Comparison of percutaneous transhepatic portal vein embolization and unilateral portal vein ligation

AIm： To compare the effect of percutaneous transhepatic portal vein embolization （PTPE） and unilateral portal vein ligation （PVL） on hepatic hemodynamics and right hepatic lobe （RHL） atrophy.mETHODS： Between march 2005 and march 2009, 13 cases were selected for PTPE （n = 9） and PVL （n = 4） in the RHL. The PTPE group included hilar bile duct carcinoma （n = 2）, intrahepatic cholangiocarcinoma （n = 2）, hepatocellular carcinoma （n = 2） and liver metastasis （n = 3）. The PVL group included hepatocellular carcinoma （n = 2） and liver metastasis （n = 2）. In addition, observation of postoperative hepatic hemodynamics obtained from computed tomography and Doppler ultrasonography was compared between the two groups.RESULTS： mean ages in the two groups were 58.9 ± 2.9 years （PVL group） vs 69.7 ±3.2 years （PTPE group）, which was a significant difference （P = 0.0002）. Among the indicators of liver function, including serum albumin, serum bilirubin, aspartate aminotransferase, alanine aminotransferase, platelets and indocyanine green retention rate at 15 min, no significant differ- ences were observed between the two groups. Preop-erative RHL volumes in the PTPE and PVL groups were estimated to be 804.9 ±181.1 mL and 813.3 4±129.7 mL, respectively, with volume rates of 68.9% ± 2.8% and 69.2% ±4.2%, respectively. There were no significant differences in RHL volumes （P = 0.83） and RHL volume rates （P = 0.94）, respectively. At 1 mo after PTPE or PVL, postoperative RHL volumes in the PTPE and PVL groups were estimated to be 638.4±153.6 mL and 749.8 ± 121.9 mL, respectively, with no significant difference （P = 0.14）. Postoperative RHL volume rates in the PTPE and PVL groups were estimated to be 54.6% ± 4.2% and 63.7% ± 3.9%, respectively, which was a significant difference （P = 0.0056）. At 1 mo after the operation, the liver volume atrophy rate was 14.3% ± 2.3% in the PTPE group and 5.4%± 1.6% in the PVL group, which was a significant difference （P = 0.0061）.CONCLUSION： PTPE is a

Hepatectomy is an effective surgical treatment for multiple bilobar liver metastases from colon cancer;however,one of the primary obstacles to completing surgical resection for these cases is an insufficient volume of the future remnant liver,which may cause postoperative liver *** induce atrophy of the unilateral lobe and hypertrophy of the future remnant liver,procedures to occlude the portal vein have been conventionally used prior to major *** report a case of a 50-year-old woman in whom two-stage hepatectomy was performed in combination with intraoperative ligation of the portal vein and the bile duct of the right hepatic *** procedure was designed to promote the atrophic effect on the right hepatic lobe more effectively than the conventional technique,and to the best of our knowledge,it was used for the first time in the present *** successful induction of liver volume shift as well as the following procedure,the patient died of subsequent liver failure after developing recurrent *** discuss the first case in which simultaneous ligation of the portal vein and the biliary system was successfully applied as part of the first step of two-stage hepatectomy.

Otosclerosis is a well-known disease which mainly affects middle-aged female patients,causing conductive or mixed hearing *** is essentially made through a complete audiometric battery testing,while treatment substantially relies on stapes *** this study,we analyzed audiometric and Quality-of-Life data from 33 patients in order to refine the surgical indications(unilateral versus bilateral stapes surgery)and assess the best technique(stapedotomy versus partial stapedectomy)and timing of *** original questionnaire OTOS,in the Italian language,was administered to all *** this study,most of the patients were female(18/33),with a mean age of 53 years *** surgical techniques appear to be effective with no significant difference in post-operative Air Bone *** questionnaire appeared to be effective in assessing quality of life in patients treated by stapes ***-operative QoL appeared to be similar in bilaterally and unilaterally operated patients,suggesting that a contralateral intervention after effective surgery should be undertaken only if expressly requested by the patient.

Cellular reprogramming technologies have been developed with different physicochemical factors to improve the reprogramming efficiencies of induced pluripotent stem cells(iPSCs).Ultrasound is a clinically applied noncontact biophysical factor known for regulating various cellular behaviors but remains uninvestigated for cellular ***,we present a new reprogramming strategy using low-intensity ultrasound(LIUS)to improve cellular reprogramming of iPSCs in vitro and in *** 3D microenvironment conditions,increased LIUS stimulation shows enhanced cellular reprogramming of the *** cellular reprogramming process facilitated by LIUS is accompanied by increased mesenchymal to epithelial transition and histone *** stimulation transiently modulates the cytoskeletal rearrangement,along with increased membrane fluidity and mobility to increase HA/CD44 ***,LIUS stimulation with HA hydrogel can be utilized in application of both human cells and in vivo environment,for enhanced reprogrammed cells into ***,LIUS stimulation with a combinatorial 3D microenvironment system can improve cellular reprogramming in vitro and in vivo environments,which can be applied in various biomedical fields.

