Plasma copeptin concentration is a predictor of tolvaptan efficacy in patients with hepatic ascites

Aims:No solid evidence-based opinion was raised regarding predictors of the degree of ascites reduction with *** retrospective cohort study aimed to examine whether serum copeptin concentration is a useful predictor of ***:The study population consisted of 80 patients with liver cirrhosis treated with tolvaptan for hepatic ascites effusions at Nara Medical University Hospital from May 2014 to December ***-three patients who lost>1.5 kg of body weight in the first week after starting tolvaptan were classified as good responders and the remaining 37 as poor *** laboratory parameters were measured immediately before the start of tolvaptan therapy to examine factors associated with predicting its ***:In the univariate analysis,no significant differences with respect to age(67.6 vs.69.8 years,p>0.05),sex,body mass index(24.8 vs.23.7 kg/m^(2),p>0.05),Child-Pugh score(9.4 vs.9.7,p>0.05),and Model for End-stage Liver Disease score(11 vs.12,p>0.05)were found between the two ***,aspartate transferase and alanine transaminase(ALT)levels were significantly lower in the good response group(52.9±56.3 vs.68.8±50.7 U/L,p<0.05;26.2±30.6 vs.40.5±33.5 U/L,p<0.01),whereas serum copeptin and serum sodium concentrations were significantly higher(57.1±15.0 vs.45.8±16.0 pg/mL,p<0.01;136.3±3.4 vs.133.8±5.8 mEq/L,p<0.05).In the multivariate analysis,serum copeptin concentration and ALT were statistically significant factors(p<0.01,p<0.05).Regression analysis of the association between the tolvaptan efficacy for refractory ascites and pretreatment serum copeptin concentration showed that a copeptin concentration cutoff of 45.9 pg/mL could predict treatment efficacy with a sensitivity,specificity,and area under the curve of 76.7%,59.5%,and 0.71%,***:Serum copeptin concentration may be a predictor of tolvaptan efficacy for refractory ascites effusion in Japanese patients with liver cirrhosis.

BACKGROUND Angioimmunoblastic T-cell lymphoma(AITL)is a peripheral T-cell lymphoma,which is a rare subtype of *** with AITL often have skin lesions,which are observed in 50%of all cases;the chief complaint of this patient was palpable *** often complicates autoimmune or hematological disorders;however,among these,pure red cell aplasia(PRCA)is a very rare complication of *** herein report a case of AITL with *** SUMMARY A 77-year-old Japanese man presented to our hospital with complaints of loss of appetite for 2 mo and a 10-d history of palpable *** physical examination,the patient was afebrile but had bilateral multiple palpable purpuric lesions over the lower extremities,lower abdomen,and part of the upper ***,lymphadenopathy of the bilateral inguinal,cervical,and supraclavicular nodes was *** and imaging studies and skin biopsy were conducted but were *** on inguinal lymph node excisional biopsy,we diagnosed the patient with ***,the patient developed progressive normocytic normochromic anemia that necessitated almost daily blood *** clinical presentations and results of bone marrow assessment were consistent with those of PRCA,which is associated with *** was initiated but was not *** patient refused further chemotherapy and opted to continue receiving best supportive *** PRCA is an extremely rare complication of *** the pathophysiology remains unclear,further research is warranted.

Recently, novel anti-androgens and inhibitors of androgen biosynthesis have been developed through the elucidation of mechanisms of castration resistance of prostate cancer. We believe that these new developments will improve hormonal therapy. On the other hand, there has been an increase in criticism of hormonal therapy, because hormonal therapy is supposed to induce adverse effects such as cardiovascular disease. In this review, we have introduced the Japanese experience of hormonal therapy, because we believe that there may be ethnic differences between Caucasians and Asian people in the efficacy and adverse effects of hormonal therapy. First, we showed that primary hormonal therapy can achieve long-term control of localized prostate cancer in some cases and that quality of life of patients receiving hormonal therapy is rather better than previously thought. Neoadjuvant and adjuvant hormonal therapy in cases undergoing radical prostatectomy or radiotherapy are very useful for high-risk or locally advanced prostate cancer. Further clinical trials are required to confirm the efficacy of neoadjuvant or adjuvant hormonal therapy. We showed that the death from cardiovascular dis- eases in Japanese patients receiving hormonal therapy was not higher than that in the general population. However; efforts should be made to decrease the adverse effects of hormonal therapy, because life-style change may increase the susceptibility to adverse effects by hormonal therapy even in Japan. Managements of endocrine and metabolic dysfunction, such as diabetes mellitus, are essential. New hormonal compounds such as selective androgen receptor modulators capable of specifically targeting prostate cancer are expected to be developed.

