Diagnosis and Treatment of Attention-Deficit Hyperactivity Disorder in Patients with Chronic Pain

Aims: To investigate rates of attention-deficit hyperactivity disorder (ADHD) in patients with chronic pain attending a pain clinic, the effects of a screening measure for ADHD in patients with chronic pain, and the effects of ADHD drugs on both pain and ADHD symptoms. Methods: We retrospectively surveyed 110 patients with chronic pain visiting the Anesthesiology and Pain Relief Center at the University of Tokyo in Japan, who had also consulted a psychiatrist, between April 2012 and July 2015. Results: Of the total of 110 patients with chronic pain, 35 (31.8%) were also diagnosed with ADHD, and the average Wender Utah Rating Scale (WURS) score among the ADHD patients was 39.0 ± 22.1 (n = 25). Only 36.0% of these patients exceeded the cutoff value, suggesting that 64.0% of the patients with ADHD were not identified by screening with the WURS. Twenty-six patients initiated treatment with ADHD medication, with dosage adjustment completed in 21. Of these 21 patients 20 (95.0%) had improved ADHD symptoms. Improved pain symptoms were observed in 14 patients (66.6%), with a reduction in the pain numerical rating scale of 64.7% ± 30.1%. Conclusions: This is the first study investigating the comorbidity of ADHD and chronic pain at pain clinics showing a high level of comorbidity and amelioration of pain and ADHD symptoms with treatment. Careful interpretation is required when the WURS is used to screen patients with chronic pain.

Perinatal outcomes associated with meconium-stained amniotic fluid in Japanese singleton pregnancies

Introduction: We examined the perinatal outcomes in Japanese singleton pregnancies associated with meconium-stained amniotic fluid (MSAF) in relation to gestational age at delivery. Methods: We reviewed the obstetric records of all Japanese singleton deliveries after 22 weeks’ gestation managed at Japanese Red Cross Katsushika Maternity Hospital between 2002 and 2008 (n = 11,249). Results: The incidence of MSAF in the whole singleton pregnancies was 13%. The incidence of MSAF at preterm, term and post-term were 9.1%, 13% and 48%, respectively. The incidence of intrauterine fetal death, low Apgar score and low umbilical artery pH at delivery in cases with MSAF were significantly higher than those without MSAF in various gestational ages at delivery. Conclusion: Obstetric management should be affected by meconium in the amniotic fluid.

Mineral Components and Anti-oxidant Activities of Tropical Seaweeds

Seaweeds are known to hold substances of high nutritional value; they are the richest resources of minerals important to the biochemical reactions in the human body. Seaweeds also hold non-nutrient compounds like dietary fiber and polyphenols. However, there is not enough information on the mineral compounds of tropical seaweeds. Also we are interested in the antioxidant activities of seaweeds, especially those in the tropical area. In this study, Indonesian green, brown and red algae were used as experimental materials with their mineral components analyzed by using an atomic absorption spectrophotometer. The catechins and flavonoids of these seaweeds were extracted with methanol and analyzed by high performance liquid chromatography （HPLC） ; the anti-oxidant activities of these seaweeds were evaluated in a fish oil emulsion system. The mineral components of tropical seaweeds are dominated by calcium, potassium and sodium, as well as small amounts of copper, iron and zinc. A green alga usually contains epigallocatechin, gallocatechin, epigallocatechin gallate and catechin. However, catechin and its isomers are not found in some green and red algae. In the presence of a ferrous ion catalyst, all the methanol extracts from the seaweeds show significantly lower peroxide values of the emulsion than the control, and that of a green alga shows the strongest anti-oxidant extract of this alga, which is significantly different from activity. The highest chelation on ferrous ions is also found in the the other methanol extracts in both 3 and 24 h incubations.

The effect of postoperative epidural analgesia in women possessing severe gestational hypertension undergoing cesarean delivery

Introduction: The purpose of this study was to examine the clinical usefulness of postoperative epidural analgesia in patients possessing severe gestational hypertension after Cesarean delivery. Methods: We reviewed the obstetric records of 99 patients possessing severe gestational hypertension undergoing singleton Cesarean delivery at ≥ 22 weeks’ gestation. Thirty patients were received continuous epidural analgesia with 0.2% ropivacaine for pain relief after Cesarean delivery with spinal anesthesia, 69 patients were not received epidural analgesia after Cesarean delivery with spinal anesthesia. Results: During the preoperative period, there were no measurable differences in the diastolic blood pressure between the 2 groups (108 vs± 7 vs. 106 mmHg ± 10 mmHg, p = 0.29). The diastolic blood pressure at 2 and 4 hours after Cesarean section in the epidural analgesia group were significantly lower than those in the non-epidural group (2 hours after Cesarean section: 88 vs ± 13 vs. 95 mmHg ± 8 mmHg, p < 0.01;4 hours after Cesarean section: 92 vs ± 15 vs. 102 mmHg ± 9 mmHg, p < 0.01). Conclusions: The current results indicated that the postoperative epidural analgesia can inhibit the rise in diastolic blood pressure in patients possessing severe gestational hypertension after Cesarean delivery. BACKGROUND Diffuse large B-cell lymphoma(DLBCL)is the most common type of malignant lymphoma(ML),accounting for 30%-40%of cases of non-Hodgkin’s lymphoma(NHL)in *** paranasal sinus lymphoma is a rare presentation of extranodal NHL that accounts for only 0.17%of all *** from the maxillary sinus(MS)is a particularly rare presentation,and is thus often difficult to *** have reported the first known case of DLBCL originating from the MS with rapidly occurrent multiple skin *** SUMMARY An 81-year-old Japanese man visited our hospital due to continuous pain for 12 d in the left maxillary nerve *** medical history included splenectomy due to a traffic injury,an old right cerebral infarction from when he was 74-years-old,hypertension,and type 2 diabetes mellitus.A plain head computed tomography(CT)scan revealed a 3 cm×3.1 cm×3 cm sized left *** day 25,left diplopia and ptosis occurred,and a follow-up CT on day 31 revealed the growth of the left MS *** on an MS biopsy on day 50,we established a definitive diagnosis of DLBCL,non-germinal center B-cell-like originating from the left *** patient was admitted on day 62 due to rapid deterioration of his condition,and a plain CT scan revealed the further growth of the left MS mass,as well as multiple systemic metastasis,including of the skin.A skin biopsy on day 70 was found to be the same as that of the left MS *** notified the patient and his family of the disease,and they opted for palliative care,considering on his condition and *** patient died on day *** This case suggests the need for careful,detailed examination,and for careful follow-up,when encountering patients presenting with a mass.