The bacterial pathogen Helicobacter pylori (H. pylori) colonizes in over half of the world’s population. H. pylori that establishes life-long infection in the stomach is definitely associated with gastro-duodenal diseases and a wide variety of non-gastrointestinal tract conditions such as immune thrombocytopenia. Triple therapy which consists of a proton pump inhibitor and combinations of two antibiotics (amoxicillin, clarithromycin or amoxicillin, metronidazol) is commonly used for H. pylori eradication. Recently, the occurrence of drug-resistant H. pylori and the adverse effect of antibiotics have severely weakened eradication therapy. Generally antibiotics induce the disturbance of human gastrointestinal microflora. Furthermore, there are inappropriate cases of triple therapy such as allergy to antibiotics, severe complications (liver and/or kidney dysfunction), the aged and people who reject the triple therapy. These prompt us to seek alterative agents instead of antibiotics and to develop more effective and safe therapy with these agents. The combination of these agents actually may result in lower a dose of antibiotics. There are many reports world-wide that non-antibiotic substances from natural products potentially have an anti-H. pylori agent. We briefly review the constituents derived from nature that fight against H. pylori in the literature with our studies.

AIm： To examine the role of E-cadherin and betacatenin in carcinogenesis and to assess their prognostic implication in Epstein-Barr virus-associated gastric carcinomas （EBV-GCs）. mETHODS： We compared the frequency of E-cadherin and beta-catenin expression in 59 EBV-GCs and 120 non-EBV-GCs, and examined the association between patients＇ prognosis and the expressions of these proteins. RESULTS： Neither the cellular-membranous nor the cytoplasmic E-cadherin expression showed any difference between EBV-GCs and non-EBV-GCs. On the other hand, loss of membranous expression of beta- catenin occurred more frequently in non-EBV-GCs than EBV-GCs [odds ratio = 0.41; 950 confidence interval （CI）, 0.19-0.90]. Furthermore, the nuclear and/or cytoplosmic expression of beta-catenin was seen more frequently in EBV-GCs than non-EBV-GCs （odds ratio = 2.23; 95% CI, 0.97-5.09）, and was observed in a larger proportion of carcinoma cells of EBV-GCs than non-EBV-GCs （P = 0.024）. Survival analysis for non-EBV-GC revealed that lymph node metastasis was significantly associated with poor prognosis （P 〈 0.001）. Among EBV- GCs, the depth of invasion （P = 0.005）, lymph node metastasis （P = 0.004） and an intestinal type by Lauren classification （hazard ratio = 9.47; 95% CI, 2.67-33.6） were significantly associated with poor prognosis. On the other hand, nuclear and/or cytoplasmic expression of beta-catenin was associated with a better prognosis in patients with EBV-GC （hazard ratio = 0.32; 95% CI, 0.11-0.93）. CONCLUSION： We observed more frequent preservation of beta-catenin in cell membrane and accumulation in nuclei and/or cytoplasm in EBV-GCs than in non-EBV- GCs. Factors involved in the prognosis of EBV-GCs and non-EBV-GCs are different in the two conditions.

AIm： To investigate features of Epstein-Barr virus （EBV）associated gastric carcinoma （EBVaGC） among a mexican population. mETHODS： Cases of primary gastric adenocarcinoma were retrieved from the files of the Departments of Pathology at the Instituto Nacional de Cancerologia and the Instituto Nacional de la Nutrición in mexico City. The anatomic site of the gastric neoplasia was identified, and carcinomas were histologically classified as intestinal and diffuse types and subclassified as proposed by the Japanese Research Society for Gastric Cancer. EBV-encoded small non-polyadenylated RNA-1 （EBER-1） in situ hybridization was conducted to determine the presence of EBV in neoplastic cells. RESULTS： We studied 330 consecutive, non-selected, primary gastric carcinomas. Among these, there were 173 male and 157 female patients （male/female ratio 1.1/1）. EBER-1 was detected in 24 （7.3%） cases （male/ female ratio： 1.2/1）. The mean age for the entire group was 58.1 years （range： 20-88 years）, whereas the mean age for patients harboring EBER-1-positive gastric carcinomas was 65.3 years （range： 50-84 years）. Age and histological type showed statistically significant differences, when EBER-1-positive and -negative gastric carcinomas were compared. EBER-1 was detected in hyperplastic- and dysplastic-gastric mucosa surrounding two EBER-1-negative carcinomas, respectively. CONCLUSION： Among Latin-American countries, mexico has the lowest frequency of EBVaGC. Indeed, the mexican population 〉50 years of age was selectively affected. Ethnic variations are responsible for the epidemiologic behavior of EBVaGC among the worldwide population.