Two cases of pancreatic ductal adenocarcinoma with intrapancreatic metastasis

There are no standardized diagnostic criteria for intrapancreatic metastasis of pancreatic ductal adenocarcinoma(PDAC). Here, we report two cases of patients with PDAC who were pathologically diagnosed as harboring intrapancreatic metastasis. In both cases, the main lesions were located in the pancreatic body, and no other lesion was detected preoperatively. The patients were diagnosed with pancreatic body cancers and distal pancreatectomy was performed. Pathological findings revealed microscopic cancer nests, which had connections to neither the main lesion nor the premalignant lesion in the pancreatic tail parenchyma. In both cases, the histological type of the daughter lesion was quite similar to that of the main lesion. Hence, we diagnosed the daughter lesions as metastatic foci in the pancreas. Although intrapancreatic metastasis of PDAC has been regarded as a poor prognostic factor, few reports of intrapancreatic metastasis are available. This article reports two such cases and provides a review of the literature.

INTRODUCTION Surgical procedures are considered more challenging in cirrhotic patients with hepatocellular carcinoma(HCC)than in non-cirrhotic patients,because of the former’s high morbidity and mortality following surgery.[1]In open liver surgery,the extremely long incision required for mobilization and resection of the liver can result in significant intraoperative blood loss or postoperative intractable ascites,followed by reduced collateral circulation in the abdominal wall and ligaments around the *** complications may progress to postoperative hepatic failure in some patients.

相对12C束流而言,β缓发粒子衰变放射性9C束流在其Bragg峰区附近深度对细胞的致死效应明显增强,这已经在我们先前的研究中证实.9C束流的相对生物学效应(RBE)较12C束流要大1倍以上.本研究旨在探讨造成这一重要实验现象的生物物理机理.首先,建立模型计算了用于实验时产生的9C束流的阻止沉积几率密度分布,模型考虑了初始束流的动量分布,束流随贯穿深度增加由核反应导致的离子通量衰减和能量损失岐离等效应.发现9C束流对细胞致死效应增强的区域出现在入射9C离子的阻止沉积区域.其次,以9C束流入射处剂量为1Gy为例,根据计算得到的入射9C离子阻止分布几率密度推导了9C束流在不同贯穿深度上的沉积离子密度,进而得到了不同深度上每细胞内平均沉积9C离子数;同时,由相近剂量平均传能线密度(LET)深度上9C和12C束流实验上测得的细胞存活率推导出了9C及12C束流在这些深度上导致的每细胞平均致死事件数.研究结果表明,在照射剂量相同时每细胞平均沉积9C离子数与9C及12C束流每细胞平均致死事件数之差竟相吻合.由此推测,一个沉积9C离子将造成该细胞死亡.考虑到由9C衰变而发射低能粒子的性质,9C离子细胞中沉积可能会在该细胞中造成团簇损伤,因而本研究结果可作为辐射团簇损伤高效细胞致死的间接证据.

Assessment on Energy Self-Sufficiency Rate for Building Integrated Photovoltaics and Fuel Cell System in Japan

A building integrated photovoltaic (PV) and fuel cell (FC) system is proposed for assessment of the energy self-sufficiency rate in a city in Japan. The electricity consumed in the building is mainly supplied by solar panels, while the gap between the energy demand and supply is solved by the FC that is powered by the H2 produced by water electrolysis with surplus power of PV. A desktop case study of using the proposed system in Tsu city which is located in central part of Japan, has been conducted. The results found that the self-sufficiency rates of PV system to electricity demand of households (RPV) during the daytime in April and July are higher than those in January and October. The results also reveal that the self-sufficiency rate of FC system to the electricity demand (RFC) is 15% - 38% for the day when the mean amount of horizontal solar radiation is obtained in January, April, July and October. In addition, it is found the optimum tilt angle of solar panel installed on the roof of the buildings should be 0 degree, i.e., placed horizontally.

Impact of Heat Transfer Media on Performance of Solar-Hydrogen Power Generation

Solar-hydrogen system has great potential for contributing to sustainable and clean energy supply. The aim of this study is to clarify the impact of heat transfer media in solar collector such as methane, ammonium, hydrogen, air and water on the performance of solar-hydrogen system. After estimating the highest temperature attainable by each heat transfer media, the amount of thermal energy that could be saved in the production of hydrogen or preheat for power generation by fuel cell was calculated. The power generation performance of fuel cell using each heat transfer media was also investigated. As a result, it has been revealed that the temperature changes of methane, ammonium and air follow the horizontal solar radiation intensity irrespective of seasons, and their highest temperatures are almost the same among them. The temperature response of hydrogen is slower than methane, ammonium and air. This study defines the ratio of saving thermal energy which indicates the effect of solar thermal utilization for production of hydrogen or preheat for power generation by fuel cell without using utility gas. It has been found that the biggest thermal energy saving is obtained when hydrogen and air are used as the heat transfer media. The power generated by PEFC system per effective area of evacuated tube collector in the case of using methane or ammonium is 3.309×10-2 kWh/m2 and 2.076×10-2 kWh/m2, respectively, while it is 2.466×10-2 kWh in the case of using hydrogen and air.
