Real-time porosity reduction during metal directed energy deposition using a pulse laser

Porosity is a challenging issue in additive manufacturing and is detrimental to the quality of the additively manufactured *** this study,a real-time porosity reduction technique was developed by incorporating a pulse laser into a laser metal powder directed energy deposition(DED)*** incorporated pulse laser can accelerate fluid flow within the melt pool and facilitate the escape of pores before complete *** achieves this real-time porosity reduction by inducing accelerated and turbulent Marangoni flow,ultrasonic waves,and shock waves into the melt *** uniqueness and advantages of the proposed technique include the following:(1)For a laser metal powder DED process,this study proposed a noncontact,nondestructive,and real-time porosity reduction technique at the melt pool level.(2)It was experimentally and numerically validated that the developed technique did not alter the geometry and the grain structure of the manufactured Ti-6Al-4V samples.(3)because the porosity reduction is accomplished at the melt pool level,its application is not limited by the size,shape,or complexity of the printing target.(4)The developed technique can be readily incorporated into the existing DED systems without any modification of the original tool-path *** experimental results showed that the pore volume fraction decreased from 0.132%to 0.005%,no pores larger than 6×10^(4)μm^(3) were observed,and a 91%reduction in the total pore number was achieved when the pulse laser energy reached 11.5 mJ.

The etiology and disease patterns of hepatocellular carcinoma(HCC)significantly vary among regions. Modern standard treatments commonly require multidisciplinary approaches, including applications of up-to date medicine and advanced procedures, and necessitate the support of socioeconomic systems. For these reasons, a number of clinical guidelines for HCC from different associations and regions have been presented. External beam radiation therapy was contraindicated for HCC until a few decades ago, but with the development of new technologies, its application has rapidly increased as selective irradiation for tumorous lesions became possible. Most of the guidelines had been opposed or indifferent to radiotherapy in the past, but several guidelines have introduced indications and recommendations for radiotherapy in their updated versions. This review will discuss the characteristics of important guidelines and their contents regarding radiotherapy and will also provide guidance to physicians who are considering applications of locoregional modalities that include radiotherapy.

The volume of academic literature,such as academic conference papers and journals,has increased rapidly worldwide,and research on metadata extraction is ***,high-performing metadata extraction is still challenging due to diverse layout formats according to journal *** accommodate the diversity of the layouts of academic journals,we propose a novel LAyout-aware Metadata Extraction(LAME)framework equipped with the three characteristics(e.g.,design of automatic layout analysis,construction of a large meta-data training set,and implementation of metadata extractor).In the framework,we designed an automatic layout analysis using PDF *** on the layout analysis,a large volume of metadata-separated training data,including the title,abstract,author name,author affiliated organization,and keywords,were automatically ***,we constructed a pre-trainedmodel,Layout-Meta bERT,to extract the metadata from academic journals with varying layout *** experimental results with our metadata extractor exhibited robust performance(Macro-F1,93.27%)in metadata extraction for unseen journals with different layout formats.

AIM:To evaluate long-term clinical course of buddChiari syndrome(bCS) and predictive factors associated with the development of hepatocellular carcinoma(HCC) and ***:We analyzed 67 patients with bCS between June 1988 and May *** diagnosis of bCS was confirmed by hepatic venous outflow obstruction shown on abdominal ultrasound sonography,computed tomography,magnetic resonance imaging,or *** median follow-up period was 103 ± 156 [interquartile range(IQR)] ***:The median age of the patients was 47 ± 16(IQR) *** diagnosis,54 patients had cirrhosis,25(37.3%) Child-Pugh class A,23(34.3%) Child-Pugh class b,and six(9.0%) patients Child-Pugh class *** the follow-up period,HCC was developed in 17 patients,and the annual incidence of HCC in patients with bCS was 2.8%.Patients in HCC group(n = 17) had higher hepatic venous pressure gradient(HVPG) than those in non-HCC group(n = 50)(21 ± 12 mmHg vs 14 ± 7 mmHg,P = 0.019).The survival rate of bCS patients was 86.2% for 5 years,73.8% for 10 years,and 61.2% for 15 *** patients with bCS and HCC,survival was 79% for 5 years,43.1% for 10 years,and 21.5% for 15 ***:The incidence of HCC in patients with bCS was similar to that in patients with other etiologic cirrhosis in South *** HVPG is expected to provide additional information for predicting HCC development in bCS patients.

Chronic hepatitis b infection is associated with the development of cirrhosis, hepatocellular carcinoma, and finally liver-related mortality. Each year, approximately, 2%-5% of patients with hepatitis b virus (HbV)-related compensated cirrhosis develop decompensation, with additional clinical manifestations, such as ascites, jaundice, hepatic encephalopathy, and gastrointestinal bleeding. The outcome of decompensated HbV-related cirrhosis is poor, with a 5-year survival of 14%-35% compared to 84% in patients with compensated cirrhosis. because the risk of disease progression is closely linked to a patient’s serum HbV DNA level, antiviral therapy may suppress viral replication, stabilize liver function and improve survival. This article briefly reviews the role that antiviral therapy plays in cirrhosis complications, particularly, in decompensation and acute-on-chronic liver failure.

AIM: To describe the clinical features and multimodal images of unilateral foveomacular retinitis in young korean soldiers. METHODS: Ten patients having foveomacular retinitis were included. Fluorescein angiography, fundus autofluorescence(FAF), infrared reflectance(IR), and spectral-domain optical coherence tomography(SD-OCT) were ***: All patients were young males experienced insidious visual decline without exposure to bright light. Initial and final vision ranged from hand movement to 20/20(median 20/200) and 20/2000 to 20/20(median 20/500), respectively. Vision decreased in 6 patients while improved in two. Typical macular abnormality was yellow granular spots. SD-OCT showed ellipsoid zone(EZ) or interdigitation zone(IZ) disruption of fovea. The degree of EZ/IZ damage correlated with vision. Lesions were clearly visualized through IR and matched with SD-OCT ***: This is the first case series of foveomacular retinitis diagnosed with multimodal imaging. Foveomacular retinitis should be suspected in sudden unilateral visual decline especially in young soldiers. SD-OCT is the most important diagnostic modality.

Liver biopsy is the gold standard for evaluating the degree of liver fibrosis in patients with chronic liver disease. However, due to the many limitations of liver biopsy, there has been much interest in the use of noninvasive techniques for this purpose. Among these techniques real-time two-dimensional shear wave elastography(2D-SWE) has the advantage of measuring tissue elasticity with the guidance of b-mode images. Recently, many studies have been conducted on the application of 2D-SWE in patients with various liver diseases, and their validity has been confirmed. Here, we briefly discuss the role of 2D-SWE in patients with chronic liver diseases, particularly aspects of the examination techniques and clinical applications.
