Dependence of Lower Molding Temperature Limit and Molding Time on Molding Mechanism in Dental Thermoforming

Effectiveness and safety of a sports mouthguard depend on its thickness and material, and the thermoforming process affects these. The purpose of this study was to clarify the effects of differences in molding mechanisms on the lower molding temperature limit and molding time in dental thermoforming. Ethylene vinyl acetate resin mouthguard sheet and two thermoforming machines;vacuum blower molding machine and vacuum ejector/pressure molding machine were used. The molding pressures for suction molding were −0.018 MPa for vacuum blower molding and −0.090 MPa for vacuum ejector molding, and for pressure molding was set to 0.090 MPa or 0.450 MPa. Based on the manufacturer’s standard molding temperature of 95˚C, the molding temperature was lowered in 2.5˚C increments to determine the lower molding temperature limit at which no molding defects occurred. In order to investigate the difference in molding time depending on the molding mechanism, the duration of molding pressure was adjusted in each molding machine, and the molding time required to obtain a sample without molding defects was measured. The molding time of each molding machine were compared using one-way analysis of variance. The lower molding temperature limit was 90.0˚C for the vacuum blower machine, 77.5˚C for the vacuum ejector machine, 77.5˚C for the pressure molding machine at 0.090 MPa, and 67.5˚C for the pressure molding machine at 0.45 MPa. The lower molding temperature limit was higher for lower absolute values of molding pressure. The molding time was shorter for pressure molding than for suction molding. Significant differences were observed between all conditions except between the pressure molding machine at 0.090 MPa and 0.45 MPa (P < 0.01). This study evaluates the performance and reliability of a vision transformer (ViT) compared to convolutional neural networks (CNNs) using the ResNet50 model in classifying lung cancer from CT images into four categories: lung adenocarcinoma (LUAD), lung squamous cell carcinoma (LUSC), large cell carcinoma (LULC), and normal. Although CNNs have made significant advancements in medical imaging, their limited capacity to capture long-range dependencies has led to the exploration of ViTs, which leverage self-attention mechanisms for a more comprehensive global understanding of images. The study utilized a dataset of 748 lung CT images to train both models with standardized input sizes, assessing their performance through conventional metrics—accuracy, precision, recall, F1 score, specificity, and AUC—as well as cross entropy, a novel metric for evaluating prediction uncertainty. Both models achieved similar accuracy rates (95%), with ViT demonstrating a slight edge over ResNet50 in precision and F1 scores for specific classes. However, ResNet50 exhibited higher recall for LULC, indicating fewer missed cases. Cross entropy analysis showed that the ViT model had lower average uncertainty, particularly in the LUAD, Normal, and LUSC classes, compared to ResNet50. This finding suggests that ViT predictions are generally more reliable, though ResNet50 performed better for LULC. The study underscores that accuracy alone is insufficient for model comparison, as cross entropy offers deeper insights into the reliability and confidence of model predictions. The results highlight the importance of incorporating cross entropy alongside traditional metrics for a more comprehensive evaluation of deep learning models in medical image classification, providing a nuanced understanding of their performance and reliability. Objective: The purpose of this study was to examine the effects of continuous gum chewing exercise on perioral muscle strength. Methods: Thirty healthy adults (24.1 ± 2.0 years) with normal occlusion performed gum chewing exercise 3 times daily for 3 months. Each exercise session lasted 5 min and involved alternating of chewing 10 times using the left molars and then 10 times using the right molars, with the mouth closed. The effect of the exercise on oral function was evaluated by measuring tongue pressure (TP), cheek pressure (CP), and labial closure strength (LCS) immediately before starting exercise, at 2 weeks and 1, 2, and 3 months after starting exercise, and at 3 months after cessation of exercise. Changes in TP, CP, and LCS according to sex and duration of exercise were analyzed by repeated two-way ANOVA. Results: Measurements for all muscles were significantly greater in men than in women at all time points. After starting exercise, TP was markedly increased at 2 months in men and women, and both CP and LCS were markedly increased at 2 weeks in men and at 1 month in women. These effects persisted for 3 months. Three months after cessation of exercise, TP, CP, and LCS tended to decrease, but were not significantly attenuated as compared with 3 months after beginning of exercise in either sex. Conclusion: The results of this study revealed that gum chewing exercise contributed to an improvement in perioral muscle strength, and that this effect was maintained for at least 3 months after discontinuation of exercise.

Cross entropy is a measure in machine learning and deep learning that assesses the difference between predicted and actual probability distributions. In this study, we propose cross entropy as a performance evaluation metric for image classifier models and apply it to the CT image classification of lung cancer. A convolutional neural network is employed as the deep neural network (DNN) image classifier, with the residual network (ResNet) 50 chosen as the DNN archi-tecture. The image data used comprise a lung CT image set. Two classification models are built from datasets with varying amounts of data, and lung cancer is categorized into four classes using 10-fold cross-validation. Furthermore, we employ t-distributed stochastic neighbor embedding to visually explain the data distribution after classification. Experimental results demonstrate that cross en-tropy is a highly useful metric for evaluating the reliability of image classifier models. It is noted that for a more comprehensive evaluation of model perfor-mance, combining with other evaluation metrics is considered essential. .

Corroborative Activities and Recognition Between Community Comprehensive Care Unit’s Nurses and Care Managers in Supporting Discharge of Elderly from Hospital - A Secondary Publication

Objective: The purpose of this study was to clarify the collaborative activities and mutual recognition between community comprehensive care unit nurses (Ns) and care managers (CM) in supporting the discharge of the elderly from the hospital. Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support.

AIM： To prospectively investigate the efficacy of the revised Vienna Classification for diagnosing colorectal epithelial neoplastic lesions in cold biopsy ***： Patients were selected for inclusion if they had colorectal epithelial lesions that were not considered suitable for direct endoscopic resection,These included colorectal polyps ≥ 10 mm and lesions suspected of being carcinomas capable of invading the colorectal submucosa or beyond, including strictures, based on the cold biopsies obtained from each lesion prior to resection. We investigated the relationship between diagnoses based on cold biopsy samples using the revised Vienna Classification and resected specimens of the same lesions, and the therapeutic implications of diagnoses made using the revised Vienna Classification. The same cold biopsy specimens were also examined using the Japanese Group Classification guidelines, and compared with the resected specimens of the same lesions for ***： A total of 179 lesions were identified. The sensitivity, specificity, positive and negativepredictive values of the revised Vienna Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 22.2%, 100%, 100%, and 71.4%,respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 59.7%, 100%, 100%, and 37.6%,respectively. The sensitivity, specificity, positive and negative predictive values of the Japanese Group Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 83.3%, 91.4%, 83.3%. and 91.4%, respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 95.1%, 91.4%, 97.9%, and 82.1%, respectively. A total of 137 of 144 carcinomas that had invaded the submucosa or beyond and three high-grade intraepithelial neoplasias were diagnosed as ＂carcinoma＂

Hydrophobic Val derivative Schiff base copper(II) complexes and dipeptide (AlaAla, GlyGly) derivative Schiff base copper(II) complexes were introduced into egg white lysozyme. X-ray crystal structure analysis revealed amino acid derivative Schiff base copper(II) complexes were obtained. Herein we discuss primarily on the binding mode of copper(II) of the complexes obtained with egg white lysozyme. The electron density of copper(II) ions was confirmed by X-ray crystal structure analysis. The Val derivative Schiff base copper(II) complex was weakly bound at Arg114 of egg white lysozyme. In other copper(II) complexes, binding of copper(II) ions with dissociated ligands to various residues was observed. The binding sites of copper(II) ions were compared with computational scientific predictions.
