AIM To assess the efficacy of CO2 insufflation for reduction of mediastinal emphysema(ME) immediately after endoscopic submucosal dissection(ESD).METHODS A total of 46 patients who were to undergo esophageal ESD were randomly assigned to receive either CO2 insufflation(CO2 group, n = 24) or air insufflation(Air group, n = 22). Computed tomography(CT) was carried out immediately after ESD and the next morning. Pain and abdominal distention were chronologically recorded using a 100-mm visual analogue scale(VAS). The volume of residual gas in the digestive tract was measured using CT imaging. RESULTS The incidence of ME immediately after ESD in the CO2 group was significantly lower than that in the Air group(17% vs 55%, P = 0.012). The incidence of ME the next morning was 8.3% vs 32% respectively(P= 0.066). There were no differences in pain scores or distention scores at any post-procedure time points. The volume of residual gas in the digestive tract immediately after ESD was significantly smaller in the CO2 group than that in the Air group(808 m L vs 1173 m L, P = 0.013).CONCLUSION CO2 insufflation during esophageal ESD significantly reduced postprocedural ME. CO2 insufflation also reduced the volume of residual gas in the digestive tract immediately after ESD, but not the VAS scores of pain and distention.

Environmentally sensitive grain-size component (ESGSC) extracted from grain-size data of a sediment core B2, which were retrieved from mud area southwest off Cheju Island (MACI), East China Sea (ECS), can be used to indicate the variations of East Asia Winter Monsoon (EAWM), with high (low) content/mean-size of ESGCS denote to strong (weak) EAWM. Combined with AMS14C datings core B2 provides a continuous high-resolution record of EAWM changes over the past 2300 years, with an average resolution of 13 years. The results show that the variations of EAWM are con-sistent with temperature changes inferred from historical documents in eastern China over the past 2300 years, from which four climate stages may be identified. In stages before 1900 aBP (50 AD) and 1450―780 aBP (50―1170 AD) the EAWM were comparatively weak, corresponding to warm climate periods in eastern China, respectively. And in stages of 1900―1450 aBP (50―500 AD) and 780―219 aBP (1170―1731 AD) the EAWM were strongly developed, which correspond well to climate changes of two cold periods in eastern China. It is also shown from this study that the stage at 780―219 aBP (1170―1731 AD) was the coldest climate period during the last 2300 years and could be, therefore, related to the Little Ice Age (LIA). Climatic fluctuations appeared obviously in all the four stages, and two climate events of abrupt changes from warm to cold occurred at around 1900 aBP (50 AD) and 780 aBP (1170 AD), of which the latter is probably related to globe-scale changes of atmospheric circulation at that time.

BACKGROUND Hyperplastic polyps are considered non-neoplastic, whereas sessile serrated lesions(SSLs) are precursors of cancer via the ‘‘serrated neoplastic pathway’’. The clinical features of SSLs are tumor size(> 5 mm), location in the proximal colon, coverage with abundant mucus called the ‘‘mucus cap’’, indistinct borders, and a cloud-like surface. The features in magnifying narrow-band imaging are varicose microvascular vessels and expanded crypt openings. However, accurate diagnosis is often *** To develop a diagnostic score system for *** We retrospectively reviewed consecutive patients who underwent endoscopic resection during colonoscopy at the Toyoshima endoscopy clinic. We collected data on serrated polyps diagnosed by endoscopic or pathological examination. The significant factors for the diagnosis of SSLs were assessed using logistic regression analysis. Each item that was significant in multivariate analysis was assigned 1 point, with the sum of these points defined as the endoscopic SSL diagnosis score. The optimal cut-off value of the endoscopic SSL diagnosis score was determined by receiver-operating characteristic curve *** Among 1288 polyps that were endoscopically removed, we analyzed 232 diagnosed as serrated polyps by endoscopic or pathological examination. In the univariate analysis, the location(proximal colon), size(> 5 mm), mucus cap, indistinct borders, cloud-like surface, and varicose microvascular vessels were significantly associated with the diagnosis of SSLs. In the multivariate analysis, size(> 5 mm;P = 0.033), mucus cap(P = 0.005), and indistinct borders(P = 0.033) were independently associated with the diagnosis of SSLs. Endoscopic submucosal dissection(ESD) has become widely accepted as a standard method of treatment for superficial gastrointestinal neoplasms because it enables en block resection even for large lesions or fibrotic lesions with minimal invasiveness, and decreases the local recurrence rate. Moreover, specimens resected in an en block fashion enable accurate histological assessment. Taking these factors into consideration, ESD seems to be more advantageous than conventional endoscopic mucosal resection(EMR), but the associated risks of perioperative adverse events are higher than in EMR. Bleeding after ESD is the most frequent among these adverse events. Although post-ESD bleeding can be controlled by endoscopic hemostasis in most cases, it may lead to serious conditions including hemorrhagic shock. Even with preventive methods including administration of acid secretion inhibitors and preventive hemostasis, post-ESD bleeding cannot be completely prevented. In addition high-risk cases for post-ESD bleeding, which include cases with the use of antithrombotic agents or which require large resection, are increasing. Although there have been many reports about associated risk factors and methods of preventing post-ESD bleeding, many issues remain unsolved. Therefore, in this review, we have overviewed risk factors and methods of preventing post-ESD bleeding from previous studies. Endoscopists should have sufficient knowledge of these risk factors and preventive methods when performing ESD.

This study focuses on carbon emissions of the building sector in relation to local climate zone(LCZ)classification,concentrating on two major ***,we estimated carbon emissions in the building sector,which were cal-culated for weekdays and weekends real-time daily energy consumption *** estimations were divided into direct(from petroleum products consumption)and indirect emissions(from electricity consumption).Sec-ond,we examined urban carbon emissions mapping in relation to *** Metropolitan Administration(BMA)was used as the case study and 2016 as the base year for *** results illustrate that indirect emissions in Bangkok can be up to ten times higher than direct *** analysis indicates that LCZ,such as compact high-rise,large low-rise,light industry,and warehouse zones had a relatively higher carbon emission intensity than ***,we identified that the compact high-rise zone has the highest indirect emission intensity,while the light industry and warehouse zone have the greatest direct emission *** results provide insights into the dynamics of carbon emission characteristics in the building sector and the methodology purported here can be used to support low carbon city planning and policymaking in Bangkok.

Complexation of π-conjugated ligands by metal or semimetal ions leads to the enhancement of the planarity and rigidity of π-conjugated systems. Boron, especially, has played a central role in the design of luminescent main-group complexes. However, these complexes still suffer the disadvantage of aggregation-caused quenching as well as typical organic fluorophores. It has recently been reported that some types of boron complexes exhibit the aggregation-induced emission(AIE) property. Moreover, AIE behavior from complexes and organometallic compounds composed of the other group 13 elements, such as aluminum and gallium, has emerged in this decade. These observations greatly encourage us to develop advanced functional materials based on the group 13 elements. Indeed, recent research has demonstrated that these classes of materials are potentially versatile scaffolds for constructing chromic luminophores,efficiently emissive π-conjugated polymers and so on. This review mainly describes AIE-active group 13 complexes with four-coordinate structures and their application as photo-functional materials. Proposed mechanisms of the origins of AIE behavior are briefly discussed.

Retinal degenerative diseases （RDs） such as retinitis pigmentosa （RP） are characterized by slowly progressive photoreceptor cell death, but the molecular mechanism underlying RP remains unclear. Animal models for RP have led to a better understand- ing of the disease pathological mechanisms, yet it remains difficult to identify an appropriate genetic model for RDs in general because there are many causative genes （Rossmiller et al., 2012）.
