in the polymer LB monolayer was discussed.

Natural selection in vertebrate evolution under genomic and biosphere biases based on amino acid content: Primitive vertebrate hagfish (<i>Eptatretus burgeri</i>)

Cluster analyses using the amino acid content predicted from the coding regions (13 genes) of complete vertebrate mitochondrial genomes as traits grouped selected vertebrates into two clusters, terrestrial and aquatic vertebrates. Exceptions were the hagfish (Eptatretus burgeri), thought to be an early ancestor of vertebrates, and the black spotted frog (Rana nigromaculata), which is terrestrial as an adult and aquatic as a larva. These two species fall into the terrestrial and aquatic clusters, respectively. Using the nucleotide (G, C, T and A) content in the coding and non-coding regions, and in the complete genome as traits, similar results were obtained but with some additional exceptions. In addition, phylogenetic analyses of 16S rRNA sequences produced a consistent result. The results of this study indicated that vertebrate evolution is controlled by natural selection under both an internal bias as a result of nucleotide replacement genomic rules, and an external bias caused by environmental biospheric conditions.

Evidence for the independent divergence of vertebrate and high C/G ratio invertebrate mitochondria from the same origin

The examination of organelle nucleotide contents across a range of groups can provide insight into evolutionary history. We compared the nucleotide contents of a large number of vertebrate and high C/G ratio invertebrate mitochondria in both coding/non-coding regions and complete strands. For most nucleotides tested, high regression coefficients were obtained;when plotted with lines of best fit, G and T contents formed a cuneiform, and C and A contents overlapped. These findings suggest that vertebrate and high C/G ratio invertebrate mitochondria descended independently from the same origin.

Laparoscopic Choledochoduodenostomy for Biliary Alleviation

Choledochoduodenostomy (CDD) was very useful alternatives for treatment for patients with common bile duct (CBD) stones, especially recurrent stones, giant stones with choledochal dilatation, and difficult or failed cases by endoscopic treatment. Furthermore, CDD was also applied to biliary bypass by malignant obstruction. In these days, minimal invasive laparoscopic approach is adapted in these disorders. We have conducted to perform a laparoscopic CDD for biliary alleviation for patients with endoscopic management of difficult CBD stone with choledochal dilatation. A side-to-side CDD was created intra-corporeally using water-tight running absorbable suture by handmade 4-0 monofilament with double side needles, starting from the right side of choledochus, and continued along posterior wall until the left side of the choledochus followed by anterior-wall anastomosis as the same manner. Five patients were treated successfully by this laparoscopic procedure and remained well without bile leakage and reflux cholangitis for the short-term follow-up. The median operative time and intracorporeally anastomosis time were 182 (167 - 209) min and 33 (30 - 38) min, respectively. Median blood loss was 32 (little-90) ml, median hospital stay was 7 (5 - 14) days, and median follow-up time was 18 months. Although this series was relatively small, this laparoscopic technique is feasible and safe for biliary alleviation, especially for endoscopic management of difficult or failed CBD stones, and would also potentially adapt to biliary bypass by malignant obstruction.

Molecular Phylogenetic Analysis of Chemosymbiotic Solemyidae and Thyasiridae

In order to invade and adapt to deep-sea environments, shallow-water organisms have to acquire tolerance to high hydrostatic pressure, low water temperature, toxic methane and hydrogen sulfide, and feeding strategies not relying on photosynthetic products. Our previous study showed that the “evolutionary stepping stone hypothe-sis”, which assumes that organic falls can act as stepping-stones to connect shallow sea with deep sea, was supported in Mytilidae. However, it is not known whether other bivalves constituting chemosynthetic communities experienced the same evolutionary process or different processes from mytilid mussels. Therefore, here, we performed phylogenetic analyses by sequencing the nuclear 18S rRNA and mitochondrial COI genes of solemyid and thyasirid bivalves. In Solemyidae, the two genera Solemya and Acharax formed each clade, the latter of which was divided into three subgroups. The Solemya clade and one of the Acharax subgroups diverged in the order of shallow-sea residents, whale-bone residents, and deep-sea vent/seep residents, which supported the “evolutionary stepping stone hypothesis”. Furthermore, in Thyasiridae, the two genera Thyasira and Maorithyas formed a paraphyletic group and the other genera, Adontorhina, Axinopsis, Axinulus, Leptaxinus, and Mendicula, formed a clade. The “evolu-tionary stepping stone hypothesis” was not seemingly supported in the other lineages of Solemyidae and Thyasiridae.

Dispersal Ability and Genetic Structure in Mytilid Mussels of Whale-Fall Communities

Since organic falls are ephemeral and distributed sporadically, organisms in organic-fall communities must acquire high dispersal ability to migrate from one organic fall to another. However, the dispersal ability of obligate organic-fall organisms has not been investigated thoroughly and the stability of the genetic structure of their communities is unknown. In this study, in order to elucidate the dispersal ability and genetic structure in the organic-fall communities, we carried out population genetic analyses based on sequences of mitochondrial NADH dehydrogenase subunit 4 in two mytilid mussels.?Adipicola pacifica was obtained from whale and cow bones artificially settled in Japanese waters off Cape Noma (CN) and in the Nansei-Shoto Trench (NS) and Sagami Bay (SB), and Benthomodiolus geikotsucola from natural whale bones in the Torishima Seamount (TS);both species are symbiotic with chemoautotrophic bacteria. Genetic differentiation (Fst) indicated almost no annual change in genetic structure between 2003, 2004, 2005, and 2007 collections of A. pacificafrom CN (depth 225 - 229 m), although the 2010 collection had somewhat different genetic structure from the others. Similarly, there was not significant genetic differentiation between 1993 and 2005 collections of *** from TS (depth 4020 m). The Fst and gene bidirectional mean rate of gene flow (Nm) indicated high gene flow and no significant genetic differentiation between A. pacifica specimens collected from CN, NS, and SB. The results suggest that the genetic structure is stable and A. pacifica has high dispersal ability. The mismatch distribution suggests that A. pacificaexpanded their distribution from SB to NS via CN, as expansion time (τ = 2ut) decreased from SB to CN and NS.
