Clinicopathological features of alpha-fetoprotein producing early gastric cancer with enteroblastic differentiation

AIM To investigate clinicopathological features of early stage gastric cancer with enteroblastic differentiation(GCED).METHODS We retrospectively investigated data on 6 cases of early stage GCED and 186 cases of early stage conventional gastric cancer(CGC: well or moderately differentiated adenocarcinoma) who underwent endoscopic submucosal dissection or endoscopic mucosal resection from September 2011 to February 2015 in our *** was defined as a tumor having a primitive intestine-like structure composed of cuboidal or columnar cells with clear cytoplasm and immunohistochemical positivity for either alpha-fetoprotein, Glypican 3 or SALL4. The following were compared between GCED and CGC: age, gender, location and size of tumor, macroscopic type, ulceration, depth of invasion, lymphatic and venous invasion, positive horizontal and vertical margin, curative resection *** Six cases(5 males, 1 female; mean age 75.7 years; 6 lesions) of early gastric cancer with a GCED component and 186 cases(139 males, 47 females; mean age 72.7 years; 209 lesions) of early stage CGC were investigated. Mean tumor diameters were similar but rates of submucosal invasion, lymphatic invasion, venous invasion, and non-curative resection were higher in GCED than CGC(66.6% vs 11.4%, 33.3% vs 2.3%, 66.6% vs 0.4%, 83.3% vs 11% respectively, P < 0.01). Deep submucosal invasion was not revealed endoscopically or by preoperative biopsy. Histologically, in GCED the superficial mucosal layer was covered with a CGC component. The GCED component tended to exist in the deeper part of the mucosa to the submucosa by lymphatic and/or venous invasion, without severe stromal reaction. Surface molecular motions of amorphous polymeric solids have been directly measured on the basis of scanningviscoelasticity microscopic(SVM)and lateral force microscopic(LFM)*** and LFM measurements werecarried out for films of conventional monodisperse polystyrene(PS)with sec-butyl and proton-terminated end groups atroom *** the case of the number-average molecular weight,M_(n),less than ca.4.0×10^(4),the surface was in a glass-rubber transition state even though the bulk glass transition temperature,T_(g)was far above room temperature,meaning thatthe surface molecular motion was fairly active compared with that in the *** measurements of the,monodisperse PSfilms at various scanning rates and temperatures revealed that the time-temperature superposition was applicable to thesurface mechanical relaxation behavior and also that the surface glass transition temperature,T_(g)^(σ),was depressed incomparison with the bulk one even though the magnitude of M_n was fairly high at 1.40×10~*** surface molecular motionof monodisperse PS with various chain end groups was investigated on the basis of temperature-dependent scanningviscoelasticity microscopy(TDSVM).The T_(g)^(σ)s for the PS films with M_n of 4.9×10^(6)to 1.45×10^(6)measured by TDSVMwere smaller than those for the bulk one,with corresponding M_ns,and the T_(g)^(σ)s for M_ns smaller than ca.4.0×10^(4)were lowerthan room temperature(293 K).The active thermal molecular motion at the polymeric solid surface can be interpreted interms of an excess free volume near the surface region induced by the surface localization of chain end *** the case ofM_n=ca.5.0×10^(4),the T_(g)^(σ)s for theα,ω-diamino-terminated PS(α,ω-PS(NH_2)_2)andα,ω-dicarboxy-terminated PS(α,ω-PS(COOH)_2)films were higher than that of the PS *** change of T_(g)^(σ)for the PS film with various chain end groups canbe explained in terms of the depth distribution of chain end groups at the surface region depending on

The phase transition behavior and aggregating states of [smectic LCP, PS(4BC/DM)/nematic LC, E7/chiral dopant, CB 15] ternary composite system were investigated on the basis of differential scanning calorimetry (DSC) measurement, polarizing optical microscopy (POM) observation and wide angle X ray diffraction (WAXD) patterns. It was demonstrated that if the weight ratio of the chiral dopant was fixed as 4.8wt%, an induced chiral nematic (N *) phase existed in the ternary system when PS(4BC/DM) ratio was below 47.6 wt%. Moreover, smectic like long range ordering appeared in the induced N * phase in a certain temperature range just above the smectic A(S A) N * phase transition when PS(4BC/DM) ratio was between 19.1wt% and 47.6wt%.

