Detection accuracy for epithelial dysplasia using an objective autofluorescence visualization method based on the luminance ratio

The autofluorescence visualization method （AVM） uses blue excitation light to assist in the diagnosis of epithelial dysplasia. It detects epithelial dysplasia as a black area, which is known as fluorescence visualization loss （FVL）. In this study, we evaluated the detection accuracy for epithelial dysplasia of the tongue using the objective AVM and assessed its possible clinical utility. Seventy-nine tongue specimens clinically suspected to have leukoplakia or squamous cell carcinoma （SCC） were analyzed. First, the AVM was subjectively performed using the Visually Enhanced Lesion scope （VELscope）, and the iodine-staining method was then performed. After biopsy, the histopathological results and the luminance ratio between the lesion and healthy tissue were compared, and a receiver operating characteristic curve was created. The cutoff value for the objective AVM was determined; the lesion was considered FVL-positive or -negative when the luminance ratio was higher or lower than the cutoff value, respectively. The histopathological diagnoses among the 79 specimens were SCC （n = 30）, leukoplakia with dysplasia （n = 34）, and leukoplakia without dysplasia （n= 15）. The cutoff value of the luminance ratio was 1.62, resulting in 66 FVL-positive and 13 FVL-negative specimens. The luminance ratio was significantly higher in the epithelial dysplasia-positive than -negative group （P〈O.O00 1）. The objective AVM showed much higher consistency between histopathological results than did the two methods （kappa statistic = 0.656）. In conclusion, objective autofluorescence visualization has a potential as an auxiliary method for diagnosis of epithelial dysplasia.

Sedimentary Evolution of the Holocene Subaqueous Clinoform off the Southern Shandong Peninsula in the Western South Yellow Sea

Based on the stratigraphic sequence formed since the last glaciation and revealed by 3000 km long high-resolution shallow seismic profiles and the core QDZ03 acquired recently off the southern Shandong Peninsula, we addressed the sedimentary characteristics of a Holocene subaqueous clinoform in this paper. Integrated analyses were made on the core QDZ03, including sedimentary facies, sediment grain sizes, clay minerals, geochemistry, micro paleontology, and AMS 14 C dating. The result indicates that there exists a Holocene subaqueous clinoform, whose bottom boundary generally lies at 15–40 m below the present sea level with its depth contours roughly parallel to the coast and getting deeper seawards. The maximum thickness of the clinoform is up to 22.5 m on the coast side, and the thickness contours generally spread in a banded way along the coastline and becomes thinner towards the sea. At the mouths of some bays along the coast, the clinoform stretches in the shape of a fan and its thickness is evidently larger than that of the surrounding sediments. This clinoform came into being in the early Holocene(about 11.2 cal kyr BP) and can be divided into the lower and upper depositional units(DU 2 and DU 1, respectively). The unit DU 2, being usually less than 3 m in thickness and formed under a low sedimentation rate, is located between the bottom boundary and the Holocene maximum flooding surface(MFS), and represents the sediment of a post-glacial transgressive systems tract; whereas the unit DU 1, the main body of the clinoform, sits on the MFS, belonging to the sediment of a highstand systems tract from middle Holocene(about 7–6 cal kyr BP) to the present. The provenance of the clinoform differs from that of the typical sediments of the Yellow River and can be considered as the results of the joint contribution from both the Yellow River and the proximal coastal sediments of the Shandong Peninsula, as evidenced by the sediment geochemistry of the core. Retinal degenerative diseases （RDs） such as retinitis pigmentosa （RP） are characterized by slowly progressive photoreceptor cell death, but the molecular mechanism underlying RP remains unclear. Animal models for RP have led to a better understand- ing of the disease pathological mechanisms, yet it remains difficult to identify an appropriate genetic model for RDs in general because there are many causative genes （Rossmiller et al., 2012）.

Carbon dioxide insufflation in esophageal endoscopic submucosal dissection reduces mediastinal emphysema: A randomized, double-blind, controlled trial

AIM To assess the efficacy of CO2 insufflation for reduction of mediastinal emphysema(ME) immediately after endoscopic submucosal dissection(ESD).METHODS A total of 46 patients who were to undergo esophageal ESD were randomly assigned to receive either CO2 insufflation(CO2 group, n = 24) or air insufflation(Air group, n = 22). Computed tomography(CT) was carried out immediately after ESD and the next morning. Pain and abdominal distention were chronologically recorded using a 100-mm visual analogue scale(VAS). The volume of residual gas in the digestive tract was measured using CT imaging. RESULTS The incidence of ME immediately after ESD in the CO2 group was significantly lower than that in the Air group(17% vs 55%, P = 0.012). The incidence of ME the next morning was 8.3% vs 32% respectively(P= 0.066). There were no differences in pain scores or distention scores at any post-procedure time points. The volume of residual gas in the digestive tract immediately after ESD was significantly smaller in the CO2 group than that in the Air group(808 m L vs 1173 m L, P = 0.013).CONCLUSION CO2 insufflation during esophageal ESD significantly reduced postprocedural ME. CO2 insufflation also reduced the volume of residual gas in the digestive tract immediately after ESD, but not the VAS scores of pain and distention.
