Camouflage Therapy for Post-Inflammatory Hyperpigmentation on the Face Caused by Fixed Drug Eruption

Camouflage therapy has been used for permanent contour and pigmentary defects including telangiectasias, vitiligo, lentigines, nevi, atrophic scars and burn scars. The goal of the therapy is to provide new and innovative ways to normalize the appearance of patients with abnormalities. A variety of cosmetic techniques are used to assist these patients in making their irregularities as inconspicuous as possible. Post-inflammatory hyperpigmentation is a frustrating problem afflicting many dermatology patients, particularly on the face. Here we report a case of successful cosmetic camouflage using the theory of complementary colors of light in a patient with post-inflammatory hyperpigmentation of the face caused by fixed drug eruption. Our case report supports the idea that camouflage for patients with post-inflammatory hyperpigmentation on the face caused by fixed drug eruption improves their quality of life and also supports the idea that camouflage should be part of the after care for patients who have received patch testing.

Clinical Evaluation of Moisturizers with Physiological Analysis of Stratum Corneum TARC and TSLP

To verify the usefulness of moisturizers, a multi-center study was conducted on patients with atopic dermatitis who visited 3 university hospitals in Japan between November 2009 and March 2012. Thirty-seven patients with dry skin and stable symptoms who were receiving topical and oral treatments were given moisturizers for 8 weeks. The moisturizers contained components such as glycerin and seawater minerals in 3 forms: lotion, emulsion, and cream. The moisturizers were chosen by physicians based on the degree of dryness of the patients’ skin. In addition to observing the skin condition of the entire body, high-frequency conductance as a parameter of skin surface hydration and transepidermal water loss (TEWL) in the buccal region and the medial side of the forearm were measured, and those of the back were used for comparison. Furthermore, stratum corneum levels of thymic stromal lymphopoietin (TSLP) and thymus and activation-regulated chemokine (TARC), both of which were objective parameters for atopic dermatitis skin lesion severity, were analyzed using the tape stripping method. At the beginning of the study, TSLP showed a correlation with skin symptoms (dryness, itching) and high-frequency conductance, whereas TARC showed a correlation with skin symptoms (erythema, dryness, itching) and TEWL. In addition, a correlation was noted among TSLP, TARC, and itching. At the end of the study, erythema, dryness, itching, TEWL,TSLP, and TARC were significantly reduced, whereas high-frequency conductance was significantly increased. The moisturizers clearly improved the dry skin symptoms of these patients with atopic dermatitis and improved the physiology of their sensitive and damaged skin. These findings also support the involvement of cytokines/chemokines in the pathogenesis of atopic dermatitis lesions.

皮肤红斑狼疮(CLE)是一种炎症性自身免疫性疾病,其分型较多,包括急性、亚急性、慢性和间歇性CLE。其中,慢性CLE可以进一步分为红斑狼疮(LE)的几个亚型,如盘状LE、疣状LE、深在性LE、冻疮样LE和Blaschko线状LE等。为了给皮肤科医生和风湿科医生提供CLE的诊断、治疗和长期管理的实用指导建议,本证据和共识指南根据卫生保健实践指南的报告条目(国际RIGHT工作组)目录编制,并在国际实践指南注册平台注册。在亚洲皮肤科学会(ADA)、亚洲皮肤病性病学会(AADV)以及中华医学会皮肤性病学分会(CSD)红斑狼疮研究中心的共同努力下,来自亚洲、美洲、欧洲等16个国家或地区的25名皮肤科医生、7名风湿科医生、1名红斑狼疮领域专家和2名方法学家参与了本指南的制定。所有建议均取得了32名医生中至少80%的同意。作为共识,CLE的诊断主要基于临床和组织病理学表现的评估,并且通过对全身多系统损伤的评估,排除系统性红斑狼疮(SLE)。外用糖皮质激素和钙调磷酸酶抑制剂是局部CLE的一线治疗。对于泛发或严重的CLE皮损和/或局部耐药的病例,可以合并全身治疗,包括抗疟药和/或短期全身使用糖皮质激素。值得注意的是,抗疟药是所有类型CLE的一线全身治疗药物,同样可用于妊娠期患者和儿童。二线治疗用药包括沙利度胺、类维生素A类药物、氨苯砜和甲氨蝶呤,而霉酚酸酯是三线治疗用药。最后,脉冲染料激光或手术可作为四线治疗,用于局部以及发生在不可修饰部位的慢性、难治性CLE皮损。贝利尤单抗亦可作为四线治疗用药,用于活动性SLE患者的泛发性CLE皮损或糖皮质激素逐渐减量期间复发的急性CLE。对于疾病的管理,患者教育和长期随访是必要的。建议在每次随访时适当评估疾病活动度、皮肤和其他器官损伤、生活质量、合并症及可能发生的不良事件。

Endocytoscopic narrow-band imaging efficiency for evaluation of inflammatory activity in ulcerative colitis

AIM:To assess the efficacy of endocytoscopic narrowband imaging(EC-NBI)for evaluating the severity of inflammation in ulcerative colitis(UC).METHODS:This retrospective study was conducted at a single tertiary care referral *** included UC patients who underwent colonoscopy with endocytoscopy from July 2010 to December *** was performed,and the images were evaluated by assessing visibility,increased vascularization,and the increased calibers of capillaries and were classified as Obscure,Visible or *** was indicative of inactive disease,while Visible and Dilated were indicative of acute *** study received Institutional Review Board *** primary outcome measures included the diagnostic ability of EC-NBI to distinguish between active and inactive UC on the basis of histological *** conventional endoscopic images were classified according to the Mayo endoscopic score.A score of 0 or 1 indicated inactive disease,whereas a score of 2 indicated active ***:Fifty-two patients were *** was a strong correlation between the EC-NBI findings and the histological assessment(r=0.871,P<0.01).The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of EC-NBI for diagnosing acute inflammation were 84.0%,100%,87.1%,100%,and 92.3%,respectively,while those for the Mayo endoscopic score were 100%,40.7%,100%,61.0%,and 69.2%,*** with conventional endoscopy,EC-NBI was superior in diagnostic specificity,negative predictive value,and accuracy(P<0.001,P=0.001 and P=0.047,respectively).CONCLUSION:The EC-NBI finding of capillaries in the rectal mucosa was strongly correlated with histological inflammation and aided in the differential diagnosis between active and inactive UC.