Prognosis of hepatocellular carcinoma accompanied by microscopic portal vein invasion

AIM:To investigate the prognostic factors in patients with hepatocellular carcinoma(HCC) accompanied by microscopic portal vein invasion(PVI).METHODS:Of the 267 patients with HCC undergoing hepatic resection at Aso Iizuka Hospital,71 had *** excluding 16 patients with HCC that invaded the main trunk and the first and second branches of the portal vein,55 patients with microscopic PVI were ***:The patients with HCC accompanied by microscopic invasion were divided into two groups:solitary PVI(PVI-S:n = 44),and multiple PVIs(PVI-M:n = 11).The number of portal vein branches invaded by tumor thrombi was 5.4 ± 3.8(2-16) in patients with *** cumulative survival,PVI-M was found to be a significantly poor prognostic factor(P = 0.0019);while PVI-M and non-anatomical resection were significantly poor prognostic factors in disease-free survival(P = 0.0213,and 0.0115,respectively).In patients with PVI-M,multiple intrahepatic recurrence was more common than in the patients with PVI-S(P = 0.0049).In patients with PVI-S,non-anatomical resection was a significantly poor prognostic factor in disease-free survival(P = 0.0370).Operative procedure was not a significant prognostic factor in patients with ***:The presence of PVI-M was a poor prognostic factor in patients with HCC,accompanied by microscopic *** resection is recommended in these patients with *** with HCC and PVI-M may also be good candidates for adjuvant chemotherapy.

Safety and effectiveness of propofol sedation during and after outpatient colonoscopy

Prospective randomized controlled trial investigating the type of sutures used during hepatectomy

AIM: To determine whether absorbable sutures or non-absorbable sutures are better in preventing surgical site infection (SSI), in this paper we discuss the results of a randomized clinical trial which examined the type of sutures used during hepatectomy. METHODS: All hepatic resections performed from January 2007 to November 2008 at the Department of Surgery at Iizuka Hospital in Japan were included in this study. There were 125 patients randomly assigned to an absorbable sutures (Vicryl) group or non-absorbable sutures (Silk) group. RESULTS: SSI was observed in 13.6% (17/125) patients participating in this study, 11.3% in the Vicryl group and 15.8% in the Silk group. Incisional SSI including superficial and deep SSI, was observed in 8% of the Vicryl group and 9.5% of the Silk group. Organ/ space SSI was observed in 3.2% of the Vicryl group and 6.0% of the Silk group. There were no significant differences, but among the patients with SSI, the period for recovery was significantly shorter for the Vicryl group compared to the Silk ***: The incidence of SSI in patients receiving absorbable sutures and silk sutures is not significantly different in this randomized controlled study; however, the period for recovery in patients with SSI was significantly shorter for absorbable sutures.

Effectiveness of outpatient percutaneous endoscopic gastrostomy replacement using esophagogastroduodenoscopy and propofol sedation

AIM: To evaluate the effectiveness of outpatient percutaneous endoscopic gastrostomy (PEG) replacement using esophagogastroduodenoscopy (EGD) and propofol sedation. METHODS: We retrospectively assessed the outcome and complications of consecutive patients referred for PEG replacement which was performed using EGD under propofol sedation in the outpatient setting. The success rate, the mean dose of propofol, procedure time, EGD findings, discharge time from endoscopy unit, respiratory depression, and complications within 72 h of the procedure were evaluated. In a subset of these patients, the blood concentrations of propofol were measured. RESULTS: All 221 patients underwent successful PEG replacement. The mean dose of propofol was 34 mg (range, 20-60 mg) with a mean procedure time of 5.9 min (range, 3-8 min). Reflux esophagitis (12 patients), gastric ulcer (5), gastric neoplasm (2), and duodenal ulcer (1) were newly diagnosed at replacement. Discharge from endoscopy unit was possible in 100% of patients 45 min after the procedure. Only 3.6% (8) required transient supplemental oxygen. No complications occurred within 72 h of the procedure. During EGD the level of sedation and propofol blood concentrations after administration of propofol (30 mg) in these PEG patients corresponded to those of propofol (60 mg) in middle aged subjects (control). CONCLUSION: PEG replacement using EGD and propofol sedationin the outpatient setting was safe and practical.

Prognostic Impact of Carcinoembryonic Angtigen (CEA) in Locally Advanced Cervical Cancer Treated with Concurrent Chemoradiotherapy

Objective: To identify prognostic factors in patients with locally advanced cervical cancer treated with concurrent chemoradiotherapy (CCRT). Methods and Materials: We analyzed 76 patients with FIGO stage IB2 - IVb cervical cancer treated with CCRT between 2001 and 2006 at the Nagoya University Hospital. Patients with an advanced cervical cancer treated with CCRT. Overall survival (OS) and Progression-free survival (PFS) rates were estimated using the Kaplan-Meier method. The log-rank test was used to test differences in survival. Fisher's exact test was employed for univariate analysis. The Cox proportional hazard model was used for multivariate analysis. Results: The median age was 52, and the median follow-up period was 36 months. The 5 - year OS and PFS rates of all patients were 88.2% and 72.4%, respectively. Twenty-one of the 76 patients were diagnosed with recurrence. A higher serum CEA before CCRT was an independent predictive factor for a poor prognosis on multivariate analysis. Conclusions: A high level of serum CEA was a predictive factor for a poor prognosis. New strategies should be considered to control disease in this group of patients.
